BACKGROUND: Infective endocarditis (IE) increasingly involves older patients. Geriatric status may influence diagnostic and therapeutic decisions. AIM: To describe transoesophageal echocardiography (TEE) use in elderly IE patients, and its impact on therapeutic management and mortality. METHODS: A multicentre prospective observational study (ELDERL-IE) included 120 patients aged ≥75 years with definite or possible IE: mean age 83.1±5.0; range 75-101 years; 56 females (46.7%). Patients had an initial comprehensive geriatric assessment, and 3-month and 1-year follow-up. Comparisons were made between patients who did or did not undergo TEE. RESULTS: Transthoracic echocardiography revealed IE-related abnormalities in 85 patients (70.8%). Only 77 patients (64.2%) had TEE. Patients without TEE were older (85.4±6.0 vs. 81.9±3.9 years; P=0.0011), had more comorbidities (Cumulative Illness Rating Scale-Geriatric score 17.9±7.8 vs. 12.8±6.7; P=0.0005), more often had no history of valvular disease (60.5% vs. 37.7%; P=0.0363), had a trend toward a higher Staphylococcus aureus infection rate (34.9% vs. 22.1%; P=0.13) and less often an abscess (4.7% vs. 22.1%; P=0.0122). Regarding the comprehensive geriatric assessment, patients without TEE had poorer functional, nutritional and cognitive statuses. Surgery was performed in 19 (15.8%) patients, all with TEE, was theoretically indicated but not performed in 15 (19.5%) patients with and 6 (14.0%) without TEE, and was not indicated in 43 (55.8%) patients with and 37 (86.0%) without TEE (P=0.0006). Mortality was significantly higher in patients without TEE. CONCLUSIONS: Despite similar IE features, surgical indication was less frequently recognized in patients without TEE, who less often had surgery and had a poorer prognosis. Cardiac lesions might have been underdiagnosed in the absence of TEE, hampering optimal therapeutic management. Advice of geriatricians should help cardiologists to better use TEE in elderly patients with suspected IE.

BACKGROUND: European guidelines order management of patients with ST-segment elevation myocardial infarction (STEMI) less than 12 hours. They encourage healthcare networks to rapid coronary reperfusion strategy depending on the distance of the patient from the PCI center. OBJECTIVE: To determine impact of out-of-hospital presentation of patients with STEMI in remote areas within the framework of a care network, and to define the degree of compliance with local recommendations, and its prognostic impact. METHOD: Over three years (2017-2019), from the RESURCOR prospective register, 310 patients with STEMI less than 12 hours were out-of hospital managed and transferred to Metrolpole Savoie Hospital in Chambéry. Depending on the "door in to PCI center" time, patients are divided into 2 groups: "Local area" for a time ≤ 60 min and "Remote area" for a time > 60 min. RESULTS: 51 patients were in the "Local area" group and 259 patients in the "Remote area" group with an average age of 63.36 years, without significant difference between the two groups. We noted more men, smokers and a higher heart rate in the "Local area" group (p = 0.015; p = 0.005 and p = 0.035 respectively). The median "call-EMS care" delay was similar at 24 min in each group. Seventy-five patients (29%) in the "Remote area" group had fibrinolysis versus only one patient in the "Local area" group (p < 0.001). Among them, 42 patients (56%) had rescue PCI, in 40% with 90 min of fibrinolytic administration. The presence of a primary care physicians "PCP" was reported in 39 cases of the "remote area" group. PCP intervention increased the rate of bolus of fibrinolytics within 10 min from STEMI diagnosis (69.2% vs 21.8%, p < 0.001), increased the rate of bolus treatment within 10 min from STEMI diagnosis within 10 min (60% vs 16% p < 0.001) and reduced the rate of rescue PCI (44.4% vs 62.5%, p = 0.035). Total ischemia time was significantly shorter in the "Local area" group (144 min vs 175 min, p = 0.005). No significant difference concerning the in-hospital outcomes was found between 2 groups. Concerning compliance with local recommendations in the "remote area" group, among those eligible for thrombolysis, 135 patients (64,2 %) had an inappropriate primary PCI (error in estimated time from STEMI diagnosis to PCI-mediated reperfusion (< 1 h)), which was associated with a higher rate of serious arrhythmias (11.1% vs 2.7% respectively, p = 0.035). Regardless of the group, if the strategy was primary PCI 22% of patients had angiography within 60 minutes after STEMI diagnosis. CONCLUSION: In this work, the temporal distance from the PCI room does not seem to be responsible for a more pejorative in-hospital outcomes, unlike the proportion of inappropriate primary PCI which increase serious arrhythmias.

Spontaneous coronary artery dissection (SCAD) and fibromuscular dysplasia (FMD) are pathologies that appear to be closely related. This study compares the characteristics of the FMD population to the non-FMD population in a SCAD cohort. It thus assesses the involvement of the FMD phenotype in a SCAD population. From the data of the French DISCO registry, we included patients with a diagnosis of SCAD and in whom a search for FMD was performed. We collected the following characteristics of this population: the clinical and angiographic presentation, the data concerning the management, and the events occurring during the follow-up. In the 373 SCADs confirmed in the DISCO registry, we obtained imaging data for 340 of them. FMD was found in 45% of cases. The mean age was higher in the FMD group, 53.2 ± 8.8 years, versus 50.1 ± 11 years in the non-FMD group. High blood pressure and postmenopausal status were significantly higher in the FMD group. Clinical presentation, angiographic data, and management were comparable. The major adverse cardiac event rate and recurrence rate were not different between the 2 groups after 1 year of follow-up. In conclusion, we confirmed a 45% prevalence of FMD in the SCAD population. The median age was higher in the FMD group, suggesting that FMD may develop over time. The rate of major adverse cardiac events and recurrence were similar in the FMD group versus the non-FMD group after 1 year of follow-up.
