RESUMO

OBJECTIVE: To evaluate skeletal and dental changes in patients with skeletal Class III malocclusion and facial asymmetry after surgical-orthodontic treatment using cone-beam computerized tomography (CBCT). METHODS: This study included forty adult patients diagnosed with skeletal Class III malocclusion and asymmetry who underwent either isolated mandibular surgery or bimaxillary surgery. CBCT scans were taken before treatment (T0), at the completion of presurgical orthodontic treatment (T1) and after treatment (T2). Mimics 17.0 and 3-Matics 7.0 were used to measure skeletal and dental parameters. Skeletal and dental changes within each group from pretreatment to posttreatment were assessed, and Pearson correlation analysis was used to analyze the correlations among skeletal changes. RESULTS: The three-dimensional changes in condylar position were insignificant after surgical-orthodontic treatment in either group (P > 0.05). However, in the one-jaw surgery group, there were significant backward rotations of the condyle and ramus on the nondeviated side (P < 0.05), and the condyle on the deviated side rotated inward and forward significantly in the two-jaw surgery group (P < 0.05) at T2. There were no significant differences in the changes in the total alveolar bone thickness of bilateral first molars during dental decompensation (P > 0.05). The ratio between the buccal and the total bone thickness around the maxillary first molar on the deviated side decreased significantly at T1, as did those around the mandibular first molar on the nondeviated side (P < 0.05). CONCLUSIONS: Condylar angulations were less stable after treatment (7 to 9 months after surgery) in both the one-jaw and the two-jaw surgery groups, while condylar displacements were insignificant. In addition, orthodontists should keep a watchful eye to the relative position of the root in the alveolar bone during tooth decompensation.

RESUMO

A 3-year-old boy was referred to the emergency department of a medical centre because of a local swelling of the tongue. Radiographic imaging concluded that it concerned a complete tooth, which had been overlooked during an earlier visit to the same department. The foreign body was removed easily under general anaesthesia. A foreign body in the lip is a feature frequently seen by family physicians and in emergency departments of medical centres. However, a foreign body in the tongue is, by contrast, less often reported. Physical examination is the key factor in diagnostics. When in doubt, a radiograph should be made.

RESUMO

The autogenous tooth transplantation approach to replace missing teeth has been in use for a long time. Different surgical techniques have been developed to improve prognosis and longevity of transplanted teeth with complete root formation. Many factors reportedly affected short- and long-term success of the procedure: complications such as ankylosis and root resorption up to the tooth exfoliation have occurred frequently. Several studies have appeared on this issue over the years. However, outcomes of transplantations have not yet been conclusively determined as no randomized clinical trials have been published on this issue, and their effectiveness has been evaluated only in observational studies. On the other hand, interesting information from specific experimental studies on the healing pattern of the interface between the root surface and alveolar bone in the replantation/transplantation model, and from non-related studies dealing with mechanically induced periodontal trauma, has been available and potentially useful since the 1970s to 1980s. However, this information has not been incorporated into clinical literature regarding tooth autotransplantation. This commentary aims to show how information from related and unrelated experimental models was translated to a clinical setting and led to a novel approach, successfully applied, in the autotransplantation of a multirooted tooth with completed root formation.

In a study of tooth extractions in general dental practices in Ontario, Canada, 165 dental practitioners provided information on 6134 patients attending during a reference week. Of these, 11.6 per cent of patients had one or more permanent teeth extracted. Periodontal disease was given as the reason for 35.9 per cent of these extractions and caries for 28.9 per cent. Analysis by tooth type showed that third molars were the most common tooth type extracted. However, there were differences in the types of teeth extracted by age. Posterior teeth were most frequently lost by the younger age groups and anterior teeth by older subjects. There were also differences in the reasons for the loss of different tooth types. A comparison of these results with those of a similar study in Scotland suggests that age and tooth type does not account for the excess of extractions due to periodontal disease in this Canadian population. Differences in practice patterns and attitudes towards the retention of teeth may be contributing factors.

RESUMO

Prophylactic antibiotic cover for certain oral and dental procedures for patients with artificial joints is a controversial issue. This paper examines the evidence supporting the use of antibiotic cover and concludes that routinely, for the majority of patients with artificial joints, it is not justified. Certain medically compromised patients with artificial joints have a slightly increased risk of infection and, under these circumstances, antibiotic prophylaxis may be justified. The antibiotic recommended for this group is cephradine or in the case of penicillin allergy, clindamycin. Preoperative dental fitness is mandatory for all patients who are to receive artificial joints. This information will be of interest to the oral and maxillofacial surgeon in his own clinical practice, and when giving advice to local general dental practitioners.

