OBJECTIVES: The present study aimed to evaluate data from patients admitted to the first-episode psychotic (FEP) outpatient clinic at the Hospital of Clinics of the Federal University of Pernambuco from July 2018 to July 2021, seeking to identify factors related to better clinical outcomes. METHODS: This study was conducted using a convenience sample, including all patients between 15 and 65 years of age who were admitted to the FEP outpatient clinic from July 2018 to July 2021. Descriptive statistical analysis was performed using mean and standard deviation or median and interquartile range for continuous quantitative variables, and absolute number/percentage for qualitative variables. Paired T-test, a parametric test, was used to compare PANSS scores upon admission and after 6 months. Spearman's correlation test was employed to assess the correlation between duration of untreated psychosis (DUP) and treatment response with other variables. RESULTS: The sample consisted of 85.3% male individuals, with 50% of patients aged between 19 and 30 years, and 82% residing in the metropolitan area of Recife. Seventy percent of patients responded to the treatment implemented by the outpatient clinic, and only 30% required psychiatric hospitalization within 6 months of follow-up. The majority of patients had a history of psychoactive substance use (82.4%); however, the use of these substances did not impact the prognosis within the analysed sample. The median DUP was 4 weeks, and a shorter DUP was associated with a lower probability of psychiatric hospitalization and a greater treatment response (reduction >50% in PANSS). CONCLUSION: A shorter DUP was associated with a lower likelihood of psychiatric hospitalization and a greater treatment response. Furthermore, the specialized early psychosis outpatient clinic itself appears to yield positive outcomes, as 70% of the treated patients exhibited a positive treatment response.

Here, the antiviral activity of aminoadamantane derivatives were evaluated against SARS-CoV-2. The compounds exhibited low cytotoxicity to Vero, HEK293 and CALU-3 cells up to a concentration of 1,000 µM. The inhibitory concentration (IC50) of aminoadamantane was 39.71 µM in Vero CCL-81 cells and the derivatives showed significantly lower IC50 values, especially for compounds 3F4 (0.32 µM), 3F5 (0.44 µM) and 3E10 (1.28 µM). Additionally, derivatives 3F5 and 3E10 statistically reduced the fluorescence intensity of SARS-CoV-2 protein S from Vero cells at 10 µM. Transmission microscopy confirmed the antiviral activity of the compounds, which reduced cytopathic effects induced by the virus, such as vacuolization, cytoplasmic projections, and the presence of myelin figures derived from cellular activation in the face of infection. Additionally, it was possible to observe a reduction of viral particles adhered to the cell membrane and inside several viral factories, especially after treatment with 3F4. Moreover, although docking analysis showed favorable interactions in the catalytic site of Cathepsin L, the enzymatic activity of this enzyme was not inhibited significantly in vitro. The new derivatives displayed lower predicted toxicities than aminoadamantane, which was observed for either rat or mouse models. Lastly, in vivo antiviral assays of aminoadamantane derivatives in BALB/cJ mice after challenge with the mouse-adapted strain of SARS-CoV-2, corroborated the robust antiviral activity of 3F4 derivative, which was higher than aminoadamantane and its other derivatives. Therefore, aminoadamantane derivatives show potential broad-spectrum antiviral activity, which may contribute to COVID-19 treatment in the face of emerging and re-emerging SARS-CoV-2 variants of concern.

ABSTRACT Ligneous conjunctivitis is a rare chronic form of recurrent membranous inflammation and plasminogen deficiency. Ocular manifestations may be associated with sites other than mucous membranes, such as the oral cavity, internal ear, respiratory, genitals, and kidney. Treatment is extremely difficult because of the lack of topic plasminogen drops, and a high volume is required for systemic supplementation. This report aimed to present two patients with ligneous conjunctivitis treated with membrane excision, topical fresh-frozen plasma, and heparin intra-, and postoperatively. No recurrence was found in the ligneous membrane in the 12-month follow-up. The use of topical fresh-frozen plasma and heparin after membrane excision could be effective to avoid recurrence.

