RESUMEN

AIM: Based on data from the Russian REGION-IM registry, to study the features of reperfusion therapy in patients with ST-segment elevation myocardial infarction (STEMI) in real-life clinical practice. MATERIAL AND METHODS: REGION-IM is a multicenter prospective observational study. The observational period is divided into 3 stages: during the stay in the hospital and at 6 and 12 months after inclusion in the registry. The patient's records contain demographic and history data; information about the present case of MI, including the time of the first symptom onset, first contact with medical personnel, and admission to the hospital; coronary angiography (CAG) data, percutaneous coronary intervention (PCI) data, and information about the thrombolytic therapy (TLT). RESULTS: Reperfusion therapy was performed in 88.9 % of patients with STEMI. Primary PCI (pPCI) was performed in 60.6 % of patients. The median time from the onset of symptoms to pPCI was 315 minutes [195; 720]. The median time from ECG to pPCI was 110 minutes [84;150]. Isolated TLT was performed in 7.4 %, pharmaco-invasive treatment tactics were used only in 20.9 % of cases. The median time from ECG to TLT (prehospital and in-hospital) was 30 minutes [10; 59], whereas the median time from ECG to prehospital TLT was 18 minutes [10; 39], and in 63 % of patients, TLT was performed more than 10 minutes after diagnosis. PCI followed TLT in 73 % of patients. CONCLUSION: The frequency of reperfusion therapy for STEMI in the Russian Federation has increased considerably in recent years. The high frequency of pPCI is noteworthy, but the timing of pPCI does not always comply with clinical guidelines. The results of this registry confirm the high demand for pharmaco-invasive strategies in real-life clinical practice. Taking into account geographical and logistical features, implementing timely myocardial reperfusion requires prehospital TLT. However, the TLT frequency in the Russian Federation is still insufficient despite its proven maximum effectiveness in the shortest possible time from the detection of acute MI.

THE AIM OF THE STUDY: Was to assess bacterial load in oral wounds in children with recessive dystrophic epidermolysis bullosa (RDEB). MATERIALS AND METHODS: The study comprised 77 RDEB children aged 3-18 years (mean age 9.5±3.6 years) and 27 healthy children aged 4-18 years (mean age 9.8±4.1 years) who served as controls. Swabs for bacteriological study were taken from the oral wounds in RDEB patients and non-affected corresponding oral mucosa areas in controls. The microorganism growth was assessed after 24, 48 and 72 hours of incubation (37 °C and 30 °C) with subsequent identification in automatic bacteriological analyzer MicroScan Walk Away (Simens, USA). Results. The study revealed high prevalence and concentrations of Candida albicans (in 40.3% children), Staphilococcus aureus (23.4%), Enterobacter cloacae (9.1%), and Enterobacteria (10.4%) in RDEB children. From these species, only Candida albicans was present in controls (26%). The prevalence and concentration of commensal and pathogenic species correlated positively with age and significant difference was revealed between children at the age of 3-6 and 7-10 years (p=0.001). Thus, bacterial load in oral wounds correlates with the RDEB natural history and fibrosis progression. Delayed oral wound healing was associated with microbiome shift towards biofilm-producing bacteria Staphilococcus aureus and Enterobacter cloacae. CONCLUSION: Oral wounds microbiome may be an indicator of RDEB severity and tendency to oral fibrosis. The decrease of bacterial load in the oral wounds may remove one of the healing obstacles and serve as fibrosis prevention measure.

Twenty-four potato (Solanum tuberosum L.) varieties differing in ripening groups (early, middle-early, and mid-season-ripening) were studied. Potatoes were grown under the conditions of the Middle Volga region of Russia in 2019-2021. It was found statistically that the yield (t/ha) of the early and mid-season-ripening varieties was negatively correlated with the increase in average temperatures during the growing season from May to August (R = -0.97, p = 0.04). Soil moisture content at a depth of 20 cm was positively correlated with the yield of middle-early varieties (R = 0.97, p = 0.04). The average tuber weight in the early varieties was sensitive to the increase in average temperatures (R = -0.95, p = 0.04). An increase in soil moisture content was beneficial to the average tuber weight (R = 0.98, p = 0.04), though only in the middle-early and mid-season-ripening groups. However, the soil moisture content and the tuber numbers in the mid-season-ripening varieties were negatively correlated (R = -0.96, p = 0.05).

The migration of excitation energy of a number of psoralen compounds has been studied. For this, the methods of induced absorption spectroscopy, stationary electron spectroscopy, fluorescence and phosphorescence, as well as quantum chemistry were used. A comparative photostability of psoralen was achieved by exposure to a XeCl excilamp irradiation (emission wavelength λem = 308 nm) with parameters Δλ = 5-10 nm, Wpeak = 18 mW/cm2, p = 8.1 J/cm3, f = 200 kHz, pulse duration 1 µs. It was found that the singlet-triplet transition played a major role in the migration of excitation energy into triplet states. Among all tested compounds, substances with an OCH3-group in the structure have the strongest effect on the spectral-luminescent characteristics.

