RESUMEN

To evaluate the shear bond strength (SBS) of self-adhesive resin cement when used with two different computer-aided design (CAD)-computer-aided manufacturing (CAM) materials after various surface treatments. Nanoceramic resin Lava Ultimate (LU) and feldspathic ceramic Vita Mark II (VM) CAD-CAM block samples were prepared with 1.5-mm thickness, and a total of 90 samples were obtained (N=90), with five samples of each block. The samples were divided into the following five groups according to the surface treatments (n=9): group 1, untreated (control); group 2,5% hydrofluoric acid etching; group 3, Er: YAG laser irradiation; group 4, tribochemical silica coating (Cojet); and group 5, air-abrasion with Al2O3. After silane application, resin cement was applied on a transparent matrix (diameter, 3mm; height, 2mm) on the blocks. SBS was determined using a universal testing device at a crosshead speed of 1mm/min. Two-way analysis of variance (ANOVA) and Tukey's post hoc tests were used to analyze the SBS values. LU showed the highest SBS value in group 4. The average SBS values in groups 3 and were found to be lower than that in the control group (p<0.05). When VM was examined, while all surface treatments increased the SBS values significantly, the highest SBS value was observed in group 4 (p<0.05). This study revealed that all surface treatments used negatively affected the bond strength values of self-adhesive resin cement to LU, except for Cojet application. The SBS values of resin cement with VM increased in all surface treatment application groups.


Evaluar la resistencia de unión al corte (SBS) del cemento de resina autoadhesivo cuando se utiliza con dos materiales diferentes de diseño asistido por computadora (CAD) y fabricación asistida por computadora (CAM) después de varios tratamientos superficiales. Se prepararon muestras de bloques CAD-CAM de resina Lava Ultimate (LU) y cerámica feldespática Vita Mark II (VM) con un espesor de 1,5mm, y se obtuvieron un total de 90 muestras (N=90), con cinco muestras de cada bloque. Las muestras se dividieron en los siguientes cinco grupos según los tratamientos superficiales (n=9): grupo 1, sin tratar (control); grupo 2, grabado con ácido fluorhídrico al 5%; grupo 3, irradiación con láser Er: YAG; grupo 4, recubrimiento triboquímico de sílice (Cojet); y grupo 5, aire-abrasión con Al2O3. Después de la aplicación de silano, se aplicó cemento de resina sobre una matriz transparente (diámetro, 3mm; altura, 2mm) sobre los bloques. La SBS se determinó usando un dispositivo de prueba universal a una velocidad de cruceta de 1mm/min. Se utilizaron análisis de varianza bidireccional (ANOVA) y pruebas post hoc de Tukey para analizar los valores de SBS. LU mostró el valor más alto de SBS en el grupo 4. Los valores promedio de SBS en los grupos 3 y fueron más bajos que en el grupo de control (p<0,05). Cuando se examinó VM, mientras que todos los tratamientos superficiales aumentaron significativamente los valores de SBS, el valor más alto de SBS se observó en el grupo 4 (p<0,05). PURPOSE: To assess the fracture resistance of single-tooth implant-supported crown restorations made with different CAD/CAM blocks. MATERIALS AND METHODS: Thirty-six titanium abutments were put on dental implant analogs (Mis Implant). For each of three test groups (n = 12/group), implant-supported, cement-retained mandibular molar single crowns were produced. Crowns were made of lithium disilicate glass (LD) IPS e.max CAD, feldspathic glass ceramic (FEL) Vita Mark II, and resin nano-ceramic (RNC) Lava Ultimate. The crowns were cemented with self-adhesive resin cement RelyX Unicem 2. After chewing cycling, crowns were tested to failure in a universal testing machine. Fracture values were calculated as initial (F-initial) and maximum fracture (F-max). RESULTS: The study groups were ranked, in order of having highest value, (LD > FEL) > RNC for F-initial load value and (LD > RNC) > FEL for F-max load value. This demonstrated that there was no parallel change in the F-initial and F-max values presenting the fracture resistance of specimens. CONCLUSIONS: There was no accordance between the F-initial and F-max values of the LD, RNC, and FEL after chewing simulation with thermocycling resembling 5 years of clinical functional use. LD had the highest fracture resistance during the fracture test. RNC had low fracture resistance; however, it had considerably high fracture resistance during the fracture test. FEL had considerably low fracture resistance values.