BACKGROUND: Multidrug-resistant tuberculosis (MDR-TB) is a serious global public health concern associated with social vulnerability. In Brazil, the Unified Health System (SUS, Portuguese) provides free diagnosis and treatment for MDR-TB; however, other expenses may still be incurred for patients and their families which, according to the World Health Organization (WHO), can be catastrophic when these costs surpass 20.0% of the annual household income. This study aimed to assess the extent of catastrophic costs related to the diagnostic and therapeutic aspects of MDR-TB among patients receiving care at an outpatient clinic in Rio de Janeiro. METHODS: This prospective study used convenience sampling from July 2019 to June 2021. Data regarding direct and indirect costs were collected using a standardized questionnaire endorsed by the WHO. To analyze any impoverishment occurred from MDR-TB, a threshold established by the Brazilian Institute of Geography and Statistics for 2019 and 2020 of US$ 79,562 and US$ 94,5273, respectively, was applied. Descriptive statistics were used for data analysis, including mean; standard deviation; variation coefficient; median; and maximum, minimum, and interquartile ranges. RESULTS: A total of 65 patients were interviewed. Among the participants, 73.8% experienced catastrophic costs, with indirect costs exerting the most significant impact (median: US$ 3,825.9), in contrast to direct costs (median: US$ 542.7). When comparing the periods before and after diagnosis, the prevalence of poverty increased from 12.0% to 28.0%. CONCLUSIONS: Despite the support from the SUS in Brazil, diagnostic and therapeutic cascades incur additional costs, exacerbating social vulnerability among patients with MDR-TB.

The incidence and clinical characteristics of NTM diseases in Brazil remain relatively unknown. The present study describes the diagnosis of NTM isolates, the clinical presentation and treatment outcomes. We analyzed NTM isolates in patients of a tertiary hospital in the Southeast region of Brazil, from January 2008 to July 2019. The ATS/IDSA criteria for diagnosis and treatment of these patients was applied. Mycobacterium kansasii were identified in 13/113 (11.5%) patients. In 59/113 (52.2%) patients who met the ATS criteria for disease, 29/59 (49.1%) received treatment, and 22/29 (75.8%) were cured. The major species identified was M. kansasii. The most frequent symptoms among the treated patients were dyspnea and cough, and the proportion of cured patients was high.

Single-cell RNAseq has allowed unprecedented insight into gene expression across different cell populations in normal tissue and disease states. However, almost all studies rely on annotated gene sets to capture gene expression levels and sequencing reads that do not align to known genes are discarded. Here, we discover thousands of long noncoding RNAs (lncRNAs) expressed in human mammary epithelial cells and analyze their expression in individual cells of the normal breast. We show that lncRNA expression alone can discriminate between luminal and basal cell types and define subpopulations of both compartments. Clustering cells based on lncRNA expression identified additional basal subpopulations, compared to clustering based on annotated gene expression, suggesting that lncRNAs can provide an additional layer of information to better distinguish breast cell subpopulations. In contrast, these breast-specific lncRNAs poorly distinguish brain cell populations, highlighting the need to annotate tissue-specific lncRNAs prior to expression analyses. We also identified a panel of 100 breast lncRNAs that could discern breast cancer subtypes better than protein-coding markers. Overall, our results suggest that lncRNAs are an unexplored resource for new biomarker and therapeutic target discovery in the normal breast and breast cancer subtypes.

The present observational study was designed to characterize the integrative profile of serum soluble mediators to describe the immunological networks associated with clinical findings and identify putative biomarkers for diagnosis and prognosis of active tuberculosis. The study population comprises 163 volunteers, including 84 patients with active pulmonary tuberculosis/(TB), and 79 controls/(C). Soluble mediators were measured by multiplexed assay. Data analysis demonstrated that the levels of CCL3, CCL5, CXCL10, IL-1ß, IL-6, IFN-γ, IL-1Ra, IL-4, IL-10, PDGF, VEGF, G-CSF, IL-7 were increased in TB as compared to C. Patients with bilateral pulmonary involvement/(TB-BI) exhibited higher levels of CXCL8, IL-6 and TNF with distinct biomarker signatures (CCL11, CCL2, TNF and IL-10) as compared to patients with unilateral infiltrates/(TB-UNI). Analysis of biomarker networks based in correlation power graph demonstrated small number of strong connections in TB and TB-BI. The search for biomarkers with relevant implications to understand the pathogenetic mechanisms and useful as complementary diagnosis tool of active TB pointed out the excellent performance of single analysis of IL-6 or CXCL10 and the stepwise combination of IL-6 â†’ CXCL10 (Accuracy = 84 %; 80 % and 88 %, respectively). Together, our finding demonstrated that immunological networks of serum soluble biomarkers in TB patients differ according to the unilateral or bilateral pulmonary involvement and may have relevant implications to understand the pathogenetic mechanisms involved in the clinical outcome of Mtb infection.

