RESUMO

Cardiac inflammatory myofibroblastic tumor (IMT) is a rare benign tumor that occurs mainly in infants and children. We report a case of right atrial IMT in a 1-month-old girl with episodic cyanosis. Both echocardiography and cardiac computed tomography revealed a right atrial mass. The patient underwent a complete surgical resection of the mass and the postoperative histological examination confirmed the diagnosis of cardiac IMT. The patient recovered well after the operation and further follow-up evaluation was uneventful.

Preschoolers can focus on the events that they are interested in and shield other information that may still be obvious and important, and this phenomenon is termed as inattentional blindness (IB). The present study investigated the developmental differences in IB and explored its relationship with fluid intelligence in 3-to-5-year-old preschoolers. With the involvement of one hundred and thirty-five preschoolers, it was found that the IB rates decreased with age during 3-to-5-year-olds. Fluid intelligence could significantly predict the incidence of IB in 4-year-olds, which means that those with higher fluid intelligence scores experience less IB. It implied that the ability to detect unintentional stimuli increased from 3-to-5-year-olds, whereas age was an impact factor to the relationship between fluid intelligence and IB.

BACKGROUND: The proportion of abnormal electrocardiogra (ECG) in patients with coronary artery fistula (CAF) is relatively high, but the correlation between CAF and arrhythmia is mostly reported in individual case studies. This paper analyzes the correlation between imaging features and ECG features. OBJECTIVE: This paper aims to analyze the incidence and distribution characteristics of abnormal ECG in patients with CAF and further explore the difference in ECG characteristics between coronary-cameral fistula (CCF) and coronary-pulmonary artery fistula (CPAF). METHOD: A total of 144,448 patients who underwent coronary computerized tomography angiography (CTA) examination from January 2016 to December 2022 were included in this study, and 284 patients with CAF (excluding coronary atherosclerosis) were selected for analysis of their ECG and image characteristics. And divided them into the CPAF (221 cases) and CCF (63 cases) groups, the differences in ECG between the two groups was compared. The changes in the ECG after the operation were analyzed. RESULTS: The incidence of abnormal ECG in patients with CAF was approximately 72.9%. There were significant differences in the proportion of ECG block, myocardial ischemia and structural ECG changes between the CPAF group and CCF group (P < 0.05). CCF was more likely to cause conduction block and ischemic and structural ECG changes. A total of 53 patients with CAF underwent surgical treatment, 28 patients with improved ECG (52%). CONCLUSION: CCF especially CCF patients often have abnormal ECG findings such as conduction block, myocardial ischemia, and structural changes, which can often be restored to normal through surgery.

In this study, the outcome serves to establish a measurement model to calculate the value of ecosystem services in Lhasa River Basin, China. Following on the principle of resource scarcity, a study paradigm is established according to the influence of natural and socio-economic location differences on the value of ecosystem services. Results show that the total value of ecosystem services in Lhasa River Basin is 104.503 billion yuan. The unit ecosystem service value of natural grassland in the lower reaches is 5.6 times and 1.07 times larger than that in the upper and middle reaches, respectively. The value of ecosystem services is clearly affected by the scarcity of ecosystem services and dependence of economic and social development on the ecosystem. The value of ecosystem services in the upper, middle, and lower reaches were 4.17%, 21.48%, and 74.35%, respectively. It is found that the value of ecosystem services in this particular region differs significantly in the upper, middle, and lower reaches of the basin. The scarcity of ecological resources has increased the value of ecosystem services, which is reflected in the highest value of ecosystem services in the middle of Lhasa River Valley and both sides of the river where population, economy, and ecological demand are relatively concentrated. According to the estimated results, this study proposes a variety of methods to improve the ecosystem service value and analysis of the ecological compensation mechanism, derived from the contribution made by ecological services.

The alpine regions of Tibet are biogeographically unique and highly biodiverse. As the political, economic, and cultural center of Tibet, the city of Lhasa's population growth and economic development have further weakened the region's already fragile ecological environment. Coordinating relationships between stable economic development, sustained population growth, rational resource use, and environmental protection has become an urgent issue. This paper establishes an ecological resistance surface based on the ecological resistance factor index to evaluate Lhasa's ecological security level. The obtained results show that the city's ecological security level is good, with high security level in the north, northwest, and northeast, and low-level in the south and the middle of city. High-level ecological security areas accounted for 34.5% of the city's total area, and low-level areas accounted for 9.0%. The overall Moran's I index of the city's ecological security was 0.518. According to a LISA clustering chart, Lhasa's ecological security grades are mainly high-high (HH) and low-low (LL). These two grades showed an apparent flaky spatial clustering in the city. We elected eight large-scale nature reserves in the city as ecological sources, constructed a resistance surface of the ecological accumulation of ecological sources, used the MCR (minimum cumulative resistance) model and gravity model to extract potential ecological corridors, and finally identified potentially important ecological corridors. A total of 51 ecological nodes and 80 potential ecological corridors were extracted, with a total length of about 3449.7 km. The length of the primary and secondary corridors accounted for 32.32% of the total length. Combining the development of Lhasa's ecological economy with tourism and cultural industry planning, a layout of ecological network model with one ring and three belts is proposed. An ecological space development strategy of agglomeration within the ring and axial drive should be implemented. This study provides a decision-making reference for the spatial layout of the ecological industry in Lhasa.

