RESUMEN

Radiomics features (RFs) studies have showed limitations in the reproducibility of RFs in different acquisition settings. To date, reproducibility studies using CT images mainly rely on phantoms, due to the harness of patient exposure to X-rays. The provided CadAIver dataset has the aims of evaluating how CT scanner parameters effect radiomics features on cadaveric donor. The dataset comprises 112 unique CT acquisitions of a cadaveric truck acquired on 3 different CT scanners varying KV, mA, field-of-view, and reconstruction kernel settings. Technical validation of the CadAIver dataset comprises a comprehensive univariate and multivariate GLM approach to assess stability of each RFs extracted from lumbar vertebrae. The complete dataset is publicly available to be applied for future research in the RFs field, and could foster the creation of a collaborative open CT image database to increase the sample size, the range of available scanners, and the available body districts.

BACKGROUND: Systemic inflammatory response syndrome (SIRS) and sepsis are the most common causes of in-hospital death. However, the characteristics associated with the improvement in the patient conditions during the ICU stay were not fully elucidated for each population as well as the possible differences between the two. GOAL: The aim of this study is to highlight the differences between the prognostic clinical features for the survival of patients diagnosed with SIRS and those of patients diagnosed with sepsis by using a multi-variable predictive modeling approach with a reduced set of easily available measurements collected at the admission to the intensive care unit (ICU). METHODS: Data were collected from 1,257 patients (816 non-sepsis SIRS and 441 sepsis) admitted to the ICU. We compared the performance of five machine learning models in predicting patient survival. Matthews correlation coefficient (MCC) was used to evaluate model performances and feature importance, and by applying Monte Carlo stratified Cross-Validation. RESULTS: Extreme Gradient Boosting (MCC = 0.489) and Logistic Regression (MCC = 0.533) achieved the highest results for SIRS and sepsis cohorts, respectively. In order of importance, APACHE II, mean platelet volume (MPV), eosinophil counts (EoC), and C-reactive protein (CRP) showed higher importance for predicting sepsis patient survival, whereas, SOFA, APACHE II, platelet counts (PLTC), and CRP obtained higher importance in the SIRS cohort. CONCLUSION: By using complete blood count parameters as predictors of ICU patient survival, machine learning models can accurately predict the survival of SIRS and sepsis ICU patients. Interestingly, feature importance highlights the role of CRP and APACHE II in both SIRS and sepsis populations. In addition, MPV and EoC are shown to be important features for the sepsis population only, whereas SOFA and PLTC have higher importance for SIRS patients.

The study of emotions through the analysis of the induced physiological responses gained increasing interest in the past decades. Emotion-related studies usually employ films or video clips, but these stimuli do not give the possibility to properly separate and assess the emotional content provided by sight or hearing in terms of physiological responses. In this study we have devised an experimental protocol to elicit emotions by using, separately and jointly, pictures and sounds from the widely used International Affective Pictures System and International Affective Digital Sounds databases. We processed galvanic skin response, electrocardiogram, blood volume pulse, pupillary signal and electroencephalogram from 21 subjects to extract both autonomic and central nervous system indices to assess physiological responses in relation to three types of stimulation: auditory, visual, and auditory/visual. Results show a higher galvanic skin response to sounds compared to images. Electrocardiogram and blood volume pulse show different trends between auditory and visual stimuli. The electroencephalographic signal reveals a greater attention paid by the subjects when listening to sounds compared to watching images. In conclusion, these results suggest that emotional responses increase during auditory stimulation at both central and peripheral levels, demonstrating the importance of sounds for emotion recognition experiments and also opening the possibility toward the extension of auditory stimuli in other fields of psychophysiology. Clinical and Translational Impact Statement- These findings corroborate auditory stimuli's importance in eliciting emotions, supporting their use in studying affective responses, e.g., mood disorder diagnosis, human-machine interaction, and emotional perception in pathology.

