OBJECTIVE: To explore the effects of a multidisciplinary team (MDT) continuous care model on psychological behavior and quality of life in children with ß-thalassemia major (ß-TM). METHODS: A retrospective study was conducted on 90 children with ß-TM who were hospitalized in the Department of Pediatrics, Affiliated Hospital of Youjiang Medical University for Nationalities from October 2021 to May 2022. According to nursing methods, the children were divided into a routine group and an experimental group, with 45 cases in each group. The routine group was intervened with routine nursing approaches, and the experimental group was intervened with the MDT continuous nursing mode on the basis of the control group. The psychological behavior [Positive and Negative Affect Scale for Children (PANAS-C)], quality of life (QoL) [Pediatric Quality of Life Inventory TM (PedsQLTM)] and satisfaction were compared between the two groups before and 6 months after intervention. The hemoglobin level and ferritin level of children after 3 months and 6 months of intervention were measured. The occurrence of complications and nursing satisfaction were recorded. The prognostic factors were further analyzed. RESULTS: After the intervention, the positive emotion score, QoL score, hemoglobin level, satisfaction score in the experimental group were all higher than those in the conventional group, and the negative emotion score and ferritin level in the experimental group were all lower than those in the conventional group (all P < 0.05). The results of Cox analysis showed that the use of iron-removing drugs and arrhythmia/heart rate failure were risk factors affecting the prognosis of children with ß-TM, while the MDT continuous nursing mode was a protective factor (all P < 0.05). CONCLUSION: The MDT continuous care model can effectively promote mental health in children with ß-TM, improve their quality of life, medical satisfaction, ameliorate the degree of anemia in children, reduce the incidence of complications and improve the prognosis; thus, it is worthy of wide clinical application.

BACKGROUND: Maintaining normal patellar alignment is important for knee health. Altered activation of individual quadriceps muscles have been found related to patellar alignment. However, the relationships between strength and passive stiffness of the quadriceps and patellar alignment remains unexplored. METHODS: Participants aged between 60 and 80 years with activity-induced knee pain were recruited. Knee pain was quantified using an 11-point numeric rating scale. Quadriceps strength was assessed using a Cybex dynamometer and passive stiffness of rectus femoris, vastus lateralis, and vastus medialis were measured by shear-wave ultrasound elastography. Patellar alignments were assessed using MR imaging. Linear regression was used to examine relationships between quadriceps properties and patellar alignments with and without controlling for potential covariates. FINDINGS: Ninety-two eligible participants were assessed (71.7% females, age: 65.6 ± 3.8 years; pain scale: 4.6 ± 2.0), most of whom had knee pain during stair climbing (85.9%). We found that 17% of patellar lateral tilt angle could be explained by lower quadriceps strength (adjusted R2 = 0.117; P < 0.001), especially in females (R2 = 0.281; P < 0.001; adjusted R2 = 0.211; P < 0.001). In addition, a higher stiffness ratio of vastus lateralis/medialis accounted for 12% of patellar lateral displacement (adjusted R2 = 0.112; P = 0.008). INTERPRETATION: Quadriceps strength and relative stiffness of lateral to medial heads are associated with patellar alignment in older adults with knee pain. It suggests that quadriceps weakness and relatively stiffer lateral quadriceps may be risk factors related to patellar malalignments in the elderly.

BACKGROUND: Knee pain is one of the main problems associated with knee osteoarthritis. The peak external knee adduction moment (KAM) in gait is commonly used to estimate medial knee loading, and elevated KAM has been related to a higher risk of developing knee pain in older adults. Although knee flexion moment (KFM) also contributes to medial knee loading, its role in the development of knee pain remains unclear. AIM: To investigate the association between knee moments and the incidence of knee pain in 24 months in asymptomatic older adults. DESIGN: Prospective cohort study. SETTING: University laboratory. POPULATION: Community-dwelling adults aged 60-80 years were recruited. We excluded participants with knee pain/known arthritis, knee injury, knee/hip joint replacement, cognitive impairment, or neurological conditions. METHODS: Three-dimensional gait analysis was conducted to compute the peak KFM and KAM. Telephone surveys were conducted 12 and 24 months after the baseline assessment. Self-reported knee pain and its intensity and frequency were captured. Logistic regression with generalized estimating equations was used to examine associations between knee moments and the risk of knee pain. RESULTS: Of the 162 eligible participants who completed the baseline assessment (age: 65.8±4.0 years, 61.1% females), 157 and 138 were also assessed for incident knee pain after 12 and 24 months, respectively. Compared with the lowest tertile, the highest tertile of KFM was significantly related to a lower incidence of frequent knee pain (RR=0.25, 95% CI: 0.08-0.85, P=0.027) in 24 months. In addition, a higher KFM was significantly associated with the lower intensity of incident knee pain after 24 months (ß=-1.513; 95% CI: -2.879, -0.147; P=0.030). We also observed trends showing that a higher peak KAM was related to higher risks of developing any (RR=2.48, 95% CI: 0.99-6.20, P=0.053) and frequent (RR=3.82, 95% CI: 0.96-15.1, P=0.057) knee pain in 24 months. CONCLUSIONS: A higher sagittal knee moment is associated with a lower risk of knee pain developing in 24 months in older adults. CLINICAL REHABILITATION IMPACT: Interventions for promoting sagittal knee moment may be considered in preventative training programs for reducing knee pain among older adults.