To investigate the capacity of biphasic cardiac CT (CCT) for qualitative and quantitative evaluation of different grades of left atrial appendage spontaneous echo contrast (LAASEC). The study included 267 inpatients with confirmed atrial fibrillation who underwent both CCT and transesophageal echocardiography (TEE). CT numbers for LAA, ascending aorta (AA), and left atrium (LA) were identified, and ROC curves for LAA, LAA/AA, and LAA/LA were plotted. With TEE as the standard, the sensitivity, specificity, PPV, NPV, and accuracy of CCT for LAASEC grade ≥ 1 were 60.3, 92.9, 92.4, 64.8, and 75.7%; and for grade ≥ 2 were 100.0, 84.4, 71.4, 100.0, and 88.8% respectively. The values of LAA, LAA/AA, and LAA/LA were significantly larger in LAASEC grade 0 versus 1 and in grade 1 versus 2, but were similar in grades 2 and 3 or in grades 3 and 4. The values of LAA/AA were larger in grade 2 versus 4. When the cutoff value for LAA/AA = 0.897, sensitivity, specificity, PPV, NPV, and accuracy of CCT for LAASEC grade ≥ 1 was 89.6, 83.2, 87.9, 85.5, and 86.9% and with a cutoff for LAA/AA of 0.524, the sensitivity, specificity, PPV, NPV, and accuracy for LAASEC grade ≥ 2 was 98.7, 92.7, 84.1, 99.4, and 94.4%. Although CCT showed limited diagnostic accuracy for grade 1 LAASEC, grade ≥ 2 LAASEC could be excluded when there was no LAA filling defect on first-phase CCT, and TEE can be avoided. CCT has an excellent accuracy in diagnosing LAASEC, and quantitative analysis (in particular LAA/AA) is superior.

BACKGROUND: Neuropathic pain is a complex state of chronic pain that is usually accompanied by peripheral and central nervous system damage or dysfunction. Previous studies have indicated that neuroinflammation in the spinal cord is an important contributor to neuropathological and behavioral abnormalities. A series of early inflammatory markers, such as IL-1, TNF-α, and IFN-γ, and advanced inflammatory markers, such as high-mobility group box 1 (HMGB1), are involved in neuroinflammation. STUDY DESIGN: A randomized, double blind, controlled animal trial. OBJECTIVE: In this study, a lentivirus delivering human IL-10 (LV/hIL-10) was administered intrathecally to determine the effects of IL-10 on allodynia and hyperalgesia in a chronic constriction injury-induced (CCI) rat model of neuropathic pain. METHODS: Sprague-Dawley rats weighting 260 - 320 g were randomly divided into 4 groups. Group Sham (Sham), Group CCI±Normal Saline (NS), Group CCI±LV/hIL-10 (LV/hIL-10), and Group CCI±LV/control (vector). Rats in each group were intrathecally delivered with NS, LV/control, or recombinant vector LV/hIL-10 in a total volume of 10 µl. Paw withdrawal mechanical thresholds (PWMT) and paw withdrawal thermal latency PWTL were measured one day before CCI (baseline) and 0, 3, 7, 14, and 28 days after intrathecal administration. Cerebrospinal fluid (CSF) samples were collected during surgical plane anesthesia and the collected CSF samples were used to assay for human IL-10, rat IL-1ß, rat IL-6, and rat TNF-α by enzyme-linked immunosorbent assay (ELISA). Animals were sacrificed and the L4-5 lumbar segment of the spinal cord was removed for determination of green fluorescent protein (GFP) expression. Immunohistochemical analysis was performed using anti HMGB1 antibodies and the expression of HMGB1 protein in the spinal cord was determined by western blot analysis after intrathecal delivery (n = 8 each). RESULTS: The results show that intrathecal LV/hIL-10 reverses enhanced pain states. Moreover, the increased level of HMGB1 exhibited in a late stage of CCI was inhibited by exogenous overexpression of hIL-10 in the CCI model. Expression of HMGB1, RAGE, and pAkt were lower in CCI-induced rats treated with LV/hIL-10 than in those treated with LV/control (vector) or saline (NS). Our results showed that IL-10 inhibits activation of the inflammatory HMGB1-RAGE pathway in the CCI rat model. LIMITATIONS: Further experimental investigations are needed to clarify the specific biological roles played by HMGB1 in IL-10-mediated regulation of neuropathic pain. CONCLUSION: Our results indicate that intrathecal lentiviral-mediated transfer of IL-10 attenuates CCI-induced neuropathic pain in rats. The anti-thermal hyperalgesia and anti-mechanical allodynia may be partly attributable to the decreased expression of HMGB1 and inhibition of HMGB1-RAGE pathway.
