BACKGROUND: Opportunistic infections (OIs) are common causes of mortality among people living with HIV (PLHIV). We determined prevalence and 30-day mortality due to histoplasmosis, cryptococcosis, and TB in PLHIV with advanced HIV disease (AHD). METHODS: PLHIV 18 years and older, with a CD4 + T-cell count of less than 350 cells/mm3 newly diagnosed with HIV infection or re-engaged in care after being without ART for more than 90 days (Group A). The second group included symptomatic PLHIV regardless of ART status or CD4 + T-cell count (Group B); all followed for 30 days. Detection of Histoplasma Ag (HisAg) in urine was done by enzyme immunoassay (EIA), Cryptococcus antigen (CrAg) was detected in serum and cerebrospinal fluid (CSF) specimens by lateral flow assay (LFA), and lipoarabinomannan (LAM) detection in urine was by LFA (TB LAM) and in sputum by GeneXpert for diagnosis of Mycobacterium infections. RESULTS: From August 2021 to June 2022, 491 PLHIV were enrolled; 482 (98%) had a CD4 + T-cell result, and 381 patients (79%) were classified with AHD according to CD4 + T-cell count (< 200 CD4/mm3). Frequency of an OI was 38% (n = 145/381). Antigen test positivity rate was 16% (72/467) for TB-LAM, 9% (43/464) for HisAg, and 11% (51/484) for CrAg. Twenty-one of 34 (62%) patients receiving CSF CrAg tests were positive, confirming meningitis. Significant differences in 30-day mortality were observed in patients with an OI (16%) vs. no OI (7%) (p = 0.002). Mortality was highest in patients with histoplasmosis (25%), co-infection (22%), cryptococcosis (18% overall; 19% for cryptococcal meningitis), and TB (10%). CONCLUSIONS: TB and fungal OIs, including co-infection, were common in PLHIV in Paraguay and had high associated mortality. Laboratories and health facilities need access to CD4 + T-cell testing and rapid diagnostic assays.

Objective: To assess the compliance in secondary and tertiary level hospitals with monthly reporting of antibiotic consumption to the Colombian National Public Health Surveillance System (SIVIGILA-INS), and to describe reported antibiotic consumption during 2018-2020. Methods: This study involved a secondary analysis of antibiotic consumption data reported to SIVIGILA-INS. Frequency of hospital reporting was assessed and compared against expected reports, disaggregated by intensive care units (ICU)/non-ICU wards and geographical regions. Consumption was expressed as defined daily dose (DDD) per 100 occupied beds for seven antibiotics. Results: More than 70% of hospitals reported antibiotic consumption at least once in each of the three years (79% in ICU and 71% in non-ICU wards). Of these, ICU monthly reporting was complete (12 monthly reports per year) for 59% in the period 2018-2019 but only 4% in 2020. Non-ICU reporting was complete for 52% in 2019 and for 2% in 2020. Most regions had an overall decrease in reporting in 2020. Analysis of antibiotic consumption showed an increase for piperacillin/tazobactam, ertapenem, and cefepime from 2019 to 2020. Conclusions: There were gaps in the consistency and frequency of reporting. Efforts are needed to improve compliance with monthly reporting, which declined in 2020, possibly due to the COVID-19 pandemic. Non-compliance on reporting and data quality issues should be addressed with the hospitals to enable valid interpretation of antibiotic consumption trends.

Objective: To describe antimicrobial resistance profiles of Escherichia coli and Salmonella spp. isolated from chicken carcasses and the antimicrobials commonly used in animals in Ecuador and provide information on antimicrobial resistance patterns for implementing evidence-based corrective measures. Methods: Meat samples were collected from chicken carcasses in 199 slaughterhouses across Ecuador as part of a national pilot study for monitoring antimicrobial resistance in agricultural sources in 2019. Samples were tested for E. coli and Salmonella spp. Sensitivity to 10 critically important and three highly important antimicrobials (from a human health perspective) was assessed. The country report submitted to the World Organization for Animal Health was accessed to extract the quantity of antimicrobials produced or imported for use in animals. Results: Of 383 samples, E. coli was isolated from 148 (39%) and Salmonella spp. from 20 (5%) samples. Ninety percent of the isolates were resistant to at least one critically important antimicrobial. Resistance was highest to erythromycin (E. coli 76%; Salmonella spp. 85%) and tetracycline (E. coli 71%; Salmonella spp. 90%). Critically or highly important antimicrobials (colistin, tetracycline, trimethoprim/sulfamethoxazole) formed the bulk (87%) of antimicrobials used in animals as per the World Organization for Animal Health report. Conclusions: High prevalence of antimicrobial resistance in poultry in Ecuador calls for the development of guidelines and regulations on the use of antimicrobials and for engagement with livestock producers. The existing surveillance system needs to be strengthened to improve the monitoring of antimicrobial use and evolving resistance patterns.

