RESUMEN

This study determined the occurrence of legionellae in private houses for which there were no available data on aquatic environments other than the water supply system. From June 2013 to November 2014, we collected 138 water and 90 swab samples from aquatic environments in 19 houses. Legionella DNA was detected via a loop-mediated isothermal amplification assay in 66 (47·8%) water and 17 (18·9%) swab samples. High Legionella DNA detection rates were observed in water samples from washing machines and aquariums. Legionella spp. was isolated from 9 (6·5%) water and 3 (3·3%) swab samples. Legionella pneumophila SG 1 was detected from the outlet water of a bathtub spout and a bath sponge. Use of amoebic co-culture effectively increased legionellae and Legionella DNA detection rates from all sample types. A logistic regression analysis revealed that the heterotrophic plate count was significantly related to Legionella contamination. Our findings indicate that there is a risk of legionellosis from exposure to Legionella spp. in a variety of aquatic environments in residential houses. Control measures for legionellae in houses should include frequent cleaning and disinfecting to reduce heterotrophic bacteria in water and, where possible, preventing aerosolization from aquatic environments.

RESUMEN

We describe a novel assay for simple, rapid and high-sensitive detection of Cryptosporidium oocysts in water samples using a reverse transcription-loop-mediated isothermal amplification (RT-LAMP). The assay is based on the detection of 18S rRNA specific for Cryptosporidium oocysts. The detection limit of the developed RT-LAMP assay was as low as 6 x 10(-3) oocysts/test tube, which theoretically enables us to detect a Cryptosporidium oocyst and perform duplicated tests even if water samples contain only one oocyst. The developed RT-LAMP assay could more sensitively detect Cryptosporidium oocysts in real water samples than the conventional assay based on microscopic observation.

Carbon and nitrogen stable isotope ratios were measured in hair samples of the Asiatic black bear (Ursus thibetanus) inhabiting the Northern Japanese Alps (NJA) (n = 20) and the periphery of Nagano City (NC) (n = 6), in Nagano Prefecture, Japan. The hair of NJA bears, which did not have access to anthropogenic foods, showed lower values of d13C and d15N than that of NC bears which had access to garbage and corn fields, especially during the summer. These results reflect somewhat differing diets between the NJA and NC bears. We attempted to assess the feeding history during the hair growth cycle using the growth section analysis method. Each hair sample had been cut into 3?mm lengths from root to tip, labeled, and analyzed along the hair growth. We measured the carbon and nitrogen stable isotope ratios of each 3?mm length of hair sample from one NC bear which had been killed while raiding a corn field. The sections showed wide ranges of isotope ratios, from -23.2% to -14.6% for delta13C, and from 0.3% to 4.6% for delta15N. It was shown that the diet of this bear shifted dramatically from principally C3 plants to more C4 plants and to foods of animal origin. An analysis of the whole hair reflects just the average feeding habit during hair growth, but the present method can trace its diet history. This method can contribute to obtain precise ecological information of wildlife.

Cryptosporidium oocysts, morphologically identified as Cryptosporidium parvum, were isolated from 22 human and 14 bovine cases in Japan, and were genotyped by means of a PCR/RFLP analysis of the polythreonine gene. DNA profiles of human isolates gave three distinct genotypes, namely an anthroponotic genotype 1, zoonotic genotype 2 and a new genotype. Isolates from bovine samples gave zoonotic genotype 2. The unusual genotype of Cryptosporidium was isolated from the feces of three immunologically healthy adults, and was further characterized by the sequence analysis of the 18S rRNA gene. The third genotype was identified as Crypto sporidium meleagridis, demonstrating that C. meleagridis, which occurs worldwide, has the potential to infect humans regardless of their immunological condition.

Fecal samples from 232 weaned piglets (1 and 3 months old) and 252 fattening porkers (6 months old) in 8 stock-raising farms located in Kanagawa Prefecture, Japan, from June 1998 to June 2000 were examined to determine the prevalence of Cryptosporidium infection. Detection of oocysts was performed using the ethyl acetate fecal concentration method and immunofluorescent staining. C. parvum oocysts were identified in 77 (33.2%) 1-3 months old weaned piglets from four farms. The odds of excreting among 1-3 months old piglets were more than 100 times greater than among 6 months old porkers (95% confidence interval: 17-902). This strongly suggests that weaned piglets are important reservoirs of pathogenic microbes whose potential contamination of drinking water has epidemiological implications for human health.

