RESUMO

We report on an 888 nm-pumped Nd:GdVO4 ring laser operational over a wavelength range from 1340.3 nm to 1342.1 nm with a maximum output power of 7.4 W at 1341.2 nm and a beam quality parameter M2<1.1. To our knowledge this is the highest single-longitudinal-mode power obtained with a Nd:GdVO4 crystal laser. We use a commercial frequency-doubling cavity to achieve 1.2 W at 671.0 nm and 4.0 W at 670.6 nm for use in lithium cold atom experiments. Respectively, these wavelengths are approximately resonant with and 250 GHz blue-detuned from the lithium D-lines. Thus, this source provides ample power for laser cooling of lithium atoms while also offering substantial power for experiments requiring light 10's to 100's of GHz blue-detuned from the primary lithium transitions.

RESUMO

Translation fracture of the lumbar spine is a rare but serious condition that necessitates prompt medical attention. This injury can cause nerve damage, spinal cord compression, and other complications that can affect motor function. The motoric outcomes of this fracture type depend on a variety of factors, including the severity and location of the fracture, the age and general health of the patient, and the timeliness and effectiveness of treatment. Accurate diagnosis and treatment of these injuries is important to prevent further neurological damage and improve motoric outcomes. Here we present the case of a male patient with a translation fracture at the L1-L2 level with AO spine type C who underwent immediate realignment and posterior stabilization, and subsequently participated in an early rehabilitation program, resulting in improved neurologic function. Thoracolumbar fracture with lateral dislocation is very rare and significant experience is needed to determine which management strategy can ensure the best outcome.

This paper presents the generation of orbital angular momentum (OAM) vortex waves with mode +1 using dielectric resonator antenna (DRA) array. The proposed antenna was designed and fabricated using FR-4 substrate to generate OAM mode +1 at 3.56 GHz (5G new radio band). The proposed antenna consists of 2 × 2 rectangular DRA array, a feeding network, and four cross slots etched on the ground plane. The proposed antenna succeeded in generating OAM waves; this was confirmed by the measured radiation pattern (2D polar form), simulated phase distribution, and intensity distribution. Moreover, mode purity analysis was carried out to verify the generation of OAM mode +1, and the purity obtained was 53.87%. The antenna operates from 3.2 to 3.66 GHz with a maximum gain of 7.3 dBi. Compared with previous designs, this proposed antenna is low-profile and easy to fabricate. In addition, the proposed antenna has a compact structure, wide bandwidth, high gain, and low losses, thus meeting the requirements of 5G NR applications.

Nuclear energy is a choice to meet the electricity needs of West Kalimantan, Indonesia to support new industrial areas. Nuclear Power Plants (NPPs) require an appropriate location in terms of biogeophysical, socio-economic, and disaster considerations. This study aims to determine potential sites for NPPs using spatial-weighted multicriteria analysis. GIS and analytical hierarchical processes (AHP) were combined to produce suitable NPPs site. This study showed that West Kalimantan has a very high suitability area covering 16,321.66 Km2 (25.81 %) for NPPs. The NPPs are proposed to operate with water reactor coolants, thereby prioritizing areas of about five kilometres from the shoreline. We discovered twelve sites, but only two suitable priority sites for NPPs were found in Ketapang after overlapping with national/regional planning and the existing state. These sites will not significantly impact the environment, socio-economic, and policies, also will support electricity for the new capital region for Indonesia.

BACKGROUND: Complex surgical site infections (SSIs) and revisions for these infectious complications following total knee and hip arthroplasties are associated with significant economic costs. AIM: To evaluate the cost of one-stage and two-stage revisions; debridement, antibiotic, and implant retention (DAIR) and DAIR with liner exchange for complex hip or knee SSIs in Alberta, Canada. METHODS: We used the Alberta Health Services Infection Prevention and Control database to identify individuals >18 years old from the two major urban centers in Alberta, Calgary, and Edmonton zone, with complex hip or knee SSIs who underwent surgical intervention between April 1, 2012, and March 31, 2019. Micro-costing and gross-costing methods were used to estimate 12 and 24-month costs following the initial hospital admission for arthroplasty. Subgroup, inverse gaussian and gamma regression analysis were used to evaluate the associations of the revision procedure, age, sex, and comorbidities on cost. FINDINGS: A total of 382 patients with complex SSIs were identified with a mean age of 66.1 years. DAIR and DAIR with liner exchange resulted in the lowest 12- and 24-month costs at $53,197 (95% CI, $38,006 - $68,388) and $57,340 (95% CI, $48,576 - $66,105), respectively; two-stage revision was the costliest procedure. Most of the incurred costs (>98%) were accrued within the first 12 months following the initial procedure. CONCLUSIONS: Medical costs are highest 12 months following initial arthroplasty and for two-stage revisions in hip and knee complex SSI.