Objective: To estimate the burden of Chlamydia trachomatis (CT), Neisseria gonorrhoeae (NG), Treponema pallidum (TP), and human papillomavirus (HPV) infections among people aged 10 to 25 in Latin America and the Caribbean. Methods: The MEDLINE, EMBASE, and LILACS databases were searched, as well as documents from regional organizations or national health Institutions. Population-based studies that reported prevalence or incidence of CT, NG, TP, and HPV detected through confirmatory tests in adolescents and young people were included. Two reviewers independently selected studies and extracted data. The quality of studies was assessed using the Newcastle-Ottawa Scale. Pooled estimators were calculated in cases where heterogeneity was <70%; when not feasible, prevalence ranges were reported. Results: Out of a total of 3 583 references, 15 prevalence studies complied with the inclusion criteria. Due to substantial heterogeneity (>70%), it was not possible to pool frequency estimators. Among the general population, the prevalence of CT infection ranged between 2.1% and 30.1% (9 studies, 5 670 participants); for NG, prevalence ranged between 0% and 2.9% (8 studies, 5 855 participants); for TP, prevalence varied between 0% and 0.7% (3 studies, 11 208 participants), and for HPV infection, prevalence ranged between 25.1% and 55.6% (8 studies, 3 831 participants). Conclusions: Reliable, population-based data on sexually transmitted infections (STIs) in adolescents and youth in Latin America and the Caribbean are limited. Additional studies are needed to better understand the burden of STIs in this population. However, given the substantial prevalence of STIs detected, countries need public health policies for prevention, early diagnosis, and treatment of STIs in young people.

OBJECTIVE: Evaluate primary health care functions from the perspective of patients with tuberculosis from slums in the city of Buenos Aires, Argentina. METHODS: Cross-sectional observational study with adult patients with tuberculosis (TB) and without TB (NoTB), living in slums (S) and outside them (NoS). Participants' perceptions were evaluated using the Primary Care Assessment Tool for users (abbreviated version), which measures four main domains (first contact, ongoing care, coordination with specialists, and comprehensiveness) and selected secondary domains. A Likert scale was used, ranging from "No, not at all" (1 point) to "Yes, definitely" (4 points). Scores ≥ 3 were considered to indicate adequate performance of functions. Averages were calculated for each domain, as well as two overall scores: with and without secondary domains. RESULTS: 83 participants were included (20 TB-S, 21 TB-NoS, 19 NoTB-S, and 23 NoTB-NoS). The evaluated functions were perceived as inadequate. The TB-S group gave the lowest overall scores, not reaching 3 points in any domain. There were no significant differences in domains or overall scores between groups. Participants with TB gave lower scores in all domains, except in family-centered care, where they gave a significantly higher score than NoTB participants. The overall score without secondary domains was lower for TB participants than for the NoTB groups. CONCLUSIONS: According to the perception of participants with TB and without TB, primary health care functions are not satisfactory, either in slums or outside them.

OBJECTIVE: To identify patient- and provider-related factors associated with the success of multidrug-resistant tuberculosis (MDR-TB) treatment in the six municipalities of Colombia with the highest number of MDR-TB cases. METHODS: Bivariate and multivariate logistic regressions were used to analyze the association between treatment success (cure or treatment completion) and characteristics of the patients and physicians, nursing professionals, and psychologists involved in their treatment. The importance of knowledge in the management of MDR-TB cases was explored through focus groups with these providers. RESULTS: Of 128 cases of TB-MDR, 63 (49.2%) experienced treatment success. Only 52.9% of the physicians and nursing professionals had satisfactory knowledge about MDR-TB. Logistic regression showed that being HIV negative, being affiliated with the contributory health insurance scheme, being cared for by a male physician, and being cared for by nursing professionals with sufficient knowledge were associated with a successful treatment outcome (p ≤ 0.05). Qualitative analysis showed the need for in-depth, systematic training of health personnel who care for patients with MDR-TB. CONCLUSIONS: Some characteristics of patients and healthcare providers influence treatment success in MDR-TB cases. Physicians' and nurses' knowledge about MDR-TB must be improved, and follow-up of MDR-TB patients who are living with HIV and of those affiliated with the subsidized health insurance scheme in Colombia must be strengthened, as these patients have a lower likelihood of a successful treatment outcome.


