RESUMEN

The social media podium offers a communal perspective platform for web marketing, advertisement, political campaign, etc. It structures like-minded end-users over the explicit group as a community. Community structure over social media is the collaborative group of globally spread users having similar interests regarding a communal topic, product or any other axis. In recent years, researchers have widely used clustering techniques of data mining to structure communities over social media. Still, due to a lack of network and implicit communal information, researchers cannot bind mutually robust and modular community structures. The collaborative features of social media are inherent with implicit and explicit end-users. The explicit nature of both active and passive users is easily extracted from the graphical structure of social media. On the other hand, the degree of information inclusion of implicit features depends upon end-users participation. The Implicit features of frequently active users are diversely available, while integrating passive and silent users' implicit features over the community is tedious. This work proposed a social theory based influence maximization (STIM) framework for community detection over social media. It combines user-generated content with profile information, extracts passive social media users through influence maximization, and provides the user space for influencing inactive users. The STIM framework clusters identical nodes over the maximum influencing node axis based on their graphical parameters such as node degree, node similarity, node reachability, modularity, and node density. This framework also provides the structural, relational and mathematical concept for the functional grouping of like-minded people as a community over social media through social theory. Finally, an evaluation has been carried out over six real-time datasets. It analyses that convolution neural network over STIM structure more dense and modular communities via influence maximization. STIM acquired around 93% modularity and 94% Normalized Mutual Information (NMI), resulting in approximately 2.23% and 5.69% improvements in modularity and NMI, respectively, over the best-acquired result of the benchmark approach.

RESUMEN

Avulsion fractures of the anterior inferior iliac spine (AIIS) are rare injuries in adolescent athletes. We present a case of a 15-year-old male who sustained an avulsion injury to his right AIIS when kicking a soccer ball. The patient had chronic pain and extra-articular subspinal impingement leading to decreased hip flexion and rotation. The injury occurred 1.5 years prior to symptom onset, and we were the first health care providers to manage the injury. We attempted six months of nonoperative management including activity modifications and nonsteroidal anti-inflammatory (NSAID) therapy without improvement. Although this injury can often be managed nonoperatively, his symptoms required excision of the AIIS and associated heterotopic ossification. He had an excellent outcome with return to soccer and no pain at his final follow-up visit two years after surgery. Due to the limited literature guiding the surgeon's management of AIIS avulsion injuries with associated heterotopic ossification, we provide a review of the literature detailing pre- and postoperative ranges of motion, surgical approach, fixation or excision of the avulsion fragment, and return to sport in this patient population.

RESUMEN

Dermatosis neglecta is a chronic, dermatologic disorder results in ignored, neglected body parts due to chronic disability or painful conditions. There is scarcity of literature supporting the existence of dermatosis neglecta in the context of psychiatric illnesses. In this case report, we attempts to highlight, dermatosis neglecta in a homeless patient suffering from schizophrenia.

RESUMEN

Psychosis with phenytoin use has earlier been reported only in the context of Vitamin B12 or folic acid deficiency. We report a rare case of phenytoin toxicity manifesting as psychosis in the absence of Vitamin deficiency. The importance of recognition of psychosis as a harbinger of phenytoin toxicity and implications for management are discussed.

RESUMEN

Although generally regarded as a disease of young adults, schizophrenia does occur in older ages. Doubts have been raised about the validity of diagnosing schizophrenia in very old age. We have described herein a rare and unique case of a woman who had onset of "schizophrenia" as per ICD-10 and DSM-IV-TR criteria at the age of 100 years. We have discussed about the validity of diagnosing schizophrenia in older age.

RESUMEN

Priapism is defined as a prolonged and persistent, painful erection of the penis without sexual stimulation or arousal. There have been a few case report of priapism resulting from the use of antipsychotics. In our case, a 48-year-old male patient with a bipolar affective disorder was experiencing recurrent priapism with sodium valproate. The condition was found to resolve on stopping sodium valproate. There are no known case reports of similar complication with sodium valproate alone.

RESUMEN

Rotationplasty is a reconstructive, limb-sparing option for management of lower extremity bone deficiency. This technique involves an intercalary resection, followed by 180° rotation of the distal limb to allow the ankle to function as a knee joint when it is fitted with a modified below-knee prosthesis. Gait analysis and functional outcome studies have reported favorable results with rotationplasty compared with those of above-knee amputation. Moreover, patient satisfaction with rotationplasty is higher than with other limb salvage procedures. The primary drawback of this procedure is patient acceptance of the limb's appearance.
