RESUMO

Root resorption is a challenging endodontic case in terms of the management of both hard and soft tissues in patients. It requires thorough knowledge, the proper choice of material, and regular follow-ups. Several etiological factors are responsible for the susceptibility of the tooth to resorption. The most common are dental caries and trauma. This case report sheds light on the etiopathogenesis of the development of internal root resorption and the clinical management of the resorptive defect. It also focuses on the need for proper diagnostic methodology for treating such complex defects.

This case report documents the diagnosis and successful management of a substantial periapical lesion located in the lower left region of the jaw. The patient presented with clinical symptoms indicative of periapical pathology, and radiographic examination revealed an extensive radiolucent lesion. The chosen treatment approach involved endodontic intervention coupled with surgical decompression, leading to the resolution of the lesion and restoration of oral health. This case underscores the significance of an accurate diagnosis and a multidisciplinary treatment approach in addressing large periapical lesions.

The management of teeth with open apices poses unique challenges in endodontics, requiring effective strategies to promote continued root development and maintain pulp vitality. This abstract explores the utilization of bioactive materials in the treatment of open apices, specifically focusing on their role in achieving optimal outcomes. Bioactive materials, such as Biodentine (Septodont, Saint-Maur-des-Fossés, France), have gained prominence for their favourable physiochemical properties, biocompatibility, and ability to stimulate dentinogenesis. The application of a bioactive material as an apical plug not only addresses immediate concerns but also contributes to long-term health and stability. This abstract reviews relevant literature, discusses clinical cases, and emphasizes the importance of tailoring treatment plans to the individual characteristics of open apex cases. The findings underscore the promising role of bioactive materials in reshaping the landscape of endodontic interventions for teeth with open apices, highlighting their potential to enhance both clinical and radiographic success.

The dental pulp, essential for tooth vitality, often becomes inflamed when exposed due to caries, fractures, or dislodged restorations. Untreated inflammation can lead to pulpal death, necessitating vital pulp therapies (VPTs) such as pulp capping and pulpotomy. Recent trends favor partial caries removal to avoid overtreatment and preserve pulp health. This shift is illustrated through two cases of young female patients with dislodged restorations and deep caries. Both underwent direct pulp capping using platelet-rich fibrin (PRF) and mineral trioxide aggregate (MTA), followed by composite restorations. These cases underscore the importance of biocompatible materials like MTA and PRF in maintaining pulp vitality and promoting dental tissue repair.

Patients have been recently observed to have undergone a noticeable increase in the concern over tooth discoloration making it a common cosmetic issue. This cumulative number of visits to dental experts is in search of ways to whiten their teeth because they desire to improve their looks. Often, the desire for a brighter smile is associated with health and beauty perceptions. Thus, patients tend to go after what they think is the perfect form of an unflawed grin. Diverse treatment modalities exist for tooth discolorations, such as composite laminate and veneers, porcelain veneers, and partial crowns. Amongst these options, bleaching, therefore, appears to be a conservative manner of dealing with teeth discoloration. This twin case report shows that discoloration can be managed best through bleaching using McInnes solution, resulting in good outcomes. Using McInnes solution instead makes bleaching an affordable and conservative technique for removing stains.

As periodontal and endodontic tissues have a close association, they come into close touch and have a lot of possible places for communication. In a clinical setting, this correlation promotes infection spread and results in the typical endo-perio lesion appearance. Because the two tissues are in close touch with one another, managing such lesions can be difficult. The success of treatment depends on a thorough examination and careful planning, with the sole focus on repair and regeneration. In these situations, bone graft materials with such characteristics have demonstrated encouraging outcomes. The treatment outcome along with a follow-up for a case of an endo-perio lesion with furcation involvement is shown in the accompanying case report. In treating such instances, a multidisciplinary approach is necessary, emphasizing regeneration.

Focusing on the Masserann technique, the study presents a success rate of 55% in retrieving separated instruments. The technique's effectiveness is demonstrated through a case involving retrieving an instrument from the mandibular first premolar. The technique utilizes various tools, including trephine burs and an extractor, providing a reliable means to dislodge tightly wedged fragments. Despite limitations in specific tooth types and the necessity for frequent radiographic monitoring, the Masserann kit proves effective and underscores the importance of attempting retrieval before considering surgical interventions. The presented case exemplifies the technique's clinical applicability and positive outcomes in intricate scenarios, emphasizing its significance in endodontic practice.

Probiotic therapy represents a novel concept in dentistry. The microbial nature of dental plaque can be altered, or the probiotic strategy can efficiently inhibit oral pathogens. Probiotics are dietary supplements that are vital for boosting immunity as they include beneficial bacteria and yeast. In dentistry and medicine, the interest in probiotics, prebiotics, and synbiotics is increasing. By forming a biofilm and assisting in preventing dental cavities, probiotics play a crucial role in dentistry and significantly impact immunity. Prebiotics are non-digestible dietary supplements that enhance health by increasing the quantity and activity of beneficial bacteria such as Lactobacilli and Bifidobacteria. It has been demonstrated that prebiotics, in addition to probiotics, can help treat oral diseases. They promote the growth and activity of beneficial organisms while inhibiting potentially harmful bacteria's growth and activity. Synbiotics are dietary supplements that combine probiotics and prebiotics, believed to work in tandem through a process known as synergism. Studies have indicated that synbiotics, or a combination of probiotics with a prebiotic, may have greater efficacy than either supplement alone.