OBJETIVO: Identificar os fatores associados ao êxito do tratamento da tuberculose multirresistente (TBMR) relacionados ao paciente e à equipe de saúde nos seis municípios da Colômbia com o maior número de casos. MÉTODOS: Mediante regressão logística bifatorial e multifatorial, analisou-se a associação entre o êxito do tratamento (cura ou completude do tratamento) e as características dos pacientes e dos médicos, profissionais de enfermagem e psicólogos envolvidos neste. Explorou-se a importância do conhecimento no manejo de casos de TBMR mediante grupos focais com os mesmos profissionais. RESULTADOS: Dos 128 casos de TBMR, 63 (49.2%) lograram êxito no tratamento. Somente 52.9% dos médicos e profissionais de enfermagem tinham conhecimentos satisfatórios sobre TBMR. A regressão logística demonstrou que soronegatividade para o HIV, cobertura pelo sistema de saúde sob o regime de contribuinte, atendimento por um médico do sexo masculino e atendimento por profissionais de enfermagem com conhecimento suficiente foram fatores associados ao êxito do tratamento (p ≤ 0,05). A análise qualitativa demonstrou necessidade de aprofundar e sistematizar a capacitação do pessoal de saúde que atende casos de TBMR. CONCLUSÕES: Algumas características do paciente e da equipe de saúde influenciam no êxito do tratamento de casos de TBMR. É preciso fortalecer os conhecimentos dos médicos e profissionais de enfermagem sobre a TBMR e reforçar o seguimento dos pacientes com TBMR que vivem com HIV e os filiados ao sistema de saúde colombiano pelo regime subsidiado, os quais têm menor probabilidade de êxito do tratamento.

OBJECTIVE: Construct and evaluate the care cascade for pulmonary tuberculosis in the indigenous population of the department of Cauca (Colombia) and identify existing gaps. METHODS: Mixed-methods sequential explanatory design. In the first phase, the pulmonary tuberculosis care cascade for the indigenous population of Cauca was evaluated. Data were obtained from secondary sources and all cases diagnosed from 1 January 2016 to 31 December 2017 were included. In the second phase, semi-structured interviews were done with nine program coordinators and 11 nursing auxiliaries to explain identified gaps. Absolute and percentage values were estimated for each of the steps and gaps in the care cascade. Quantitative and qualitative results were triangulated. RESULTS: In 2016 and 2017, an estimated 202 patients with respiratory symptoms were expected to be positive and 106 cases of pulmonary tuberculosis were reported among the indigenous population of the department of Cauca. A gap of 47.5% was found for diagnosis, since only 52.5% of subjects were diagnosed in health services. This gap was explained by poor quality of samples and flawed smear techniques; flaws in correct identification of patients with respiratory symptoms; limited access to diagnostic methods, such as culture and molecular tests; and limited training and high turnover of personnel in health service provider institutions. CONCLUSIONS: The tuberculosis control program should focus actions on bridging the gap in case detection in the indigenous population.

OBJETIVOS: Avaliar as funções da atenção primária à saúde da perspectiva de pacientes com tuberculose (TB) provenientes de comunidades desfavorecidas na cidade de Buenos Aires, Argentina. MÉTODOS: Estudo observacional transversal com pacientes adultos com ou sem TB (TB, NãoTB), residentes ou não de comunidades desfavorecidas (C, NãoC). Avaliamos as percepções dos participantes utilizando questionário Primary Care Assessment Tool-usuários (versão reduzida), que mede quatro dimensões principais (primeiro contato, longitudinalidade da atenção, coordenação entre serviços e integralidade), bem como algumas dimensões secundárias. Utilizamos uma escala de Likert de 4 pontos, variando de "definitivamente não" (1 ponto) a "definitivamente sim" (4 pontos). Considerou-se que pontuações ≥3 indicavam o cumprimento adequado das funções. Calculamos as médias para cada domínio e duas pontuações globais: com e sem domínios secundários. RESULTADOS: Ao todo, 83 participantes foram incluídos no estudo (20 TB-C, 21 TB-NãoC, 19 NãoTB-C e 23 NãoTB-NãoC). As funções avaliadas foram percebidas como inadequadas. O grupo TB-C apresentou a pontuação global mais baixa, não alcançando 3 pontos em nenhum domínio. Não houve diferenças significativas entre os grupos nos domínios nem na pontuação global. Os participantes com TB deram pontuações mais baixas em todos os domínios, exceto no enfoque familiar, no qual a pontuação foi significativamente mais alta que a dos participantes sem TB; a pontuação global sem domínios secundários foi mais baixa nos participantes com TB que nos sem TB. CONCLUSÕES: De acordo com as percepções dos participantes com e sem TB, as funções da atenção primária à saúde são insatisfatórias, tanto dentro como fora das comunidades desfavorecidas.