BACKGROUND: Quadriceps weakness is a known risk factor for the onset of knee osteoarthritis (OA). In addition to muscle weakness, increased passive stiffness of the quadriceps may affect knee biomechanics and hence contribute to the pathogenesis of knee OA. However, the association between quadriceps stiffness and the risk of knee OA development has not been prospectively investigated. AIM: The aim of this study was to investigate how baseline quadriceps passive stiffness predicts the incidence of clinical knee OA at the 12-month follow-up. DESIGN: Prospective cohort study. SETTING: University laboratory. POPULATION: Community-dwelling adults aged 60-80 years were recruited. We excluded participants with: 1) knee pain or known arthritis; 2) knee injury; 3) knee or hip joint replacement, 4) cognitive impairment; or 5) neurological conditions. METHODS: At baseline, passive stiffness of the three superficial quadriceps muscle heads (rectus femoris [RF], vastus lateralis [VL], and vastus medialis oblique [VMO]) was evaluated using shear-wave ultrasound elastography. Knee muscle (quadriceps and hamstrings) strength was tested using a Cybex dynamometer. Knee OA was defined based on clinical criteria 12 months after baseline measurements. Generalized estimating equations were used to examine the associations of quadriceps stiffness and knee muscle strength with the risk of knee OA, controlling for age, sex, Body Mass Index, comorbidities, and activity level. RESULTS: The analyses included 158 knees (58.2% females, age: 65.6±4.1 years). Twenty-eight knees (17.7%) were classified as having clinical OA at 12 months. Compared with the lowest stiffness tertiles, the highest stiffness tertiles of the RF (relative risk =5.31, 95% CI: 1.34-21.0), VMO (4.15, 1.04-16.6), and total superficial quadriceps (6.35, 1.48-27.3) at baseline were significantly associated with a higher risk of knee OA at the follow-up. The highest strength tertile of quadriceps has a trend of association with a lower risk of knee OA than the lowest tertile (0.18, 0.03-1.25, P=0.083). CONCLUSIONS: Greater passive stiffness of the quadriceps at baseline was associated with a higher risk of clinical knee OA incidence at the 12-month follow-up. CLINICAL REHABILITATION IMPACT: Interventions for reducing the passive stiffness of the quadriceps should be included in preventative training programs for older adults.

Neurofeedback training is a common treatment option for attention deficit hyperactivity disorder (ADHD). Given theta/beta-based neurofeedback (T/B NF) training targets at the electrophysiological characteristics of children with ADHD, benefits for attention may be expected. PsycINFO, PubMed, ScienceDirect, Scopus, and Web of Science were searched through December 31, 2020. Studies were evaluated with Risk of Bias tools. Within-group effects based on Pre- and Post-treatment comparisons of the Intervention Group, and Between-group effects based on the between-group differences from Pre-treatment to Post-treatment were calculated. Nineteen studies met selection criteria for systematic review, 12 of them were included in meta-analysis. Within-group effects were medium at Post-treatment and large at Follow-up. Between-group analyses revealed that T/B NF was superior to waitlist control and physical activities, but not stimulant medication. Results showed that T/B NF has benefits for attention in children with ADHD, however, cautions should be taken when interpreting the findings.