PURPOSE: The bond strength of soft denture liner to a recently introduced denture base resin after thermocycling has not been compared to traditional denture base materials. The objective of this study was to investigate the effects of thermocycling on the tensile bond strength of soft denture liners to two chemically different denture base resins, polymethyl methacrylate (PMMA) and urethane dimethacrylate (UDMA). MATERIALS AND METHODS: A total of 48 PMMA and UDMA tensile test specimens were fabricated by attaching two different soft denture liners (Molloplast-B, Permaflex) according to the manufacturers' instructions and assigned to two groups. Half of the specimens for each group were stored in water for 1 week, and the other half were thermocycled (5000 cycles) between baths of 5°C and 55°C. Specimens were mounted on a universal testing machine with a 5 mm/min crosshead speed. The data were analyzed with three-way ANOVA and post hoc Tukey-Kramer multiple comparisons tests (α = 0.05). RESULTS: The highest bond strength was measured in the specimens from the UDMA/Molloplast groups, and the lowest was seen in the PMMA/Permaflex group. No significant difference in bond strength was detected in PMMA/Permaflex groups after thermocycling (p = 0.082), whereas other groups exhibited significant differences after thermocycling (p < 0.05). CONCLUSIONS: Thermocycling decreased the bond strength values in both the PMMA and UDMA groups. Regardless of types of soft liners, PMMA specimens presented lower bond strength values than UDMA specimens, both before and after thermocycling.

INTRODUCTION: Pneumoconiosis in dental technicians' has rarely been evaluated. OBJECTIVES: To evaluate clinical, functional and radiological impacts of exposure to dust on respiratory functions via chest X-ray (CXR), high-resolution computed tomography (HRCT) and spirometry in dental technicians. METHODS: Demographic data, respiratory symptoms, spirometry results, CXR and HRCT scans of 32 dental technicians were evaluated. The opacities on the radiological images were categorized based on their intensity. We investigated the relation of radiological scores with clinical, radiological and functional findings. RESULTS: The mean age of the study population was 31 ± 9 years and mean employment duration was 14 ± 9 years. Twenty-two (69%) technicians had a history of smoking. The most common symptom was phlegm, while dyspnea prevalence was higher in those with an elevated International Labour Office (ILO) profusion score (P < 0.01). Parenchymal opacities were determined in 10 (31%) technicians by CXR and in 22 (69%) technicians by HRCT (P < 0.01). There was a positive correlation between ILO profusion score and HRCT score (r = 0.765, P < 0.01). ILO profusion score and HRCT score showed positive correlation with employment duration (r = 0.599, P = 0.01; r = 0.514, P = 0.01, respectively), while exhibiting negative correlation with FVC (r = -0.509, P < 0.05; r = -0.627, P = 0.01 respectively), FVC% (r = -0.449, P < 0.05; r = -0.457, P < 0.05, respectively) and forced expiratory volume in 1 s (r = -0.473, P < 0.05; r = -0.598, P = 0.01, respectively). CONCLUSIONS: We believe that a combined approach including spirometry, CXR and HRCT modalities should be employed in demonstrating respiratory disorders associated with exposure to inorganic dusts in dental technicians.

OBJECTIVE: The present study was to assess the effect of surface treatments of Er:YAG, Nd:YAG, and Ho:YAG lasers on the tensile bond strength of a silicone-based resilient liner to an acrylic denture in an in vitro setting. METHODS: Experimental dumbbell-shaped specimens (75 mm) were produced by combining two acrylate pieces fabricated from heat-polymerized acrylic resin (36 mm) with 3 mm of Molloplast(®)-B filling between them. The specimens (n=200) were randomly divided in half for thermocycling, and each 100 specimen set was randomized into five groups (n=20) with different surface treatments: control (no surface treatment), sandblasting, Er:YAG laser, Nd:YAG laser, and Ho:YAG laser. A tensile bond strength test was performed. The effect of the laser surface treatments was examined with scanning electron microscopy. RESULTS: Only the Er:YAG laser increased the tensile bond strength compared with the other treatments. The other laser groups showed lower bond strengths. The Ho:YAG laser resulted in considerably reduced tensile bond strength. The scanning electron microscopy images showed that applying laser surface treatments modified the surface of the denture base resin. CONCLUSIONS: There was not an overall improvement with the use of the studied laser modalities in the adhesion quality of resilient denture liner to acrylic resin, although Er:YAG laser showed a potential to improve their adhesion. These laser modalities need to be subjected to further studies to determine optimal setup for use in prosthodontics.

This study aims to compare the effect of low-level laser therapy (LLLT) and ozone therapy on the bone healing. Thirty-six adult male Wistar albino rats were used for this study. Monocortical defects were shaped in right femur of all rats. Defects were filled with nano-hydroxyapatite graft. The animals were divided into 3 groups and each group was than divided into 2 subgroups. Then, LLLT with a diode laser was applied to the first group (G1), ozone therapy was applied to the second group (G2), and no treatment was applied to the third group as a control group (G3). Animals were sacrificed after 4th and 8th weeks and the sections were examined to evaluate the density of the inflammation, the formation of connective tissue, the osteogenic potential, and osteocalcin activity. As a result, there were no significant differences among the groups of 4 weeks in terms of new bone formation. In the immunohistochemical assessment, the number of osteocalcin-positive cells was higher in the laser group compared to the other group of 4 weeks; this difference was statistically significant in the LLLT and ozone groups (P < 0.05). Histomorphometric assessment showed that the new bone areas were higher in the LLLT and ozone groups; furthermore, there was a statistically significant difference in the LLLT in comparison with the control group at 8th week (P < 0.05). At the same time immunohistochemical assessment showed that osteocalcin-positive cells were considerably higher in G2 than G1 at 8th week (P < 0.05). The findings of this study may be the result of differences in the number of treatment sessions. Further studies are therefore needed to determine the optimal treatment modality.