In clinical environments, such as the Intensive Care Unit (ICU), continuous and uninterrupted monitoring of vital signs is critical for the early detection of patient deterioration and prompt intervention. Since data collected in these settings are often corrupted by noise, artifacts, or recording gaps, it is important to estimate missing data for a more accurate signal assessment.In this study, we propose an automatic algorithm for reconstructing of arterial blood pressure signal waveforms. The methodological core of the algorithm is based on the idea of statistical shape modeling, which basically estimates the shape variation of beat waveforms in order to reconstruct them in noisy segments. The waveform reconstruction is achieved by combining the average beat template from a 90-second segment of clean signal preceding the gap with the main shape variations of the estimated waveform.The algorithm was validated using arterial blood pressure recordings from 9 subjects admitted in the ICU and collected in the MIMIC-III Waveform Database, each lasting 1 hour and sampled at 125 Hz. For each record, ten fictitious gaps were created, and the reconstructed segments were compared to the original signals with the metrics proposed within the PhysioNet / Computing in Cardiology Challenge 2010. Results demonstrate the excellent performance of the proposed algorithm, with overall averages of both Q1 and Q2 metrics greater than 0.85.

Introduction: Research on the neural mechanisms of perceptual decision-making has typically focused on simple categorical choices, say between two alternative motion directions. Studies on such discrete alternatives have often suggested that choices are encoded either in a motor-based or in an abstract, categorical format in regions beyond sensory cortex. Methods: In this study, we used motion stimuli that could vary anywhere between 0° and 360° to assess how the brain encodes choices for features that span the full sensory continuum. We employed a combination of neuroimaging and encoding models based on Gaussian process regression to assess how either stimuli or choices were encoded in brain responses. Results: We found that single-voxel tuning patterns could be used to reconstruct the trial-by-trial physical direction of motion as well as the participants' continuous choices. Importantly, these continuous choice signals were primarily observed in early visual areas. The tuning properties in this region generalized between choice encoding and stimulus encoding, even for reports that reflected pure guessing. Discussion: We found only little information related to the decision outcome in regions beyond visual cortex, such as parietal cortex, possibly because our task did not involve differential motor preparation. This could suggest that decisions for continuous stimuli take can place already in sensory brain regions, potentially using similar mechanisms to the sensory recruitment in visual working memory.

BACKGROUND: The autonomic response to transcutaneous auricular vagus nerve stimulation (taVNS) has been linked to the engagement of brainstem circuitry modulating autonomic outflow. However, the physiological mechanisms supporting such efferent vagal responses are not well understood, particularly in humans. HYPOTHESIS: We present a paradigm for estimating directional brain-heart interactions in response to taVNS. We propose that our approach is able to identify causal links between the activity of brainstem nuclei involved in autonomic control and cardiovagal outflow. METHODS: We adopt an approach based on a recent reformulation of Granger causality that includes permutation-based, nonparametric statistics. The method is applied to ultrahigh field (7T) functional magnetic resonance imaging (fMRI) data collected on healthy subjects during taVNS. RESULTS: Our framework identified taVNS-evoked functional brainstem responses with superior sensitivity compared to prior conventional approaches, confirming causal links between taVNS stimulation and fMRI response in the nucleus tractus solitarii (NTS). Furthermore, our causal approach elucidated potential mechanisms by which information is relayed between brainstem nuclei and cardiovagal, i.e., high-frequency heart rate variability, in response to taVNS. Our findings revealed that key brainstem nuclei, known from animal models to be involved in cardiovascular control, exert a causal influence on taVNS-induced cardiovagal outflow in humans. CONCLUSION: Our causal approach allowed us to noninvasively evaluate directional interactions between fMRI BOLD signals from brainstem nuclei and cardiovagal outflow.

The WHISPER (Widespread Hearing Impairment Screening and PrEvention of Risk) platform was recently developed for screening for hearing loss (HL) and cognitive decline in adults. It includes a battery of tests (a risk factors (RF) questionnaire, a language-independent speech-in-noise test, and cognitive tests) and provides a pass/fail outcome based on the analysis of several features. Earlier studies demonstrated high accuracy of the speech-in-noise test for predicting HL in 350 participants. In this study, preliminary results from the RF questionnaire (137 participants) and from the visual digit span test (DST) (78 participants) are presented. Despite the relatively small sample size, these findings indicate that the RF and DST may provide additional features that could be useful to characterize the overall individual profile, providing additional knowledge related to short-term memory performance and overall risk of HL and cognitive decline. Future research is needed to expand number of subjects tested, number of features analyzed, and the range of algorithms (including supervised and unsupervised machine learning) used to identify novel measures able to predict the individual hearing and cognitive abilities, also including components related to the individual risk.

