RESUMEN

Ventilator-associated pneumonia (VAP) is the most frequent nosocomial infection in critically ill-ventilated patients. Oropharyngeal and lung microbiota have been demonstrated to be associated with VAP occurrence, but the involvement of gut microbiota has not been investigated so far. Therefore, the aim of this study is to compare the composition of the gut microbiota between patients who subsequently develop VAP and those who do not. A rectal swab was performed at admission of every consecutive patient into the intensive care unit (ICU) from October 2019 to March 2020. After DNA extraction, V3-V4 and internal transcribed spacer 2 regions deep-sequencing was performed on MiSeq sequencer (Illumina) and data were analyzed using Divisive Amplicon Denoising Algorithm 2 (DADA2) pipeline. Among 255 patients screened, 42 (16%) patients with invasive mechanical ventilation for more than 48 h were included, 18 (43%) with definite VAP and 24 without (57%). Patients who later developed VAP had similar gut bacteriobiota and mycobiota α-diversities compared to those who did not develop VAP. However, gut mycobiota was dissimilar (ß-diversity) between these two groups. The presence of Megasphaera massiliensis was associated with the absence of VAP occurrence, whereas the presence of the fungal genus Alternaria sp. was associated with the occurrence of VAP. The composition of the gut microbiota, but not α-diversity, differs between critically ill patients who subsequently develop VAP and those who do not. This study encourages large multicenter cohort studies investigating the role of gut-lung axis and oropharyngeal colonization in the development of VAP in ICU patients. IMPORTANCE The composition of the gut microbiota, but not α-diversity, differs between critically ill patients who subsequently develop ventilator-associated pneumonia (VAP) and those who do not. Investigating gut microbiota composition could help to tailor probiotics to provide protection against VAP.

BACKGROUND: Hyperoxia is associated with increased morbidity and mortality in the intensive care unit. Classical noninvasive measurements of oxygen saturation with pulse oximeters are unable to detect hyperoxia. The Oxygen Reserve Index (ORI) is a continuous noninvasive parameter provided by a multi-wave pulse oximeter that can detect hyperoxia. Primary objective was to evaluate the diagnostic accuracy of the ORI for detecting arterial oxygen tension (PaO2) > 100 mmHg in neurocritical care patients. Secondary objectives were to test the ability of ORI to detect PaO2 > 120 mmHg and the ability of pulse oximetry (SpO2) to detect PaO2 > 100 mmHg and PaO2 > 120 mmHg. METHODS: In this single-center study, we collected ORI and arterial blood samples every 6 h for 3 consecutive days. Diagnostic performance was estimated using the area under the receiver operating characteristic curve (AUROC). RESULTS: There were 696 simultaneous measurements of ORI and PaO2 in 62 patients. Considering the repeated measurements, the correlation between ORI and PaO2 was r = 0.13. The area under the receiver operating characteristic curve (AUROC), obtained to test the ability of ORI to detect PaO2 > 100 mmHg, was 0.567 (95% confidence interval = 0.566-0.569) with a sensitivity of 0.233 (95%CI = 0.230-0.235) and a specificity of 0.909 (95%CI = 0.907-0.910). The AUROC value obtained to test the ability of SpO2 to detect a PaO2 > 100 mmHg was 0.771 (95%CI = 0.770-0.773) with a sensitivity of 0.715 (95%CI = 0.712-0.718) and a specificity of 0.700 (95%CI = 0.697-0.703). The diagnostic performance of ORI and SpO2 for detecting PaO2 > 120 mmHg was AUROC = 0.584 (95%CI = 0.582-0.586) and 0.764 (95%CI = 0.762-0.766), respectively. The AUROC obtained for SpO2 was significantly higher than that for ORI (p < 0.01). Diagnostic performance was not affected by sedation, norepinephrine infusion, arterial partial pressure of carbon dioxide, hemoglobin level and perfusion index. CONCLUSION: In a specific population of brain-injured patients hospitalized in a neurointensive care unit, our results suggest that the ability of ORI to diagnose hyperoxia is relatively low and that SpO2 provides better detection.