RESUMEN

This paper explores the potential of recent research on metacognition to offer new avenues to assess and address the phenomenon of fragmentation in schizophrenia, which was described by E.Bleuler as «splitting¼. The concepts of metacognition characterize and quantify alterations or decrements in the processes by which fragments or pieces of information are integrated into a coherent sense of self and others. A method for assessing metacognition is presented along with research examining the presence and importance of metacognitive deficits in schizophrenia. Greater levels of metacognitive deficits have been detected in different phases of schizophrenia and linked to poorer psychosocial outcomes. These data were obtained both in foreign and preliminary Russian studies. The authors suggest that treatments, which successfully target metacognitive capacity, may uniquely promote wellness and recovery in schizophrenia.

THE AIM OF THE STUDY: Was to assess changes in the oral microbiota when using octenidine dihydrochloride mouthwash. MATERIALS AND METHODS: Quantitative and qualitative changes in the oral microbiota were studied and evaluated after a single rinse, course application and long-term dates. RESULTS: A single rinse resulted in a significant decrease in the number of pathogenic microorganisms: S. aureus, C. albicans, and Enterobacteriae. After a seven-day course of mouthwash application, the complete disappearance of pathogenic microorganisms: S. aureus, S. albicans, and Enterobacteriae and an increase in the number of Lactobacillus spp were revealed. 7 days after the mouthwash was canceled, there was some recovery of pathogenic (S. aureus, Candida), the number of Lactobacillus spp. remained the same, normal microflora almost completely restored. CONCLUSION: Octenidine dihydrochloride mouthwash may be recommended for wide use in dental practice in the treatment of various oral pathological conditions, including candidiasis and dysbiosis.

RESUMEN

The article the literature review on dental materials for the manufacture of gum formers used in dental implantology. Adhesion of microorganisms to titanium, PMMA and PEEK resins is discussed. According to published studies PEEK polymer is characterized by a similar bacterial contamination compared to titanium but shows lower degree of contamination when compared to acrylic resins in equal conditions.

RESUMEN

The paper presents a rare clinical case of an infant with KID (Keratitis, Ichthyosis, Deafness) syndrome (about 100 patients reported so far) admitted for histological verification of oral mucosa lesions. Disease pathogenesis defines inadequate reparation and skin and mucosa innate immunity defect leading to higher incidence of bacterial and fungal infections, so the 4-years old girl received treatment for vegetating candidiasis of the oral mucosa for several weeks with no clinical improvement. Initial examination showed that the oral lesions resulted from sharp edges of severely affected carious teeth. Histological study of multifocal biopsy revealed pyogenic granulomas and no signs of SCC. Teeth extraction and symptomatic treatment leaded to significant clinical improvement and some remained mucosal changes may be attributed to syndrome manifestations.

BACKGROUND: Older adults with anxiety and/or depression experience additional memory dysfunction beyond that of the normal aging process. However, few studies have examined test bias in memory assessments due to anxiety and/or depressive symptoms. The current study investigated the influence of self-reported symptoms of anxiety and depression on the measurement equivalence of memory tests in older adults. METHOD: This is a secondary analysis of the Advanced Cognitive Training for Independent and Vital Elderly dataset, a randomized controlled trial of community-dwelling older adults. Baseline data were included in this study (n = 2802). Multiple indicators multiple causes modeling was employed to assess for measurement equivalence, differential item functioning (DIF), in memory tests. RESULTS: The DIF was present for anxiety symptoms but not for depressive symptoms, such that higher anxiety placed older adults at a disadvantage on measures of memory performance. Analysis of DIF impact showed that compared with participants scoring in the bottom quartile of anxious symptoms, participants in the upper quartile exhibited memory performance scores that were 0.26 standard deviation lower. CONCLUSION: Anxious but not depressive symptoms introduce test bias into the measurement of memory in older adults. This indicates that memory models for research and clinical purposes should account for the direct relationship between anxiety symptoms and memory tests in addition to the true relationship between anxiety symptoms and memory construct. These findings support routine assessments of anxiety symptoms among older adults in settings in which cognitive testing is being conducted. Copyright © 2016 John Wiley & Sons, Ltd.

The 76 strains of Clostridium related to 14 species including 28 strains of C.difficile, were isolated from 74 samples of biological material taken out of oncologic patients. The species identification of strains was implemented by MALDI-TOF mass spectrometry using technique of direct application without extraction. The 74 strains (97.4%) were identified up to specie with reliability evaluation 2.0-2.5. Among C.non-difficile the most frequently were separated C.perfringens (42%), group of C.clostridioforme (24%). The sensitivity of separated strains to anti-microbial preparations was established with E-tests technique. The different species of Clostridia had different sensitivity to antibiotics. The strains C.perfringens are sensitive to all testing antibiotics whereas species related to group C.clostridioforme are resistant to fluoroquinolones (100%)б penicillin (67%). The resistance to fluoroquinolones was observed in C.scindens, C.ramosum, C.symbiosum. The strains C.scindens, C.tertium are resistant to penicillin. Among C.difficile, 93% of strains were resistant to penicillin, 57% - to imipenem and 100% - to fluoroquinolones.
