RESUMEN

AIM: To search for simple clinical criteria for prediction of obstructive sleep apnoea syndrome (OSAS). MATERIAL AND METHODS: Case histories of 389 patients with uncomplicated snoring and OSAS (321 males and 68 females, mean age 55.2 +/- 10.9 years) were analysed retrospectively. The OSAS diagnosis was made according to polysomnographic evidence using somnological systems Minisimno (Sefam, France) and Compumedix S-series (Compumedix, Australia). Correlations between OSAS severity and some clinical parameters were studied. RESULTS: It was found that gain in body mass after onset of snoring significantly correlates with apnea/hypopnea index and with OSAS severity. If the body mass gain was > 10 kg, there is a 95% probability of the existence of moderate or severe OSAS. If the gain was > 20 kg, severe OSAS is highly probable. CONCLUSION: Body mass gain after onset of snoring can help in prediction of OSAS severity.

RESUMEN

AIM: To assess cerebral hemodynamics during sleep in patients with syndrome of obstructive sleep apnea. METHODS: Long term monitoring of rheoencephalogram. Data processing consisted in construction of trends (curves of changes of rheographical parameters during any selected time interval). RESULTS AND CONCLUSION: Cyclical periods of apnea are associated with negative changes of cerebral hemodynamics. At the end of period of apnea despite compensatory vasodilatation cerebral blood flow decreases. This occurs during pronounced hypoxemia and can predispose to ischemic disturbances. In ventilation phase rapid rise of heart rate and blood pressure at the background of continuing cerebral vasodilatation can increase risk of hemorrhagic complications.

RESUMEN

Tolerance, subjective and objective effectiveness of domestic intraoral applicator UPLH-01 were studied in the treatment of snoring and obstructive sleep apnea (OSA). In 44 patients with uncomplicated snoring the above parameters were assessed using questionnaire, in 20 patients with uncomplicated snoring and OSA paired polysomnographic investigations were made. Tolerance of the treatment was 50%. Subjective positive effect was registered in 65% of the patients. The applicator was found effective in the majority of patients with uncomplicated snoring. In mild and moderate OSA the applicator was effective only in a few cases. In severe OSA no effect was found. Patients with chronic disorders of nasal breathing exhibited deterioration of respiration and blood saturation with oxygen. Thus nasal obstruction is a contraindication to applicator use.

RESUMEN

AIM: To try validity of polysomnography in differential diagnosis of obstructive and central sleep apnea (SA). MATERIALS AND METHODS: A retrospective analysis was made of 138 cases of sleep apnea (SA) subjected to polysomnography. 79 of them were treated with continuous positive pressure. RESULTS: In clinical practice it is rather difficult to distinguish between central and obstructive SA. In this respect, polysomnography was also unreliable as it is predisposed to hyperdiagnosis of central SA. CONCLUSION: In SA diagnosis, polysomnography should be used in combination with other diagnostic methods. In indeterminate diagnosis, a test course of the positive pressure is recommended to exclude obstructive SA.

RESUMEN

A study of the impact of 10 sessions of hyperbaric oxygenation on the heart's pump function and contractility as evidenced echocardiographically in 46 patients with angina of effort, functional class 2-3, and 36 matched controls demonstrated that hyperbaric oxygenation can normalize hemodynamics in cases of hyper- and hypokinetic circulation, its effect being particularly good in patients with pre-existing left-ventricular dilatation.

RESUMEN

A study of the effect of hyperbaric oxygenation (HBO), applied as single sessions or a treatment course, on cardiac rhythm in 82 coronary patients with chronic extrasystole demonstrated positive therapeutic effect to be achieved after the first 5 or 6 HBO sessions in all patients. Further treatment may provoke an aggravation of arrhythmia in some patients. Positive antiarrhythmic effect of a complete HBO course is doubtful, where ectopic activity is increased following the 7th session. There is a correlation between the results of ECG monitoring from the hyperbaric chamber, from session 7 onwards, and 24-hour ECG recording before and after a HBO course. As shown by 24-hour ECG monitoring, positive effect was obtained in 67% of patients with ventricular extrasystole, the number of ventricular extrasystoles remained basically unchanged after a HBO course in 17%, and ectopic activity increased in 15.4%.
