RESUMEN

The effect of hormonal contraception on cervix of the uterus of young nullipara women with ectopia was investigated. Cohort study was carried out by using simple blind method. By means of statistical data manipulation correlation between the application of hormonal contraceptives and changes of colposcopy pictures in the zone of pathology was displayed. Excluton showed low percent anomalous colpscopy picture in pathology zone. Excluton was declared as medicine of choice for nullipara women with ectopia. Still it is necessary to conduct a strong dynamic control of cervix of the uterus (colposcopy and pap-smear test) in order to avoid complications.

RESUMEN

We have studied the relationship between the hypoxic change and mitochondrial redox-potential disturbances in the mechanism of pheto-placental insufficiency in pregnancy with cytomegalovirus infection (CMV), detected by the positive anti-CMV-IgG titer and more then 4-fold increase of low avid anti-CMV-IgG. It was shown, that chronic CMV infection induces production of active forms of oxygen, peroxidation of structures and concurrently damage of mitochondria with essential decrease of ATP level. Results of the study have shown the important diagnostic value of estimation of hypoxic-oxidative damage induced by CMV infection. The results also revealed important relationships between the activity of the CMV infection and intensity of mitochondrial damage. On the basis of our investigations we suggest the additional diagnostic test (the determination of citozol NADH dependent isocitratdehydrogenaze activity) to evaluate the depth of CMV induced metabolic disturbances.

An early initiation of sexual life as the cause of problem concerning hormonal contraception with young women not given birth acquires a special actuality. It is known that no matter how carefully the operation of abortion is held, it is inevitably accompanied by the traumatization of cervix, especially in women with first pregnancy. According to the reference data the consequences of the first abortion in every third woman are the inflammatory diseases of reproductive organs, the infringement of menstruation, infertility, ectopic pregnancy, endometriosis, etc. It is known that the most effective method of avoiding the first pregnancy and in particular the first abortion is the hormonal contraception (HC). It is necessary to mention that there are numerous and diverse investigations dedicated to the study of condition of the cervix in the process of hormonal contraception. The detailed analysis of the literature in last 5-8 years show that the frequency of cervical cancer in women who use HC does not exceed the overall rate in the population. Besides the right choice of oral contraception (OC) for present contingent of women promotes the epithelization of the pseudo-erosions of the cervix.

RESUMEN

A new method of treatment of endometriosis includes combination of surgical and hormonal therapy. Surgical method included excision of tissue with endometriosis, while the hormonal therapy involved the combination of dufaston and reaferon-EC (recombinant interferon alpha 2b). The aim of this study was to develop effective method of treatment of infertility caused by endometriosis using combination of dufaston and reaferon. There were 114 patients under observation who suffered from endometriosis and infertility. At the first stage of treatment thermokouterisation of area with endometrosis has been used. At the second stage of treatment all patients were divided into different clinical groups. To 34 patients from the study group reaferom-EC has been administered on the 5th, 8th, 10th, 12th, 14th days of menstrual cycle. The control group was represented by 80 patients, which was divided into two subgroups, 40 patients in each group. The patients from the first subgroup got hormonal therapy 20 mg dufaston from the 5th to 25th days of menstrual cycle, while the patients from the second subgroup got 20 mg dufaston from the 15th-20th to 25th days. The treatment was successful among the patients who got the combination of reaferon and dufaston, the frequency of pregnancy in the study group reached 52% vs. 27.3% in the control group (p>0.05). Those patients who had so called "small form" of endometriosis became pregnant in 34% cases in the study group and in 22.7% cases in the control group. Patients who had endometriosal cystoma became pregnant in 18.7% in the study group, but only in 9.4% in the test group, which shows that the treatment is less effective among these categories of patients. As a result of our study it is proved that the method of combination of reaferon and dufaston gives a chance to increase therapeutic effect and the number of pregnancy among the patients with endometriosis.
