OBJECTIVES: To demonstrate that deterioration in thyroid function tests can serve as an indicator of severity and prognosis in acute pancreatitis despite a healthy thyroid gland. METHODS: This study is a retrospective, single-center study. Patients diagnosed with acute pancreatitis between May 2020 and June 2021 were evaluated. Acute pancreatitis was diagnosed and classified according to the 2012 revised Atlanta criteria. Patients were categorized into Non-Thyroidal Illness Syndrome and euthyroid groups and compared in terms of biochemical parameters and scoring systems such as Ranson, Glasgow, Balthazar and BISAP scores. RESULTS: A total of 152 patients were included in the study. Eighty-three patients (54%) were euthyroid, with free triiodothyronine (T3), free thyroxine (T4), and Thyroid-stimulating hormone (TSH) levels within normal limits. Sixty-nine patients (46%) had Non-Thyroidal Illness Syndrome with low serum free T3 levels and low/normal TSH levels. As expected, free T3 was significantly lower in the Non-Thyroidal Illness Syndrome group than in the euthyroid group (1.5 ± 0.04 vs 2.6 ± 0.04, respectively, p < 0.0001). In the Non-Thyroidal Illness Syndrome group, Ranson score (3.35 ± 0.2 vs 2.11 ± 0.18 p < 0.0001), Glasgow (2.4 ± 0.2 vs 1.3 ± 0.1, p < 0.0001), Atlanta (p = 0.007), and Balthazar (2.1 ± 0.1 vs 1.4 ± 0.1, p = 0.001) scores were significantly higher than euthyroid group. CONCLUSION: Non-Thyroidal Illness Syndrome provides insight into the prognosis of acute pancreatitis. Free T3 values are a significant parameter that may indicate the prognosis of acute pancreatitis. We believe that free T3 could be incorporated into an ideal scoring system in a disease such as acute pancreatitis, where early determination of prognosis is known to significantly reduce mortality.

Alzheimer's, Parkinson's, and dementia are the leading neurodegenerative diseases that threaten the world with the aging population. Although the pathophysiology of each disease is unique, the steps to be taken to prevent diseases are similar. One of the changes that a person can make alone is to gain the habit of an antioxidant-rich diet. Phytochemicals known for their antioxidant properties have been reported to prevent neurodegenerative diseases in various studies. Phytochemicals with similar chemical structures are grouped. Accordingly, there are two main groups of phytochemicals, flavonoid and non-flavonoid. Various in vitro and in vivo studies on phytochemicals have proven neuroprotective effects by increasing cognitive function with their anti-inflammatory and antioxidant mechanisms. The purpose of this review is to summarize the in vitro and in vivo studies on phytochemicals with neuroprotective effects and to provide insight.

Multiple Gateways (GWs) provide network connectivity to Internet of Things (IoT) sensors in a Wide Area Network (WAN). The End Nodes (ENs) can connect to any GW by discovering and acquiring its periodic beacons. This provides GW diversity, improving coverage area. However, simultaneous periodic beacon transmissions among nearby GWs lead to interference and collisions. In this study, the impact of such intra-network interference is analyzed to determine the maximum number of GWs that can coexist. The paper presents a new collision model that considers the combined effects of the Medium Access Control (MAC) and Physical (PHY) layers. The model takes into account the partial overlap durations and relative power of all colliding events. It also illustrates the relationship between the collisions and the resulting packet loss rates. A performance evaluation is presented using a combination of analytical and simulation methods, with the former validating the simulation results. The system models are developed from experimental data obtained from field measurements. Numerical results are provided with Gaussian Frequency Shift Keying (GFSK) modulation. This paper provides guidance on selecting GFSK modulation parameters for low bit-rate and narrow-bandwidth IoT applications. The analysis and simulation results show that larger beacon intervals and frequency hopping help in reducing beacon loss rates, at the cost of larger beacon acquisition latency. On the flip side, the gateway discovery latency reduces with increasing GW density, thanks to an abundance of beacons.

