Aspergillus galactomannan immunoassay is a main diagnostic and monitoring tool in medical mycology. However, the specificity of the method can be skewered by the presence of several other fungi. Trying to diagnose a possible fungal infection of the lower respiratory tract in a haematology patient, it appeared that the fungus Trichoderma longibrachiatum is an additional probable cause of positive galactomannan results. Although, that Trichoderma is a rare but emerging pathogen in immunocompromised patients, the above information could be a caution point in the clinical evaluation of diagnostic results.

Pneumocystis jirovecii is the causative agent of Pneumocystis pneumonia (PcP), a common and often life-threatening opportunistic infection in HIV-infected patients. However, non-HIV, immunocompromised patients are at risk of PcP as well, whereas the mortality appears to be higher among these patients. Pneumocystis co-infections with other microorganisms are less frequent and only sparse reports of combined PcP and invasive pulmonary fungal infections exist in the literature, especially in the non-HIV patients. Two cases of pulmonary co-infections by P. jirovecii and Aspergillus fumigatus are presented. Both patients were non-HIV infected, the first one was suffering from crescentic IgA nephropathy under immunosuppressive treatment and the second from resistant non-Hodgkin lymphoma under chemotherapy. Both patients were treated with intravenous trimethoprim/sulphamethoxazole (TMP/SMX) combined with voriconazole. The first patient showed gradual clinical improvement while the outcome for the second patient was unfavourable. In addition, a literature review of the previous published cases of co-infection by P. jirovecii and other fungi in non-HIV patients was performed. Our target was to provide comprehensive information on this kind of infections, highlighting the importance of clinical suspicion.

Our objective was to study the outcome of femtosecond-assisted lamellar keratoplasty (FSLK) in stromal corneal diseases. This is a retrospective chart review of 17 patients (20 eyes) who underwent FSLK for anterior corneal pathologies. Main outcome measures were refractive results following FSLK, complications, and graft survival. Mean follow-up time was 42 ± 15 (7-58) months. Preoperative best spectacle-corrected visual acuity (BSCVA) was ≤20/40 in 17 eyes. Postoperative BSCVA ≥ 20/40 was achieved in 12/14 at 12 months, 11/12 at 24 months, and 10/12 eyes at 36 months; postoperative BSCVA ≥ 20/25 was achieved in 8/14, 8/12, and 5/12 eyes at 12, 24, and 36 months, respectively. One eye had vertical gas break through the epithelium during the FSLK. One eye had postoperative epithelial rejection and two eyes had stromal rejection treated successfully with topical steroids. Another eye had epithelial ingrowth that was not progressive; however, the same eye developed bacterial keratitis and scarred graft 32 months post-FSLK. One eye had graft dehiscence and one eye developed excessive interface fibrosis. Five out of 20 grafts failed due to the recurrence of the original disease (3), corneal scarring (1), and excessive interface fibrosis (1). FSLK provides many advantages over conventional PK and DALK, with faster visual rehabilitation and emmetropization of the manifest refraction rather than inducing ametropia and irregular astigmatism.

BACKGROUND: Biological control with predatory mites is applied against pests in greenhouse crops. Chemical control with the use of selective, reduced-risk pesticides, is an important component of Integrated Pest Management (IPM) programs, that often needs to be combined with biological control. Here, we evaluated the effect of plant pollen when used as supplementary food on the survival, reproduction and predation of the predatory mite Amblydromalus limonicus (Acari: Phytoseiidae) after exposing young larvae and adults to flonicamid, an insecticide of moderate toxicity to phytoseiids. Pollen is an important alternative food for generalist phytoseiids ensuring survival and supporting populations build-up during periods of prey scarcity. Two regimes of cattail (Typha angustifolia L.) pollen differing in application frequency were used. In the first, the total amount of pollen was supplied once, within 30 min after insecticide application, whereas in the second regime, the same amount of pollen was supplied gradually, i.e., every 48 h. RESULTS: Regardless of the frequency of application, pollen provisioning results in a reduction in prey (thrips) consumption relative to the control (no pollen provisioning). Nevertheless, when adult mites were directly exposed to flonicamid residues, pollen provisioning attenuated the reduction in prey consumption as compared to the control. In addition, the gradual (every 48 h) provisioning of pollen to adult predators exposed to flonicamid residues impacted positively the intrinsic rate of population increase (rm) of A. limonicus as compared to when feeding on prey. CONCLUSION: Our results reveal an unexpected role of pollen provisioning in alleviating pesticides side-effects on phytoseiids. © 2024 The Authors. Phytoseiid mites of the genus Phytoseius are natural enemies of tetranychid and eriophyid herbivorous mites mostly found on hairy plants where they feed on prey, as well as on pollen. Nevertheless, the nutritional ecology and the role of these predators in biological pest control are only rarely addressed. In the present study, we evaluated the potential of Phytoseius finitimus to feed and reproduce on three major greenhouse pests, the two-spotted spider mite, the greenhouse whitefly and the western flower thrips. Additionally, we estimated the effect of cattail pollen when provided to the predator alone or in mixed diets with prey. Contrary to thrips larvae, both spider mite larvae and whitefly crawlers sustained the development of P. finitimus. In addition, females consumed more spider mite eggs and larvae, as well as whitefly crawlers than thrips larvae, but laid eggs when feeding on all prey. When provided alone, cattail pollen sustained the development and reproduction of the predator. The addition of pollen in mixed diets with prey reduced prey consumption, though it increased the predator's egg production. We discuss the implications of our findings for biological pest control.