The efficacy of the late-evening snack (LES) has been extensively studied due to the impact of the longest intermeal duration occurring at night in patients with cirrhosis. While actual clinical guidelines on nutrition in chronic liver disease recommend an LES, no specific nutritional compositions have been reported by the European Association for the Study of the Liver (EASL) and the European Society for Clinical Nutrition and Metabolism (ESPEN). Late-evening snacks vary greatly among studies, including natural foods and/or nutritional supplements, yet oral supplements still need to fully meet the LES's nutritional composition. In addition, many hepatologists need to gain experience in nutritional approaches and have access to registered dieticians who can help them manage patients with liver disease. Therefore, this review study aims to summarise evidence regarding using LESs and the mechanisms behind long starvation in patients with cirrhosis. It also provides a practical nutritional guide with several LES options based on common natural foods tailored to special patients' nutritional requirements and geographical backgrounds. In preventing accelerated starvation and related protein malnutrition and sarcopenia in patients with cirrhosis, the nutritional composition of LESs is essential. The proper and straightforward application of the LES's rational nutrition is an advantage to cirrhotic patients and should be carried out by healthcare professionals to enhance the overall liver function and nutritional status of patients with cirrhosis.

In an observational study, we analyzed 1293 healthcare workers previously infected with severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2), of which 34.1% developed postacute sequelae of SARS-CoV-2 infection (also known as long COVID). Using a multivariate logistic regression model, we demonstrate that the likelihood of developing long COVID in infected individuals rises with the increasing of duration of infection and that 3 doses of the BNT162b2 vaccine are protective, even during the Omicron wave.

PURPOSE: Radiomics of vertebral bone structure is a promising technique for identification of osteoporosis. We aimed at assessing the accuracy of machine learning in identifying physiological changes related to subjects' sex and age through analysis of radiomics features from CT images of lumbar vertebrae, and define its generalizability across different scanners. MATERIALS AND METHODS: We annotated spherical volumes-of-interest (VOIs) in the center of the vertebral body for each lumbar vertebra in 233 subjects who had undergone lumbar CT for back pain on 3 different scanners, and we evaluated radiomics features from each VOI. Subjects with history of bone metabolism disorders, cancer, and vertebral fractures were excluded. We performed machine learning classification and regression models to identify subjects' sex and age respectively, and we computed a voting model which combined predictions. RESULTS: The model was trained on 173 subjects and tested on an internal validation dataset of 60. Radiomics was able to identify subjects' sex within single CT scanner (ROC AUC: up to 0.9714), with lower performance on the combined dataset of the 3 scanners (ROC AUC: 0.5545). Higher consistency among different scanners was found in identification of subjects' age (R2 0.568 on all scanners, MAD 7.232 years), with highest results on a single CT scanner (R2 0.667, MAD 3.296 years). CONCLUSION: Radiomics features are able to extract biometric data from lumbar trabecular bone, and determine bone modifications related to subjects' sex and age with great accuracy. However, acquisition from different CT scanners reduces the accuracy of the analysis.