PURPOSE: The purpose of this study was to analyze the functional stresses around implants and supporting tissues placed in different combinations in the grafted and nongrafted atrophic posterior maxilla and to consider the acceptability of various fixed partial denture treatment options. MATERIALS AND METHODS: A computer model of the atrophic posterior maxilla was created from the computed tomography images of an actual patient. Three different treatment scenarios were modeled with partial denture restorations, grafted and nongrafted maxillary sinuses, and various implant inclinations. Oblique forces were applied to simulate chewing movements. Stress analyses were performed with a three-dimensional finite element analysis computer program, and the von Mises and minimum principal stresses on the implants and supporting tissues were compared. RESULTS: In all models, minimum principal (compressive) stress peak points were the highest within the crestal cortical bone (49.761 MPa), lower within sinus cortical (14.144 MPa) and trabecular bone (4.347 MPa), and lowest within grafted bone (0.049 MPa). The second molar implant in the third model (5×11-mm implant, inclined 45 degrees) showed the highest von Mises stresses (499.50 MPa), and the second molar implant in the first model (6×5-mm implant) showed the lowest (219.63 MPa) von Mises stresses. CONCLUSION: The stress absorption capacity of graft material is not sufficient and is much lower than that of other supporting tissues. For a fixed partial prosthesis, the use of short, wide implants with sinus floor bicortical fixation was found to be the most feasible approach for the atrophied posterior maxilla.

OBJECTIVES: The aim of this 7-year follow-up study was to determine respiratory changes in dental technicians. MATERIAL AND METHODS: In our region, in the year 2005, 36 dental technicians were evaluated with a cross-sectional study on respiratory occupational diseases, and in 2012 we evaluated them again. Inclusion of information on respiratory symptoms and demographic features questionnaires was applied. Pulmonary function tests (PFT) were performed. Chest X-rays (CXR) were evaluated according to the ILO-2000 classification. For the comparisons of the technicians' findings in 2005 and 2012, data analyses were performed with the Wilcoxon test in addition to descriptive statistical procedures. RESULTS: In 2012, 19 out of the 36 technicians continued to work in the same place, so we were able to evaluate their findings. The prevalence of respiratory symptoms in dental technician was as follows: dyspnea 7 (37%), cough 6 (32%), and phlegm 5 (26%). According to ILO classifications in 2005, among the 36 technicians, 5 (13.8%) had pneumoconiosis. At the end of 7 years, there were 9 pneumoconiosis cases among the 19 remaining technicians (47%). Thus, there was a statistically significant progression on the profusion of the radiologic findings (p < 0.005). Also there was a significant worsening on spirometric findings (p < 0.05). CONCLUSION: In dental technicians, a determination of both radiologic and functional progressions at the end of 7 years demonstrate that the primary and secondary preventive measures are necessary for these workplaces. Workplaces must be regularly controlled for worker health and hygiene.

AIM: The aim of this study was to investigate the effect of radiation doses very close to the human dose for oral cancers on mechanical, chemical and physical properties for poly methyl-methacrylate (PMMA). METHODS: PMMA samples were divided into four different groups: no irradiated group, 25-Gy irradiated group, 50-Gy irradiated group and 75-Gy irradiated group. Each group contained nine samples. After 24 h, a three-point loading test was applied to each PMMA groups. The transverse strength and the elastic modulus were calculated using the test results. The results were analyzed statistically by using one-way analysis of variance. The structural characterizations of the PMMA samples were carried out by a Fourier Transform Infrared (FTIR) spectrophotometer to evaluate the chemical structure differences. RESULTS: The transverse strength values of 25-Gy, 50-Gy and 75-Gy radiation groups were significantly higher than that of the no radiation group (p < 0.05). There was no significant difference among the elastic modulus values of the study groups (p > 0.05). The FTIR findings demonstrated that the irradiation process did not change the chemical structure of the PMMA polymeric materials. CONCLUSION: The therapeutic radiation doses increase the mechanical properties of the PMMA; however, the chemical and structural properties have no effect. When the findings of this study are taken into account, it can be said that patients can wear dentures during the radiotherapy.
