Children are highly susceptible to environmental contaminants as their physiology and some metabolic pathways differ from adults. The present cross-sectional study aimed to assess whether exposure to benzene, toluene, ethylbenzene, o,p-xylene, and m-xylene (BTEX) affects oxidative DNA damage in street children using a biomonitoring approach. Thirty-five boys (7-13 years of age), exposed by working at a busy intersection, and 25 unexposed boys of similar age and living in the neighborhood near the busy intersection were recruited. Urinary un-metabolized BTEX levels were quantified by a headspace gas chromatography-mass spectrometry (GC-MS). Urinary malonaldehyde (MDA) was measured with spectrophotometry. Sociodemographic and lifestyle conditions information was collected by interviews using administered questionnaires. Exposed subjects provided urine before (BE) and after work exposure (AE), while unexposed boys gave a single morning sample. Urinary BTEX concentrations in BE samples were similar to unexposed. Concentrations in AE samples were 2.36-fold higher than observed in BE samples (p < 0.05) and higher than those in the unexposed group (p < 0.05). In addition, urinary MDA levels in AE samples were 3.2 and 3.07-times higher than in BE samples and in the unexposed group (p < 0.05). Environmental tobacco smoke (ETS) increased urinary BTEX and MDA levels in both groups. Our findings confirm that street children working at busy intersections are significantly exposed to BTEX, which is associated with oxidative stress. Implementing protective measures is crucial to reduce exposure and to improve health outcomes in this group.

The present systematic review aimed to evaluate the associations between welding fumes exposure and changes in oxidative stress [superoxide dismutase (SOD) and malondialdehyde (MDA)] and DNA damage [8-hydroxy-2'-deoxyguanosine (8-OHdG) and DNA-protein crosslink (DPC)] markers in professional welders (PROSPERO CRD42022298115). Six electronic bibliographic databases were searched from inception through September 2021 to identify observational epidemiological studies evaluating the association between welding fumes exposures and changes in oxidative stress and DNA damage in professional welders. Two reviewers independently assessed the risk of bias and certainty of the evidence. A narrative synthesis of results was conducted using the Synthesis Without Meta-analysis (SWiM) method. Pooled mean differences with 95% confidence intervals were calculated in a random-effects meta-analysis for the outcomes of interest in the review. From 450 studies identified through the search strategy, 14 observational epidemiological studies were included in the review. Most studies reported significantly higher welding fumes levels in welders than in controls. The narrative synthesis results of SOD showed a significant difference between welders and controls, while the meta-analysis results of MDA did not show a significant difference between the studied groups (MD = 0.26; 95% CI, -0.03, 0.55). The meta-analysis results of 8-OHdG (MD = 9.38; 95% CI, 0.55-18.21) and DPC (MD = 1.07; 95% CI, 0.14-2) revealed significantly differences between the studied groups. The included studies were at high risk of exclusion and confounding bias. The certainty of the evidence for oxidative stress and DNA damage results were very low and moderate, respectively. Exposure to welding fumes and metal particles is associated with DNA damage in professional welders, and 8-OHdG and DPC might be considered reliable markers to assess DNA damage resulting from exposure to welding fumes. We recommend, however, that the evaluation of oxidative stress resulting from welding fumes exposure not be solely based on MDA and SOD.

Concentrations of metals and metalloids derived mainly from anthropogenic activities have increased considerably in the environment. Metals might be associated with increase reactive oxygen species (ROS) damage, potentially related to several health outcomes. This study has recruited 200 adult participants, including 110 males and 90 females in Shiraz (Iran), to investigate the relationship between chronic exposure to metals and ROS damage by analyzing malondialdehyde (MDA) and 8-Oxo-2'-deoxyguanosine (8-OHdG) concentrations, and has evaluated the associations between chronic metal exposure and ROS damage using regression analysis. Our findings showed participants are chronically exposed to elevate As, Ni, Hg, and Pb levels. The mean urinary concentrations of 8-OHdG and MDA were 3.8 ± 2.35 and 214 ± 134 µg/g creatinine, respectively. This study shows that most heavy metals are correlated with urinary ROS biomarkers (R ranges 0.19 to 0.64). In addition, regression analysis accounting for other confounding factors such as sex, age, smoking status, and teeth filling with amalgam highlights that Al, Cu, Si and Sn are associated with 8-OHdG concentrations, while an association between Cr and MDA and 8-OHdG is suggested. Smoking cigarettes and water-pipe is considered a significant contributory factor for both ROS biomarkers (MDA and 8-OHdG).