Objetivo: Conhecer a percepção de pessoas em situação de rua em relação ao tratamento da tuberculose. Métodos: Estudo qualitativo, com base na história oral temática. Foram entrevistadas 24 pessoas em situação de rua no município de São Paulo, em tratamento da tuberculose e vinculados a duas Unidades Básicas de Saúde, no período de junho a julho de 2018. Resultados: Emergiram três categorias: A situação de rua como desafio para o enfrentamento da tuberculose; o impacto do tratamento na vida dos usuários; as potencialidades e os limites dos serviços de saúde no apoio ao tratamento. Os participantes destacaram que o fato de viver em situação de rua impacta no tratamento da tuberculose, pois as medidas de isolamento e monitoramento são menos efetivas. Em contrapartida, a possibilidade de cura e a relação estabelecida com os profissionais de saúde foram fundamentais para a adesão ao tratamento. Considerações finais: A tuberculose na população de rua deve ser compreendida na perspectiva da determinação social do processo saúde-adoecimento. Por outra parte, desafios inerentes aos esquemas terapêuticos, condições de vida na rua, vulnerabilidades individuais, coletivas e programáticas dos serviços e políticas públicas que precisam ser considerados na construção das estratégias de cuidado. Objective: To understand homeless people's perception in relation to the treatment of tuberculosis. Methods: A qualitative study based on thematic oral history. Twenty-four homeless individuals from the city of São Paulo undergoing treatment for tuberculosis and linked to two Basic Health Units were interviewed from June to July 2018. Results: Three catego-ries emerged: Homelessness as a challenge to coping with tuberculosis; The impact of treatment on users' lives; and The potential and limits of health services in supporting the tuberculosis treatment. The participants highlighted that the fact of living on the streets exerts impacts on their treatment, as isolation and monitoring measures are less effective. On the other hand, the possibility of cure and the relationship established with health professionals were fundamental for adherence to the treatment. Final considerations: Tuberculosis in the street population must be understood from the perspective of the social determination of the health-disease process. Added to the challenges inherent to the treat-ment of tuberculosis, life conditions on the street, as well as individual, collective and programmatic vulnerabilities of the services and public policies need to be considered in the care strategies. Drug-resistant tuberculosis (DR-TB) continues to be a serious public health problem. The objective of this study was to evaluate the sociodemographic, radiological, clinical, and outcome characteristics and assess the determinants of unfavorable outcomes in DR-TB. The descriptive-analytical study was carried out in a reference outpatient clinic in Rio de Janeiro, Brazil, among DR-TB cases that received treatment between February 2016 and October 2020, using descriptive statistics, χ2 test, and logistic regression multivariate. Of the 148 cases, 12.2% were resistant to rifampicin, 12.2% were resistant to isoniazid, 18.2% were polyresistant, 56.1% multidrug resistant, and 1.3% were extensively drug resistant. Most of the patients were men, aged up to 44 years, with brown or black skin, having up to 8 years of schooling, unemployed or working in the informal economy, and of low income. Presenting with acquired resistance or positive sputum smear microscopy in the diagnosis, taking more than four drugs, and being unemployed were associated with unfavorable outcomes. Having no income or acquired resistance doubled the chances of unfavorable outcomes. There was a high proportion of unfavorable outcomes, thereby highlighting the need to concentrate efforts on planning and executing public policies that include the severity of DR-TB and its risk factors.