We present a system for marking the skin during oral surgical operations. This system identifies teeth to be extracted or exposed under general anaesthesia. Removal of the wrong tooth can cause appreciable morbidity and leaves the surgeon and organisation liable for litigation and scrutiny by regulatory bodies. A recent review of claims to the NHS litigation authority between 1995 and 2010 showed that in the field of oral and maxillofacial surgery, dentoalveolar surgery resulted in the largest number of claims for negligence, of which removal of the wrong tooth was one of the most common. In 2010/2011 the National Reporting and Learning System (NRLS) of the National Patient Safety Agency (NPSA) were notified of 20 incidents when the wrong tooth had been extracted, which accounted for 5% of all incidents reported. We have therefore developed a robust marking system for oral surgical procedures in our hospital, which improves on the World Health Organisation (WHO) checklist. We have audited patients' perceptions and the clinical application of our marking system, and have shown that the system is welcomed by patients, and is simple and effective for clinicians to use.

A prerequisite for immediate implant placement is fully intact osseous and soft tissue, which can only be achieved by a careful and less-traumatic tooth removal that minimizes damage to the soft and hard tissue structures of the extracted tooth socket. Traditional methods of tooth extraction using forceps and elevators often results in some degree of soft and hard tissue damage, which ranges from slight soft tissue laceration to complete loss of the buccal bony plate and interdental bone crest. This paper describes an extraction technique that utilizes a systematic tooth sectioning approach to help minimize soft and hard tissue loss in the extracted tooth socket.

INTRODUCTION: Posts have been used efficiently to retain restorations for badly destructed teeth. This article critically analyzes the concerned topics related to the fracture resistance of teeth restored with dowel-retained restorations. METHODS: A systematic review of PubMed/MEDLINE, Cochrane, and Scopus databases was completed (from 1960 to 2010). Single or combined key words were used to obtain the most possible comprehensive list of articles. Checking the references of the relevant obtained sources completed the review along with a manual search to locate related articles on the topic. In vivo and ex vivo (laboratory, computer-based finite element, and photoelastic stress analysis studies) investigations related to the topic were included. RESULTS: Many factors have been proposed to influence the fracture resistance of post-restored teeth. Recognizing the significance of these factors on the fracture resistance of teeth would aid in choosing the suitable treatment modality for every individual case. Fracture resistance was improved if tooth structure loss was limited, a ferrule was obtained, a post with similar physical properties to natural dentine was used, and adhesive techniques for post luting and coronal restoration were used. Adhesively luted resin/fiber posts with composite cores appear to be the best currently available option in terms of tooth fracture and biomechanical behaviour. CONCLUSIONS: Most guidelines were based mainly on ex vivo studies and to a lesser extent on limited in vivo studies. The lack of long-term controlled randomized clinical studies was the main hindrance to reaching a conclusive and undisputable opinion regarding endodontic posts in terms of tooth fracture and biomechanical behaviour.

PURPOSE: The aim of the work was to use and to evaluate the usefulness of the slow variable magnetic fields to aid the treatment of the teeth chosen for extraction. The marginal paradontium of periapical bone of teeth was in a state of extensive destruction. The teeth were chosen for extraction. MATERIAL AND METHODS: 13 patients were chosen. 10 of them had with endo-perio changes and 3 suffered from full tooth luxation and had the teeth replanted. Those people were to have an extraction procedure or were declared as impossible to treat in other dental offices. Patients underwent non-aggressive skaling, endodontic treatment and were exposed to slow variable magnetic fields generated by Viofor JPS, accordingly to methods and parameters suggested by Department of Propaedeutics in Dentistry of Pomeranian Medical University in Szczecin. The process of healing of changes was evaluated radiologically. RESULTS: RTG done after 2 weeks and after 2 months were evaluated in respect of bone regeneration. They show the bone structure concentration. A RTG evaluation after half a year, two and three years show a preservation of the bone structure concentration. CONCLUSIONS: The use of slow variable magnetic fields contributed to bone structure regeneration and to preserve teeth with recorded endo-perio syndrome. Endodontic treatment of replanted teeth, aided with magnetostimulation has stopped the osteolisis process.

RESUMO

Eight subjects had the crowns of 21 teeth surgically amputated, and the roots were covered by alveolar mucosa. At the time of surgery, 17 teeth were vital and four had received endodontic therapy. After an average 2-year-period, 12 teeth (57%) remained submerged without complications. Two roots have been extracted, three have been successfully converted to regular overdenture abutments, and the remaining four have appeared in the mouth and are still under observation. Caution is urged as to expectations of a long-term prosthetic benefit from this procedure.
