RESUMEN

There have been numerous studies on shotcrete based on strength and durability. However, few studies have been conducted on rheological characteristics, which are very important parameters for evaluating the pumpability and shootability of shotcrete. In those studies, silica fume has been generally used as a mineral admixture to simultaneously enhance the strength, durability, pumpability, and shootability of shotcrete. Silica fume is well-known to significantly increase the viscosity of a mixture and to prevent material sliding at the receiving surface when used in shotcrete mixtures. However, the use of silica fume in shotcrete increases the possibility of plastic shrinkage cracking owing to its very high fineness, and further, silica fume increases the cost of manufacturing the shotcrete mixture because of its cost and handling. Colloidal silica is a new material in which nano-silica is dispersed in water, and it could solve the above-mentioned problems. The purpose of this research is to develop high-performance shotcrete with appropriate levels of strength and workability as well as use colloidal silica for normal structures without a tunnel structure. Thereafter, the workability of shotcrete with colloidal silica (2, 3, and 4%) was evaluated with a particle size of 10 nm and silica fume replacement (4 and 7%) of cement. In this study, an air-entraining agent for producing high-performance shotcrete was also used. The rheological properties of fresh shotcrete mixtures were estimated using an ICAR rheometer and the measured rheological parameters such as flow resistance and torque viscosity were correlated with the workability and shootability. More appropriate results will be focusing on the Bingham model properties such that the main focus here is to compare all data using the Bingham model and its performance. The pumpability, shootability, and build-up thickness characteristics were also evaluated for the performance of the shotcrete. This research mainly focuses on the Bingham model for absolute value because it creates an exact linear line in a graphical analysis, which provides more appropriate results for measuring the shotcrete performance rather than ICAR rheometer relative data.

BACKGROUND: Owing to an increased number of colonoscopy screenings, the incidence of diagnosed rectal neuroendocrine tumors (NETs) has also increased. Tumor size is one of the most frequently regarded factors when selecting treatment; however, it may not be the determinant prognostic variable. We aimed to evaluate oncological outcomes according to the treatment modality based on the size of rectal NETs. METHODS: A retrospective analysis was performed on patients who were treated for rectal NETs between March 2000 and January 2016 at the Asan Medical Center, Seoul, Korea. Patients who underwent endoscopic removal, local surgical excision, and radical resection were included. The primary outcome was recurrence-free survival (RFS). Data were specified and analyzed following the 2019 World Health Organization classification (WHO). RESULTS: A total of 644 patients were categorized under three groups according to the treatment modality used: endoscopic removal (n = 567), surgical local excision (n = 56), and radical resection (n = 21). Of a total of 35 recurrences, 27 were local, whereas eight were distant. The RFS rate did not differ significantly between the treatment groups in the same tumor-size group ([Formula: see text]1 cm group: P = .636, 1-2 cm group: P = .160). For T1 tumors, RFS rate was not different between local excision and radical resection ([Formula: see text]1 cm group: P = .452, 1-2 cm group: P = .700). Depth of invasion, a high Ki-67 index, and margin involvement were confirmed as independent risk factors for recurrence. Among patients treated with endoscopic removal, endoscopic biopsy was a significant factor for worse RFS (P < .001), while tumor size did not affect the RFS. CONCLUSION: The current guideline recommends treatment options according to tumor size. However, more oncologically important prognostic factors include muscularis propria invasion and a higher Ki-67 index.

BACKGROUND: The microsatellite instability (MSI) test and immunohistochemistry (IHC) are widely used to screen DNA mismatch repair (MMR) deficiency in sporadic colorectal cancer (CRC). For IHC, a two-antibody panel of MLH1 and MSH2 or four-antibody panel of MLH1, MSH2, PMS2, and MSH6 are used. In general, MSI is known as a more accurate screening test than IHC. AIM: To compare two- and four-antibody panels of IHC in terms of accuracy and cost benefit on the basis of MSI testing for detecting MMR deficiency. METHODS: We retrospectively analyzed patients with CRC who underwent curative surgery between 2015 and 2017 at a tertiary referral center. Both IHC with four antibodies and MSI tests were routinely performed. The sensitivity and specificity of a four- and two types of two-antibody panels (PMS2/MSH6 and MLH1/MSH2) were compared on the basis of MSI testing for detecting MMR deficiency. RESULTS: High-frequency MSI was found in 5.5% (n = 193) of the patients (n = 3486). The sensitivities of the four- and two types of two-antibody panels were 97.4%, 92.2%, and 87.6%, respectively. The specificities of the three types of panels did not differ significantly (99.6% for the four-antibody and PMS2/MSH6 panels, 99.7% for the MLH1/MSH2 panel). Based on Cohen's kappa statistic (κ), four- and two-antibody panels were in almost perfect agreement with the MSI test (κ > 0.9). The costs of the MSI test and the four- and two-antibody panels of IHC were approximately $200, $160, and $80, respectively. CONCLUSION: Considering the cost of the four-antibody panel IHC compared to that of the two-antibody panel IHC, a two-antibody panel of PMS2/MSH6 might be the best choice in terms of balancing cost-effectiveness and accuracy.