OBJECTIVE: Monitoring trends in key population health indicators is important for informing health policies. The aim of this study was to examine population health trends in Canada over the past 30 years in relation to other countries. METHODS: We used data on disability-adjusted life years (DALYs), years of life lost (YLL), years lived with disability, life expectancy (LE), and child mortality for Canada and other countries between 1990 and 2019 provided by the Global Burden of Disease Study. RESULTS: Life expectancy, age-standardized YLL, and age-standardized DALYs all improved in Canada between 1990 and 2019, although the rate of improvement has leveled off since 2011. The top five causes of all-age DALYs in Canada in 2019 were neoplasms, cardiovascular diseases, musculoskeletal disorders, neurological disorders, and mental disorders. The greatest increases in all-age DALYs since 1990 were observed for substance use, diabetes and chronic kidney disease, and sense organ disorders. Age-standardized DALYs declined for most conditions, except for substance use, diabetes and chronic kidney disease, and musculoskeletal disorders, which increased by 94.6%, 14.6%, and 7.3% respectively since 1990. Canada's world ranking for age-standardized DALYs declined from 9th place in 1990 to 24th in 2019. CONCLUSION: Canadians are healthier today than in 1990, but progress has slowed in Canada in recent years in comparison with other high-income countries. The growing burden of substance abuse, diabetes/chronic kidney disease, and musculoskeletal diseases will require continued action to improve population health.


RéSUMé: OBJECTIF: La surveillance des tendances des indicateurs clés de la santé de la population est importante pour éclairer les politiques de santé. Dans cette étude, nous avons examiné les tendances de la santé de la population au Canada au cours des 30 dernières années par rapport à d'autres pays. MéTHODES: Nous avons utilisé des données sur les années de vie ajustées en fonction de l'incapacité (DALY), les années de vie perdues (YLL), les années vécues avec un handicap, l'espérance de vie (LE) et la mortalité infantile pour le Canada et d'autres pays entre 1990 et 2019, fournies par l'Étude mondiale sur le fardeau de la maladie. RéSULTATS: L'espérance de vie, les YLL ajustées selon l'âge et les DALY ajustées selon l'âge ont tous connu une amélioration au Canada entre 1990 et 2019, bien que le taux d'amélioration se soit stabilisé depuis 2011. Les cinq principales causes des DALY pour tous les âges au Canada en 2019 étaient les néoplasmes, les maladies cardiovasculaires, les affections musculosquelettiques, les affections neurologiques et les troubles mentaux. Les plus fortes augmentations des DALY pour tous les âges depuis 1990 ont été observées pour l'usage de substances, le diabète et les maladies rénales chroniques, ainsi que les troubles des organes sensoriels. OBJECTIVES: Polycyclic Aromatic Hydrocarbons (PAHs) are ubiquitous, toxic environmental chemicals that can cause adverse reproductive health effects. The objectives of this mini-review are to highlight the adverse reproductive outcomes due to PAH exposure with the main focus on polycystic ovary syndrome (PCOS) and premature ovarian failure (POF) and to provide perspectives on future research needs. CONTENT: We reviewed studies that have reported the adverse reproductive outcomes associated with PAHs exposures in women through a comprehensive search of bibliographic databases and gray literature sources. In addition, potentially modifiable sources of exposure to PAHs and associated reproductive outcomes were also investigated. SUMMARY: A total of 232 papers were retrieved through a comprehensive search of bibliographic databases, out of which three studies met the eligibility criteria and were included in the review. Results showed that exposure to PAHs is associated with adverse reproductive outcomes defined as PCOS, POF, and reproductive hormone imbalance. Sources of PAH exposure associated with adverse reproductive outcomes include active and passive tobacco smoking, specific cooking methods, and pesticides. OUTLOOK: Future studies are warranted to examine the mechanisms by which PAHs result in adverse reproductive endpoints in women. Further, environmental exposures that are potentially modifiable such as exposure to tobacco smoke, may contribute to PAH exposure, and these exposures should be targeted in future policies and interventions.