BACKGROUND: Pancreatic surgery has been associated with important postoperative morbidity, mortality and prolonged length of hospital stay. In pancreatic surgery, the effect of poor preoperative nutritional status and muscle wasting on postsurgery clinical outcomes still remains unclear and controversial. MATERIALS AND METHODS: A total of 103 consecutive patients with histologically proven carcinoma undergoing elective pancreatic surgery from June 2015 through to July 2020 were included and retrospectively studied. A multidimensional nutritional assessment was performed before elective surgery as required by the local clinical pathway. Clinical and nutritional data were collected in a medical database at diagnosis and after surgery. RESULTS: In the multivariable analysis, body mass index (OR 1.25, 95% CI 1.04-1.59, p = 0.039) and weight loss (OR 1.16, 95% CI 1.06-1.29, p = 0.004) were associated with Clavien score I-II; weight loss (OR 1.13, 95% CI 1.02-1.27, p = 0.027) affected postsurgery morbidity/mortality, and reduced muscle mass was identified as an independent, prognostic factor for postsurgery digestive hemorrhages (OR 0.10, 95% CI 0.01 0.72, p = 0.03) and Clavien score I-II (OR 7.43, 95% CI 1.53-44.88, p = 0.018). No association was identified between nutritional status parameters before surgery and length of hospital stay, 30 days reintervention, 30 days readmission, pancreatic fistula, biliary fistula, Clavien score III-IV, Clavien score V and delayed gastric emptying. CONCLUSIONS: An impaired nutritional status before pancreatic surgery affects many postoperative outcomes. Assessment of nutritional status should be part of routine preoperative procedures in order to achieve early and appropriate nutritional support in pancreatic cancer patients. Further studies are needed to better understand the effect of preoperative nutritional therapy on short-term clinical outcomes in patients undergoing pancreatic elective surgery.

Comparisons among the different vaccines against SARS-CoV-2 are important to understand which type of vaccine provides more protection. This study aimed to evaluate the real-life efficacy through symptomatic infection and the humoral response of six different vaccines against SARS-CoV-2-BNT162b2, mRNA-1273, ChAdOx1-S, CoronaVac, Ad26.COV2, and Ad5-nCoV. This multicentric observational longitudinal study involved hospitals from Mexico and Brazil in which volunteers who received complete vaccination schemes were followed for 210 days after the last dose. SARS-CoV-2 Spike 1-2 IgG levels were taken before receiving the first vaccine, 21 days after each dose, and the last sample at six months (+/-1 month) after the last dose. A total of 1132 individuals exposed to five COVID-19 waves were included. All vaccines induced humoral responses, and mRNA vaccines had the highest antibody levels during follow-up. At six months, there was a decline in the SARS-CoV-2 Spike 1-2 IgG antibody titers of 69.5% and 36.4% in subjects with negative and positive history of infection respectively. Infection before vaccination and after complete vaccination scheme correlated with higher antibody titers. The predictors of infection were vaccination with CoronaVac compared to BNT162b2 and ChAdOx1-S. In the presence of comorbidities such as diabetes, rheumatoid arthritis, or dyslipidemia, CoronaVac lowered the risk of infection.

Physiologic dead space is a well-established independent predictor of death in patients with acute respiratory distress syndrome (ARDS). Here, we explore the association between a surrogate measure of dead space (DS) and early outcomes of mechanically ventilated patients admitted to Intensive Care Unit (ICU) because of COVID-19-associated ARDS. Retrospective cohort study on data derived from Italian ICUs during the first year of the COVID-19 epidemic. A competing risk Cox proportional hazard model was applied to test for the association of DS with two competing outcomes (death or discharge from the ICU) while adjusting for confounders. The final population consisted of 401 patients from seven ICUs. A significant association of DS with both death (HR 1.204; CI 1.019-1.423; p = 0.029) and discharge (HR 0.434; CI 0.414-0.456; p [Formula: see text]) was noticed even when correcting for confounding factors (age, sex, chronic obstructive pulmonary disease, diabetes, PaO[Formula: see text]/FiO[Formula: see text], tidal volume, positive end-expiratory pressure, and systolic blood pressure). These results confirm the important association between DS and death or ICU discharge in mechanically ventilated patients with COVID-19-associated ARDS. Further work is needed to identify the optimal role of DS monitoring in this setting and to understand the physiological mechanisms underlying these associations.

An important issue that is often neglected is the difference between male and female genders in response to medical treatments. In the context of COVID-19 vaccine administration, despite identical protocol strategies, it has been observed that females often suffer more adverse consequences than males. Here, we analyzed the adverse events (AEs) of the Comirnaty vaccine in a population of 2385 healthcare workers as a function of age, sex, COVID-19 history and BMI. Using logistic regression analysis, we showed that these variables may contribute to the development of AEs, particularly in young subjects, females and individuals with a BMI below 25 kg/m2. Moreover, partial dependence plots indicate a 50% probability of developing a mild AE for a long period of time (≥7 days) or a severe AE of any duration in women below 40 years old and with a BMI < 20 kg/m2. As this effect is more evident after the second dose of the vaccine, we propose to reduce the amount of vaccine for any additional booster dose in relation to age, sex and BMI. This strategy might reduce adverse events without affecting vaccine efficacy.