A specialized genetic algorithm (GA) is used to search the structural space of samarium-doped ceria (SDC) for the most energetically stable configurations which will predominate in low temperature fuel cells. A systematic investigation of all configurations of 3.2% SDC and a GA investigation of 6.6% SDC are presented for the first time at the DFT+U level of theory. It was found that Sm atoms prefer to occupy the nearest neighbor (NN) position relative to the oxygen vacancy at 3.2%, while at 6.6%, a balance exists between various Sm-vacancy interactions and the vacancies prefer to be separated by ∼6 Å. Also, the migration barriers for oxygen diffusion are calculated amongst the best structures in 3.2% and 6.6% SDC and are found to be comparable to the barriers in Gd-doped ceria at the DFT+U level of theory. While the migration calculations provide insight on the oxygen diffusion mechanism in this material, the favored configurations from our GA enable future research on concentrated SDC and contribute to the atomistic understanding of the influence of dopant-vacancy and vacancy-vacancy interactions on ionic conductivity.

BACKGROUND: The maternal deaths due to obstetrical cases declined, but the maternal deaths that caused by nonobstetrical cases still increase. The study reported that traumatic cases in pregnancy are the highest causes of mortality in pregnancy (nonobstetrical cases) in the United States. Another study reported that 1 in 12 pregnant women that experienced traumatic accident and as many as 9.1% of the trauma cases were caused by traumatic brain injury (TBI). The female sex hormone has an important role that regulates the hemodynamic condition. Anatomical and physiological changes during pregnancy make the examination, diagnosis, and treatment of TBI different from non-pregnant cases. Therefore, it is very important to lead the algorithm for each institution based on their own resources. CASE SERIES: A 37-year-old woman with a history of loss of consciousness after traffic accident. She rode a motorbike then hit the car. She was referred at 18 weeks' gestation. Glasgow Coma Scale (GCS) E1V1M4, isochoric of the pupil, reactive to the light reflex, and right-sided hemiparesis. The non-contrast head computed tomography (CT) scan revealed subdural hematoma (SDH) in the left frontal-temporal-parietal region, SDH of the tentorial region, burst lobe intracerebral hemorrhage, and cerebral edema. There was not a fetal distress condition. The next case, a 31 years old woman, in 26 weeks gestation, had a history of unconscious after motorcycle accident then she fell from the height down to the field about 3 m. GCS E1V1M3, isochoric of the pupil, but the pupil reflex decreased. Noncontrast CT scan revealed multiple contusion, subarachnoid hemorrhage, and cerebral edema. She had a good fetal condition. DISCUSSION: We proposed the algorithm of TBI in pregnancy that we already used in our hospital. The main principle of the initial management must be resuscitating the mother and that also the maternal resuscitation. The primary and secondary survey is always prominent of the initial treatment. CONCLUSION: The clinical decision depends on the condition of the fetal, the surgical lesion of the intracranial, and also the resources of the neonatal intensive care unit in our hospital.

Classical force field simulations and genetic algorithms are used to navigate low-energy configurations of samarium-doped ceria (SDC) at a number of concentrations, up to 20% SDC, such that the experimentally observed peak in ionic conductivity is mapped out in its entirety and fresh insight into samarium's role is reported.

RESUMO

We present a case of persistent Staphylococcus aureus bacteremia (SAB) with delayed diagnosis of deep-seated asymptomatic abscesses leading to an unnecessarily prolonged hospital stay and significant morbidity. This case suggests that early empiric nuclear imaging might have led to earlier source control and, in turn, reduced morbidity. In addition, the case illustrates that even with a presumed superficial source of infection, one should still search thoroughly for additional and perhaps clinically occult secondary foci in patients with high-risk SAB.


Les auteurs présentent un cas de bactériémie à Staphylococcus aureus (BSA) persistante avec diagnostic tardif d'abcès asymptomatiques profonds qui a donné lieu à une hospitalisation inutilement prolongée et à une morbidité importante. Ce cas laisse croire qu'une imagerie nucléaire empirique précoce aurait pu permettre d'en contrôler la source plus rapidement et ainsi réduire la morbidité. De plus, il démontre que, malgré la présomption d'une source d'infection superficielle, il faut tout de même faire une recherche minutieuse de foyers secondaires et peut-être occultes sur le plan clinique chez les patients à haut risque de BSA.