Microbes are found in the environment, possibly more often as biofilms than in planktonic forms. Biofilm formation has been described for several important fungal species. The presence of a dermatophytoma in a dermatophytic nail infection was the basis for the proposal that dermatophytes form biofilms as well. This could explain treatment failure and recurrent dermatophytic infections. Several investigators have performed in vitro and ex vivo experiments to study the formation of biofilms by dermatophytes and their properties. The nature of the biofilm structure itself contributes to fungal protection mechanisms against many harmful external agents, including antifungals. Thus, a different approach should be carried out regarding susceptibility testing and treatment. Concerning susceptibility testing, methods to evaluate either the inhibition of biofilm formation, or the ability to eradicate it, have been introduced. As for treatment, in addition to classical antifungal agents, some natural formulations, such as plant extracts or biosurfactants, and alternative approaches, such as photodynamic therapy, have been proposed. Studies that connect the results of the in vitro and ex vivo experimentation with clinical outcomes are required in order to verify the efficacy of these approaches in clinical practice.

The two-spotted spider mite Tetranychus urticae is a polyphagous herbivore with a worldwide distribution, and is a serious pest in tomato and other crops. As an alternative to chemical pesticides, biological control with the release of natural enemies such as predatory mites represent an efficient method to control T. urticae in many crops, but not in tomato. Other biological control agents, such as beneficial microbes, as well as chemical compounds, which can act as plant defense elicitors that confer plant resistance against pests and pathogens, may prove promising biological solutions for the suppression of spider mite populations in tomato. Here, we assessed this hypothesis by recording the effects of a series of fungal and bacterial strains and the plant strengthener acibenzolar-s-methyl for their plant-mediated effects on T. urticae performance in two tomato cultivars. We found significant negative effects on the survival, egg production and spider mite feeding damage on plants inoculated with microbes or treated with the plant strengthener as compared to the control plants. Our results highlight the potential of beneficial microbes and plant strengtheners in spider mite suppression in addition to plant disease control.

OBJECTIVES: Infections caused by dermatophytes affect a high percentage of the population. Antifungal susceptibility testing (AST) can offer useful information about the susceptibility profiles of the pathogens as well as the concomitant documentation of the appropriate treatment. However, the slow growth rate of these fungi and their poor sporulation are factors that can delay and affect the performance of the AST. The proposed methods by the CLSI or the EUCAST are both laborious for the everyday routine. There are alternative applications which propose the use of an inoculum, consisting of a conidia-mycelium mixture or even plain mycelia, as well as the use of resazurin in order to facilitate the reading. The aim of this study was to compare these approaches to the EUCAST method and evaluate their performance. METHODS: Three alternative methods were compared to the EUCAST proposed methodology for conidia forming molds. The last was defined as the reference method. The methods under evaluation were (a) a fragmented mycelia method, (b) the EUCAST method with the addition of resazurin sodium salt solution and (c) the fragmented mycelia method with the addition of resazurin sodium salt solution. Twenty-two isolates (8 Trichophyton interdigitale, 8 T. rubrum, and 6 Microsporum canis) were tested against the antifungal agents of griseofulvin, terbinafine, fluconazole, and itraconazole. RESULTS: The essential agreement between the methods was calculated in percentages and a statistical analysis of the results was performed. Data evaluation revealed sufficient overall agreement of the methods with the addition of resazurin to the initial "uncolored" methods (98.9 and 97.5% for the EUCAST and the fragmented mycelia methods, respectively). The fragmented mycelia method exhibited a relatively sufficient overall agreement in comparison to the EUCAST method (90%) and not a satisfactory correlation, probably as a result of various issues of standardization. CONCLUSION: The EUCAST method was found to be the more reliable one, whereas the addition of resazurin sodium salt solution facilitates the reading and provides a reliable and objective evaluation. The fragmented mycelia method could serve as an alternative that should be applied only in cases of poor or no sporulating dermatophytes.