Purpose: Retinopathy of prematurity (ROP) is a retinal vascular disease affecting premature infants that can culminate in blindness within days if not monitored and treated. A disease stage for scrutiny and administration of treatment within ROP is "plus disease" characterized by increased tortuosity and dilation of posterior retinal blood vessels. The monitoring of ROP occurs via routine imaging, typically using expensive instruments ($50 to $140 K) that are unavailable in low-resource settings at the point of care. Approach: As part of the smartphone-ROP program to enable referrals to expert physicians, fundus images are acquired using smartphone cameras and inexpensive lenses. We developed methods for artificial intelligence determination of plus disease, consisting of a preprocessing pipeline to enhance vessels and harmonize images followed by deep learning classification. A deep learning binary classifier (plus disease versus no plus disease) was developed using GoogLeNet. Results: Vessel contrast was enhanced by 90% after preprocessing as assessed by the contrast improvement index. In an image quality evaluation, preprocessed and original images were evaluated by pediatric ophthalmologists from the US and South America with years of experience diagnosing ROP and plus disease. All participating ophthalmologists agreed or strongly agreed that vessel visibility was improved with preprocessing. Using images from various smartphones, harmonized via preprocessing (e.g., vessel enhancement and size normalization) and augmented in physically reasonable ways (e.g., image rotation), we achieved an area under the ROC curve of 0.9754 for plus disease on a limited dataset. Conclusions: Promising results indicate the potential for developing algorithms and software to facilitate the usage of cell phone images for staging of plus disease.

ABSTRACT Objective This study aimed to measure the anxiety levels of mothers during the pandemic period and to investigate the correlation between anxiety levels and children's eating behaviors in Turkey. Methods Mothers with children aged 3-6 years (n=303) living in Turkey were recruited from different social media channels until February and March 2021 due to the pandemic. Mothers completed an online questionnaire that included the Child Feeding Questionnaire, Beck Anxiety Inventory, and Coronavirus Anxiety Scale. Pearson correlation analysis and Kruskal-Wallis tests were applied to determine the existing relationship between the scales. Results Although 21.2% of mothers had moderate and severe anxiety, only 0.3% of mothers had coronavirus anxiety. No significant correlation was found between any of the Child Feeding Questionnaire subscales and anxiety levels evaluated according to Beck Anxiety Inventory. Only a correlation was observed between the Beck Anxiety Inventory score and Coronavirus Anxiety Scale score (r=0.47, p<0.001). Conclusion Although coronavirus anxiety was found in a small group in our study, more comprehensive studies are needed to see the effect of the anxiety created by the pandemic on child nutrition. Considering that human beings will face various pandemics in the coming years, the negative effects of the pandemic on the individual should be determined and precautions should be taken. In this way, it can be ensured that future generations are healthy in all aspects.
RESUMO Objetivo Este estudo teve como objetivo medir os níveis de ansiedade das mães durante o período de pandemia e investigar a correlação entre os níveis de ansiedade e os comportamentos alimentares das crianças na Turquia. Métodos Mães com filhos de 3 a 6 anos (n=303) residentes na Turquia foram recrutadas em diferentes canais de mídia social até fevereiro e março de 2021 devido à pandemia. As mães preencheram um questionário online que incluía o Child Feeding Questionnaire, Beck Anxiety Inventory, e a Coronavirus Anxiety Scale. A análise de correlação de Pearson e os testes de Kruskal-Wallis foram aplicados para determinar a relação existente entre as escalas. Resultados Embora 21,2% das mães tivessem ansiedade moderada e grave, apenas 0,3% das mães tinham ansiedade por coronavírus. Não foi encontrada correlação significativa entre nenhum dos subfatores do Child Feeding Questionnaire e os níveis de ansiedade avaliados de acordo com o Beck Anxiety Inventory. Apenas uma corre Conclusão Embora a ansiedade por coronavírus tenha sido encontrada em um pequeno grupo em nosso estudo, são necessários estudos mais abrangentes para ver o efeito da ansiedade criada pela epidemia na nutrição infantil. This paper quantifies the coverage area of Low-Power Wide-Area Networks (LPWAN) for Packet Success Rates (PSR) above 85%, where acceptable Quality of Service (QoS) can be achieved. The network consists of battery-operated end-nodes (ENs) and multiple stationary gateways (GWs). We consider asynchronous communication that uses ALOHA-based random channel access. Each transmission from the ENs can be received by multiple GWs. Such spatial diversity results in favorable Signal-to-Noise ratios (SNR). The LoRa modulation is assumed and its specific features, such as IQ inversion, further contribute to decreasing the impact of interference. An increase in the GW density improves network performance, which allows support for a larger density of end-nodes as well as increasing the coverage area. Our simulation results show that a suburban area of up to 1.44 km2 can be covered with five GWs with up to fifty end-nodes with a PSR greater than 86%.

