RESUMO

The use of the water-soluble radiocontrast substance Dimer-X in lumbosacral myelography in 22 patients with a stable pain syndrome in the lumbosacral region showed that this method makes it possible to disclose the cause of the disease exactly and to reveal or rule out any neurosurgical pathology of this region. This examination is tolerated well by the patients, does not produce serious complications, and may be conducted in any neurosurgical institution.

The authors studied the effect of the drug emoxypin on the brain functional asymmetry (A) in 36 patients with craniocerebral trauma attended by occurrence of focal traumatic injuries (FTI) to the brain (experimental group). The control group consisted of 61 patients who received the traditional intensive therapy for FTI (isolated brain contusion of moderate and severe degree, intracerebral hematomas measuring 30-50 cm3 in volume in the contusion focus). Favorable changes of the brain FA indices in the individual asymmetry profiles were noted, respectively, in 76.7% and 40.9% of patients given and not given emoxypin. Complete normalization of brain FA indices by the 25th-30th day after the beginning of treatment was recorded in 60.9% of patients in the control group and in 37% of those in the experimental group. The dynamics of individual asymmetry profiles in patients with FTI provides evidence that emoxypin improves the attention, mental efficiency, memory capacity, and selectivity of mnemonic processes.

The content of the products of lipid peroxide oxidation in the plasma of blood flowing from the brain was higher in patients with moderate and severe craniocerebral injury than in patients with no marked functional and metabolic disorders. The concentration of these products in the cerebrospinal fluid was still higher. In patients with severe craniocerebral injury there was also an increase in the content of free fatty acids in blood flowing from the brain and in the content of ferrous iron ions in the cerebrospinal fluid. The intensification of the processes of lipid peroxide oxidation in the brain may play an essential role in determining the severity of the traumatic damage.

RESUMO

From experience in operations on 19 patients the authors show that hemilaminectomy provides for an adequate approach to spinal cord tumors of various localization and histostructure. In all 10 patients benign extradural tumors were totally removed. Removal of intramedullary tumors is also possible. Some advantages of hemilaminectomy over routine laminectomy are pointed out.

RESUMO

The working capacity of 1,855 patients after craniocerebral trauma (CCT) was analysed. A unified approach to the diagnosis of the type and severity of the CCT in all stages of the follow-up and to active complex treatment was recorded. The chances of restoration of working capacity are higher in young individuals, males, those engaged in intellectual work, and in persons with trauma suffered in everyday life. The possibility of disability is high in persons of prepension age, females, persons engaged in qualified physical work, and in trauma suffered at work. Medical factors, the severity of the trauma and reversibility of the deficiency signs have a more marked and significant effect on the condition of the working capacity at the outcome of CCT.

RESUMO

Radionuclide cisternomyelography performed in 324 patients with posttraumatic injuries revealed the abnormality of the brain liquor tracts. The radiological picture of craniocerebral injury sequelae showed a great diversity of radionuclide signs, detected both morphological changes in the liquor tracts, and liquorodynamic disorders. The morphological disorders recorded by radionuclide cisternomyelography appeared as hydrocephalus, porencephalitis, arachnoidal cysts and liquorrheas. The liquorodynamic disorders recorded by this tool manifested themselves as altered liquor circulation and resorption. Radionuclide cisternomyelography provides valid data on the performance of the drainage system during operations on the liquor tracts of the brain and spine.

RESUMO

A method of a long-term retrograde catheterization of the superficial temporal artery for performance of a repeated angiographic investigation following operations for saccular aneurysms of the cerebral vessels is described. The proposed procedure was successfully empolyed in 7 patients without any complications.