Assessment of heartbeat dynamics provides a promising framework for non-invasive monitoring of cardiovascular and autonomic states. Nevertheless, the non-specificity of such measurements among clinical populations and healthy conditions associated with different autonomic states severely limits their applicability and exploitation in naturalistic conditions. This limitation arises especially when pathological or postural change-related sympathetic hyperactivity is compared to autonomic changes across age and experimental conditions. In this frame, we investigate the intrinsic irregularity and complexity of cardiac sympathetic and vagal activity series in different populations, which are associated with different cardiac autonomic dynamics. Sample entropy, fuzzy entropy, and distribution entropy are calculated on the recently proposed sympathetic and parasympathetic activity indices (SAI and PAI) series, which are derived from publicly available heartbeat series of congestive heart failure patients, elderly and young subjects watching a movie in the supine position, and healthy subjects undergoing slow postural changes. Results show statistically significant differences between pathological/old subjects and young subjects in the resting state and during slow tilt, with interesting trends in SAI- and PAI-related entropy values. Moreover, while CHF patients and healthy subjects in upright position show the higher cardiac sympathetic activity, elderly and young subjects in resting state showed higher vagal activity. We conclude that quantification of intrinsic cardiac complexity from sympathetic and vagal dynamics may provide new physiology insights and improve on the non-specificity of heartbeat-derived biomarkers.

Emotions significantly shape decision-making, and targeted emotional elicitations represent an important factor in neuromarketing, where they impact advertising effectiveness by capturing potential customers' attention intricately associated with emotional triggers. Analyzing biometric parameters after stimulus exposure may help in understanding emotional states. This study investigates autonomic and central nervous system responses to emotional stimuli, including images, auditory cues, and their combination while recording physiological signals, namely the electrocardiogram, blood volume pulse, galvanic skin response, pupillometry, respiration, and the electroencephalogram. The primary goal of the proposed analysis is to compare emotional stimulation methods and to identify the most effective approach for distinct physiological patterns. A novel feature selection technique is applied to further optimize the separation of four emotional states. Basic machine learning approaches are used in order to discern emotions as elicited by different kinds of stimulation. Electroencephalographic signals, Galvanic skin response and cardio-respiratory coupling-derived features provided the most significant features in distinguishing the four emotional states. Further findings highlight how auditory stimuli play a crucial role in creating distinct physiological patterns that enhance classification within a four-class problem. When combining all three types of stimulation, a validation accuracy of 49% was achieved. The sound-only and the image-only phases resulted in 52% and 44% accuracy respectively, whereas the combined stimulation of images and sounds led to 51% accuracy. Isolated visual stimuli yield less distinct patterns, necessitating more signals for relatively inferior performance compared to other types of stimuli. This surprising significance arises from limited auditory exploration in emotional recognition literature, particularly contrasted with the pleathora of studies performed using visual stimulation. In marketing, auditory components might hold a more relevant potential to significantly influence consumer choices.

The advent of vaccines against SARS-CoV-2 has drastically reduced the level of hospitalization with severe COVID-19 disease in infected individuals. However, the diffusion of variants of concern still challenge the protection conferred by vaccines raised against the wild-type form of the virus. Here, we have characterized the antibody response to the BNT162b2 (Comirnaty) mRNA vaccine in patients infected with the Omicron variant. We analyzed a population of 4354 vaccinated healthcare workers (HCW) from 7 different hospitals in Italy and monitored infection with SARS-CoV-2 Omicron. We correlated infection with the antibody response after vaccination. We found that a lower level of IgG, younger age, and the presence of allergies correlate with increased infection during the Omicron wave, and that infections correlate with wild-type spike protein antibody titers below 350 BAU/mL. These results support the necessity of a fourth booster dose, particularly for individuals with lower levels of antibodies.