Methods: A retrospective study was conducted on the clinical records of 148 children diagnosed with severe beta thalassemia who were admitted to our hospital between October 2018 and September 2021. The patients were separated into two groups, a control group and an intervention group, with 74 cases in each group, according to the various care approaches. The basic treatment regimen was given to all of the children: deferoxamine mesylate combined with deferiprone. During treatment, the control group received routine care, and the intervention group adopted the FCC model based on a mobile app. The quality of life scale for children and adolescents (QLSCA) score, the family assessment device (FAD) score, the exercise of self-care agency scale (ESCA) score, and the medication compliance scale score were compared between the two groups. Results: The QLSCA score, ESCA score, and medication compliance scale score of the intervention group were significantly higher than those of the control group and showed a significant difference (intergroup effect: F = 198.400, 259.200, and 129.800, all P < 0.001). Scores in both groups increased over time (time effect: F = 19.350, 40.830, and 12.130, all P < 0.001), and there was an interaction effect between grouping and time (interaction effect: F = 3.937, 12.020, and 5.028). The P values were 0.020, <0.001, and 0.007. The FAD score of the intervention group was significantly lower than that of the control group (intergroup effect: F = 177.200, P < 0.001). The FAD scores of both groups decreased over time (time effect: F = 7.921, P = 0.005). There was an interaction effect between groups and time (interaction effect: F = 5.206, P = 0.006). Conclusion: The application effect of the mobile app-based FCC model combined with the comprehensive iron removal treatment program in children with severe beta thalassemia is significant, which can significantly improve the quality of life, family function, self-care ability, and medication compliance of children, and has high clinical application value.

Precipitation intensity estimation is a critical issue in the analysis of weather conditions. Most existing approaches focus on building complex models to extract rain streaks. However, an efficient approach to estimate the precipitation intensity from surveillance cameras is still challenging. This study proposes a convolutional neural network known as the signal filtering convolutional neural network (SF-CNN) to handle precipitation intensity using surveillance-based images. The SF-CNN has two main blocks, the signal filtering block (SF block) and the gradually decreasing dimension block (GDD block), to extract features for the precipitation intensity estimation. The SF block with the filtering operation is constructed in different parts of the SF-CNN to remove the noise from the features containing rain streak information. The GDD block continuously takes the pair of the convolutional operation with the activation function to reduce the dimension of features. Our main contributions are (1) an SF block considering the signal filtering process and effectively removing the useless signals and (2) a procedure of gradually decreasing the dimension of the feature able to learn and reserve the information of features. Experiments on the self-collected dataset, consisting of 9394 raining images with six precipitation intensity levels, demonstrate the proposed approach's effectiveness against the popular convolutional neural networks. To the best of our knowledge, the self-collected dataset is the largest dataset for monitoring infrared images of precipitation intensity.

Neuropilin-1 (NRP1) is an essential transmembrane receptor with a variety of cellular functions. Here, we identify two human NRP1 splice variants resulting from the skipping of exon 4 and 5, respectively, in colorectal cancer (CRC). Both NRP1 variants exhibit increased endocytosis/recycling activity and decreased levels of degradation, leading to accumulation on endosomes. This increased endocytic trafficking of the two NRP1 variants, upon HGF stimulation, is due to loss of N-glycosylation at the Asn150 or Asn261 site, respectively. Moreover, these NRP1 variants enhance interactions with the Met and ß1-integrin receptors, resulting in Met/ß1-integrin co-internalization and co-accumulation on endosomes. This provides persistent signals to activate the FAK/p130Cas pathway, thereby promoting CRC cell migration, invasion and metastasis. Blocking endocytosis or endosomal Met/ß1-integrin/FAK signaling profoundly inhibits the oncogenic effects of both NRP1 variants. These findings reveal an important role for these NRP1 splice variants in the regulation of endocytic trafficking for cancer cell dissemination.

Loss of 4E-BP1 expression has been linked to cancer progression and resistance to mTOR inhibitors, but the mechanism underlying 4E-BP1 downregulation in tumors remains unclear. Here we identify Snail as a strong transcriptional repressor of 4E-BP1. We find that 4E-BP1 expression inversely correlates with Snail level in cancer cell lines and clinical specimens. Snail binds to three E-boxes present in the human 4E-BP1 promoter to repress transcription of 4E-BP1. Ectopic expression of Snail in cancer cell lines lacking Snail profoundly represses 4E-BP1 expression, promotes cap-dependent translation in polysomes, and reduces the anti-proliferative effect of mTOR kinase inhibitors. Conversely, genetic and pharmacological inhibition of Snail function restores 4E-BP1 expression and sensitizes cancer cells to mTOR kinase inhibitors by enhancing 4E-BP1-mediated translation-repressive effect on cell proliferation and tumor growth. Our study reveals a critical Snail-4E-BP1 signaling axis in tumorigenesis, and provides a rationale for targeting Snail to improve mTOR-targeted therapies.