OBJETIVO: Elaborar e avaliar a cascata de atenção da tuberculose pulmonar na população indígena do Departamento de Cauca (Colômbia) e identificar as lacunas existentes. MÉTODOS: Metodologia mista com desenho sequencial explicativo. Na primeira fase, avaliamos a cascata de atenção da tuberculose pulmonar para a população indígena de Cauca. Os dados foram obtidos de fontes secundárias, incluindo todos os casos diagnosticados entre 1 de janeiro de 2016 e 31 de dezembro de 2017. Na segunda fase, realizamos entrevistas semiestruturadas com nove coordenadores do programa e 11 auxiliares de enfermagem para explicar as lacunas identificadas. Estimamos os valores absolutos e percentuais em cada uma das etapas e as lacunas na cascata de atenção. Os resultados quantitativos e qualitativos foram triangulados. RESULTADOS: Nos anos de 2016 e 2017, foi estimada a ocorrência de 202 casos com sintomas respiratórios com diagnóstico esperado positivo; no entanto, os serviços de saúde só diagnosticaram e notificaram 106 casos de tuberculose pulmonar na população indígena do Departamento de Cauca. Portanto, identificamos uma lacuna diagnóstica de 47,5%, já que apenas 52,5% dos casos receberam um diagnóstico nos serviços de saúde. As explicações para esta lacuna foram a má qualidade das amostras e falhas na técnica de esfregaço, falhas na identificação correta dos sintomas respiratórios, acesso limitado aos métodos de diagnóstico, tais como cultura e testes moleculares, bem como capacitação deficiente e alta rotatividade de pessoal nas instituições de saúde. CONCLUSÕES: As ações do programa de controle da tuberculose devem se concentrar em reduzir a lacuna na detecção de casos na população indígena.

OBJECTIVE: Since the 1980s, dengue incidence has increased 30-fold. However, in 2017, there was a noticeable reduction in reported dengue incidence cases within the Americas, including severe and fatal cases. Understanding the mechanism underlying dengue's incidence and decline in the Americas is vital for public health planning. We aimed to provide plausible explanations for the decline in 2017. METHODS: An expert panel of representatives from scientific and academic institutions, Ministry of Health officials from Latin America and PAHO/WHO staff met in October 2017 to propose hypotheses. The meeting employed six moderated plenary discussions in which participants reviewed epidemiological evidence, suggested explanatory hypotheses, offered their expert opinions on each and developed a consensus. RESULTS: The expert group established that in 2017, there was a generalised decreased incidence, severity and number of deaths due to dengue in the Americas, accompanied by a reduction in reported cases of both Zika and chikungunya virus infections, with no change in distribution among age groups affected. This decline was determined to be unlikely due to changes in epidemiological surveillance systems, as similar designs of surveillance systems exist across the region. Although sudden surveillance disruption is possible at a country or regional level, it is unlikely to occur in all countries simultaneously. Retrospective modelling with epidemiological, immunological and entomological information is needed. Host or immunological factors may have influenced the decline in dengue cases at the population level through immunity; however, herd protection requires additional evidence. Uncertainty remains regarding the effect on the outcome of sequential infections of different dengue virus (DENV) types and Zika virus (ZIKV), and vice versa. Future studies were recommended that examine the epidemiological effect of prior DENV infection on Zika incidence and severity, the epidemiological effect of prior Zika virus infection on dengue incidence and severity, immune correlates based on new-generation ELISA assays, and impact of prior DENV/other arbovirus infection on ZIKV immune response in relation to number of infections and the duration of antibodies in relation to interval of protection. Follow-up studies should also investigate whether increased vector control intensification activities contributed to the decline in transmission of one or more of these arboviruses. Additionally, proposed studies should focus on the potential role of vector competence when simultaneously exposed to various arboviruses, and on entomological surveillance and its impact on circulating vector species, with a goal of applying specific measures that mitigate seasonal occurrence or outbreaks. CONCLUSIONS: Multifactorial events may have accounted for the decline in dengue seen in 2017. Differing elements might explain the reduction in dengue including elements of immunity, increased vector control, and even vector and\or viruses changes or adaptations. Most of the results of this expert consensus group meeting are hypothetical and based on limited evidence. Further studies are needed.