Although the fundamental reasons for cognitive function disorders have been well documented, little is known about the impact of environmental exposures, such as pesticides, on children's cognitive function development. This study investigated the effect of exposure to organophosphate pesticides on children's cognitive function. In order to determine various factors of exposure, hair samples were collected from 114 elementary school children who lived in Boyer-Ahmad County in the province of Kohgiluyeh and Boyer-Ahmad, Iran. A detailed questionnaire was utilized to gather demographic information and exposure profile. Pesticides were detected in hair samples using a gas chromatography-mass spectrometer (GC-MS); also, cognitive function was assessed using the trail-making test (TMT), which was divided into two parts: TMT-part A and TMT-part B. Participants in the study were 10.12 ± 1.440 years old on average. Children in rural areas had higher mean total pesticide concentrations (13.612 ± 22.01 ng/g) than those who lived in the urban areas (1.801 ± 1.32). The results revealed that boys (46.44 s and 92.37 s) completed the TMT-part A and part B tests in less time than girls (54.95 s and 109.82 s), respectively, and showed better performance (2.14) on the cognitive function exam than girls (2.07). Diazinon and TMT-part B were positively correlated (p < 0.05). With the increase in pesticides, there was no discernible difference in cognitive function. Pesticide use throughout a child's development may affect certain cognitive function indicators. In order to assess causal relationships, group studies and case studies are required because the current research was cross-sectional in nature.

Human biomonitoring (HBM) data measured in specific contexts or populations provide information for comparing population exposures. There are numerous health-based biomonitoring guidance values, but to locate these values, interested parties need to seek them out individually from publications, governmental reports, websites and other sources. Until now, there has been no central, international repository for this information. Thus, a tool is needed to help researchers, public health professionals, risk assessors, and regulatory decision makers to quickly locate relevant values on numerous environmental chemicals. A free, on-line repository for international health-based guidance values to facilitate the interpretation of HBM data is now available. The repository is referred to as the "Human Biomonitoring Health-Based Guidance Value (HB2GV) Dashboard". The Dashboard represents the efforts of the International Human Biomonitoring Working Group (i-HBM), affiliated with the International Society of Exposure Science. The i-HBM's mission is to promote the use of population-level HBM data to inform public health decision-making by developing harmonized resources to facilitate the interpretation of HBM data in a health-based context. This paper describes the methods used to compile the human biomonitoring health-based guidance values, how the values can be accessed and used, and caveats with using the Dashboard for interpreting HBM data. To our knowledge, the HB2GV Dashboard is the first open-access, curated database of HBM guidance values developed for use in interpreting HBM data. This new resource can assist global HBM data users such as risk assessors, risk managers and biomonitoring programs with a readily available compilation of guidance values.

Welders are exposed to high levels of metal fumes, which could be resulting in various health impairments. Respirators became a practical protective option in workplaces, as they are lightweight and easy to use. This systematic review attempts to explore the field effectiveness of using respirators to reduce metal particle exposure in workplaces. We reviewed papers published from 1900 to April 2019 in five major bibliographic databases, including Embase, Web of Science, Medline, Scopus, and CINAHL, along with organizational websites to cover gray literature. In total, 983 references were identified from the databases, out of which, 520 duplicates were removed from the EndNote database. The remaining 463 references were screened for their title and abstract. Out of 463, 70 references went through the full-text screening. Finally, eight papers, including 19 workplace respirator studies, satisfied all the inclusion criteria and were reviewed in this report. The geometric means for metal levels in workers' breathing zone with and without respirators were 9.4 and 1,777 µg/m3 for iron, 1.1 and 139 µg/m3 for lead, 2.1 and 242 µg/m3 for zinc, and 27 and 1,398 µg/m3 for manganese oxide, respectively. Most reviewed studies reported significant differences between measured metal particle levels among workers who worn respirators and who did not. In addition, results showed that N95 provided significantly less protection than elastomeric half facepieces, full-face respirators, and powered air-purifying respirators (p<0.001). More field studies are recommended to investigate Workplace Protection Factor (WPF) and fit factor (FF) of different respirators to understand the actual protection levels that they could be provided to control welding fume exposure among welders in various workplaces.