Background: The objective of this pilot study was to identify frequency-dependent effects of respiratory-gated auricular vagus afferent nerve stimulation (RAVANS) on the regulation of blood pressure and heart rate variability in hypertensive subjects and examine potential differential effects by sex/gender or race. Methods: Twenty hypertensive subjects (54.55 ± 6.23 years of age; 12 females and 8 males) were included in a within-person experimental design and underwent five stimulation sessions where they received RAVANS at different frequencies (i.e., 2 Hz, 10 Hz, 25 Hz, 100 Hz, or sham stimulation) in a randomized order. EKG and continuous blood pressure signals were collected during a 10-min baseline, 30-min stimulation, and 10-min post-stimulation periods. Generalized estimating equations (GEE) adjusted for baseline measures were used to evaluate frequency-dependent effects of RAVANS on heart rate, high frequency power, and blood pressure measures, including analyses stratified by sex and race. Results: Administration of RAVANS at 100 Hz had significant overall effects on the reduction of heart rate (ß = -2.03, p = 0.002). It was also associated with a significant reduction of diastolic (ß = -1.90, p = 0.01) and mean arterial blood pressure (ß = -2.23, p = 0.002) in Black hypertensive participants and heart rate in female subjects (ß = -2.83, p = 0.01) during the post-stimulation period when compared to sham. Conclusion: Respiratory-gated auricular vagus afferent nerve stimulation exhibits frequency-dependent rapid effects on the modulation of heart rate and blood pressure in hypertensive patients that may further differ by race and sex. Our findings highlight the need for the development of optimized stimulation protocols that achieve the greatest effects on the modulation of physiological and clinical outcomes in this population.

Sensory decision-making is frequently studied using categorical tasks, even though the feature space of most stimuli is continuous. Recently, it has become more common to measure feature perception in a gradual fashion, say when studying motion perception across the full space of directions. However, continuous reports can be contaminated by perceptual or motor biases. Here, we examined such biases on perceptual reports by comparing two response methods. With the first method, participants reported motion direction in a motor reference frame by moving a trackball. With the second method, participants used a perceptual frame of reference with a perceptual comparison stimulus. We tested biases using three different versions of random dot kinematograms. We found strong and systematic biases in responses when reporting the direction in a motor frame of reference. For the perceptual frame of reference, these systematic biases were not evident. Independent of the response method, we also detected a systematic misperception where subjects sometimes confuse the physical stimulus direction with its opposite direction. This was confirmed using a von Mises mixture model that estimated the contribution of veridical perception, misperception, and guessing. Importantly, the more sensitive perceptual reporting method revealed that, with increasing levels of sensory evidence, perceptual performance increases not only in the form of higher detection probability, but under certain conditions also in the form of increased precision.

Objective. Electrodermal activity (EDA) reflects sympathetic nervous system activity through sweating-related changes in skin conductance and could be used in clinical settings in which patients cannot self-report pain, such as during surgery or when in a coma. To enable EDA data to be used robustly in clinical settings, we need to develop artifact detection and removal frameworks that can handle the types of interference experienced in clinical settings while salvaging as much useful information as possible.Approach. In this study, we collected EDA data from 70 subjects while they were undergoing surgery in the operating room. We then built a fully automated artifact removal framework to remove the heavy artifacts that resulted from the use of surgical electrocautery during the surgery and compared it to two existing state-of-the-art methods for artifact removal from EDA data. This automated framework consisted of first utilizing three unsupervised machine learning methods for anomaly detection, and then customizing the threshold to separate artifact for each data instance by taking advantage of the statistical properties of the artifact in that data instance. We also created simulated surgical data by introducing artifacts into cleaned surgical data and measured the performance of all three methods in removing it.Main results. Our method achieved the highest overall accuracy and precision and lowest overall error on simulated data. One of the other methods prioritized high sensitivity while sacrificing specificity and precision, while the other had low sensitivity, high error, and left behind several artifacts. These results were qualitatively similar between the simulated data instances and operating room data instances.Significance. Our framework allows for robust removal of heavy artifact from EDA data in clinical settings such as surgery, which is the first step to enable clinical integration of EDA as part of standard monitoring.
