RESUMEN

Resumo Este artigo é uma narrativa pessoal de um drama familiar representado na sequência da morte do meu irmão, decorrente de um acidente de avião. Histórias "verdadeiras" como essa se encaixam no espaço entre ficção e ciências sociais, juntando escrita etnográfica e literária, e compreensão autobiográfica e sociológica. Meu objetivo é reposicionar os leitores em relação aos outros autores de textos de ciências sociais, reconhecendo o potencial para leituras opcionais e encorajando os leitores a "experienciarem uma experiência" que pode revelar não apenas como foi para mim, mas como poderia ser ou foi também para eles alguma vez. Esta forma experimental permite a pesquisadores e leitores reconhecerem e darem voz às suas próprias experiências emocionais e incentiva sujeitos etnográficos (coautores) a reivindicarem e escreverem suas próprias vidas.


Abstract: This article is a personal narrative of a family drama enacted in the aftermath of my brother's death in an airplane crash. "True" stories such as this fit in the space between fiction and social science, joining ethnographic and literary writing, and auto-biographical and sociological understanding. My goal is to reposition readers vis a vis authors of texts os social science by acknowledging potential for optional readings and encouraging readers to "experience and experience" that can reveal not only how it was for me, but how it could be or once was for them. This experimental form permits researchers and readers to acknowledge and give voice to their own emotional experiences and encourages ethnographic subjects (co-authors) to reclaim and white their own lives.


Resumen: Este artículo es la narración personal del drama familiar ocurrido tras la muerte de mi hermano en un accidente aéreo. Historias "verdaderas" como esta se encajan en el limite de la ficción y las ciencias sociales, uniendo escritura etnográfica y literaria, y la comprensión autobiográfica y sociológica. Mi objetivo es reposicionar a los lectores en relación con los demás autores de textos de ciencias sociales, reconociendo el potencial de la lectura opcional y animar a los lectores a "experimentar una experiencia" que pueda revelar no solo cómo fue para mí, sino también como podría haber sido para ellos. Esta forma experimental permite a los investigadores y lectores reconocer y expresar sus propias experiencias emocionales y anima a los sujetos etnográficos (coautores) a reclamar y escribir sus propias vidas.

RESUMEN

Calciphylaxis is a potentially fatal disease caused by metastatic calcification of the small- and medium-sized blood vessels of the dermis and subcutis. It most commonly is seen in patients with renal disease requiring dialysis, but it also may be triggered by nonuremic causes in patients with known risk factors for calciphylaxis. We report a case of nonuremic calciphylaxis (NUC) occurring in the setting of multiple risk factors, including chronic corticosteroid use, obesity, rapid weight loss, and hypotension. A review of the literature also is provided with an in-depth discussion of the known risk factors and triggers of NUC.

Angioimmunoblastic T-cell lymphoma (AITL) is an aggressive form of peripheral T-cell lymphoma that is characterized by lymphadenopathy, night sweats, fever, weight loss, and autoimmune phenomena. Cutaneous manifestations are present in up to 50% of cases, but few cases are reported in the dermatologic literature. We present a case of AITL that manifested in the skin. The patient was diagnosed with diffuse large B-cell non-Hodgkin lymphoma 3 months prior based on results from a lymph node biopsy. Reexamination and immunohistochemical staining of the previously biopsied lymph node specimen revealed the same clonal population of T cells positive for CD3, CD4, CD10, and programmed cell death protein 1 (PD-1) that was present in the skin and confirmed a diagnosis of AITL. Angioimmunoblastic T-cell lymphoma is frequently misdiagnosed due to its nonspecific clinical and histologic findings; it is not uncommon for AITL to be mistaken for other types of lymphoma. Dermatologists and dermatopathologists can play an important role in the recognition of this difficult-to-diagnose malignancy.

INTRODUCTION: Many jobs require working or exercising at low intensities for extended periods in hot-humid climates; however, in these conditions the isolated effects of relative humidity (RH) remain unclear. Therefore, the purpose of this study was to examine how RH influenced thermoregulation and perception during low-intensity exercise in the heat. METHODS: There were 13 healthy men (age = 23 +/- 2 yr, mass = 83.1 +/- 13.3 kg, height = 179.9 +/- 5.9 cm, Vo2max = 55.6 +/- 7.3 ml x kg(-1) x min(-1)) who walked 90 min at 35% Vo2max in 35 degrees C, completing trials at 40% RH (40RH), 55% RH (55RH), 70% RH (70RH), and 85% RH (85RH). Investigators obtained (1) rectal temperature (Tre), skin temperatures, heart rate, and perceptual measures every 5 min; (2) respiratory measures every 30 min; and (3) pre- and post-exercise nude body masses; these measures derived partitional calorimetry variables. RESULTS: Maximal evaporative capacity and heat loss incrementally decreased as RH increased; nonevaporative and respiratory heat loss negligibly altered heat balance. Progressively raising RH significantly increased heat storage, heat production, and Tre. Tre in 40RH and 55RH matched; 70RH exceeded 40RH and 55RH after 35 min; and 85RH exceeded all trials after 40 min. DISCUSSION: Nonevaporative and respiratory heat loss mechanisms failed to offset decreased sweat evaporation. Nonlinear increases in Tre appeared to catalyze responses in most other variables, which demonstrated similar but temporally delayed patterns. Under these circumstances, an RH threshold for increased thermal strain plausibly existed between 55-70RH; environmental characteristics indicated the threshold occurred not where heat stress became uncompensable, but instead where Tre surpassed the "balance point," triggering compensatory responses.

BACKGROUND: Notalgia paresthetica (NP) is a very common, under-recognized condition characterized by pruritus in a unilateral, dermatomal distribution in the mid-back. Chronic pruritus is sometimes accompanied by pain, paresthesias, or altered sensation to touch. OBJECTIVES: To review the current literature with regards to the cause of NP and its most appropriate treatment. METHODS: Comprehensive literature review using PubMed to inspect the available data on NP. RESULTS: The most likely cause of NP is cutaneous nerve damage. Many therapies have been tried in the treatment of NP, mostly in small case studies. CONCLUSIONS: The published cases and studies suggest symptoms of NP are due to a cutaneous sensory neuropathy. Treatments addressing the condition as such are more successful than traditional itch therapies. In many cases, a simple explanation for the persistent pruritus is satisfactory for patients. For very distressing cases, therapy should address the condition as a benign sensory neuropathy. Further studies are needed to evaluate which treatments have the greatest potential for providing symptom relief.

RESUMEN

The social consequences of appealing to age to excuse memory failure were examined in 2 vignette-based studies. In Study 1, 75 older (M = 72 years) and 78 young (M = 22 years) adults evaluated forgetful older targets in their 70s who used their age, lack of ability, lack of effort, or the situation to explain forgetting. In Study 2, 105 older (M = 72 years) and 105 young participants (M = 19 years) evaluated forgetful targets with no specific age given in 4 excuse conditions (age, ability, situation, and no excuse). In support of the prediction of positive consequences, age excuses were rated as more believable than situation in both studies and more believable and socially fluent than effort in Study 1. In support of predictions of negative consequences, both groups in Study 2 rated target persons who used an age excuse to be much older than their peers and, along with ability excuse users, as eliciting more worry and frustration than the others. Moreover, young adults showed additional sensitivity to the negative aspects of age excuses in terms of worry and frustration in Study 1 and anticipated repeat forgetting in Study 2. These results suggest that although age excuses may relieve socially awkward situations, this strategy reinforces negative age stereotyping of the older forgetter.
