RESUMEN

Background: Treatment of comminuted patellar fractures accompanied by coronal split articular and inferior pole fragments is a challenge. To treat this difficult fracture, we perform articular fragment detachment and separate fixation for coronal split articular and inferior pole fragments. We aimed to evaluate the radiological and clinical outcomes of our technique in comminuted patellar fractures at least 1 year after surgery. Methods: Between January 2019 and June 2022, 15 patients diagnosed with comminuted patellar fractures accompanied by coronal split articular and inferior pole fragments based on preoperative computed tomography underwent surgery using the articular detachment technique. The key point of this technique was anatomical reduction and subchondral fixation of the coronal split articular fragment to the superior main fragment after complete detachment of the coronal split fragment from the inferior pole. The remaining inferior pole was fixed using a separate construct. Postoperative articular gap, articular step-off, and complications, including resorption, reduction loss, and avascular necrosis of fixed articular fragments, were evaluated as radiological outcomes. Range of motion and the Lysholm scores were used to evaluate clinical outcomes. Results: Among the 15 patients, the coronal split articular fragments were fixed using Kirschner wires in 13 patients and headless screws in 2 patients. The inferior poles were fixed using separate vertical wiring in 13 patients and tension-band wiring in 2 patients. A postoperative articular gap was noted in 7 patients, with an average articular gap of 1.0 mm (range, 0.7-1.6 mm). No articular step-off was observed. Bone union and normal range of motion were achieved in all patients. On the 1-year postoperative lateral radiograph, resorption of the articular fracture site was seen in 5 patients. There was no loss of reduction or avascular necrosis of the coronal split articular fragments. The average postoperative Lysholm score at 1 year was 89.3 ± 4.1 (range, 82-95). Conclusions: The technique would be a reliable and safe option for the surgical treatment of comminuted patellar fractures accompanied by coronal split articular and inferior pole fragments in terms of anatomical reduction and stable fixation of articular fragments without risk of avascular necrosis.

Background: Recent studies on lateral knee anatomy have reported the presence of a true ligament structure, the anterolateral ligament (ALL), in the anterolateral region of the knee joint. However, its biomechanical effects have not been fully elucidated. Purpose: To investigate, by using computer simulation, the association between the ALL and anterior cruciate ligament (ACL) under dynamic loading conditions. Study Design: Descriptive laboratory study; Level of evidence, 5. Methods: The authors combined medical imaging from 5 healthy participants with motion capture to create participant-specific knee models that simulated the entire 12 degrees of freedom of tibiofemoral (TF) and patellofemoral (PF) joint behaviors. These dynamic computational models were validated using electromyographic data, muscle activation data, and data from previous experimental studies. Forces exerted on the ALL with ACL deficiency and on the ACL with ALL deficiency, as well as TF and PF contact forces with deficiencies of the ACL, ALL, and the entire ligament structure, were evaluated under gait and squat loading. A single gait cycle and squat cycle were divided into 11 time points (periods 0.0-1.0). Simulated ligament forces and contact forces were compared using nonparametric repeated-measures Friedman tests. Results: Force exerted on the ALL significantly increased with ACL deficiency under both gait- and squat-loading conditions. With ACL deficiency, the mean force on the ALL increased by 129.7% under gait loading in the 0.4 period (P < .05) and increased by 189% under high flexion during the entire cycle of squat loading (P < .05). A similar trend of significantly increased force on the ACL was observed with ALL deficiency. Contact forces on the TF and PF joints with deficiencies of the ACL, ALL, and entire ligament structure showed a complicated pattern. However, contact force exerted on TF and PF joints with respect to deficiencies of ACL and ALL significantly increased under both gait- and squat-loading conditions. Conclusion: The results of this computer simulation study indicate that the ACL and the ALL of the lateral knee joint act as secondary stabilizers to each other under dynamic load conditions.

AIM: This study aimed to introduce the circle drawing method for the evaluation of marginal impaction associated with posterior malleolar fractures (PMFs) on computed tomography (CT) images and evaluate the marginal impaction based on PMF patterns (Bartonícek classification) in rotational ankle fractures. METHODS: From January 2014 to December 2019, A total of 299 patients were reviewed retrospectively. The circle drawing method consists of matching the articular surface of the intact tibial plafond with a best-fitted circle on the sagittal CT image. The mismatch gap between the circle and the articular surface indicates the presence of marginal impaction. To validate the circle drawing method, we assessed CT images of 60 ankles without fracture at the medial, central, and lateral parts using the new method. RESULTS: Based on the review of the preoperative CT scan, 153 out of 299 patients (51.2%) were identified as having PMFs. Among them, 57 patients (37.3%) had marginal impaction associated with PMFs. For small posterolateral type 2 fractures, 58% of patients (47/81) had marginal impaction, and its incidence was most frequent among all types (P<.01). For posteromedial extended type 3 fractures, 27% of patients (8/30) had marginal impaction. Extraincisural type 1 and large posterolateral triangular type 4 fractures were rarely associated with marginal impaction. Among the 180 zones of 60 ankles without fracture, 171 zones (95%) were perfectly fitted to a best-fitted circle. CONCLUSIONS: The marginal impaction associated with PMFs is often observed in rotational ankle fractures, particularly in small posterolateral and posteromedial extended fractures.