BACKGROUND: The Neural Clinical Score for tuberculosis (NCS-TB) is a computer system developed to improve the triage of presumed pulmonary TB (pPTB). METHODS: A study was performed with cohorts of pPTB patients cared for at a reference hospital in Northeast Brazil. RESULTS: The NCS-TB sensitivity was 76.5% for TB diagnosis, which shortened the time from triage to smear microscopy results (3.3 to 2.5 days; p<0.001) and therapy initiation (6.7 to 4.1 days; p=0.045). CONCLUSIONS: Although the NCS-TB was not suitable as a screening tool, it was able to optimize laboratory diagnosis and shorten the time to treatment initiation.

BACKGROUND: In recent years, the prevalence of nontuberculous mycobacterial (NTM) infections has increased in different regions of the world. The American Thoracic Society (ATS) recommends standardized identification criteria, reinforcing the need for faster and less complicated clinical and laboratory techniques. METHODS: In this retrospective study, NTM species isolated from pulmonary, extrapulmonary, and disseminated samples from patients treated at a TB/HIV reference unit in the State of Amazonas from 2011 to 2014 were identified through a combination of molecular techniques. RESULTS: To identify the molecular technique, 50 cryopreserved NTM cultures were recovered and subcultivated in culture medium. The potentially pathogenic NTM species identified were M. avium, M. intracellulare, M. kansasii, M. chelonae, M. abscessus, M. fortuitum, and M. peregrinum. Results of GenoType® showed moderate agreement with those of genomic sequencing (kappa = 0.60), whereas the results obtained by the PRA-hsp65 technique disagreed with the results obtained by sequencing (kappa = 0.49). CONCLUSIONS: Our findings highlight that GenoType CM is a good method for the identification of NTM, as well as the need for the application of standardized criteria, such as those set forth by the ATS.

Objective: To map the scientific evidence related to the organization of palliative care practices provided for the elderly in the Healthcare System. Materials and methods: This is a scoping review following the Joanna Briggs Institute's method. The search was conducted in the PubMed, Cinahl, Web of Science, Scopus, VHL/Lilacs, Embase, and Cochrane databases, identifying 1,150 articles managed using the Endnote software. After the selection, the study sample consisted of 12 articles. The data were organized in Microsoft Excel spreadsheets, submitted to descriptive analysis, and discussed with the literature. Results: The relevance of palliative care practices for older adults to be organized in the Healthcare System was noted, focusing on primary healthcare, at home, and interdisciplinary coordination. However, there is still a fragmented offer of this care with limited considerations regarding the challenges and possibilities of its effectiveness. Conclusions: The mapping of scientific evidence on the subject shows that discussions regarding the insertion of palliative care in the Healthcare System are modest. The studies explore the role of primary care as the organizer of these practices, although in a fragmented perspective, with limited coordination between the Healthcare System services.


Objetivo: mapear las evidencias científicas relacionadas con la organización de las prácticas de cuidado paliativo al anciano en la Red de Atención de Salud. Materiales y método: scoping review por el método Joanna Briggs Institute. La búsqueda se realizó en las bases de datos PubMed, Cinahl, Web of Science, Scopus, BVS/Lilacs, Embase y Cochrane, con identificación de 1150 artículos, los cuales fueron gerenciados por el software Endnote. Luego de la selección, 12 artículos compusieron la amuestra del estudio. Los datos se ordenaron en planilla del Microsoft Excel, se sometieron a análisis descriptivo y se discutieron con la literatura. Resultados: se observó que la organización de las prácticas de cuidado paliativo al adulto mayor desde la Red de Atención de Salud, con enfoque en la atención primaria de salud, en la residencia y en la articulación interdisciplinaria es importante. Sin embargo, aun hay fragmentación en la oferta de este cuidado, con limitadas consideraciones en cuanto a los retos y posibilidades de su efectuación en todos los niveles de atención. Conclusiones: el mapeo de las evidencias científicas sobre el tema demuestra que son modestas las discusiones en torno de la inserción de los cuidados paliativos en la Red. Los estudios exploran el rol de la atención primaria de salud como ordenadora de estas prácticas, aunque en una perspectiva fragmentada, con poca articulación entre los servicios de la Red.

Os estudos exploram o papel da atenção primária como ordenadora dessas práticas, ainda que em uma perspectiva fragmentada, com pouca articulação entre os serviços da Rede.