A highly sensitive refractive index sensor is proposed by using the resonant coupling of a Tamm state to a Fabry-Perot resonance in slits periodically pierced in a metal film. This new hybrid resonance exists at the interface between a dielectric Bragg mirror and a subwavelength metal grating. Contrary to a classic Tamm plasmon, it is sensitive to the ambient media refractive index due to its confinement inside the grating slits. The proposed sensor output can be either the reflected intensity or preferably the wavelength, and its sensitivity and figure of merit are numerically investigated with the help of rigorous coupled wave analysis. The sensitivity of the studied sensor is 87 nm/RIU for a refractive index range from 1.25 to 1.38, and, at the expense of the resolution, it can reach up to 160 nm/RIU for a grating duty cycle of 50%. The figure of merit is around ${{7.5}}\;{{\rm{RIU}}^{- 1}}$ with a large measuring range. The index sensor performances and operating resonance wavelength can be modified by adjusting the grating geometric parameters (height and duty cycle). The possibility to achieve wavelength modulation in any spectral range makes the proposed grating Tamm structure an attractive candidate to design refractive index sensors and photonic components in the infrared and terahertz domains.

BACKGROUND: From a physiological viewpoint, changes in end-tidal carbon dioxide (EtCO2) could be a simple, noninvasive, and inexpensive way to monitor changes in cardiac index. This study aimed to assess the utility of changes in EtCO2 as a marker of fluid responsiveness after volume expansion in the operating room. METHODS: A prospective observational study was conducted in a tertiary university teaching hospital, from August 2018 to February 2019. A total of 109 non-consecutive, mechanically ventilated adults undergoing neurosurgery in the supine position with cardiac output monitors were included. Patients with major respiratory disease, arrhythmia, or heart failure were excluded. Volume expansion with 250 ml of saline 0.9% was performed over 10 min to maximise cardiac output during surgery, according to current guidelines. A positive fluid challenge was defined as an increase in stroke volume index of more than 10% from baseline. Changes in stroke volume index (monitored using pulse contour analysis) and EtCO2 were recorded before and after infusion. RESULTS: A total of 242 fluid challenges in 114 patients were performed, of which 26.9% were positive. Changes in EtCO2 > 1.1% induced by infusions had utility for identifying fluid responsiveness, with a sensitivity of 62.9% (95% confidence interval [CI], 62.5-63.3%) and a specificity of 77.8% (95% CI, 77.6-78.1%). The area under the receiver operating characteristic curve for changes in EtCO2 after volume expansion was 0.683 (95% CI, 0.680-0.686). CONCLUSIONS: Changes in EtCO2 induced by rapid infusion of 250 ml saline 0.9% lacked accuracy for identifying fluid responsiveness in mechanically ventilated patients in the operating room. BACKGROUND: Dynamic arterial elastance (Eadyn), defined as the ratio between pulse pressure variations and stroke volume variations, has been proposed to assess functional arterial load. We evaluated the evolution of Eadyn during volume expansion and the effects of neosynephrine infusion in hypotensive and preload-responsive patients. METHODS: In this prospective bicentre study, we included 56 mechanically ventilated patients in the operating room. Each patient had volume expansion and neosynephrine infusion. Stroke volume and stroke volume variations were obtained using esophageal Doppler, and pulse pressure variations were measured through the arterial line. Pressure response to volume expansion was defined as an increase in mean arterial pressure (MAP) ≥ 10%. RESULTS: Twenty-one patients were pressure responders to volume expansion. Volume expansion induced a decrease in Eadyn (from 0.69 [0.58-0.85] to 0.59 [0.42-0.77]) related to a decrease in pulse pressure variations more pronounced than the decrease in stroke volume variations. Baseline and changes in Eadyn after volume expansion were related to age, history of arterial hypertension, net arterial compliance and effective arterial elastance. Eadyn value before volume expansion > 0.65 predicted a MAP increase ≥ 10% with a sensitivity of 76% (95% CI 53-92%) and a specificity of 60% (95% CI 42-76%). Neosynephrine infusion induced a decrease in Eadyn (from 0.67 [0.48-0.80] to 0.54 [0.37-0.68]) related to a decrease in pulse pressure variations more pronounced than the decrease in stroke volume variations. Baseline and changes in Eadyn after neosynephrine infusion were only related to heart rate. CONCLUSION: Eadyn is a potential sensitive marker of arterial tone changes following vasopressor infusion.