OBJECTIF: Depuis les années 1980, l'incidence de la dengue a été multipliée par 30. Cependant, en 2017, il y a eu une réduction notable du nombre de cas d'incidence de dengue rapportés dans les Amériques. Nous voulions fournir des explications plausibles à la baisse en 2017. MÉTHODES: Un groupe d'experts constitué de représentants d'institutions scientifiques et académiques, d'officiels des Ministères de la Santé d'Amérique Latine et de membres du personnel de l'OPS/OMS s'est réuni en octobre 2017 pour proposer et évaluer des hypothèses. RÉSULTATS: En 2017, il y a eu une baisse généralisée de l'incidence, de la sévérité et du nombre de décès dus à la dengue dans les Amériques, accompagnée d'une réduction des cas rapportés d'infections par le virus Zika et par le virus du chikungunya, sans modification dans la répartition entre les groupes d'âge affectés. Il a été déterminé que ce déclin était peu probablement dû aux changements dans les systèmes de surveillance épidémiologique, étant donné que des systèmes de surveillance similaires existaient dans toute la région. Bien que des perturbations soudaines dans la surveillance soient possibles au niveau national ou régional, il est peu probable que cela se produise simultanément dans tous les pays. Une modélisation rétrospective avec des informations épidémiologiques, immunologiques et entomologiques est nécessaire. Des facteurs liés à l'hôte ou immunologiques peuvent avoir influencé le déclin des cas de dengue au niveau de la population par le biais de l'immunité; cependant, l'évidence d'une protection conférée par l'effet du troupeau nécessite des données supplémentaires. Une incertitude subsiste quant à l'effet sur le résultat des infections séquentielles de différents types du virus de la dengue (DENV) et du virus Zika (ZIKV), et vice-versa. Les études à venir devraient examiner (1) l'effet épidémiologique d'une infection antérieure par le DENV sur l'incidence et la sévérité du virus Zika, (2) l'effet épidémiologique d'une infection antérieure par le virus Zika sur l'incidence et la sévérité de la dengue, (3) les corrélats immunitaires basés sur des tests ELISA de nouvelle génération, (4) l' impact d'une infection antérieure à DENV/autres arbovirus sur la réponse immunitaire au ZIKV en fonction du nombre d'infections et de la durée des anticorps en fonction de l'intervalle de protection, (5) si des activités d'intensification de la lutte antivectorielle ont contribué à la diminution de la transmission d'un ou plusieurs de ces arbovirus, (6) le rôle potentiel de la compétence vectorielle lorsqu'ils sont exposés simultanément à différents arbovirus, (7) la surveillance entomologique et son impact sur la circulation d'espèces de vecteurs, dans le but d'appliquer des mesures spécifiques qui réduisent l'occurrence saisonnière d'épidémies. CONCLUSIONS: Des événements multifactoriels pourraient expliquer le déclin observé de la dengue en 2017. La plupart des résultats de cette réunion du groupe de consensus d'experts sont hypothétiques, reposent sur des données limitées et requièrent des investigations supplémentaires.

Galápagos giant tortoises are an essential component of their ecosystem and evaluation of parasites in their populations is essential for the management of conservation processes. Coccidiosis is the most common intestinal infection in free-living and captive reptiles. The aim of this study was to characterize molecularly the presence of Eimeria sp. in captive reared giant tortoises from Santa Cruz, Santiago, Española, and Pinzon Islands hatched and housed at the tortoise rearing center on Santa Cruz Island, Galápagos, by sequencing of the 18S rRNA gene. Galápagos. All samples were previously analyzed by coproparasitoscopic flotation technique and PCR for molecular identification. The results obtained by microscopy examination showed oocysts in all samples. PCR and sequencing indicated the presence Eimeria sp., showing a similarity percentage of 98% with Eimeria environmental. In conclusion, we identified a group of coccidia of the genus Eimeria sp. (MK909931) in Galápagos tortoises.

OBJECTIVE: Evaluate the control of pulmonary tuberculosis in a detention center and identify the risk factors associated with unsuccessful treatment in the largest prison in Ecuador. METHODS: Surveillance data from the prison and a cohort of inmates diagnosed with tuberculosis (TB) between 2015 and 2016 were analyzed. Records without treatment outcome information were excluded. The percentage of patients with respiratory symptoms and TB incidence rate were estimated. Factors associated with unsuccessful treatment were estimated with binomial logistic regression. RESULTS: Of 59 846 medical consultations, 3% of respiratory symptoms were identified and, of these, 326 inmates had TB; 184 of them were analyzed. The incidence rate of TB in the prison was 3 947/100 000 inhabitants. Treatment was successful in 70.4% (65.6% cured; 4.8% treatment completed) and unsuccessful in 29.4% (12.5% lost during follow-up, 5% deceased, 1.1% treatment failure, 10.8% not evaluated). Seropositivity for human immunodeficiency virus (HIV) was associated with an increased risk of unsuccessful treatment (relative risk: 1.66, 95% confidence interval: 1.33-2.07). CONCLUSION: The incidence of TB in the prison was 123 times higher than in the general population of Ecuador. Prisoners co-infected with HIV-TB are at greater risk of not having a successful treatment, and articulation is required between the ministries of health and justice that allows the proper implementation of health protocols and the End TB Strategy.


