BACKGROUND: There is a renewed interest in uncemented total knee arthroplasty to potentially provide longer durability, including the use of newer design metal-backed patellae (MBPs). The purpose of this study was to review survivorship with failure mode and time to failure of an earlier version MBP at up to 10-30 years of follow-up that may influence the desirability of using these components today. METHODS: A retrospective review was performed of patients that had uncemented total knee arthroplasty with an uncemented MBP. All-cause revision rates were obtained from chart reviews and telephone discussions with patients and family members of deceased patients. Kaplan-Meier plots were used to determine the implant survivorship. Outcome scores were compared between revised and nonrevised patients. RESULTS: The 97 knees that had an end point of an aseptic revision or last known contact with implant survivorship averaged 15 years (range, 0-32 years). There were 40 knees that underwent revision that included 37 patella component failures (38.1%). All patellar failures had polyethylene wear or fracture. None were revised due to loosening. Survivorship was 97.9% at 5 years, 88.7% at 10 years, and 53.0% at 20 years. Median time to failure was 11 years. CONCLUSION: Loosening is not a failure mode with this MBP. There were 75% of the failures occurring after 10 years. Use of contemporary MBP with improved but still thin polyethylene warrants guarded optimism when used in younger patients where longer survivorship than with a cemented all-polyethylene patellar component is the goal.

PURPOSE: To determine (1) whether platelet-rich plasma (PRP) injection significantly improves validated patient-reported outcomes in patients with symptomatic knee osteoarthritis (OA) at 6 and 12 months postinjection, (2) differences in outcomes between PRP and corticosteroid injections or viscosupplementation or placebo injections at 6 and 12 months postinjection, and (3) similarities and differences in outcomes based on the PRP formulations used in the analyzed studies. METHODS: PubMed, Cochrane Central Register of Controlled Trials, SCOPUS, and Sport Discus were searched for English-language, level I evidence, human in vivo studies on the treatment of symptomatic knee OA with intra-articular PRP compared with other options, with a minimum of 6 months of follow-up. A quality assessment of all articles was performed using the Modified Coleman Methodology Score (average, 83.3/100), and outcomes were analyzed using 2-proportion z-tests. RESULTS: Six articles (739 patients, 817 knees, 39% males, mean age of 59.9 years, with 38 weeks average follow-up) were analyzed. All studies met minimal clinical important difference criteria and showed significant improvements in statistical and clinical outcomes, including pain, physical function, and stiffness, with PRP. All but one study showed significant differences in clinical outcomes between PRP and hyaluronic acid (HA) or PRP and placebo in pain and function. Average pretreatment Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC) scores were 52.36 and 52.05 for the PRP and HA groups, respectively (P = .420). Mean post-treatment WOMAC scores for PRP were significantly better than for HA at 3 to 6 months (28.5 and 43.4, respectively; P = .0008) and at 6 to 12 months (22.8 and 38.1, respectively; P = .0062). None of the included studies used corticosteroids. CONCLUSIONS: In patients with symptomatic knee OA, PRP injection results in significant clinical improvements up to 12 months postinjection. Clinical outcomes and WOMAC scores are significantly better after PRP versus HA at 3 to 12 months postinjection. There is limited evidence for comparing leukocyte-rich versus leukocyte-poor PRP or PRP versus steroids in this study. LEVEL OF EVIDENCE: Level I, systematic review of Level I studies.

PURPOSE: To perform a systematic review to identify macroscopic and microscopic patterns and differences in hip capsule innervation between normal hips and hips with osteoarthritis (OA), femoroacetabular impingement (FAI) syndrome, and developmental dysplasia of the hip (DDH). METHODS: A systematic review was performed using Preferred Reporting Items for Systematic reviews and Meta-Analyses guidelines. Multiple databases were searched for both clinical and basic science laboratory studies on hip capsule innervation. Non-innervation capsule and non-human animal studies were excluded. Macroscopic and microscopic differences in capsular innervation between normal hips, and hips with OA, FAI, and DDH were analyzed. Methodological quality assessment of all studies included in this review was completed using the Methodological Index for Non-randomized Studies. RESULTS: Ten articles were analyzed (263 specimens; 211 patients, 52 cadavers; mean Methodological Index for Non-randomized Studies 10/16). The hip capsule is innervated by the sciatic and superior gluteal nerves posterosuperiorly, nerve to quadratus femoris and inferior gluteal nerve posteroinferiorly, and femoral and obturator nerves anteriorly. The anterior-superior capsule between 1:00 and 2:30 o'clock on a right hip is a safe internervous zone. The superolateral capsule has the greatest density of mechanoreceptors and sensory fibers. OA is associated with a greater expression of nerve fibers compared with normal hips but does not correlate with pain or disability. No significant differences were found in nerve fiber expression among patients with DDH, FAI, or normal hips. A negative correlation is seen with aging and pain fiber expression. CONCLUSIONS: The hip capsule has a complex macroscopic and microscopic innervation pattern with varying nerve fiber expression from at least 6 separate peripheral nerves. OA is associated with a greater expression of nerve fibers, although nerve fiber expression does not correlate with painful pathology. LEVEL OF EVIDENCE: IV, Systematic review of level I-IV studies.