The knees of 72 patients with unilateral anterior-cruciate- ligament (ACL) injury were analyzed before ACL reconstruction as well as by follow-up arthroscopy on the day of staple removal. At ACL reconstruction 31 lateral menisci and 40 medial menisci were found to be normal. 28 lateral menisci and 24 medial menisci were treated surgically, while 13 lateral menisci and 8 medial menisci with small or incomplete meniscal tearing were not treated. At follow-up arthroscopy there were 3 new cases of lateral meniscal tearing and 3 new cases of medial meniscal tearing in the groups diagnosed as normal prior to surgery. Two of the 13 cases with small or incomplete lateral meniscal tearing required resection, 8 healed and the other 3 demonstrated no progressive change. Four of the 8 cases with small or incomplete medial meniscal tears healed, 3 exhibited no progressive change and one required surgical treatment. There was no correlation between meniscal tearing and knee instability as indicated by a positive Lachman test or a positive pivot shift sign. The results of the present study indicate that ACL reconstruction prevents progressive changes in meniscal tears and will prevent secondary osteoarthritis, and that some small tears of the lateral meniscus require no surgical treatment.

This study was designed to evaluate the efficacy, toxicity, and quality of life (QOL) of outpatient chemotherapy with 5-FU and LV in advanced gastrointestinal cancer. Treatment consisted of CI 5-FU 450-500 mg/body/day, days 1-28, IVB LV 30 mg/body q wk. 5-FU was administered with the Baxter infusor (0.5 ml/hr). LV was not administered if Grade 2 mucositis occurred. Seven patients (pts) with advanced gastrointestinal cancer (esophageal -2, gastric -1, colorectal -4) have been treated to maintain the efficacy of prior inpatient chemotherapy, and two pts with colorectal cancer instead of oral UFT. And to evaluate toxicity and QOL, six pts (bile duct -1, gastric -2, colorectal -3) treated as adjuvant chemotherapy were added. The median duration of response of the nine pts was 3.03 months. Because of mucositis, the administration of LV was restricted. The mean administration of it was 2.1 times (62 mg). Grade 3 or 4 toxicities of mucositis were seen in 40% of the cases. Grade 1 or 2 skin toxicities were seen in all pts. Mean score of QOL in these outpatients chemotherapy was 78.0 +/- 11.5, ranked between inpatient chemotherapy (5-FU+CDDP 59.0 +/- 13.8) and oral UFT (91.8 +/- 6.1). In conclusion, this schedule of 5-FU and LV combination offers a high patient QOL and is suitable for advanced gastrointestinal cancer treatment.

Addition of diet fiber to meals is reported to result in significant reduction of postprandial glucose levels in diabetic patients as well as in normal subjects. In the present study effect of guar intake (12 g) on plasma somatostatin-like immunoreactivity (SRIF-LI) was studied in non-insulin dependent diabetes (NIDDM) and in insulin-dependent diabetes (IDDM) to see if somatostatin plays a role in reducing postprandial glucose. In 6 of 7 normal controls plasma SRIF-LI increased and reached a maximum at 30 min after guar intake. In 5 of 7 NIDDM patients SRIF-LI reached a maximum at 90 min after guar intake, while in five IDDM patients SRIF-LI significantly decreased through 120 min after guar intake. These results suggest the possibility that guar induced somatostatin secretion contributes to reduction of postprandial glucose levels at least in NIDDM patients as well as in normal subjects, for somatostatin is reported to inhibit glucose uptake from gastrointestinal tract.

N-benzoyl-L-tyrosyl-p-aminobenzoic acid (Bz-ty-PABA) was orally administered to 11 controls, 10 patients with chronic pancreatitis, 7 patients with diabetes mellitus and 6 patients with liver cirrhosis. The cumulative 6 h recovery rate of PABA in the urine was significantly lower (P less than 0.005) in patients with chronic pancreatitis (49.1 + or - 10.1 percent), diabetes mellitus (50.4 + or - 20.4 percent) and liver cirrhosis (52.5 + or - 13.0 percent) than in the control group (79.5 + or - 12.0 percent) (mean + or -S.D.). This test is considered to be useful in the diagnosis of pancreatic exocrine insufficiency, especially in chronic pancreatitis. Patients with diabetes mellitus frequently has demonstrable abnormality of pancreatic exocrine function. Liver cirrhosis causing severe impairment of liver functions seemed to interfer with the elimination of PABA.