OBJETIVO: Avaliar o controle da tuberculose (TB) pulmonar e identificar os fatores de risco associados ao tratamento malsucedido na maior prisão do Equador. MÉTODOS: Foram analisados os dados de vigilância da prisão e de uma coorte de reclusos da mesma prisão, diagnosticados com TB em 2015 e 2016. Foram excluídos os registros sem resultado do tratamento. A porcentagem de sintomas respiratórios identificados e a taxa de incidência de TB foram estimadas. Os fatores associados ao tratamento malsucedido foram estimados com regressão logística binomial. RESULTADOS: Em 59 846 consultas médicas, 3% dos reclusos foram identificadas com sintomas respiratórios. Entre esses, 326 tinham TB e 184 foram analisados. A taxa de incidência de TB na prisão foi de 3 947/100 000 habitantes. Observou-se tratamento bem-sucedido em 70,4% (65,6% curados e 4,8% com tratamento completo) e malsucedido em 29,4% (12,5% perdidos durante o acompanhamento, 5% morreram, 1,1% com falhas do tratamento e 10,8% sem avaliação). A soropositividade para HIV foi associada a risco aumentado de tratamento malsucedido (risco relativo 1,66; intervalo de confiança de 95%: 1,33 a 2,07). CONCLUSÃO: A incidência de TB na prisão foi 123 vezes maior do que na população geral do Equador. Prisioneiros coinfectados com HIV-TB correm maior risco de não ter tratamento bem-sucedido. É necessária uma articulação entre os ministérios da saúde e da justiça para implementar de forma adequada os protocolos de saúde e da Estratégia pelo Fim da TB.

OBJECTIVES: To estimate the tuberculosis (TB) burden in children under 15 years of age and to describe the clinical and epidemiological characteristics and the results of the anti-tuberculosis treatment in Ecuador. METHODS: A retrospective study was carried out using data from the national TB programm for 2015 and 2016. The rate and percentage of cases of childhood TB were estimated and the disease characteristics and treatment outcome were described according to age categories: 0-4, 5-9 and 10-14 years. RESULTS: Of the 10 991 cases of TB diagnosed, 223 (2.03%) were under 15 years of age; depending on the region, this burden ranged from 0 to 5.5%. Of the 223 cases, 213 had their treatment outcome registered and were included in the study; 78 (37%) were younger than 5 years and 147 (69%) had no record of contact screening. Sixty-five (68%) of the adolescents and 40 (51%) of the children under 5 had a diagnosis of pulmonary TB. HIV prevalence was 11.5% in children under 5 and 6.3% in the 10-14 age group. Treatment was succesful in 93% of cases (cure, 36.6%, treatment completed, 56.8%). CONCLUSIONS: Ecuador presents a high percentage of under-diagnosis of childhood TB and a lower than expected burden, mainly in children under 5 years of age. The high prevalence of HIV and the lack of adequate systematization of adolescent contact screening suggest the need to consider family-centered strategies that involve training health personnel in the management of pediatric patients, with a focus on the specific needs of each population.

MÉTODOS: Foi realizado um estudo retrospectivo utilizando dados do programa nacional de TB para os anos de 2015 e 2016. A taxa e a porcentagem de casos de tuberculose infantil foram estimadas. As características da doença e o resultado do tratamento foram descritos de acordo com as categorias de idade: 0 a 4 anos, 5 a 9 anos e 10 a 14 anos. RESULTADOS: Dos 10 991 casos de TB diagnosticados, 223 (2,03%) tinham menos de 15 anos. A carga de TB variou de 0 a 5,5%, dependendo da região do país. Em 213 dos 223 casos, o resultado do tratamento havia sido registrado, possibilitando a inclusão no estudo. Desses, 78 (37%) tinham menos de 5 anos; em 147 (69%) não havia registro da investigação de contato. Sessenta e cinco (68%) dos adolescentes e 40 (51%) dos menores de 5 anos foram diagnosticados com TB pulmonar. A prevalência de HIV foi de 11,5% em crianças menores de 5 anos e de 6,3% no grupo de 10 a 14 anos. O tratamento foi satisfatório em 93% dos casos (cura, 36,6%; tratamento concluído, 56,8%). CONCLUSÕES: O Equador tem uma elevada porcentagem de subdiagnóstico de TB infantil e a carga da doença ficou abaixo do esperado, principalmente em crianças menores de 5 anos. A alta prevalência do HIV e a falta de sistematização da pesquisa de contatos em adolescentes revelam a necessidade de considerar estratégias centradas na família, que envolvam o treinamento de profissionais de saúde no manejo do paciente pediátrico, com foco nas necessidades específicas de cada população.

OBJECTIVE: To identify factors associated with sputum smear nonconversion in patients with pulmonary tuberculosis (PTB) in Suriname. METHODS: A case-control study was conducted using routinely-collected surveillance data of PTB cases reported in January 2010 - December 2015 and recorded in the database of the National Tuberculosis Program of Suriname. Cases were smear-positive PTB patients whose sputum results were negative 2 months after treatment initiation. Controls were the smear-positive PTB patients whose sputum results were negative in the same timeframe. Multivariate logistic regression analysis was used to examine associations between potential risk factors and smear conversion. RESULTS: The two age groups ≥ 35 years (35 - 54 years, AOR: 2.7, 95%CI: 1.2 - 6.1; and 55+ years, AOR: 2.5, 95%CI: 1.1 - 5.9) and high bacillary load at baseline (AOR 2.34, 95%CI: 1.2 - 4.8) were significantly associated with delayed smear conversion. CONCLUSION: The National TB program of Suriname should develop strategies to address patients at higher risk for delayed smear conversion to prevent further spreading and unfavorable treatment outcomes. To better inform decision-making and future studies, the NTP should expand its data collection to include all risk factors for delayed smear conversion.

