RESUMO

The status of parsley as a well-known folk medicine noted for its nutritional and medicinal properties prompted the exploration of its potential as a functional food and natural remedy. The paper aims to investigate the potential of parsley to enhance muscle function and alleviate psoriasiform dermatitis, eventually establishing it as a natural, well-tolerated alternative with specific benefits for both muscles and skin. This study examines the tolerability of parsley in a cohort of 937 participants by assessing immunoglobulin G (IgG) reactions. The findings reveal high tolerability, as 96.26% of participants experienced no adverse effects. Among the 902 individuals lacking hypersensitivity, 37.02% reported muscle cramps, with a notable 15.02% reduction observed in the subgroup consuming parsley juice. In the subset of 32 subjects with dermatitis, the application of parsley extract ointment led to a significant decrease in dermatological parameters (redness, thickness, scaling). While the control group exhibited improvements, statistical significance was not observed. Notably, four categories of affected area reduction were identified, with scaling demonstrating the most pronounced impact. The results propose that parsley holds promise for favorable tolerability, contributing to the alleviation of muscle cramps and presenting an effective alternative in dermatitis treatment. Nonetheless, sustained validation through long-term studies is imperative to substantiate these preliminary findings.

Stroke represents a serious illness and is extremely relevant from the public health point of view, implying important social and economic burdens. Introducing new procedures or therapies that reduce the costs both in the acute phase of the disease and in the long term becomes a priority for health systems worldwide. The present study quantifies and compares the direct costs for ischemic stroke in patients with thrombolysis treatment versus conservative treatment over a 24-month period from the initial diagnosis, in one of the 7 national pilot centres for the implementation of thrombolytic treatment. The significant reduction (p < 0.001) of the hospitalization period, especially of the days in the intensive care unit (ICU) for stroke, resulted in a significant reduction (p < 0.001) of the total average costs in the patients with thrombolysis, both at the first hospitalization and for the subsequent hospitalizations, during the period followed in the study. It was also found that the percentage of patients who were re-hospitalized within the first 24-months after stroke was significantly lower (p < 0.001) among thrombolyzed patients. The present study demonstrates that the quick intervention in cases of stroke is an efficient policy regarding costs, of Romanian Public Health System, Romania being the country with the highest rates of new strokes and deaths due to stroke in Europe.

Cereals, as the world's most consumed food, face challenges related to nutrient quality due to climate change and increased production impacting soil health. In this study, we investigated the vitamin and mineral content, polyphenols, and antioxidant activity in cereals from Western Romania, analyzing whole and hulled wheat, rye, oat, and soybeans before and after heat treatment. Samples from 2022 crops were processed into dough and subjected to 220 °C for 30 min. The results reveal that, despite efforts to optimize nutrient content, cereals, particularly after heat processing, exhibited lower vitamin and mineral levels than the recommended daily intake. The decrease in polyphenols and antioxidant capacity was notable, with rye flour experiencing the largest decline (15%). Mineral analysis showed copper levels in decorticated wheat decreased by 82.5%, while iron in rye decreased by 5.63%. Soy flour consistently displayed the highest calcium, magnesium, and potassium levels, whereas oat flour had the highest zinc and copper levels before and after heat processing. The study highlights the concerningly low vitamins and minerals contents in cereals, as well as in the final products reaching consumers in the Western part of Romania, and contributes to the assessment of measures that are meant to improve the contents of these minerals.

Infective spondylodiscitis (ISD), the infection of vertebral bodies and surrounding tissues, is a rare complication with major impact on the long-term survival of hemodialysis (HD) patients. Although the most frequent etiology is staphylococcal, identifying these pathogens in blood cultures and biopsy cultures is often difficult. This paper aims to present suitable antibiotic combinations for the treatment of these patients, which is usually challenging in the case of an unidentified pathogen. We presented the therapies applied for 13 HD patients and 19 patients without chronic kidney disease (CKD), diagnosed with ISD between 2013 and 2023 in Bihor County. The percentage of positive blood cultures was low in both groups (30.78% HD vs. 15.78% non-HD). The average length of antibiotic therapy was 5.15 weeks in HD patients and 6.29 weeks in non-HD patients. The use of Carbapenem alone (e.g., Meropenem) for an average of 19.6 days for patients in HD when the pathogen was not identified has proven to be efficient in most cases, similarly to using Vancomycin and Fluoroquinolone/Cephalosporines in combination. Regarding the non-CKD patients, the use of Clindamycin in various combinations for an average of 30.3 days has proven to be efficient in more than 90% of cases of ISD with a nonidentified pathogen. Within 2 years after ISD was diagnosed, 12 of the 13 HD patients passed away, mainly due to cardiovascular causes. Unfortunately, there are no guidelines in the literature concerning the empiric treatment of ISD in the particular case of HD patients. Upon checking the literature on PubMed and Google Scholar, only 10 studies provided relevant data regarding ISD treatment for HD patients. More data about the treatment and evolution of these patients is needed in order to elaborate a truly relevant metanalysis.

