BACKGROUND: Though the dosimetric criteria for the gastrointestinal tract were met, late gastrointestinal toxicity was seen in several cases. Therefore, we thought that it was caused by the positional variation of gastrointestine surrounding pancreatic cancer because of peristalsis. METHOD: They were confirmed by CT image regularly. And we evaluated that how much the difference of matching methods for correcting the positional variation influenced dose distribution. RESULT: The fiducial markers could follow the position of pancreatic cancer and the duodenum. But it could reproduce the dose distribution to pancreatic cancer and the duodenum. DISCUSSION: In proton therapy, the reproducible improvement of the duodenum position did not make the dose of the duodenum same as planning dose because the matching of fiducial markers made the positional relations between beam compensator and the duodenum change. CONCLUSION: The fiducial markers are useful for correcting the position of pancreatic cancer and the duodenum. But in proton therapy, it could not reproduce the dose distribution to pancreatic cancer and the duodenum.

AIM: The aim of the present study was to evaluate the clinicopathological features and the efficacy of endoscopic treatments in treating gastric antral vascular ectasia (GAVE) in association with liver diseases. METHODS: Thirty-four patients with the characteristic endoscopic findings of GAVE were enrolled. Endoscopic treatments were carried out for all 34 patients, including argon plasma coagulation (APC) in 22 patients and endoscopic band ligation (EBL) in 12 patients. RESULTS: All 34 patients had iron-deficiency anemia and 21 patients also had a history of tarry stools. The underlying pathologies of chronic liver diseases were liver cirrhosis in 26 patients, liver cirrhosis associated with hepatocellular carcinoma in six, and idiopathic portal hypertension in two. The liver function was classified by Child-Pugh classification: class A (n=6), class B (n=21), and class C (n=7). Antral motility was frequent and intense in all 34 GAVE patients. In the 22 patients who received APC, endoscopies revealed the recurrence of GAVE in 15 patients requiring further treatment by APC (recurrence rate, 68.2%). Seven patients died during the follow-up period, including two cases with bleeding-related deaths. In the 12 patients who received EBL, endoscopies revealed the recurrence of GAVE in one patient requiring further treatment by EBL (recurrence rate, 8.3%). Two patients died during the follow-up period, neither were bleeding-related deaths. CONCLUSIONS: The results suggest that GAVE is related to severe liver damage and portal hypertension. APC has a high recurrence rate of GAVE in the medium term after treatment. EBL may be useful as a treatment for GAVE.

AIMS: To evaluate the feasibility of integrating three-dimensional images created by intra-procedural cone-beam computed tomography (CBCT) into three-dimensional electroanatomical maps (EAM) and compare its accuracy with that of pre-procedural multi-slice CT (MSCT). METHODS AND RESULTS: In 24 patients with drug-refractory atrial fibrillation (AF), atriography using CBCT with pulmonary arterial contrast injection was performed at the beginning of the AF ablation procedure. Intra-procedural CBCT images and pre-procedural MSCT images were individually imported into the EAM system and compared their integration accuracy (point-to-surface distance) of each image and EAM just before ablation. The CBCT images were assessed qualitatively and quantitatively in comparison with MSCT images. All CBCT images were graded as optimal or useful in delineating the left atrium-pulmonary vein anatomy and were successfully integrated with the EAM. Overall, integration accuracy was similar for CBCT and MSCT. However, in 11 patients, the MSCT was performed 5 or more days prior to EAM, resulting in significantly shorter surface-to-point distance in CBCT than that in MSCT (P = 0.047). Radiation exposure with CBCT was significantly reduced compared with MSCT (P < 0.001). CONCLUSION: It is feasible to integrate CBCT image into EAM, and the integration is relatively accurate. Intra-procedural atriography by CBCT may replace pre-procedural MSCT as the imaging source for image integration.