BACKGROUND: Although several renal biopsy registry reports have been published worldwide, there are no data on primary glomerular disease trends in Turkey. METHODS: Three thousand eight-hundred fifty-eight native kidney biopsy records were assessed in the Turkish Society of Nephrology Primary Glomerulopathy Working Group (TSN-GOLD) Registry. Secondary disease and transplant biopsies were not recorded in the registry. These records were divided into four periods, before 2009, 2009 to 2013, 2013-2017, and 2017-current. RESULTS: A total of 3858 patients (43.6% female, 6.8% elderly) were examined. Nephrotic syndrome was the most common biopsy indication in all periods (58.6%, 53%, 44.1%, 51.6%, respectively). In the whole cohort, IgA nephropathy (IgAN) (25.7%) was the most common PGN with male predominance (62.7%), and IgAN frequency steadily increased through the periods (× 2 = 198, p < 0.001). MGN was the most common nephropathy in the elderly (> 65 years), and there was no trend in this age group. An increasing trend was seen in the frequency of overweight patients (× 2 = 37, p < 0.0001). Although the biopsy rate performed with interventional radiology gradually increased, the mean glomeruli count in the samples did not change over the periods. CONCLUSIONS: In Turkey, IgAN is the most common primary glomerulonephritis, and the frequency of this is increasing.

PURPOSE: To report a case of recurrent chalazia and Herpes Simplex conjunctivitis in a patient with DOCK8 deficiency. CASE REPORT: We report the case of a 15-year-old male patient with genetically diagnosed DOCK8 deficiency who presented with a left lower eyelid mass and left eye conjunctivitis. The lesion did not resolve despite aggressive measures, including topical steroids, topical antibiotics, and multiple debulking procedures but eventually resolved after the initiation of intravenous acyclovir. The diagnosis of HSV infection was supported by pathology findings from the eyelid mass. CONCLUSION: Patients with DOCK8 deficiency can present with a variety of cutaneous viral infections. In our case, a DOCK8-deficient patient with chalazia and conjunctivitis resistant to antibiotics, steroids and debulking responded to intravenous acyclovir.

Drawing on 20 semi-structured interviews with women garment workers in a low-income neighbourhood of Istanbul, and observations in the ateliers where they worked, this article examines their work experiences in the gendered and sexualised work atmosphere of garment workshops. There are three interrelated levels upon which the gender-related issues emerge in women garment workers' stories. The first set of discourses portrays young female garment workers in highly sexualised terms, and the second concerns the use of kinship vocabulary and avoidance of impersonal work relationships. That is, women workers' experiences in capitalist production sites were trivialised and regulated through the sexualisation of their bodies and the deployment of kinship idioms while addressing their role at the workplace. The third level analyses women's submissive, subversive or contradictory responses to these gendered disciplinary techniques and representations, i.e. the construction of their subjectivities. These three levels point to two things: first, cultural presumptions about marriage, women's sexuality and reproductive cycles are materialised at the workplace. Second, gendered instantiations of these presumptions in a specific work environment are both informed by their familial roles (such as daughter, wife, mother, widowed) and inform their future reproductive preferences (whether they marry, have a child, get a divorce, etc.). This article shows how the ways in which women's difference is construed and acted upon in the garment industry are inseparable from women's reproductive decisions.

PURPOSE: To investigate the use of spectral domain optical coherence tomography (SD-OCT) findings in pediatric acute lymphoblastic leukemia (ALL) patients. METHODS: Children that were diagnosed with precursor B-cell ALL and classified as belonging to the medium-risk group for relapse were selected for this study. Individuals who were in continuous remission and on maintenance therapy were included in the study group. Cases that had central nervous system involvement were excluded. Age-matched, otherwise healthy children were selected for the control group. Each study participant underwent a comprehensive eye examination and SD-OCT evaluation. Thickness measurements were made within the retinal nerve fiber layer (RNFL), central macula, posterior polar, and peripapillary choroid. RESULTS: A total of 112 eyes of 56 children were included: 54 eyes in the study group and 58 in the control group. Compared to the control group, subfoveal and temporal choroidal thicknesses of the posterior pole were significantly thinner in the study group (P < 0.005). Similarly, peripapillary choroidal thicknesses were significantly thinner in most sectors of the study group (P < 0.005). There were no major differences between groups in terms of central macular thicknesses and overall RNFL thicknesses. CONCLUSIONS: Evidence of choroidal attenuation was found in this subgroup of pediatric ALL patients. Further studies are warranted to clarify the utility of SD-OCT in detecting subclinical ocular involvement and monitoring treatment response and risk of relapse in patients with pediatric leukemia.

