ABSTRACT
The decrease in systolic/diastolic blood pressure of 100/50 mmHg obtained by antihypertensive therapy is associated with a 22% reduction of coronary events and 41% of cerebrovascular events. The benefit is independent of initial blood pressure level without threshold effect. The benefit is linked to lower blood pressure itself. The doctrine of comparable effectiveness of all antihypertensive drugs is challenged by more recent works. Although patients at high cardiovascular risk in clinical trials included are treated better than before, receiving as many other preventive treatments (statins, aspirin), it seems to remain a high residual cardiovascular risk that seems incompressible.
