RESUMEN

BACKGROUND: Cardiac Resynchronization Therapy (CRT) is widely used for treating selected heart failure patients, but patients with myocardial scar respond worse to treatment. The Selvester QRS scoring system estimates myocardial scar burden using 12-lead ECG. This study's objective was to investigate the scores correlation to mortality in a CRT population. METHODS AND RESULTS: Data on consecutive CRT patients was collected. 401 patients with LBBB and available ECG data were included in the study. QuAReSS software was used to perform Selvester scoring. Mean Selvester score was 6.4, corresponding to 19% scar burden. The endpoint was death or heart transplant; outcome was analyzed using Cox proportional hazards models. A Selvester score >8 was significantly associated with higher risk of the combined endpoint (HR 1.59, p=.014, CI 1.09-2.3). CONCLUSION: Higher Selvester scores correlate to mortality in CRT patients with strict LBBB and might be of value in prognosticating survival.

RESUMEN

Water surged from Puget Sound sometime between 1000 and 1100 years ago, overrunning tidal marshes and mantling them with centimeters of sand. One overrun site is 10 kilometers northwest of downtown Seattle; another is on Whidbey Island, some 30 kilometers farther north. Neither site has been widely mantled with sand at any other time in the past 2000 years. Deposition of the sand coincided-to the year or less-with abrupt, probably tectonic subsidence at the Seattle site and with landsliding into nearby Lake Washington. These findings show that a tsunami was generated in Puget Sound, and they tend to confirm that a large shallow earthquake occurred in the Seattle area about 1000 years ago.

A study has been undertaken to examine the effect of peripheral substitution on the photophysics of platinum tetraarylporphyrins. The aim of the study was to provide better dyes for oxygen sensing. Substitution of electron rich aryl groups results in red shifts for both the absorption and emission spectra. These observations are explained using simple molecular orbital theory. The lifetime of the excited singlet state for platinum tetraphenylporphyrin is calculated to be shorter than 0.7 psec. The radiative rate constants are on the order of 10(3) sec(-1) while nonradiative rate constants are on the order of 10(4) sec(-1). The model used is sufficiently flexible to be applied to all Pt and Pd porphyrins.

RESUMEN

Intertidal mud has buried extensive, well-vegetated lowlands in westernmost Washington at least six times in the past 7000 years. Each burial was probably occasioned by rapid tectonic subsidence in the range of 0.5 to 2.0 meters. Anomalous sheets of sand atop at least three of the buried lowlands suggest that tsunamis resulted from the same events that caused the subsidence. These events may have been great earthquakes from the subduction zone between the Juan de Fuca and North America plates.