BACKGROUND: The associations between toll-like receptor 2 (TLR2) and toll-like receptor 4(TLR4) polymorphisms and inflammatory bowel disease (IBD) susceptibility remain controversial. A meta-analysis was performed to assess these associations. METHODS: A systematic search was performed to identify all relevant studies relating TLR2 and TLR4 polymorphisms and IBD susceptibility. Odds ratios (ORs) and 95% confidence intervals (CIs) were calculated. Subgroup analyses were performed by ethnicity and publication quality. RESULTS: Thirty-eight eligible studies, assessing 10970 cases and 7061 controls were included. No TLR2 Arg677Trp polymorphism was found. No significant association was observed between TLR2 Arg753Gln polymorphism and Crohn's disease (CD) or ulcerative colitis (UC) in all genetic models. Interestingly, TLR4 Asp299Gly polymorphism was significantly associated with increased risk of CD and UC in all genetic models, except for the additive one in CD. In addition, a statistically significant association between TLR4 Asp299Gly polymorphism and IBD was observed among high quality studies evaluating Caucasians, but not Asians. Associations between TLR4 Thr399Ile polymorphisms and CD risk were found only in the allele and dominant models. The TLR4 Thr399Ile polymorphism was associated with UC risk in pooled results as well as subgroup analysis of high quality publications assessing Caucasians, in allele and dominant models. CONCLUSIONS: The meta-analysis provides evidence that TLR2 Arg753Gln is not associated with CD and UC susceptibility in Asians; TLR4 Asp299Gly is associated with CD and UC susceptibility in Caucasians, but not Asians. TLR4 Thr399Ile may be associated with IBD susceptibility in Caucasians only. Additional well-powered studies of Asp299Gly and other TLR4 variants are warranted.

Cylindrocladium leaf blight is one of the most severe diseases in Eucalyptus plantations and nurseries. There are Eucalyptus cultivars with resistance to the disease. However, little is known about the defense mechanism of resistant cultivars. Here, we investigated the transcriptome and proteome of Eucalyptus leaves (E. urophylla×E. tereticornis M1), infected or not with Calonectria pseudoreteaudii. A total of 8585 differentially expressed genes (|log2 ratio| ≥1, FDR ≤0.001) at 12 and 24hours post-inoculation were detected using RNA-seq. Transcriptional changes for five genes were further confirmed by qRT-PCR. A total of 3680 proteins at the two time points were identified using iTRAQ technique.The combined transcriptome and proteome analysis revealed that the shikimate/phenylpropanoid pathway, terpenoid biosynthesis, signalling pathway (jasmonic acid and sugar) were activated. The data also showed that some proteins (WRKY33 and PR proteins) which have been reported to involve in plant defense response were up-regulated. However, photosynthesis, nucleic acid metabolism and protein metabolism were impaired by the infection of C. pseudoreteaudii. This work will facilitate the identification of defense related genes and provide insights into Eucalyptus defense responses to Cylindrocladium leaf blight. BIOLOGICAL SIGNIFICANCE: In this study, a total of 130 proteins and genes involved in the shikimate/phenylpropanoid pathway, terpenoid biosynthesis, signalling pathway, cell transport, carbohydrate and energy metabolism, nucleic acid metabolism and protein metabolism in Eucalyptus leaves after infected with C. pseudoreteaudii were identified. This is the first report of a comprehensive transcriptomic and proteomic analysis of Eucalyptus in response to Calonectria sp.

OBJECTIVE: To determine soil fertility of the Pseudostellariac Radix planting base of Shibing County and Huangping County in Guizhou Province, and to provide experimental basis for soil improvement and balanced fertilization. METHODS: 12 soil samples from Shibing County and 10 soil samples from Huangping County were involved in the detection. RESULTS: In the soil samples from Shibing County, the contents of organic matter, total N, available N, total P, available P and available K were higher, while the content of total P was at middle level. In the soil samples from Huangping County, the contents of total N, available P, total K and available K were higher,the contents of organic matter, total K and available N were at middle level. CONCLUSION: The level of soil fertility of the Pseudostellariae Radix planting base in Guizhou Province is common. The soil fertility of Shibing County is superior to that of Huangping County. The soil pH value of Shibing and Huangping County is low. In Shibing County, lime, farmyard manure and neutral fertilizer can be used to increase the pH of soil. Potash fertilizer can be increased, while nitrogen fertilizer and phosphate fertilizer can be reduced in the soil of Shibing County. Potash fertilizer and phosphate fertilizer will be increased, while nitrogen fertilizer can be reduced in the soil of Huangping County. The soil fertility can be improved by these measures.
