RESUMEN

PURPOSE: AML is a heterogeneous hematologic malignancy. Region-specific recommendations for AML management can enhance patient outcomes. This article aimed to develop recommendations for the Gulf Cooperation Council (GCC) countries. METHODS: Ten AML panel members from Kuwait, Oman, Qatar, and the United Arab Emirates (KOQU) participated in a modified two-round Delphi process. The panel first identified the unmet regional needs and finalized a list of core variables. Next, they voted on iterative statements drawn from international recommendations and provided feedback via a questionnaire. Consensus voting ≤70% was discussed, and additional clinical decision making statements were suggested. At round closure, a consensus vote took place on revised statements. RESULTS: The panel reached ≥97.8% consensus on AML management. The panel agreed to use international risk stratification categories for personalized treatment of AML. The presence of ≥10% blasts for recurrent genetic abnormalities was required for a diagnosis of AML. Key consensus was reached for different treatment stages. The panel noted that older patients pose a challenge because of poor cytogenetics and genetic anomalies and require different treatment approaches. The panel recommended venetoclax-hypomethylating agents; fludarabine, cytarabine, idarubicin, and granulocyte colony-stimulating factor; and targeted therapy for AML relapsed/refractory disease. Supportive care is considered on the basis of prevailing organisms and drug resistance. CONCLUSION: The GCC KOQU's consensus-based recommendations for managing AML include an evidence-based and region-specific framework.

Studies on chronic myeloid leukemia (CML) in the Gulf region are scarce, consisting of a survey and expert meeting that included 15 experts in 2023 which discussed CML diagnosis, testing, treatment objectives, toxicities, and discontinuation in the Gulf region. Most patients were reported to be in first-line therapy, and the most common treatments were imatinib/imatinib generic in first-line and dasatinib in second- and third-lines. Mutation analysis was not reported to be routinely performed at the time of diagnosis but rather in case of progression to accelerated/blast phase or any sign of loss of response. While all participants were aware that BCR-ABL should be monitored every three months during the first year of treatment, 10% reported monitoring BCR-ABL every six months in practice due to test cost and lab capability. The most important first-line therapy objective was "achievement of major molecular response" (MMR) in younger patients and "overall survival" in older ones. The most important treatment objectives were "MMR" and "early molecular response followed by prolongation of overall survival" in the short term and "treatment-free remission" in the long term. The current practices in CML in the Gulf region appear to be similar to global figures.

Hypomethylating agents are the most widely used upfront therapy for patients with myelodysplastic syndrome (MDS) who are not suitable for hematopoietic stem cell transplantation. In Australia, azacitidine was, until recently, the only approved and subsidized treatment for patients with intermediate-2 and high-risk MDS, chronic myelomonocytic leukemia, and low blast acute myeloid leukemia. We analyzed prescription data to evaluate the real-world persistence and overall survival (OS) of patients prescribed azacitidine for the first time in Australia. A retrospective cohort analysis of patients who had been prescribed Pharmaceutical Benefits Scheme (PBS)-listed azacitidine for the first time, between January 2016 and April 2021, was conducted using the PBS 10% dataset. Treatment persistence and OS were estimated using Kaplan-Meier methods. The impact of the number of treatment cycles and treatment adherence on OS was also estimated. There were 351 patients in the PBS 10% dataset who initiated treatment with azacitidine. The average age (standard deviation [SD]) at azacitidine initiation was 71.9 (11.1) years and the average number (SD) of azacitidine prescriptions was 5.6 (0.2). The median persistence on azacitidine was 15.6 months, and the OS was 13.4 months. The median OS for patients who had six or more cycles of azacitidine treatment was greater compared to patients who had five or less cycles of treatment. The data from this real-world study illustrate the unmet medical needs of patients with MDS treated with azacitidine in Australia. The majority of patients are not treated with the optimal number of cycles of azacitidine, which is negatively correlated with patient outcomes.