RESUMEN

BACKGROUND: Rapid molecular methods such as the line probe assay (LPA) and Xpert® MTB/RIF assay (Xpert) have been recommended by the World Health Organization for drug-resistant tuberculosis (DR-TB) diagnosis. We conducted an interventional trial in DR-TB reference centers in Brazil to evaluate the impact of the use of LPA and Xpert. METHODS: Patients with DR-TB were eligible if their drug susceptibility testing results were available to the treating physician at the time of consultation. The standard reference MGITTM 960 was compared with Xpert (arm 1) and LPA (arm 2). Effectiveness was considered as the start of the appropriate TB regimen that matched drug susceptibility testing (DST) and the proportions of culture conversion and favorable treatment outcomes after 6 months. RESULTS: A higher rate of empirical treatment was observed with MGIT alone than with the Xpert assay (97.0% vs. 45.0%) and LPA (98.2% vs. 67.5%). Patients started appropriate TB treatment more quickly than those in the MGIT group (median 15.0 vs. 40.5 days; p<0.01) in arm 1. Compared to the MGIT group, culture conversion after 6 months was higher for Xpert in arm 1 (90.9% vs. 79.3%, p=0.39) and LPA in arm 2 (80.0% vs. 83.0%, p=0.81). CONCLUSIONS: In the Xpert arm, there was a significant reduction in days to the start of appropriate anti-TB treatment and a trend towards greater culture conversion in the sixth month.

Ethanol is the main biofuel produced by fermentation route and the search for new feedstocks to produce fuel ethanol is still a great challenge. This work aims to compare the ethanol production from a new irrigated rice cultivar BRS AG to the conventional cultivar BRS PAMPA applied in Brazil. Six different commercial strains of Saccharomyces cerevisiae (BG-1, CAT-1, FT-858, JP-1, PE-2, and SA-1) were applied in fermentation reactions. Fermentations performed with BRS PAMPA rice revealed that the highest yields were achieved with strain SA-1, corresponding to 93.0% of the theoretical maximum and final ethanol concentration of 58.92 g L-1, and with CAT-1, a yield of 92.7% and final ethanol concentration of 58.93 g L-1. For the fermentations with BRS AG rice, the highest yields were obtained with strain FT-858, exhibiting a 89.6% yield and final ethanol concentration of 62.45 g L-1, and with CAT-1, 87.9% yield and final ethanol concentration of 61.25 g L-1 were achieved. The most appropriate microorganism for ethanol production using BRS PAMPA rice and BRS AG rice was CAT-1. Comparatively, the ethanol yield and productivity using BRS AG were higher than those observed for BRS PAMPA for all strains, except for PE-2 and SA-1 that led to very similar results. The experimental results showed that the giant rice BRS AG is an excellent feedstock for fuel ethanol production in lowland fields.

The study aimed to assess the dominant sensory attributes and preference for cold brews prepared with different coffee species under different brewing conditions. Four beverages were brewed using Arabica and Robusta coffees with either a coarser coffee grind and extraction at 5 °C or a finer grind and extraction at 15 °C. Coffee flavor and bitter taste were dominant sensory attributes of all beverages, regardless of material and preparation. While there were no differences in preference, the composition and dominant sensory attributes differed among the cold brews. The use of different coffee species had a greater impact on the composition and the perception of the attributes of the cold brews than brewing conditions. Sourness and coffee flavor were more dominant in Arabica cold brews, which had higher trigonelline content and acidity, but bitter taste dominance was further reported. Meanwhile, bitterness and astringent sensation were more dominant in Robusta cold brews, with higher caffeine and total chlorogenic acid content; however, a high coffee flavor dominance was also observed.

