RESUMEN

The agreement of experts of the Eurasian Association of Therapists (EAT) discusses pathogenesis and treatment of COVID-19. Modern data on the characteristics of cardiovascular, kidney, respiratory damage in SARS-infected CoV-2 are presented. The tactics of managing patients initially having cardiovascular diseases, diabetes mellitus, chronic obstructive pulmonary disease, bronchial asthma, chronic kidney disease are discussed in detail. The article presents data on drug interaction of drugs.

RESUMEN

The authors present their experience in treating 142 patients with severe viral respiratory infection caused by influenza A (H1N1), describe its clinical picture, and identify major syndromes observed in the treatment of these patients at an intensive care unit. A rapid development of acute respiratory distress syndrome, significant hypoxemia and hypercapnia with the low efficiency of various therapeutic measures and hence progressive organ dysfunction determine the essence of the severe course of the disease. Uniform guidelines for intensive care in this patient population are presented.

RESUMEN

The quality of the diagnosis and treatment of lung diseases was assessed in the polyclinics of the Northern Administrative District of Moscow by 2 methods: (1) selective examination of 960 case histories of bronchial asthma and chronic obstructive pulmonary diseases and (2) questionnaire survey and spirometry of 2132 patients aged 35 to 75 years who have visited a polyclinic for no respiratory diseases. Random check of the case histories revealed a large number of diagnostic and medical errors suggesting the undertraining of therapists in pulmonology. The performed cycles of educational programs could considerably reduce the number of the errors found on recheck. Questionnaire survey and spirometry could increase the number of diagnosed cases of chronic lung diseases by 10 times.

RESUMEN

The efficacy of ciprofloxacin was studied in the treatment of 22 patients with hospital infection of the lower respiratory tract: 10 patients with nosocomial pneumonia, 8 with exacerbation of chronic obstructive bronchitis, 3 with exacerbation of mucoviscidosis and 1 with bronchiectasis. The drug was administered intravenously in a dose of 200 to 400 mg or orally in a dose of 500 to 750 mg twice a day for 6 to 21 days. The clinical and bacteriological efficacies of ciprofloxacin amounted to 90.9 and 86.4 per cent respectively. The treatment failed in 2 patients: 1 with pneumonia due to Streptococcus pneumoniae and 1 with mucoviscidosis and persistence of Pseudomonas aeruginosa. Ciprofloxacin was well tolerated. The side effects were observed in 2 patients and required no discontinuation of the drug use.

RESUMEN

In spite of great progress in antiasthmatic therapy, bronchial asthma affects more and more people. More and more patients die of this disease. The authors hold that one of the reasons of incomplete response to BA treatment is unsatisfactory compliance of the patients. To understand their disease and doctor's efforts better, the patients need detailed knowledge of their disease. Now, a great choice of educational programs for asthmatics are available. These vary in the scope of information, form (lecture, seminar, video film, guide, etc.), are intended for groups and individual study. The assessment of the efficacy of such programs show that they are able to reduce the number of hospitalizations, critical episodes, exacerbations. Educational programs are now actively lectured to asthmatics in the Research Institute of Pulmonology and Pediatric Medical Institute in Moscow.

The authors review the principles of the organization of the management of pulmonologic patients at the ambulatory stage. Describe the structure of the pulmonologic room, its functions and personnel. Based on the experience gained in the work of the pulmonologic rooms within the system of the All-Union Institute of Pulmonology, USSR Ministry of Health, give an account of the efficient organization of the work of the physician, the scope of diagnostic and treatment measures performed at the room. Describe the principles of the follow-up of patients afflicted with respiratory diseases. Give a critical review of the current standardized documents on pulmonology. Provide a schedule for the patients' diagnosis and treatment.