Background: Globally, multiple myeloma (MM) ranks 24th among the most common cancers. The Middle East and Africa are affected by an increasing trend in MM incidence, owing to several underlying factors. This systematic review aims to assess the epidemiology, patient characteristics, and treatment outcomes associated with MM in selected countries in the Middle East and Africa. Methods: An electronic search was performed in the PubMed/MEDLINE database. Abstracts presented at the annual meetings of the American Society of Clinical Oncology, American Society of Hematology, and European Society for Medical Oncology and the GLOBOCAN registry were searched. Qualitative analysis was performed. Results: A total of 412 articles were screened, and 14 were selected. The five-year prevalence per 100,000 gathered from country-wise GLOBOCAN data ranged between 155 in Kuwait and 5,625 in North Africa. The identified treatment options were proteasome inhibitors such as bortezomib, drugs such as thalidomide, lenalidomide, dexamethasone, melphalan, and cyclophosphamide, and newer drugs such as daratumumab. Conclusion: Improved diagnostic capability has increased the incidence of MM in this region. However, advanced drugs and treatment regimens remain unaffordable in many countries of these regions. Therefore, understanding the trends of the disease and improving healthcare settings are imperative.

INTRODUCTION: Divorce is one of the harshest realities in Eastern societies worldwide because it is an intact component of the elementary social institution of the family. Grievously, divorce rates are escalating with profound ramifications for divorcees in Asia, including Pakistan. Therefore, exploring the challenges after divorce with gender-based dimensions in the Pakistani context was necessary, particularly in Hazara Division. RESEARCH METHODOLOGY: The study followed a pragmatic approach through snowball sampling and recruited 75 respondents. The data were collected through a semi-structured questionnaire and analysed using IBM SPSS 25. For descriptive statistics, frequencies of quantitative responses were determined using percentages, means, and standard deviations. Then Chi-Square Test of Independence, Principal Component Analysis, and Multivariate Analysis of Covariance were performed to find an association between the dependent and independent variables. RESULTS: Results demonstrated that most divorcees face children-related issues followed by economic, social, and psychological issues that impede post-divorce adjustment of divorcees. Results showed that the immaturity of divorcees and gender-specific violence specifically for women make it challenging for them to cope with the situation and impede their growth after divorce. Results revealed that more than half of women and a few of men have custody of children after divorce; however, fight over custody of children is another major cause of delayed adjustment. Results presented that gender significantly influences women's intensity of post-divorce adjustment constraints. CONCLUSION: Therefore, it is concluded that regardless of gender, ongoing conflicts with ex-spouses or in-laws made the post-divorce adjustment of divorcees difficult. Divorcees are in a constant tug-of-war between fighting internal dilemmas, pursuing individualistic ideals, and fulfilling societal norms, values, and expectations. This battle complicates and prolongs their adjustments after divorce. The study suggests that institutional, psychosocial, and family support is critical to proactively relieve divorcees from resources and their children.

COVID-19 indiscriminately impacted all the segments of the global society. Due to the unstructured job market, women in the informal sector were at high risk to experience the adverse effects of the pandemic. This paper aims to explore the impact of COVID-19 on women domestic workers and their families. Semi structured interviews conducted with fifty-four women domestic workers in three districts of Khyber Pakhtunkhwa, Pakistan were analyzed in five themes: disruption caused by lockdown, loss of livelihood, economic hardships, social support mechanism, and challenges faced during the pandemic. The study underlines that the pandemic left severe impacts on access to basic services, employment, food security at household level and pattern of expenditures. The plummeting economic activities led to sudden drops in earnings that forced families to sell their assets and incur debts. Respondents lamented over the social support system and considered it a necessary but not sufficient condition for uplifting the lives of the poor. Strains in marital relationships led to stress, anxiety and domestic violence among families. The utmost concern was the restoration of economic activities and urgent policy interventions to strengthen social safety measures for the vulnerable segments of society.

Food wastage is a critical and world-wide issue resulting from an excess of food supply, poor food storage, poor marketing, and unstable markets. Since food quality depends on consumer standards, it becomes necessary to monitor the quality to ensure it meets those standards. Embedding sensors with active nanomaterials in food packaging enables customers to monitor the quality of their food in real-time. Though there are many different sensors that can monitor food quality and safety, pH sensors and time-temperature indicators (TTIs) are the most critical metrics in indicating quality. This review showcases some of the recent progress, their importance, preconditions, and the various future needs of pH sensors and TTIs in food packaging for smart sensors in food packaging applications. In discussing these topics, this review includes the materials used to make these sensors, which vary from polymers, metals, metal-oxides, carbon-based materials; and their modes of fabrication, ranging from thin or thick film deposition methods, solution-based chemistry, and electrodeposition. By discussing the use of these materials, novel fabrication process, and problems for the two sensors, this review offers solutions to a brighter future for the use of nanomaterials for pH indicator and TTIs in food packaging applications.

