AIM: Report a new case of primary Synovialosarcoma. CASE: We report a case of a 16 year old girl, who presented with a tumefaction on the right clavicle evolving for 3 months. Various explorations carried out, in particular thoracic scanner and imagery by magnetic resonance, showed a multilocular cystic formation of the seat and the clavicle reaching the mediastinum. A pathological examination of the part (piece) highlighted a biphasic synovialosarcoma of grade III. This diagnosis was validated by the immunohistochemistry study and the detection of transcribed specific fusion of the synovialosarcoma, SYT-SSX1. CONCLUSION: The unusual locations of the primitive SS must be known because the diversity of their microscopic aspects may cause confusion in the diagnosis. The immunohistochemistry, and more recently the cytogenetic studies, helps to solve these problems of differential diagnosis.

INTRODUCTION: Abscess of the nasal septum is a rare and serious infection, because of the complications which it can generate. The authors report a new case of nasal septum abscess probably of dental origin. OBSERVATION: A 47-year-old man consulted for bilateral nasal obstruction and rhinorrhea for 3 days with fever. The clinical examination as well as TDM imagery revealed a septal abscess. The patient underwent surgical drainage associated to triple antibiotherapy, with a good evolution at the end of 10 days. The etiologic assessment suggested dental infection. CONCLUSION: This is a rare and serious infection of the nasal septum. This new case of rare abscess of the nasal septum gives us the opportunity to describe out the various etiologies, their management, and to insist on the preventive treatment of this infection.

The patient had a surgical drainage associated with a triple antibiotherapy with favourable evolution at the end of 10 days. The etiologic investigation is in favour of a dental etiology. CONCLUSION: It is about a rare and serious infectious pathology of the nasal septum. We point out its various etiologies, its management, while insisting on the preventive treatment of this affection.

67 patients with prosthetic valvular replacement have been followed up for a period ranging from 6 months to 9 1/2 years; the overall number of valves was: 53 mitral, 24 aortic, 10 tricuspid valves. The average age of our patients was young (23.6 years). Many patients, because of their age or by lack of discipline or because of their social condition, had an inefficient anticoagulant treatment; they were subdivided into three groups. Those who stopped their anticoagulant treatment 6, those treatment was inefficient 25; in these two groups, no thrombo-embolic accident was noted, those whose treatment was efficient with a prothrombin time permanently lower than or equal to 40%: 36. It is in this very series that three cases of thrombosis occurred, two of which were lethal. Thus, although the indication for an anticoagulant treatment in patients with prosthetic valvar replacement remains indisputable, one may wonder, in front of such results, if there are not other factors, besides anticoagulant treatment, that result in thrombo-embolic accidents.

Tumors of ceruminous glands, called "ceruminoma" are rare. They arise from the ceruminous glands of the external auditory canal. They realise a group of glandular tumors diversified in histologic pattern and prognosis. Three patients with ceruminous gland tumors are reported: one case of ceruminous adenoma, one case of adenocarcinoma and adenoid cystic carcinoma in the third one.

Based on seventeen cases of upper digestive and respiratory tract tuberculosis, the authors review the different localizations of the disease. The mean age of the patients is 34 years old. The diagnostic is based on bacteriological and especially histological examination. The prognostic of the affection is excellent if tuberculosis chemotherapy is used for a minimum of 9 months.

Our work consists of a comparative study of 4 therapeutic protocols for treatment of otological infection from the pyocyanic bacillus, and more specifically of 19 patients with a necrotizing malignant otitis, 24 patients with perichondritis and 12 patients suffering from a post-operative infection. The association of Cefsulodin (a 3rd generation Cephalosporin) and Gentamicin gave the best results. A major decrease of the time of treatment and a total absence of infectious complications were in fact recorded. The adjuvant therapies were duly observed for the 4 protocols, in particularly the surgical treatment and the treatment of the site (diabetes).

The authors point out three cases of appendicular taeniasis disease into an analytical retrospective study of 5,000 appendicectomies. In these 3 cases the parasite does not induce important processes level with the appendix. The localisation of the parasite is very rare.