OBJECTIVE: To determine factors associated with delays in pulmonary tuberculosis diagnosis and treatment initiation in the city of Cali, Colombia. METHODS: This was a retrospective cohort study of cases of tuberculosis (TB) reported in the TB control program of Cali between January and December 2016. The information was collected from the databases of the TB control program, individual treatment cards, and clinical histories. The variables considered were sociodemographic factors, clinical factors, substance use, and performance of the health service. RESULTS: A total of 623 cases were identified, of which 57.0% were male. The median age was 42 years (interquartile range (IQR): 27-60). The median time from onset of symptoms to TB diagnosis was 57 days (IQR: 21-117), and from onset of symptoms to TB treatment initiation was 72 days (IQR: 35-145). A factor associated with longer time from the onset of symptoms to TB treatment was being a previously treated TB patient (coefficient: 123.8 days, 95% confidence interval (CI): 48.3 to 199.3). In contrast, being incarcerated was a protective factor for earlier TB treatment initiation (coefficient: -57.3 days; 95% CI: -92.4 to -22.3). CONCLUSIONS: Our results provide important information concerning risk factors that are associated with delays in the diagnosis and treatment of tuberculosis, and that are subject to future interventions. Health insurance program managers must work together with health care providers on issues that include patient care, health promotion, and updating TB protocols and standards.

OBJECTIVE: To identify socio-demographic and clinical factors associated with mortality among persons with tuberculosis (TB) and TB/HIV co-infection in Suriname. METHODS: This was a retrospective cohort study using data from the national TB and HIV databases for 2010 - 2015. The survival probability of TB and TB/HIV co-infected patients was analyzed using the Kaplan-Meier estimates and the log-rank test. A Cox proportional hazard model was applied. RESULTS: The study showed that HIV-seropositivity (aHR: 2.08, 95%CI: 1.48 - 2.92) and older age (aHR: 5.84, 95%CI: 3.00 - 11.4) are statistically associated with higher mortality. For the TB/HIV co-infected patients, TB treatment (aHR: 0.43, 95%CI: 0.35 - 0.53) reduces the risk of death. Similarly, HIV treatment started within 56 days (aHR: 0.15, 95%CI: 0.12 - 0.19) and delayed (aHR: 0.25, 95%CI: 0.13 - 0.47) result in less hazard for mortality; Directly-Observed Treatment (aOR: 0.16, 95%CI: 0.09 - 0.29) further reduces the risk. CONCLUSIONS: The Ministry of Health of Suriname should develop strategies for early case-finding in key populations, such as for HIV and TB in men 60 years of age and older. Implementation of Isoniazid Preventive Therapy for HIV should be pursued. Scaling up TB and HIV treatment, preferably through supervision, are essential to reducing the TB/HIV mortality.

In 2015, Brazil faced a Zika virus epidemic that spread to other countries in the world. As a result, recommendations regarding reporting criteria for congenital Zika syndrome (CZS) were issued in the form of protocols. The frequent changes in these recommendations may have affected clinical management and the access to post-diagnostic support by children who were affected by CZS, but who ended up not being identified. In the present study, 39 cases of CZS reported in the state of Espírito Santo, Brazil, from 2015 to 2016 were re-classified in terms of diagnosis using the current protocol, which is different from the protocol used in 2015. According to this re-classification, only eight out of 36 cases would be confirmed, based on the criterion of two or more signs or symptoms of CZS with or without microcephaly plus positive serologic results. Also, considering the decrease in the head circumference cut-off point defining microcephaly, 17 cases would no longer meet the definition for this condition. Even though the current protocol does not rely on head circumference alone for CZS reporting and confirmation, it should be noted that this is still the main sign considered by health care teams, and therefore the decrease in the cut-off point might have compromised early CZS detection. A review of "ruled out" cases would be advisable in moments of protocol transition to determine whether these cases have been correctly classified.