Multi-parameter water quality monitoring is crucial in resource-limited areas to provide persistent water safety. Conventional water monitoring techniques are time-consuming, require skilled personnel, are not user-friendly and are incompatible with operating on-site. Here, we develop a multi-parameter water quality monitoring system (MWQMS) that includes an array of low-cost, easy-to-use, high-sensitivity electrochemical sensors, as well as custom-designed sensor readout circuitry and smartphone application with wireless connectivity. The system overcomes the need of costly laboratory-based testing methods and the requirement of skilled workers. The proposed MWQMS system can simultaneously monitor pH, free chlorine, and temperature with sensitivities of 57.5 mV/pH, 186 nA/ppm and 16.9 mV/°C, respectively, as well as sensing of BPA with <10 nM limit of detection. The system also provides seamless interconnection between transduction of the sensors' signal, signal processing, wireless data transfer and smartphone app-based operation. This interconnection was accomplished by fabricating nanomaterial and carbon nanotube-based sensors on a common substrate, integrating these sensors to a readout circuit and transmitting the sensor data to an Android application. The MWQMS system provides a general platform technology where an array of other water monitoring sensors can also be easily integrated and programmed. Such a system can offer tremendous opportunity for a broad range of environmental monitoring applications.

Free chlorine is widely used as a disinfectant in the water industry. Accurate monitoring of the residual free chlorine concentration in water cycles is critical to maintain public health safety. Here, we report on a thin gold film-based reusable and reagent-less free chlorine sensor. A gold thin film of 300 nm thickness was deposited on a polyimide tape, which was placed on a glass substrate and a simple Styrofoam adhesive tape was used to cover the film and expose 0.36 cm2 circular area as the sensing surface. The sensor showed a high sensitivity of 0.327 µA ppm-1, with a linear range of 0 to 6 ppm, and an accuracy of <0.1 ppm with high selectivity in the presence of commonly interfering ions. The sensor response time was 50 s with a negligible hysteresis of 0.06 ppm. The sensor showed very little change in output current in the pH range between 5.2 to 8.4, and temperature range of 20 to 30 °C. Therefore, the sensor operation is reagent-less, does not need frequent calibration, and showed consistent sensing performance with real water samples. The simple fabrication, ease-of-use and reliable sensing performance of the proposed sensor shows feasibility for mass-production and application in remote and resource-limited areas.

Smart packaging of fresh produce is an emerging technology toward reduction of waste and preservation of consumer health and safety. Smart packaging systems also help to prolong the shelf life of perishable foods during transport and mass storage, which are difficult to regulate otherwise. The use of these ever-progressing technologies in the packaging of fruits has the potential to result in many positive consequences, including improved fruit quality, reduced waste, and associated improved public health. In this review, we examine the role of smart packaging in fruit packaging, current-state-of-the-art, challenges, and prospects. First, we discuss the motivation behind fruit quality monitoring and maintenance, followed by the background on the development process of fruits, factors used in determining fruit quality, and the classification of smart packaging technologies. Then, we discuss conventional freshness sensors for packaged fruits including direct and indirect freshness indicators. After that, we provide examples of possible smart packaging systems and sensors that can be used in monitoring fruits quality, followed by several strategies to mitigate premature fruit decay, and active packaging technologies. Finally, we discuss the prospects of smart packaging application for fruit quality monitoring along with the associated challenges and prospects.

Bisphenol A, an endocrine disrupting compound, is widely used in food and beverage packaging, and it then leaches in food and source water cycles, and thus must be monitored. Here, we report a simple, low-cost and sensitive electrochemical sensor using graphene oxide and ß-cyclodextrin functionalized multiwalled carbon nanotubes for the detection of BPA in water. This sensor electrode system combines the high surface area of graphene oxide and carbon nanotubes, and the superior host-guest interaction capability of ß-cyclodextrin. A diffusion-controlled oxidation reaction involving equal numbers of protons and electrons facilitated the electrochemical sensing of BPA. The sensor showed a two-step linear response from 0.05 to 5 µM and 5-30 µM with a limit of detection of 6 nM. The sensors also exhibited a reproducible and stable response over one month with negligible interference from common inorganic and organic species, and an excellent recovery with real water samples. The proposed electrochemical sensor can be promising for the development of simple low-cost water quality monitoring system for monitoring of BPA in water.