Individuals who get involved in the disinfection of public settings using sodium hypochlorite might suffer adverse health effects. However, scarce information is available on the potential oxidative stress damage caused at low concentrations typically used for disinfection. We aimed to assess whether exposure to sodium hypochlorite during the COVID-19 pandemic causes oxidative stress damage in workers engaged in disinfection tasks. 75 operators engaged in the disinfection of public places were recruited as the case group, and 60 individuals who were not exposed to disinfectant were chosen as the control group. Spot urine samples were collected before (BE) and after exposure (AE) to disinfectants in the case group. Likewise, controls provided two spot urine samples in the same way as the case group. Urinary malondialdehyde (MDA) levels were quantified by forming thiobarbituric acid reactive substances in the urine. In addition, the concentration of 8-hydroxy-2'-deoxyguanosine (8-OHdG) in the urine was determined using an ELISA kit. Results showed significant differences in the urinary levels of oxidative stress markers, where median 8-OHdG (AE case: 3.84 ± 2.89 µg/g creatinine vs AE control 2.54 ± 1.21 µg/g creatinine) and MDA (AE case: 169 ± 89 µg/g creatinine vs AE control 121 ± 47 µg/g creatinine) levels in case group AE samples were 1.55 and 1.35-times higher than the control group AE samples (P < 0.05), respectively. Besides, urinary levels of oxidative stress markers in AE samples of the case group were significantly higher than in BE samples (8-OHdG BE 3.40 ± 1.95 µg/g creatinine, MDA BE 136 ± 51.3 µg/g creatinine, P < 0.05). Our results indicated that exposure to even low levels of sodium hypochlorite used in disinfection practices might cause oxidative stress related damage. With this in mind, implementing robust protective measures, such as specific respirators, is crucial to reduce the health burdens of exposure to disinfectants.

Prenatal exposure to polycyclic aromatic hydrocarbons (PAHs) evokes extensive concerns regarding public health. Monitoring the pregnant women's exposure can be considered a suitable alternative to assess the fetus's exposure. This study aimed to monitor pregnant women's exposure (n = 126) to PAHs using a biomonitoring approach to evaluate effects on anthropometric indices and neonatal thyroid-stimulating hormone (TSH) in Shiraz, Iran. PAHs priority compounds were measured by gas chromatography-mass spectrometry (GC-MS) after separating blood serum and liquid-liquid extraction (LLE) method. Information on anthropometric indices, neonatal TSH levels, and data from the respondents was obtained from medical records and questionnaires. The mean PAHs concentrations ranged from 0.29 to 327.91 ng/g lipid. There was no significant difference between the measured PAHs in maternal serum at the seventh month and pregnancy termination except for ACY (p-Value<0.05). Regression analysis results showed a significant correlation (p-value<0.05) between exposure to passive smoke and total PAHs concentrations. There was no significant relationship between exposure to PAHs and weight, height, head circumference, and Apgar score of newborns. The results showed TSH decreased by 0.99 units as ACE increased per unit (ß = -0.001). This study is the first to evaluate relationships between prenatal exposure to PAHs and effects on newborn health indicators, including TSH levels in a Middle Eastern population. Future studies are suggested to perform detailed assessments of PAHs intake sources, especially in vulnerable populations such as pregnant women and children.

Veterinary staff are frequently exposed to various occupational hazards. The present study was aimed to investigate the air characteristics of veterinary clinics in terms of fungal bioaerosols and particulate matters. Air samples were taken every six days from the operating room, examination room and outdoor air of three veterinary clinics in Shiraz, southwest Iran. The concentrations of fungal bio-aerosols ranged from 8.05 CFU/m 3 in the outdoor air of clinic B to 47.21 CFU/m 3 in the operating room of clinic A. The predominant fungal genera identified in the studied clinics were Penicillium and Aspergillus niger, respectively. The concentrations of PM2.5 ranged from 41.88 µg/m 3 in the operating room of clinic C to 60.31 µg/m 3 in the outdoor air of the same clinic. The corresponding values for PM10 ranged from 114.40 µg/m 3 in the operating room of clinic C to 256.70 µg/m 3 in the outdoor air of the same clinic. The results of this study showed a positive correlation between the concentration of fungal bioaerosols and relative humidity (p < 0.05; r = 0.622). Besides, a negative correlation was found between the concentration of fungal bioaerosols and temperature (p < 0.05; r = 0.369). To better assess the individual exposure of veterinarians and staff in veterinary clinics, tests including nasopharyngeal sampling are recommended.