Cowden syndrome (CS) is a rare disease that was first described in 1963 and later included in the large group of genodermatoses. It is the most common syndrome among the PTEN-associated hamartomatous tumor syndromes (PHTS). CS has an autosomal dominant inheritance pattern, with increased penetrance and variable expressivity, making early diagnosis difficult. Mutations in the PTEN gene (phosphatase and TENsin homolog) are involved in its pathogenesis, involving many organs and systems originating in the three embryonic layers (ectodermum, endodermum, and mesodermum). The consequence is the development of hamartomatous lesions in various organs (brain, intestines, thyroid, oropharyngeal cavity, colon, rectum, etc.). Multiple intestinal polyps are common in patients with CS, being identified in over 95% of patients undergoing colonoscopy. The authors describe the case of a patient who presented the first signs of the disease at 3 ½ years (tonsil polyp) but was diagnosed only at the age of 20 following a colonoscopy that revealed hundreds of intestinal polyps, suggesting further molecular testing. A heterozygous frameshift mutation was identified in the PTEN gene, classified as a potentially pathogenic variant (c.762del.p(Val255*)). The authors present this case to highlight the path taken by the patient from the first symptoms to the diagnosis and to emphasize the clinical aspects of this mutational variant that have still not been identified in other patients with this syndrome.

Soil contamination represents a serious and significant issue, especially when it comes to soil used in agricultural practices. This research was carried out in order to investigate the accumulation level of potentially toxic trace elements (Cr, Cd, Cu, Mn, Ni, Pb and Zn) in soil and vegetables (Solanum lycopersicum and Daucus carota). The transfer of the trace elements from soil to vegetables and the potential risk assessment were studied as well. Results indicated relatively high levels of heavy metals. Cd, Cu and Pb exceeded the alert limits established by the Romanian legislation. Zn was high as well. Positive correlations between the Cr, Cu and Pb indicated similar source of pollution, possibly related to the activities occurred in the non-metallic facility, nearby the study area. The heavy metals determined in the Solanum lycopersicum fruits and Daucus carota roots were below the maximum allowable concentrations, according to the WHO/FAO guideline. Slightly higher amounts of Cr and Cu were measured in tomatoes, compared to the carrots. Nevertheless, carrots were richer in Ni and Mn. The applied pollution indices indicated a contamination with heavy metals in 90% of the soil samples, with 9% probability of toxicity, the remaining 10% being classified into the precaution domain category. The plant bioconcentration of heavy metals into the Solanum lycopersicum fruits and Daucus carota roots is characterized using transfer factors. Generally, the results indicate that Daucus carota was the most susceptible to uptake Cu and Mn, while Solanum lycopersicum would rather uptake Cd and Zn. The estimated non-carcinogenic risk, based on the human health risk indices, indicates that the studied vegetables are safe for consumption with no impact on the human health. The results are lower than the critical value. Similarly, the carcinogenic risk indices results showed acceptable risks of cancer developing. It is important to assess and monitor the heavy metals levels in soil and in the vegetables intended to be consumed, in order to prevent contamination and potential negative effects on the environment and implicitly on the human health. The obtained data can be used in remediation techniques, as well as in implementing control measures of heavy metal contamination in soil and vegetables.

In 1848 pharmacist Horváth Mihály established the first pharmacy in Alesd, called Speranta (Remény). Following the brief history of this pharmacy we will notice that in 1874 the pharmacy comes into the possession of Kocsiss József. In 1906 the personal rights of the pharmacy are transcribed to Kocsiss Béla, and since 1938 the his son, Kocsiss Dezso, pharmacist, became the new owner. In 1949 the pharmacy was nationalized and became the property of the Pharmaceutical Office Oradea, the pharmacy got the name Farmacia nr. 22 of Alesd, and continued its activity throughout the whole communist period. Starting with the year 1991 it entered into private system as Angefarm, as the property of Mermeze Gheorghe, pharmacist, and from 2003 until now works under the name Vitalogy 3, as the property of Ghitea Sorin. A second pharmacy, Sfântul Anton was founded in 1937 by pharmacist Herceg Dobreanu Atena, which however had no continuity during the communist period.