Rapid, accurate and inexpensive monitoring of water quality parameters is indispensable for continued water safety, especially in resource-limited areas. Most conventional sensing systems either can only monitor one parameter at a time or lack user-friendly on-site monitoring capabilities. A fully integrated electrochemical sensor array is an excellent solution to this barrier. Electrochemical sensing methods involve transduction of water quality parameters where chemical interactions are converted to electrical signals. The challenge remains in designing low-cost, easy-to-use, and highly sensitive sensor array that can continuously monitor major water quality parameters such as pH, free chlorine, temperature along with emerging pharmaceutical contaminants, and heavy metal without the use of expensive laboratory-based techniques and trained personnel. Here, we overcame this challenge through realizing a fully integrated electrochemical sensing system that offers simultaneous monitoring of pH (57.5 mV/pH), free chlorine (186 nA/ppm), and temperature (16.9 mV/°C) and on-demand monitoring of acetaminophen and 17ß-estradiol (<10 nM) and heavy metal (<10 ppb), bridging the technological gap between signal transduction, processing, wireless transmission, and smartphone interfacing. This was achieved by merging nanomaterials and carbon nanotube-based sensors fabricated on microscopic glass slides controlled by a custom-designed readout circuit, a potentiostat, and an Android app. The sensing system can be easily modified and programmed to integrate other sensors, a capability that can be exploited to monitor a range of water quality parameters. We demonstrate the integrated system for monitoring tap, swimming pool, and lake water. This system opens the possibility for a wide range of low-cost and ubiquitous environmental monitoring applications.

We studied the outcome of 47 adult patients with relapsed acute leukaemia (AML = 25 and ALL = 22) treated with FLAG-mitoxantrone regimen. Median time to relapse was 10.7 months (range 1.9-27.7). Complete remission (CR2) was 60.1% which was significantly more frequent in ALL compared to AML (P = 0.049). WBC count < 100 × 109/L at initial diagnosis and time to relapse > 1 year were significantly predictor for CR2 in AML (P = 0.005 for both). Induction death was significantly higher in ALL compared to AML (P = 0.039). Median follow-up was 4.0 months (0.9-119.8) for AML and 2.1 months (range 0.6-118.1) for ALL. Nine patients underwent allogeneic stem-cell transplantation (allo-SCT). Estimated overall survival (OS) at 12 and 18 months was 60.5 and 34.6%, respectively, for AML, and 39.9 and 29.9%, respectively, for ALL. For AML patients failure to achieve CR, WBC count at initial diagnosis > 5 × 109/L and poor cytogenetic risk group was significant predictors of poor OS (P = 0.010, P = 0.025, and P = 0.015, respectively). For ALL patients failure to achieve of CR, WBC count at relapse < 5 × 109/L (CR patients) and lack of any type of consolidation therapy were significant predictor of poor OS (P < 0.001, P = 0.008, P = 0.008, respectively).

Monitoring of trace amount of acetaminophen and estrogen in drinking water is of great importance because of their potential links to gastrointestinal diseases and breast and prostate cancers. The sensitive and accurate detection of acetaminophen and estrogen requires the development of advanced sensing materials that possess appropriate number of analyte-capturing sites and suitable signal conduction path. This can be achieved by implementing appropriate chemical attachment of multiwalled carbon nanotubes (MWCNTs) and ß-cyclodextrin (ßCD). Here, we report a systematic investigation of four types of modified MWCNT-ßCD: (1) physical mixing, (2) "click reaction", (3) thionyl chloride esterification, and (4) Steglich esterification. The Steglich esterification is a one-step approach with shorter reaction time, lower reaction temperature, and eliminates handling of air/moisture-sensitive reagents. MWCNT-ßCD prepared by Steglich esterification possessed moderate ßCD loading (5-10 wt %), large effective surface area, and fast electron transfer. The host-guest interaction of ßCD and redox properties of MWCNT enabled sensitive detection of acetaminophen and 17ß-estradiol (E2 is a primary female sex hormone) in the range of 0.005-20 and 0.01-15 µM, with low detection limits of 3.3 and 2.5 nM, respectively. We demonstrated accurate detection results of pharmaceutical compositions in water and urine samples. These results indicate that Steglich esterification method may be applied in fabricating pharmaceutical contaminants sensors for health and environmental applications.

