RESUMEN
Unilateral separate anchor--wiring osteosynthesis of the forearm bones has essential advantages over the accepted methods for osteosynthesis of metaepiphyseal fractures of the radius. For adaptation to modern requirements an apparatus is suggested with external fixation of the radial diaphysis and the second metacarpal bone by means of bundles of short Kirschner wires with Y-shaped insertion of the pointed ends. The apparatus provides for longitudinal and transverse movement with fixation of the fragments at any level of the forearm and maintenance of rotational movements and movements at the interphalangeal joints. The method was applied in operations on 18 patients with neglected metaepiphyseal deformities of the radius; the outcome was positive.

An external fixation device with separate unilateral insertion of shortened Kirschner's rods with V-shape intraosseous location of the sharp ends has been proposed by the authors for the treatment of inveterate posttraumatic deformations of the bones of the forearm. The apparatus has a transversal reposing unit for displacement of the fragments in the same or antilateral directions. The apparatus has been applied in 33 patients. The results of the treatment are positive in 28 patients, and 5 patients are still under treatment with a favourable outlook.

The author has used an improved two-stage method of autoplasty in 27 patients mainly with posttraumatic bone defects of the forearm in the conditions of latent infection. Preliminary longitudinal distraction of the fragments allowed to reconstruct distal radioulnar syndesmosis and simultaneous transversal traction of the fragments extended the interosseal space. At the second stage it allowed to carry out surgical treatment of the deformed avascular ends of the fragments in optimal conditions and to adequately replace the defect by a self-fixing figure autograft from the tibial crest which was modelled according to the shapes and to the form of the lost segment of the radius or of the ulna. The long-term results were positive.