PURPOSE: To determine whether augmented renal clearance (ARC) impacts negatively on piperacillin-tazobactam unbound concentrations in critically ill patients receiving 16 g/2 g/day administered continuously. MATERIAL AND METHODS: Fifty nine critically ill patients without renal impairment underwent 24-h creatinine clearance (CrCL) measurement and therapeutic drug monitoring during the first three days of antimicrobial therapy by piperacillin-tazobactam. The main outcome was the rate of piperacillin underexposure, defined by at least one of three samples under 16 mg/L. Monte Carlo simulation was performed to predict the distribution of piperacillin concentrations for various CrCL and minimal inhibitory concentration (MIC) values. RESULTS: The rate of piperacillin underexposure was 19%, significantly higher in ARC patients (0 vs. 31%, p = .003). A threshold of CrCL ≥ 170 mL/min had a sensitivity and specificity of 1 (95%CI: 0.79-1) and 0.69 (95%CI: 0.61-0.76) to predict piperacillin underexposure. In ARC patients, a 20 g/2.5 g/24 h PTZ dosing regimen was associated with the highest probability to reach the 16 mg/L empirical target, without risk of excessive dosing. CONCLUSIONS: When targeting a theoretical MIC at the upper limit of the susceptibility range, the desirable target (100%fT>16) may not be achieved in patients with CrCL ≥ 170 mL/min receiving PTZ 16 g/2 g/day administered continuously.

RESUMEN

Bloch surface waves (BSWs) are recently developing alternative to surface plasmon polaritons (SPPs). Due to dramatically enhanced propagation distance and strong field confinement these surface states can be successfully used in on-chip all-optical integrated devices of increased complexity. In this work we propose a highly miniaturized grating based BSW coupler which is gathering launching and directional switching functionalities in a single element. This device allows to control with polarization the propagation direction of Bloch surface waves at subwavelength scale, thus impacting a large panel of domains such as optical circuitry, function design, quantum optics, etc.

A two-dimensional periodic sub-wavelength array of vertical dielectric cylinders on a glass substrate is studied numerically using three different electromagnetic approaches. It is shown that such structure can present a narrow-band spectral resonance characterized by large angular tolerances and 100% maximum in reflection. In particular, in a two-nanometer spectral bandwidth the reflectivity stays above 90% within angles of incidence exceeding 10 degrees for unpolarized light. Bloch modal analysis shows that these properties are due to the excitation of a hybrid mode that is created in the structure by a guided-like mode and a localized cavity mode. The first one is due to the collective effect of the array, while the second one comes from the mode(s) of a single step-index fiber.

OBJECTIVE: The aim of this study was to determine the frequency and characteristics of antisynthetase syndrome (ASS) revealed by polyarthritis. METHODS: First we conducted a retrospective single-centre study to assess the frequency of ASS patients who presented with polyarthritis without pulmonary and/or muscle symptoms. Secondly, we conducted a larger, multicentre study in order to describe the clinical characteristics of these patients. Exclusion criteria were the presence of RF, the presence of ACPA and overlap with another CTD. RESULTS: In the single-centre study, polyarthritis was the first manifestation in 12 of 45 ASS patients (27%). An additional 28 patients were collected for the multicentre study, resulting in a total population of 40 ASS patients who presented with polyarthritis. The mean delay from polyarthritis onset to ASS diagnosis was 27 months (s.d. 40). Pulmonary and muscle symptoms were uncommon at ASS diagnosis (40% and 32.5%, respectively) and were dramatically delayed [mean delay after polyarthritis onset of 41 months (s.d. 53) and 21 months (s.d. 14), respectively]. Mechanic's hands and cutaneous signs of DM occurred in 25% and 22.5%, respectively, with a mean delay of 10 months (s.d. 10) and 31 months (s.d. 21), respectively. When present (32%), RP was the earliest non-articular manifestation [mean delay 3 months (s.d. 23) after polyarthritis onset]. On HEp-2 cells, antinuclear and/or cytoplasmic fluorescence was found in 70% of cases, with specificity for various anti-aminoacyl tRNA synthetase (anti-ARS) antibodies. CONCLUSION: ASS may be revealed by polyarthritis. To decrease the delay in diagnosis of ASS, pulmonary and muscle symptoms and anti-ARS antibodies might usefully be searched for in seronegative polyarthritis patients, especially in those with RP.