Welding fumes vary in composition depending on the materials and processes used, and while health outcomes in full-time welders have been widely studied, limited research on apprentices exists. Besides, few data are available for metals such as vanadium and antimony. This study aimed to look at individual metals present in welding fumes in the learning environment of apprentice welders. Forty-three welders and 41 controls were chosen from trade programmes at the Northern Alberta Institute of Technology. Ambient and personal air samples were collected at days 0, 1, 7, and 50 of their training and analysed for mass and metal concentrations using Inductively Coupled Plasma Mass Spectrometry. Results showed increases in particle and metal concentrations as apprentices progressed throughout their education and that concentrations at day 50 were similar to levels found in the literature for professional welders. Variable concentrations indicate that some individuals may not properly use the local exhaust ventilation system. Other possible explanation for variations are the position of the sampler on the shoulder, the time spent welding and in each welding position, and the skills of the welders. Strong relationships were observed between particle and metal concentrations, suggesting that these relationships could be used to estimate metal exposure in welders from particle exposure. Welding processes were the most important determinant of exposure in apprentice welders, with Metal Core Arc Welding producing the largest particle concentrations followed by oxyacetylene cutting, and Gas Metal Arc Welding. Health risk assessment showed that welder apprentices are at risk for overexposure to manganese, which suggests that professional welders should be monitored for manganese as they are exposed more than apprentices. Training in proper positioning of local exhaust ventilation system and proper use of respirators are recommended in training facilities.

Urinary benzene, toluene, ethylbenzene, and xylenes (BTEX) can be used as a reliable biomarker of exposure to these pollutants. This cross-sectional study aimed to employ biomonitoring to assess BTEX exposure among South Pars Gas Field (SPGF) workers in Assaluyeh, Iran. Forty employees who were working on the site were recruited as the case group. Besides, 31 administrative employees were recruited as the control group. Pre-shift and post-shift spot urine samples were collected from the subjects in the case group, while the subjects in the control group provided mid-morning urine samples. Overall, 111 urine samples, including 80 samples from the case group and 31 samples from the controls, were collected. Gas chromatography-mass spectrometry (GC/MS) was used to determine the urinary levels of BTEX compounds. The median urinary levels of benzene, toluene, ethylbenzene, m,p-xylene, and o-xylene in the post-shift samples of the exposed group were 1.24, 2.28, 0.5, 1.32, and 1.5 µg/l, respectively. Significant differences were observed in urinary BTEX levels among smokers and non-smokers in both studied groups (p < 0.05). Accordingly, the median urinary BTEX concentrations in smokers were 2 to 6.5 times higher than the corresponding values in non-smoker subjects. Smoking status was the only predictor of the urinary BTEX concentration. Our findings revealed that refinery workers are exposed to significant levels of BTEX compounds. Considering the health risks associated with BTEX exposure for refinery workers, implementation of suitable control strategies, such as using appropriate personal protective equipment and improving on-site ventilation systems, are recommended reducing their exposure to BTEX via the inhalation.

It has been proven that exposure to bioaerosols is associated with several health effects, such as pulmonary diseases and allergies. The present cross-sectional study was aimed to investigate fungal contamination in indoor air and on the surfaces of four traditional baths in Shiraz, Iran, one of the most historical cities in the world. Samples were taken from indoor air, using a microbial air sampler, as well as the surfaces of the shower, hallway, and dressing rooms of studied baths for 3 months. Totally 180 samples, including 45 air and 135 surfaces samples, were collected from studied baths. The concentrations of fungi collected from the air of studied baths were ranged from 22.6 to 34.6 CFU/m3. Besides, the levels of fungi collected from the surface samples of studied baths were ranged from 21.2 to 60 CFU/m2. The highest and lowest fungi species detected both in air and surfaces samples of the studied baths were Penicillium spp. and Mucor spp. respectively. Although the levels of fungi in the studied baths were lower than the levels recommended by the World Health Organization, some environmental health measures such as washing and disinfecting surfaces and tools after each working shift and periodic inspections are recommended ensuring the safety of costumers who are visiting such places.