Patient-specific design (PSD) total knee arthroplasty implants are marketed to restore neutral mechanical-axis alignment (MAA) and provide better anatomic fit compared with standard off-the-shelf (OTS) total knee arthroplasty designs. The purpose was to compare the Knee Society scores, radiographic outcomes, and complications of PSD and OTS implants. METHODS: Retrospective study analyzing PSD and OTS by a single surgeon. Implant design change in PSD occurred during the period of data collection leading to PSD-1 and PSD-2 subgroups. Radiographic data including MAA, femorotibial angle, coronal-tibial angle, tibial slope and patella-sulcus angle, and complications were analyzed. Minimum follow-up was 2 years or until revision, and patients completed Knee Society scores preoperatively and postoperatively at 3, 6, 12, 24 weeks, and final follow-up. RESULTS: There were 136 patients (154 knees), average age (62.76 +/- 8.4 years), and follow-up (3.1 +/- 1.5 years). The groups included PSD-1 (77 knees), PSD-2 (36 knees), and OTS (41 knees). The PSD-2 group had better Knee Society function scores compared with PSD-1 and OTS at all timepoints except final follow-up. PSD-2 had significantly shorter hospital stay (P = 0.000012) and less hemoglobin drop (P = 0.032) compared with PSD-1 and OTS. No differences were observed in MAA (P = 0.349) or final range of motion (P = 0.629) between the 3 groups. PSD-2 had more normal femorotibial angle, coronal-tibial angle, and tibial slope compared with PSD-1 and OTS. Failures requiring revision were 23% (18/77) PSD-1, 0% PSD-2, and 3% (1/35) OTS. Most common modes of failure were tibial subsidence (56%) and polyethylene locking mechanism failure (22%) in PSD-1. CONCLUSION: Catastrophic failure was seen in the PSD-1 group with tibial subsidence and polyethylene locking mechanism failure. PSD-2 had better early Knee Society function scores, shorter hospital stay, lower hemoglobin drop, radiographic alignment, and no failures compared with PSD-1 and OTS.

This study performs a systematic review to determine (1) if a significant difference exists in return to preinjury activity level between one- and two-stage treatment of combined anterior cruciate ligament (ACL) and patellar tendon (PT) tears; and (2) if a significant difference exists in the number of postoperative complications between the two differing surgical treatment approaches. A systematic review was performed using Preferred Reporting Items for Systematic reviews and Meta-Analyses (PRISMA) guidelines and registered on PROSPERO. MEDLINE, Cochrane Central Register of Controlled Trials, SCOPUS, and Sport Discus were searched for English language level I-IV evidence studies on either one- (simultaneous) or two-stage (sequential) surgical treatment of simultaneously sustained ipsilateral ACL and PT tears. The approach to initial evaluation, diagnosis, treatment, and outcomes were qualitatively analyzed. Methodological quality assessment of all included studies was completed using the Methodological Index for Non-randomized Studies (MINORS). The Grading of Recommendations, Assessment, Development, and Evaluation (GRADE) tool was used to assess quality of evidence and provide strength of recommendation. Statistical analyses were done using Fischer's exact test. Eleven articles (18 patients; 83% males; mean age, 31.1 ± 10.1 years; mean follow-up, 2.2 ± 1.7 years; and mean MINORS 7.8/16) were analyzed. Eight patients had a one-stage procedure (primary PT repair and ACL reconstruction), and 10 patients underwent a two-stage procedure (primary PT repair first followed by ACL reconstruction) with mean 28 ± 45.7 weeks (5 weeks-3 years) between surgeries. The rate for return to preinjury activity level after surgery was not significantly different between one- (88%) and two-stage (100%) (p = 0.444). There was a significantly higher complication rate (p = 0.023) in the one-stage (stiffness, instability, and patella baja) versus two-stage surgery (no complications). There was no significant difference in return to preinjury activity level between one- and two-stage PT repair and ACL reconstruction. However, the one-stage combined surgery had a significantly higher complication rate compared with two-stage surgery. The level of evidence is IV.