We deduce by the monomode modal method the analytical expressions of the transmission and reflection Jones matrices of an infinitely conducting metallic screen periodically pierced by subwavelength holes. The study is restricted to normal incidence and to the case of neglected evanescent fields (far-field), which covers many common cases. When only one nondegenerate mode propagates in cavities, they take identical forms to those of a polarizer, with Fabry-Perot-like spectral resonant factors depending on the bigrating parameters. The isotropic or birefringent properties are then obtained when holes support two orthogonal polarization modes. This basic formalism is finally applied to design compact and efficient metallic half-wave plates.

We extend the differential theory to anisotropic cylindrical structures with an arbitrary cross section. Two cases have to be distinguished. When the anisotropic cylinders do not contain the origin, the scattering matrix of the device is calculated from the extended differential theory with the help of the scattering matrix propagation algorithm. The fields outside the cylinders are described by Fourier-Bessel expansions. When the origin is located in one cylinder, the fields inside the cylinder are expressed from a semi-analytical theory related to a homogeneous anisotropic medium. In this second case, the formalism of the scattering matrix propagation algorithm is not exactly the same and requires suitable change. The numerical results are in good agreement with the ones obtained for the diffraction by one circular cylinder. The theory is then applied on the diffraction by an elliptical cylinder.

RESUMEN

We propose a study of a generalized Airy-like formula of the transmittance through planar resonators. A complete and analytical analysis of total transmission conditions is given according to physical parameters. The homogeneous problem related to free oscillations of resonators, which leads to complex resonance frequencies and quality factors, is also resolved. After a quick validation on well-known parallel-plate dielectric layers behaving as Fabry-Perot resonators, our discussion is applied on subwavelength metallic lamellar gratings from an analytical modal theory assuming perfectly electric conductors.

The differential theory of diffraction by arbitrary cross-section cylindrical objects is developed for the most general case of an incident field with a wave vector outside the cross-section plane of the object. The fast Fourier factorization technique recently developed for studying gratings is generalized to anisotropic and/or inhomogeneous media described in cylindrical coordinates; thus the Maxwell equations are reduced to a first-order differential set well suited for numerical computation. The resolution of the boundary-value problem, including an adapted S-matrix propagation algorithm, is explained in detail for the case of an isotropic medium. Numerical applications show the capabilities of the method for resolving complex diffraction problems. The method and its numerical implementation are validated through comparisons with the well-established multipole method.

Using two different modal methods, the multipole method and the more recent fast Fourier factorization method, we exhibit and explain a core mode transition induced by avoided crossing between a core localized leaky mode and an high-index cylinder leaky mode in anti-resonant reflecting optical waveguide microstructured optical fibers (ARROW MOFs). Due to its wavelength selectivity and to the leaky nature of the involved modes, this transition doesn't seem to have already been described in detail and analyzed as done in this work in spite of several already published studies on core mode dispersion properties. The main properties of this transition are also described. We also revisite the already mentionned cut-off phenomena limiting the transmission band in ARROW MOFs in terms of mode coupling between the core mode and one or several high- index cylinder modes.

The diffraction of an electromagnetic wave by a cylindrical object with arbitrary cross section is studied by taking advantage of recent progress in grating theories. The fast Fourier factorization method previously developed in Cartesian coordinates is extended to cylindrical coordinates thanks to the periodicity of both the diffracting object and the incident wave with respect to the polar angle theta. Thus Maxwell equations in a truncated Fourier space are derived and separated in TE and TM polarization cases. The new set of equations for TM polarization is resolved numerically with the S-matrix propagation algorithm. Examples of elliptic cross sections and cross sections including couples of nonconcentric circles show fast convergence of the results, for both dielectric and metallic materials, as well as good agreement with previous published results. Thus the method is suitable for an extension to conical (out-of-plane) diffraction, which will allow studying mode propagation along microstructured fibers.