Iodine deficiency in soils and plants is a common problem in the hilly areas and has still been ignored. The aim of the current study is to investigate associated health issues, socio-economic characteristics and people's perception regarding iodine deficiency. Furthermore, a follow-up study was conducted to determine iodine and nutrients concentration in soil and crops at District Diamer, Gilgit-Baltistan, Pakistan. A descriptive and empirical analysis was examined. Most importantly, male was significantly observed as more affected for iodine deficiency than female, as contrary to the literature, socio-economic factors such as households' income and awareness (education) are reported as negative and significant contributor to reduce the iodine deficiency among the people irrespective of the gender. Majority of people (84%) heard about the iodine, but about 51.6% people argue that iodine has no effect on the human body and 56% of the respondents do not know why iodine is necessary for human. It was found that 11.5% of the community was affected from the IDD and majority of them were females. Wheat crops were the principal crop since it supplies 75% of calorific energy in an average Pakistani diet. The concentrations of iodine in soil samples range from 4.21 to 5.45 mg kg-1. The concentration of iodine in wheat crop plant sampled were considerably varied as Boner > Gais > Goher Abad with 1.12, 0.91 and 0.81 mg kg-1. respectively. Likewise, grain iodine concentration was varied as Gais > Boner > Goher Abad with 0.126, 0.102 and 0.078 mg kg-1, respectively. This study exposed that community using the cereal crops could face iodine deficiency in their diet which can cause endemic goiter among the population and control the iodine deficiency by empowering the rural community to raise the income level and providing the awareness to the people.

Although acute promyelocytic leukemia (APL) is a curable hematologic malignancy, early death (ED) remains a significant cause of treatment failure especially in developing countries. In a retrospective data analysis of 67 adult APL patients diagnosed in United Arab Emirates we report an ED rate of 11.9% which is comparable to that reported from more developed countries. We identified the following parameters at presentation as significant predictor of increased ED: Age >40 years (P = 0.015), fever (P = 0.030), WBC count >20 × 109/L (P = 0.010), the breakpoints other than bcr1 (P = 0.043) and fibrinogen level <1.5 g/L (P = 0.025). Delay in ATRA administration beyond 24 h from admission and fibrinogen <150 mg/dL were also significant predictors of ED, but only among high-risk patients (P = 0.035 and P = 0.033, respectively). WBC count >10 × 109/L and expression of HLA-DR (P = 0.018) or CD2 (P = 0.017) were significant predictors for differentiation syndrome (DS) which was found to be a predictor of ED (P = 0.002). Reducing the APL related ED rate in centers with limited resources is feasible provided early initiation of ATRA administration and early correction of coagulopathy in high-risk patients in addition to prompt treatment of DS. To our knowledge this is the first report from the Arabian Gulf describing ED in APL.

The effect of mercury (Hg) on the growth and survival of parsley (Petroselinum crispum) was explored at various treatments. The plants were grown in pots having Hoagland's solution to which various Hg treatments were applied and placed under greenhouse conditions. The treatments were: no metal applied (control) and six doses of Hg as mercuric chloride for 15 days. Linear trend of Hg accumulation was noted in roots, stems, and leaves with increasing Hg treatments. The maximum Hg concentration in root, stem and leaf was 8.92, 8.27, and 7.88 at Hg treatments of 25 mg l(-1), respectively. On the whole, Hg accumulation in different plant parts was in the following order: leaves > stem > roots. Linear trend was also observed for Bioaccumulation Factor (BF) and Translocation Factor (TF) with increasing Hg concentrations in the growth medium. The highest respective BFHg and TFHg values were 9.32 and 2.02 for the Hg treatments of 25 and 50 mg l(-1). In spite of the reduced growth in the presence of Hg, the plant has phytoremediation potential. It is recommended that parsley should not be cultivated in Hg contaminated sites in order to avoid dietary toxicity.

