RESUMO

Magnetoencephalography (MEG) detects synchronized activity within a neuronal network by measuring the magnetic field changes generated by intracellular current flow. Using MEG data, we can quantify brain region networks with similar frequency, phase, or amplitude of activity and thereby identify patterns of functional connectivity seen with specific disorders or disease states. In this review, we examine and summarize MEG-based literature on functional networks in dystonias. Specifically, we inspect literature evaluating the pathogenesis of focal hand dystonia, cervical dystonia, embouchure dystonia, the effects of sensory tricks, treatment with botulinum toxin and deep brain stimulation, and rehabilitation approaches. This review additionally highlights how MEG has potential for application to clinical care of patients with dystonia.

BACKGROUND: Parkinson's Disease (PD) leads to poor quality of life and caregiver burden. Mindfulness-based stress reduction (MBSR) may improve these symptoms. We assessed the impact of a 9-week MBSR course on people with PD (PwP) and their care partners (CPs). METHODS: Participants completed questionnaires at screening, at the end of the course, and at 3-month follow-up: Parkinson's Disease Quality-39 (PDQ-39, PD only), Zarit Burden Inventory (ZBI, CP only) and Mindful Attention Awareness Scale (MAAS, both). The primary outcome measure was change in PDQ-39 (for PwP) or ZBI (for CP). Patient-reported scales were analyzed quantitatively; qualitative data on perceived effectiveness was collected. RESULTS: 53.8% PwP and 100% CPs completed the course. Among PwP, there was a significant reduction in MAAS(p < 0.001) and in PDQ-39 (p = 0.008). CPs experienced an increase in MAAS (p = 0.02) but no change in ZBI (p = 0.239). Qualitatively, both PwP and CPs expressed satisfaction with the course. DISCUSSION: MBSR improves mindful awareness in CPs and improves health-related quality of life in PwP.

RESUMO

A 76-year-old Caucasian woman initially presented to the Duke Memory Disorders clinic with a 9-month history of a rapid decline in cognitive, motor, and neuropsychiatric function. On initial presentation, the patient required assistance with activities of daily living. On neurological examination, she was found to have Gerstmann's syndrome along with appendicular apraxia. A positional tremor was noted without myoclonus or fasciculations. She had a paucity of speech and was unable to write her own name. Snout and grasp reflexes were present. Episodes of inappropriate laughter were noted during the exam. She was admitted to the inpatient neurology service for further evaluation. The Diffusion Weighted Imaging sequence on Magnetic Resonance Imaging of the brain was negative for restricted diffusion. An electroencephalogram was unremarkable. Cerebrospinal fluid analysis for Real-Time Quaking-Induced Conversion assay was positive along with an elevated 14-3-3 and increased total Tau protein levels. There was no family history of Creutzfeldt-Jakob disease. The cerebral spinal fluid results were consistent with a diagnosis of Creutzfeldt-Jakob disease, despite the negative MRI brain findings.