En el 2015, Brasil enfrentó una epidemia de infección por el virus del Zika que se propagó por varios países del mundo. Posteriormente, se divulgaron recomendaciones acerca de los criterios de notificación de casos del síndrome congénito por el virus del Zika (SCZ) por medio de protocolos. Los cambios frecuentes de esas recomendaciones podrían haber afectado el manejo clínico y el acceso al apoyo posterior al diagnóstico de los niños afectados, pero no identificados. En el presente estudio, se reclasificó el diagnóstico de 39 casos del SCZ notificados en el estado de Espírito Santo en el período 2015-2016, de acuerdo con el protocolo vigente en la actualidad, que es distinto del que regía en el 2015. Por causa de la reclasificación, se confirmaron únicamente ocho de los 36 casos, con observancia del criterio de dos o más signos o síntomas del SCZ acompañados o no de microcefalia y con confirmación serológica. Además, por la disminución del perímetro cefálico que define la microcefalia, 17 casos no correspondieron a esa afección. A pesar de que en el protocolo vigente no se utiliza solamente el perímetro cefálico como criterio para la notificación y confirmación del SCZ, cabe resaltar que este hallazgo es, con todo, la mayor señal para los equipos de salud, puesto que indica un riesgo de falta de detección temprana del SCZ. Convendría examinar los casos "descartados" en el momento de la transición entre protocolos, con el fin de determinar si se clasificaron correctamente.

OBJECTIVE: Describe the sociodemographic profile of mothers of children with congenital Zika syndrome. METHODS: This is a descriptive, cross-sectional, quantitative study. The 39 mothers of live born babies among the cases notified in 2015 and 2016 with confirmed diagnosis of congenital Zika syndrome in the state of Espírito Santo, Brazil, were invited to participate. Of these, 25 mothers were enrolled. Data were collected using a socioeconomic questionnaire and a field diary. RESULTS: Skin color was self-reported as non-white by 19/25 mothers (74.0%), and 16 (64.0%) reported having a partner. Regarding schooling, 12 (48.0%) had completed high school. Per capita income varied from no income to US$ 1 111.11, and ranged from none to US$ 61.72 for 12 women (48.0%). Seventeen women (68.0%) lived in underprivileged periphery areas with poor living conditions. Of the 25 women, 16 (64.0%) had a job prior to the pregnancy; and of these, 12 (75.0%) were fired or quit after the baby was born. CONCLUSIONS: The profile of mothers shows that the Zika epidemics was not equanimous and preferentially affected women with low income and lower social class. In this sense, we believe that congenital Zika syndrome may be determined by social inequalities in Brazil. The data described in the present study may be useful for the planning of effective actions to ensure a social protection network for children with congenital Zika syndrome and their families.


OBJETIVO: Describir el perfil sociodemográfico de las madres de niños con síndrome congénito por el virus del Zika. MÉTODOS: Estudio descriptivo, transversal, de abordaje cuantitativo. Se invitó a participar en el estudio a 39 madres de niños nacidos vivos pertenecientes al grupo de casos notificados entre 2015 y 2016 con diagnóstico confirmado de síndrome congénito por el virus del Zika en el estado de Espírito Santo. De las 39 mujeres, 25 aceptaron participar. Para la recolección de datos se utilizó un formulario para identificar el perfil sociodemográfico y un diario de campo. RESULTADOS: Se identificaron 107 estudios. Al final del proceso de clasificación se incluyeron 18 artículos que presentaban datos sobre la asociación entre el péptido C y el riesgo cardiovascular. Cinco estudios proporcionaron datos sobre la relación entre el péptido C y la mortalidad cardiovascular y general. El péptido C presentó una asociación positiva con el índice de masa corporal en población china, y una asociación inversa con el colesterol HDL en muestras poblacionales de Asia, Oriente Medio y Estados Unidos. Sin embargo, no fue posible realizar un metanálisis para los componentes de riesgo cardiovascular. Por otro lado, el péptido C se asoció con la mortalidad cardiovascular (RR = 1,62, IC95%: 0,99 a 2,66) y general (RR = 1,39, IC95%: 1,04 a 1,84). CONCLUSIONES: De las 25 madres, 19 (74,0%) manifestaron no ser blancas y 16 (64,0%) tener una pareja. En cuanto a la escolaridad, 12 (48,0%) poseían eduación media completa. El ingreso domiciliario per cápita varió desde ningún ingreso hasta US$ 1 111,11; 12 mujeres (48,0%) reportaron desde no tener ingresos hasta un ingreso de US$ 61,72. En cuanto a la vivienda, 17 (68,0%) vivían en zonas periféricas con condiciones precarias. De las 25 mujeres, 16 (64,0%) presentaban vínculo laboral antes de la gestación, y de estas 12 (75,0%) fueron despedidas o renunciaron después del nacimiento del niño. Conclusiones. El perfil de las madres reveló que la epidemia no fue equitativa y alcanzó mayormente a mujeres de bajos ingresos y de estratos sociales desfavorecidos. En ese sentido, la enfermedad podría ser determinada por las desigualdades sociales de salud presentes en el país. Los datos encontrados son útiles para planificar acciones efectivas enfocadas a garantizar una red de protección social para niños con síndrome congénito por el virus del Zika y sus familias.