PURPOSE: To compare stromal riboflavin absorption after 20% alcohol application and partial or complete epithelial removal by analyzing light transmission properties of porcine corneas after riboflavin/ultraviolet A (UVA) corneal collagen cross-linking. METHODS: Riboflavin 0.13% eye drops were applied to 18 porcine eyes (6 in which 20% alcohol solution had been applied for 40 seconds, 6 eyes with a grid pattern of full-thickness epithelial trauma, and 6 with the central epithelium fully removed) at 5-minute intervals for 35 minutes. In all eyes, the corneal surface was exposed to UVA light for 30 minutes during riboflavin administration. The light transmission spectra of the corneas were analyzed with a spectrophotometer and compared to those of 9 untreated controls (4 corneas with epithelium and 5 without) and to the spectra of riboflavin 0.13% solution. RESULTS: The spectra of riboflavin-treated corneas in the alcohol group were similar to controls. Those with grid-pattern epithelial trauma showed a dip in light transmission between 400 and 490 nm, but this was significantly less than that in eyes for which epithelial removal was complete, where the spectrum was similar to that of riboflavin 0.13% solution. CONCLUSIONS: Complete removal of the corneal epithelium appears to be necessary to allow sufficient riboflavin absorption into the stroma to alter the normal light transmission properties of the porcine cornea. Although partial grid-pattern epithelial removal allows some riboflavin penetration, uptake is limited and non-homogeneous, which may affect the efficacy of the cross-linking process.

Values of spirometry indices vary among subjects of similar age, gender and somatometrics but of different ethnic origins. Low socioeconomic status in childhood is inversely related to lung growth. The aim of this investigation was to assess spirometry values in Gypsy children and compare them to reported values for Caucasians. Gypsy students attending primary schools in Central Greece were recruited. Spirometry indices were measured using a portable spirometer. Regression analysis was applied to construct prediction equations for forced vital capacity (FVC) and other spirometric indices (FEV(1), FEF(50), FEF(25), FEF(25-75)) based on standing height. Predicted spirometric values were compared to values for Caucasians from published studies. In 152 children (ages 5-14 years; 57 girls) lung function increased linearly with height: spirometry index=intercept+[slopexheight], (r(2)=0.68 for FVC and FEV(1) in girls; r(2)=0.78 for FVC and r(2)=0.74 for FEV(1) in boys). Excluding boys-but not girls-in puberty increased fit for FVC (r(2)=0.83) and FEV(1) (r(2)=0.79). Mean predicted values were 5-10% lower than values for Caucasians. In Gypsy children, FVC and expiratory flow function increase linearly with standing height and predicted values are lower than those for Caucasians of similar height.

BACKGROUND: Iphiseius degenerans (Berlese) (Acari: Phytoseiidae) is a common predatory mite in citrus orchards in some areas of the Mediterranean basin and an important biological control agent of the thrips Frankliniella occidentalis (Pergande) in Integrated Pest Management (IPM) programmes in greenhouse crops. In this study, we evaluated the effects of the 'reduced-risk' insecticides acetamiprid, chlorantraniliprole, flubendiamide, metaflumizone, methoxyfenozide, spinetoram and thiamethoxam on I. degenerans, as a means of testing their compatibility in IPM programmes. RESULTS: Although all pesticides decreased immature survival, high mortality was only recorded for young larvae when exposed to acetamiprid, while metaflumizone, thiamethoxam and spinetoram resulted in intermediate lethal effects. The estimated LC50 values of acetamiprid, spinetoram and thiamethoxam for I. degenerans females were 0.52, 0.84 and 0.16-fold lower than the respective maximum recommended doses of the pesticides for field application. Although all pesticides tested significantly decreased fecundity, highest rates corresponded to the three pesticides already mentioned. CONCLUSION: Chlorantraniliprole, flubendiamide and methoxyfenozide may preliminarily be included in IPM programmes, whereas the effects of acetamiprid, metaflumizone, spinetoram and thiamethoxam on I. degenerans should be clarified in further field toxicological tests. This information could be useful for rationally planning and implementing pest management on a sustainable basis.

AIMS: A blind, randomised, prospective, bilateral study to investigate the efficacy of riboflavin/ultraviolet A corneal collagen cross-linkage to halt the progression of keratoconus. METHODS: 24 patients with early/moderate bilateral keratoconus with recent progression were recruited. One eye was randomly assigned to undergo collagen cross-linkage following epithelial removal with riboflavin 0.1% and ultraviolet A (370 nm at 3 mW/cm(2)). The other remained untreated as a control. The follow-up was 18 months in 22 patients. RESULTS: At 18 months, Orbscan II 3 mm, 5 mm keratometry and simulated astigmatism and cone apex power and wave-front measurements (Keraton Scout), including root mean square, coma and pentafoil showed significant reductions from baseline in treated compared with untreated eyes (p=0.04). In treated eyes at 18 months, the best spectacle-corrected acuity improved (p=0.01), and Orbscan II-simulated keratometry (p<0.001), 3 mm keratometry (p=0.008), simulated astigmatism (p=0.007), cone apex power (p=0.002), root mean square, coma, spherical aberration, secondary astigmatism and pentafoil (p=0.05) decreased from baseline. One treated eye experienced transient recurrent corneal erosions; otherwise there were no complications attributable to the treatment. CONCLUSIONS: Corneal collagen cross-linkage appears to be an effective and safe modality to halt the progression of keratoconus. Improvements in visual and topographic parameters are seen in some eyes.
