RESUMO

Despite the increasing popularity of total ankle replacement, ankle arthrodesis remains the gold standard for the treatment of end-stage ankle arthritis. Historically, open techniques have been utilized for ankle arthrodesis. There have been many variations and techniques described, including transfibular, anterior, medial, and miniarthrotomy. Inherent disadvantages to these open techniques include postoperative pain, delayed or nonunion, wound complications, shortening, prolonged healing times, and prolonged hospital stays. Arthroscopic ankle arthrodesis provides the foot and ankle surgeon with an alternative to the traditional open techniques. Arthroscopic ankle arthrodesis has demonstrated faster union rates, decreased complications, reduced postoperative pain, and shorter hospital stays.

RESUMO

INTRODUCTION: Peroneus brevis flaps provide a viable option to achieve soft tissue coverage in hard-to-heal lower extremity wounds, specifically those to the lateral ankle and hindfoot. CASE REPORT: The authors present a unique case of a patient with a 20-year-old wound dehiscence complicated by osteomyelitis. The wound was a complication from a lateral extensile incision utilized during prior calcaneal open reduction and internal fixation. Due to many factors, including multiple comorbidities, the patient could not obtain complete healing despite IV antibiotics, vascular optimization, local wound care, surgical debridement, and grafting. Wound closure was ultimately achieved with a PB muscle flap. Adjunctive therapies also utilized included multilevel ring external fixation, negative pressure wound therapy, and hyperbaric oxygen therapy. On follow-up 32 months after the procedure, the patient continued to be wound free and satisfied with the results. CONCLUSIONS: This case report demonstrates the utility of PB muscle flaps for hard-to-heal lower extremity wounds in patients with comorbidities.

RESUMO

Understanding new theories of the epidemiology of Charcot neuroarthropathy is practice changing. Treatment of Charcot neuroarthropathy is evolving from a passive approach to one that sees the urgency of proactive, early recognition, thereby avoiding the cascading events that lead to the complex, limb-threatening deformities. Preventive medicine is the most efficient at avoiding severe deformity, with prolonged offloading and immobilization as the current mainstay of treatment. However, with recent advancements in medical and surgical modalities, this may become the treatment of the past as clinicians begin to favor medical management and early surgical intervention.