PURPOSE: To compare the results of conventional 1064 nm neodymium-doped yttrium-aluminum garnet laser goniopuncture (Nd:YAG-GP) and selective 532 nm Nd:YAG laser (selective laser trabeculoplasty [SLT]) âgonioreconditioning (GR) on trabeculo-Descemet's membrane in eyes resistant to viscocanalostomy surgery. METHODS: Thirty-eight eyes of 35 patients who underwent laser procedure after successful viscocanalostomy surgery were included in the study. When postoperative intraocular pressure (IOP) was above the individual target, the eyes were scheduled for laser procedure. Nineteen eyes underwent 532 nm SLT-GR (Group 1), and the remaining 19 eyes underwent conventional 1064 nm Nd:YAG-GP (Group 2). IOPs before and after laser (1 week, 1 month, 3 months, 6 months, 1 year, and last visit), follow-up periods, number of glaucoma medications, and complications were recorded for both groups. RESULTS: Mean times from surgery to laser procedures were 17.3 ± 9.6 months in Group 1 and 13.0 ± 11.4 months in Group 2. Mean IOPs before laser procedures were 21.2 ± 1.7 mmHg in Group 1 and 22.8 ± 1.9 mmHg in Group 2 (P = 0.454). Postlaser IOP measurements of Group 1 were 12.1 ± 3.4 mmHg and 13.8 ± 1.7 mmHg in the 1 st week and last visit, respectively; in Group 2, these measurements were 13.6 ± 3.7 mmHg and 14.9 ± 4.8 mmHg, respectively. There were statistically significant differences (P < 0.001) in IOP reduction at all visits in both groups; the results of the two groups were similar (P > 0.05). Mean follow-up was 16.6 ± 6.4 months after SLT-GR and 18.9 ± 11.2 months after Nd:YAG-GP. CONCLUSIONS: While conventional Nd:YAG-GP and SLT-GR, a novel procedure, are both effective choices in eyes resistant to viscocanalostomy, there are fewer complications with SLT-GR. SLT-GR can be an alternative to conventional Nd:YAG-GP.

In June 2013, protests that erupted in Gezi Park in Istanbul, Turkey were met with state violence, mobilizing hundreds of native physicians to deliver emergency medical care. Drawing on ethnographic fieldwork in makeshift clinics during these protests, interviews with Gezi physicians and analyses of recent laws restricting emergency care provision, in this article I explore the criminalization of clinical practice through legal and coercive means of the government and the delegitimization of state violence through clinical and expert witnessing practices of physicians. As I show, material, legal, and discursive articulations of the idiom of medical neutrality revolve around the tension between medical praxis as neutrality and medical praxis as political participation. I offer a reconsideration of medical humanitarian and human rights regimes in terms of their consequences for inciting, documenting and restricting state violence.

State authorities invested in developing official expert discourses and practices to deny torture in post-1980 coup d'état Turkey. Documentation of torture was therefore crucial for the incipient human rights movement there in the 1980s. Human rights physicians used their expertise not only to treat torture victims but also to document torture and eventually found the Human Rights Foundation of Turkey (HRFT) in 1990. Drawing on an ethnographic and archival research at the HRFT, this article examines the genealogy of anti-torture struggles in Turkey and argues that locally mediated intimacies and/or hostilities between victims of state violence, human rights physicians, and official forensics reveal the limitations of certain universal humanitarian and human rights principles. It also shows that locally mediated long-term humanitarian encounters around the question of political violence challenge forensic denial of violence and remake the legitimate levels of state violence.

PURPOSE: To study the concentrations of amino acids and vascular endothelial growth factor (VEGF) in subretinal fluid (SRF) of cases with rhegmatogenous retinal detachment (RRD). The relevance of the results with postoperative anatomic and functional success in RRD was investigated. METHODS: Fifty-three patients were included in this prospective study. The study group consisted of 46 patients who had scleral buckling surgery with the diagnosis of RRD, and SRF was obtained during the surgery. The control specimens consisted of vitreous samples of seven patients who were diagnosed with pars plana vitrectomy without RRD. Study cases were divided into three groups, corresponding to the duration of retinal detachment. Clinical characteristics, including best corrected visual acuity (BCVA) and anatomic status at month 6, were recorded. Concentrations of 15 selected amino acids were quantified by using high performance liquid chromatography, and VEGF levels were measured with enzyme immunoassay. RESULTS: When compared with the control group, SRF concentrations of aspartate, citrulline, glutamate, and glycine increased significantly in the study group (p<0.05). Statistical analysis showed that concentrations of alanine, isoleucine, leucine, methionine, phenylalanine, threonine, tyrosine, and valine decreased (p<0.05). SRF levels of glutamine, taurine, and serine had no significant change. SRF VEGF levels were significantly higher than the vitreous samples of the controls (p<0.001). Time-dependent changes and interactions between VEGF and amino acids were observed. There was no correlation between the concentrations of amino acids or VEGF with the parameters of BCVA and anatomical success. CONCLUSIONS: Significant changes occur in concentrations of amino acids and VEGF in SRF of cases with RRD. Our results suggest that several mechanisms contribute to the pathophysiology.