BACKGROUND: Postoperative neutral alignment may be an important factor for longevity of total knee arthroplasty (TKA). In knees with severe varus deformity, greater soft tissue release and bone resection were required to achieve neutral alignment. We investigated the relationship between the severity of preoperative varus deformity and longevity of neutral-aligned TKAs. METHODS: Of the 723 knees in patients who underwent primary TKA for varus-type osteoarthritis between November 1998 and June 2009, 496 knees aligned neutrally (the postoperative mechanical hip-knee-ankle [HKA] axis angle ranged between -3° and 3°) and followed up for at least 5 years were included in the study. The mean follow-up period was 9.28 years. Patients were divided into 4 groups based on their preoperative HKAs: mild (0° < HKA ≤ 5°, n = 79), moderate (5° < HKA ≤ 10°, n = 204), severe (10° < HKA ≤ 15°, n = 149), and very severe (HKA > 15°, n = 64) groups. Failure was defined as need for revisional TKA for mechanical reason. Survival was analyzed by Kaplan-Meier method and log-rank test. RESULTS: The overall failure rate was 2.02% (10 of 496 prostheses). The cumulative survival rates of neutral-aligned TKAs at 10 years were 97.4% (95% confidence interval [CI], 93.9%-100%), 99.0% (95% CI, 97.6%-100%), 97.8% (95% CI, 95.4%-100%), and 96.9% (95% CI, 92.6%-100%) in mild, moderate, severe, and very severe varus groups, respectively. There were no significant differences between group survival rates (P = .395). CONCLUSION: The severity of preoperative varus deformity did not affect survival rates of neutral-aligned TKAs over 10 years.

BACKGROUND: The notion that neutral alignment is mandatory to assure long-term durability after TKA has been based mostly on short-film studies. However, this is challenged by recent long-film studies. QUESTIONS/PURPOSES: We conducted this long-film study to know (1) whether the risk of aseptic revision for nontraumatic reasons was greater among knees with greater than 3° varus or valgus (defined as "outliers") than those that were aligned within 3° of neutral on long-standing mechanical axis (hip to knee) radiographs; and (2) what the failure mechanisms were and whether the malalignment was femoral or tibial in origin, or both, among those in the outlier group. METHODS: Between November 1998 and January 2009 we performed 1299 cemented, posterior cruciate ligament-substituting TKAs in 867 patients for primary osteoarthritis. We had inadequate long-standing radiographs to analyze postoperative alignment for 124 of those knees, and an additional 24 were excluded for prespecified reasons. Consequently, 1151 knees were enrolled in our study. Of these, 982 (85%) in 661 patients (620 women and 41 men) who had followup greater than 24 months were analyzed. The knees were divided according to whether the postoperative mechanical axis was neutral (0° ± 3°), varus (> 3°), or valgus (< -3°) alignment on long-standing radiographs. The survivorships free from aseptic revision for nontraumatic reasons were compared among groups. The mechanical femoral and the tibial component alignment (MFCA and MTCA, respectively) were investigated to know the origin of overall mechanical malalignment of the outlier knees. The mean duration of followup was 8 ± 4 years (range, 2-17 years). Thirty-five knees (4%) showed aseptic loosening at 7 ± 4 years (range, 0.1-14 years) and five (1%) showed polyethylene wear at 12 ± 1 years (range, 10-13 years). Tibial loosening (73%) was the most common reason for aseptic revision followed by femoral loosening (30%). Of this cohort, 687 (70%), 250 (25%), and 45 (5%) knees had overall mechanical neutral, varus, and valgus alignment, respectively. Factors associated with the risk of aseptic revision were identified by Cox regression. RESULTS: The varus outliers (but not the valgus outliers) failed more often than the neutral knees (10% [25 of 250] versus 2% [13 of 687]; odds ratio [OR], 5.8, 95% CI, 2.9-11.5; p < 0.001). Ten-year survivorship free from aseptic revision was lower among varus outliers than among knees with neutral alignment (87% [95% CI, 80%-93%] versus 98% [95% CI, 97%-99%]; p = 0.001). Femoral component varus malpositioning was the main origin of the varus outliers (MFCA = 4.2° ± 2.0°; MTCA = 0.9° ± 1.7°) and was a risk factor for aseptic revision compared with neutral femoral positioning (OR, 14.0; 95% CI, 1.9-105.6; p < 0.001). CONCLUSIONS: This long-film study corresponds to previous short-film studies for the notion that varus malalignment is associated with inferior long-term implant survivorship. Although aseptic loosening occurred most commonly on the tibial side, the primary origin of the overall varus malalignment was femoral component varus malpositioning. Aiming for neutral alignment in TKA still seems to be a reasonable strategy in clinical practice. LEVEL OF EVIDENCE: Level III, therapeutic study.