Highly sensitive, easy-to-fabricate, and low-cost pH sensors with small dimensions are required to monitor human bodily fluids, drinking water quality and chemical/biological processes. In this study, a low-temperature, solution-based process is developed to prepare palladium/palladium oxide (Pd/PdO) thin films for pH sensing. A precursor solution for Pd is spin coated onto pre-cleaned glass substrates and annealed at low temperature to generate Pd and PdO. The percentages of PdO at the surface and in the bulk of the electrodes are correlated to their sensing performance, which was studied by using the X-ray photoelectron spectroscope. Large amounts of PdO introduced by prolonged annealing improve the electrode's sensitivity and long-term stability. Atomic force microscopy study showed that the low-temperature annealing results in a smooth electrode surface, which contributes to a fast response. Nano-voids at the electrode surfaces were observed by scanning electron microscope, indicating a reason for the long-term degradation of the pH sensitivity. Using the optimized annealing parameters of 200°C for 48 h, a linear pH response with sensitivity of 64.71±0.56 mV/pH is obtained for pH between 2 and 12. These electrodes show a response time shorter than 18 s, hysteresis less than 8 mV and stability over 60 days. High reproducibility in the sensing performance is achieved. This low-temperature solution-processed sensing electrode shows the potential for the development of pH sensing systems on flexible substrates over a large area at low cost without using vacuum equipment.

Implantable electrodes with high flexibility, high mechanical fixation and low electrochemical impedance are desirable for neuromuscular activation because they provide safe, effective and stable stimulation. In this paper, we report on detailed materials and electrical analyses of three metal implantable electrodes - gold (Au), platinum (Pt) and titanium (Ti) - using X-ray photoelectron spectroscopy (XPS), scanning acoustic microscopy, drop shape analysis and electrochemical impedance spectroscopy. We investigated the cause of changes in electrochemical impedance of long-term immersed Au, Pt and Ti electrodes on liquid crystal polymers (LCPs) in phosphate buffered saline (PBS). We analyzed the surface wettability, surface and interface defects and the elemental depth profile of the electrode-adhesion layers on the LCP. The impedance of the electrodes decreased at lower frequencies, but increased at higher frequencies compared with that of the short-term immersion. The increase of impedances was influenced by the oxidation of the electrode/adhesion-layers that affected the double layer capacitance behavior of the electrode/PBS. The oxidation of the adhesion layer for all the electrodes was confirmed by XPS. Alkali ions (sodium) were adsorbed on the Au and Pt surfaces, but diffused into the Ti electrode and LCPs. The Pt electrode showed a higher sensitivity to surface and interface defects than that of Ti and Au electrodes. These findings may be useful when designing electrodes for long-term implantable devices.

Environmental quality indicators are crucial for responsive and cost-effective policies. The objective of the study is to examine the relationship between environmental quality indicators and financial development in Malaysia. For this purpose, the number of environmental quality indicators has been used, i.e., air pollution measured by carbon dioxide emissions, population density per square kilometer of land area, agricultural production measured by cereal production and livestock production, and energy resources considered by energy use and fossil fuel energy consumption, which placed an impact on the financial development of the country. The study used four main financial indicators, i.e., broad money supply (M2), domestic credit provided by the financial sector (DCFS), domestic credit to the private sector (DCPC), and inflation (CPI), which each financial indicator separately estimated with the environmental quality indicators, over a period of 1975-2013. The study used the generalized method of moments (GMM) technique to minimize the simultaneity from the model. The results show that carbon dioxide emissions exert the positive correlation with the M2, DCFC, and DCPC, while there is a negative correlation with the CPI. However, these results have been evaporated from the GMM estimates, where carbon emissions have no significant relationship with any of the four financial indicators in Malaysia. The GMM results show that population density has a negative relationship with the all four financial indicators; however, in case of M2, this relationship is insignificant to explain their result. Cereal production has a positive relationship with the DCPC, while there is a negative relationship with the CPI. Livestock production exerts the positive relationship with the all four financial indicators; however, this relationship with the CPI has a more elastic relationship, while the remaining relationship is less elastic with the three financial indicators in a country. Energy resources comprise energy use and fossil fuel energy consumption, both have distinct results with the financial indicators, as energy demand have a positive and significant relationship with the DCFC, DCPC, and CPI, while fossil fuel energy consumption have a negative relationship with these three financial indicators. The results of the study are of value to both environmentalists and policy makers.