Heavy metals are neurotoxic, associated with brain dysfunction, and have been linked with cognitive decline in adults. This study was aimed to characterize chronic exposure to metals (Cd, Be, Co, Hg, Sn, V, Al, Ba, Cr, Cu, Fe, Li, Mn, Ni, Pb, and Zn) and metalloids (As, B, Sb) and assess its impact on cognitive performance of Tehran's residents, capital of Iran. Scalp hair samples gathered from 200 volunteered participants (110 men and 90 women), aged 14-70 years and quantified by inductively coupled plasma atomic emission spectroscopy (ICP-OES). Attention and executive function, two measures of cognitive performance, were characterized using the trail making test (TMT) part A and B, respectively. Mental flexibility was characterized as the Delta TMT B-A scores and cognitive efficiency or dissimulation as the ration between TMT B and A scores. A comprehensive questionnaire was used to gather information on demographic and socioeconomic as well as lifestyle and health status. The highest and lowest mean concentrations were observed for B (325 µg/g) and As (0.29 µg/g), respectively. Results indicated that chronic metal exposure measured in hair changed significantly based on gender and age (p < 0.05). The levels of Cr, Fe, Ni, Si, Hg, Pb and B were significantly higher in males' hair, whereas those of Ag and Ba were greater in females' hair (p < 0.05). The results of the cognitive TMT test were significantly different between gender and age groups (p < 0.05). Moreover, results revealed that As, Hg, Mn, and Pb levels in hair were significantly associated with poorer participants' performance scores in the TMT test (p < 0.05). Age, gender, cigarette smoking, water-pipe smoking, traffic density in the area of residence, and dental amalgam filling were significant factors affecting the TMT test scores. The results suggest that chronic exposure to metals has detrimental effects on attention, executive function, mental flexibility and cognitive efficiency.

Composting facilities workers are potentially exposed to different volatile organic compounds (VOCs). This study aims to investigate the potential exposure to benzene, toluene, ethylbenzene and xylenes (BTEX) compounds among workers of composting facilities by measuring un-metabolized BTEX in urine and to investigate the effect that several lifestyle factors (i.e. smoking and residential traffic), using personal protective equipment, and religious practices such as Ramadan fasting can have on the urinary BTEX concentrations. We assessed concentrations of BTEX in the urine of a composting facility workers. Samples were collected in May 2018. Overall, 25 workers chosen as the exposed group and 20 inhabitants living close to the composting facility as a control group. The urine samples were collected from studied subjects. Identification and quantification of un-metabolized BTEX was performed using a headspace gas chromatography-mass spectrometry (GC-MS). Detailed information of participants was gathered by a comprehensive questionnaire. The geometric mean levels of urinary benzene, toluene, ethylbenzene, m­p xylene, and o­xylene in the exposed subjects were 1.27, 2.12, 0.54, 1.22 and 1.51 µg/L, respectively; 1.4 to 3.7-time higher than values in control group (p < 0.05). Post-shift levels were significantly higher than pre-shift for all chemicals (p < 0.05). Smoking habits, exposure to environmental tobacco smoke, and Ramadan fasting predicted urinary BTEX levels. Personal protective equipment which included a simple N95 mask did not protected workers from BTEX emissions. Composting facilities represent a significant source BTEX emissions and exposure for staff. More effective protective strategies are required to minimize exposure and related occupational hazards.

Urinary benzene, toluene, ethylbenzene, and xylenes (BTEX) can be used as a reliable biomarker of exposure to these pollutants. This study was aimed to investigate the urinary BTEX concentration in operators of healthcare waste (HCW) autoclaves. This cross-sectional study was conducted in selected hospitals in Tehran, Iran between April and June 2017. Twenty operators (as the case group) and twenty control subjects were enrolled in the study. Personal urine samples were collected at the beginning and end of the work shift. Urinary BTEX were measured by a headspace gas chromatography-mass spectrometry (GC/MS). A detailed questionnaire was used to gather information from subjects. Results showed that the median of urinary benzene, toluene, ethylbenzene, m-p xylene, and o-xylene levels in the exposed group were 3.26, 3.36, 0.84, 3.94 and 4.48 µg/L, respectively. With the exception of ethylbenzene, subjects in the exposed group had significantly higher urinary BTEX levels than control group (p < 0.05). Urinary BTEX concentrations in the exposed case group were 2.5-fold higher than in the control group. There was a significant relationship between the amount of generated waste per day and the urinary BTEX in the exposed group. Smoking status and type of autoclave used were also identified as predictors of urinary BTEX concentrations. The healthcare waste treatment autoclaves can be considered as a significant BTEX exposure source for operators working with these treatment facilities. The appropriate personal protection equipment and control measures capable in reducing BTEX exposure should be provided to HCW workers to reduce their exposures to BTEX.