INTRODUCTION: This study evaluates the performance of individual and combinations tests used for pediatric tuberculosis diagnosis at a reference center. MATERIALS AND METHODS: Diagnostic test outcomes from children with presumed pulmonary tuberculosis evaluated from January 2005 - July 2010 were compared to a standard diagnosis made by an expert panel of physicians. RESULTS: Presence of at least one sign/symptom, history of contact, or abnormal chest X-ray (aCXR) individually showed the highest sensitivity (85.7%). While the combination of history of contact, at least one sign/symptom, positive tuberculin skin test, and aCXR had low sensitivity of 20%, but the specificity and a positive predictive value were 100%, respectively. The combination of tests used in the International Union Against Tuberculosis and Lung Disease and the Brazilian Ministry of Health systems showed sensitivity of 28.6% and 71.4% and specificity of 95.8% and 97.0%, respectively. CONCLUSIONS: In the absence of a gold standard, the combination of clinical history, tuberculin skin test, and aCXR, as well as the Brazilian scoring system serve as simple, low-cost approach that can be used for pediatric TB diagnosis by first-contact care providers.

Resumo: A teoria da objetificação propõe que as experiências de objetificação sexual vivenciadas pelas mulheres levam à auto-objetificação: uma preocupação excessiva e prejudicial com a própria aparência em detrimento dos estados e competências corporais. A pesquisa teve como objetivo investigar as propriedades psicométricas de uma escala de auto-objetificação corporal para universitárias brasileiras. A amostra foi composta por 400 mulheres, com idades entre 17 e 53 anos (M = 21,65; DP = 5,46), sendo a maioria graduandas do curso de psicologia (49%, n = 196). Análises fatoriais exploratórias (AFE) e confirmatórias (AFC) foram realizadas para verificar a estrutura do modelo. Os resultados suportaram uma estrutura com 13 itens e duas dimensões: Vigilância corporal e Vergonha Corporal, mostrando bons indicadores para validade e confiabilidade interna. Não houve diferenças significativas entre graduandas em psicologia e estudantes de outros cursos. Discute-se futuras aplicações do instrumento e possibilidades frente a investigação da auto-objetificação em contextos variados.


Abstract: The objectification theory proposes that experiences of sexualization lead to self-objectification in women: a harmful concern for their appearance at the expense of body states and skills. The research aimed to investigate the psychometric properties of the Body Objectification Consciousness Scale for female Brazilian university students. The sample was composed by 400 women, aged between 17 and 53 years old (M = 21.65; SD = 5.46), the majority of them were undergraduate psychology students (49%, n = 196). Exploratory (AFE) and confirmatory (AFC) factor analyzes were performed to verify the model's structure. The results supported a structure with 13 items and two dimensions: Body Surveillance and Body Shame, showing good indicators for internal validity and reliability. There were no significant differences between undergraduate psychology students and students from other courses. Future applications of the instrument and possibilities regarding the investigation of self-objectification in different contexts are discussed.


Resumen: La teoría de la objetivación propone que las experiencias de objetivación sexual vividas por las mujeres conducen a la auto-objetivación: una preocupación excesiva y dañina por su apariencia a expensas de los estados y competencias corporales. La investigación tuvo como objetivo investigar las propiedades psicométricas de una escala de auto-objetivación corporal en una muestra de estudiantes universitarias brasileñas. Participaron 400 mujeres, de entre 17 y 53 años (M = 21.65; DT = 5.46), la mayoría egresadas de la carrera de psicología (49%, n = 196). Se realizaron análisis factoriales exploratorios (AFE) y confirmatorios (AFC) para verificar la estructura del modelo. Los resultados apoyaron un modelo con 13 ítems y dos dimensiones: Vigilancia corporal y Vergüenza corporal, mostrando buenos indicadores de validez interna y confiabilidad. No hubo diferencias significativas entre los estudiantes de psicología y los estudiantes de otros cursos. Se discuten futuras aplicaciones del instrumento y posibilidades en la investigación de la auto-objetivación en diferentes contextos.

ABSTRACT Background: The Neural Clinical Score for tuberculosis (NCS-TB) is a computer system developed to improve the triage of presumed pulmonary TB (pPTB). Methods: A study was performed with cohorts of pPTB patients cared for at a reference hospital in Northeast Brazil. Results: The NCS-TB sensitivity was 76.5% for TB diagnosis, which shortened the time from triage to smear microscopy results (3.3 to 2.5 days; p<0.001) and therapy initiation (6.7 to 4.1 days; p=0.045). Conclusions: Although the NCS-TB was not suitable as a screening tool, it was able to optimize laboratory diagnosis and shorten the time to treatment initiation.