The aim of this study was to investigate environmental and lifestyle factors affecting exposure to PAHs in the general population in a large city of the Middle East (Tehran) by measuring urinary monohydroxy polycyclic aromatic hydrocarbons (OH-PAHs) and establishing relationships between PAHs exposure and related factors. Urine samples were collected from 222 randomly chosen subjects who were living in the urban area of Tehran, Iran. Subjects were required to complete a detailed questionnaire aimed to document their personal and sociodemographic information, activities, cooking-related appliances, smoking history/exposure, and consumed foodstuff. Identification and quantification of six OH-PAHs was carried out using a gas chromatography with mass spectrometry (GC-MS). The geometric means for 1-OHP, 1-NAP, 2-NAP, 2-FLU, 9-FLU, and 9-PHE for whole population study were 310, 1220, 3070, 530, 330, and 130 ng/g creatinine, respectively. The two naphthalene metabolites contributed on average 77% of the total concentration of six measured OH-PAHs, followed by the 2-FLU, 1-OHP, 9-FLU, and 9-PHE. The most important predictors of urinary PAHs were consumption of grilled/barbecued foods, smoking, and exposure to environmental tobacco smoking. Water pipe smoking was linked to urinary OH-PAH metabolite in a dose-response function. Residential traffic was also related with OH-PAH metabolite concentrations. Other factors including gender, age, exposure to common house insecticides, open burning, and candle burning were found to be statistically associated with the urinary levels of some OH-PAHs. High exposure to PAHs among general population in Middle Eastern large cities and its associated health implications calls for public health measures to reduce PAHs exposure.

BACKGROUND: Improper treatment of infectious waste can cause numerous adverse environmental and health effects such as transmission of diseases through health personnel and other susceptible groups,who come in contact with such wastes. On the other hand, selection of appropriate treatment alternatives in infectious waste management has become a challenging task for public health authorities especially in developing countries. The objective of this paper is to select the best infectious waste treatment alternative by the modified Sustainability Assessment of Technologies (SAT) methodology, developed by the International Environmental Technology Center of the United Nations Environment Program (IETC-UNEP). METHODS: SAT methodology consists of three main components, including screening, scoping and detailed assessment. In screening, different infectious waste treatment alternatives undergo screening using the finalized environmental and technical criteria. Short-listed treatment options from the previous step, then go through the comprehensive scoping and detailed assessment (2nd and 3rd components) which is more qualitative and quantitative in nature. An empirical case in Tehran, the largest city in Iran, is provided to illustrate the potential of the proposed methodology. RESULTS: According to the final score, "Hydroclave", was the most suitable infectious treatment technology. The ranking order of the treatment alternatives were "Autoclave with a shredder", "Autoclave", "Central Incineration" and "chemical treatment" on the basis of technical, economical, social and environmental aspects and their related criteria. CONCLUSIONS: According to the results it could be concluded that the top ranking technologies basically have higher scores in all the aspects. Hence it is easier to arrive at a decision for the final technology selection based on the principles of sustainability.

In this study, atmospheric concentrations of particulate-bound polycyclic aromatic hydrocarbons (PAHs) in Tehran megacity were determined to investigate the concentration, distribution, and sources of PAHs in PM10. The health risk from exposure to airborne BaPeq through inhalation pathway was also assessed. Toxic equivalency factors (TEFs) approach was used for quantitative risk estimate, and incremental lifetime cancer risk (ILCR) was calculated. PM10 samples were collected at ten sampling locations during the summer 2013 and winter 2014 by using two independent methods of field sampling. The PM10 concentration in winter (89.55 ± 15.56 µg m(-3)) was 1.19 times higher than that in summer (75.42 ± 14.93 µg m(-3)). Sixteen PAHs were measured with the total average concentrations of PAHs ranged from 56.98 ± 15.91 to 110.35 ± 57.31 ng m(-3) in summer and from 125.87 ± 79.02 to 171.25 ± 73.94 ng m(-3) in winter which were much higher than concentrations measured in most similar studies conducted around the world. Molecular diagnostic ratios were used to identify PAH emission sources. The results indicated that gasoline-driven vehicles are the major sources of PAHs in the study area. Risk analysis showed that the mean and 90 % probability estimated inhalation ILCRs were 7.85 × 10(-6) and 16.78 × 10(-6), respectively. Results of a sensitivity analysis indicated that BaP concentration and cancer slope factor (CSF) contributed most to effect on ILCR mean.