PURPOSE: Carcinoma arising from Crohn disease (CD) is rare, and there is no clear guidance on how to properly screen for at-risk patients and choose appropriate care. This study aimed to evaluate the clinicopathological characteristics, treatment, and oncologic outcomes of CD patients diagnosed with colorectal cancer (CRC). METHODS: Using medical records, we retrospectively enrolled a single-center cohort of 823 patients who underwent abdominal surgery for CD between January 2006 and December 2015. CD-associated CRC patients included those with adenocarcinoma, lymphoma, or neuroendocrine tumors of the colon and rectum. RESULTS: Nineteen patients (2.3%) underwent abdominal surgery to treat CD-associated CRC. The mean duration of CD in the CD-associated CRC group was significantly longer than that in the benign CD group (124.7 ± 77.7 months vs. 68.9 ± 60.2 months, P = 0.006). The CD-associated CRC group included a higher proportion of patients with a history of perianal disease (73.7% vs. 50.2%, P = 0.035) and colonic location (47.4% vs. 6.5%, P = 0.001). Among 19 CD-associated CRC patients, 17 (89.5%) were diagnosed with adenocarcinoma, and of the 17 cases, 15 (88.2%) were rectal adenocarcinoma. On multivariable analyses for developing CRC, only colonic location was a risk factor (relative risk, 7.735; 95% confidence interval, 2.862-20.903; P = 0.001). CONCLUSION: Colorectal malignancy is rare among CD patients, even among patients who undergo abdominal surgery. Rectal adenocarcinoma accounted for most of the CRC, and colonic location was a risk factor for developing CRC.

Few radiation-induced bowel perforations have been reported to date. Furthermore, perforation after ileal restoration in asymptomatic patients is rare. We report the case of a 61-year-old man who was administered preoperative chemoradiotherapy for advanced rectal cancer. The patient underwent ultra-low anterior resection with ileal diversion, followed by ileal restoration. Perforation was detected 9 days after restoration, and he underwent a right hemicolectomy. The histologic evaluation indicated ileal perforation caused by acute radiation enteritis.

We analyzed oncologic outcomes according to pre-/post-LPLN enlargement. Rectal cancer patients who underwent resection post-PCRT during 2008-2012 were enrolled. Magnetic resonance imaging pre-/post-PCRT were re-evaluated. LNs with short axis (SA) ≥7 mm pre-PCRT and ≥4 mm post-PCRT were defined as enlarged nodes. Of 798 patients enrolled, recurrence occurred in 55 (6.9%) local, 17 (2.1%) lateral, and 179 (22.4%) distal regions. Patients with LPLN SA ≥7 mm pre-PCRT showed worse local recurrence-free survival (RFS), lateral RFS, and distant RFS (p < 0.001, 0.002, and 0.005, respectively). LN shrinkage post-PCRT to SA<4 mm showed better 5-year local RFS (83.5% vs. 78.3%, p = 0.045), but distant RFS was similar irrespective of LN shrinkage to <4 mm. Among patients with pre-PCRT SA ≥7 mm, node shrinkage to SA<4 mm after PCRT presented with lower incidence of local recurrence but did not benefit in distant recurrence. Lateral node sampling did not improve local recurrence control, resulting in a 5-year local RFS of 75.4% in patients undergoing lateral node sampling and 83.2% in those not undergoing lateral node sampling (p = 0.722). Four (66.7%) patients had lateral recurrence in the same area of the enlarged nodes identified pre-PCRT. For patients assessed with pre-PCRT nodes ≥7 mm, response to PCRT did not guarantee better outcomes.

Direct grazing-angle X-ray scattering evidence of the order-disorder transition and interdigitation of side chains in a conjugated polymer poly(3-hexylthiophene) (P3HT) is presented. The free methyl ends of the side chains exhibit closest packing, as in n-alkane crystallization, and cause a structural mismatch due to the difference between their packing density and the areal density of the attached ends. This mismatch is resolved by increases in the tilt angle of the side chains and local interdigitation. In situ X-ray scattering and electrical measurements show that the structural transition and interdigitation of these side chains strongly affect its surface morphology as well as the charge transport properties of the resulting P3HT-based organic field-effect transistor. Since most conjugated polymers have side chains, the results of this study provide a deeper understanding of the effects of side chains on the structural and electrical properties of conjugated backbones. These results also provide a new perspective on the formation of a metastable polymorph consisting of interdigitated P3HT.

We explored the photocatalytic capacities of cementitious materials (cement paste and mortar) incorporating titanium dioxide (TiO2). P-25 is a commercial TiO2 preparation which, if incorporated into large civil buildings, is extremely expensive. It is essential to produce low-cost TiO2. A cheap anatase form of TiO2 powder, NP-400, manufactured under relatively low burning temperature, was considered in this paper. Addition of NP-400 to 0, 5, 10, and 20 wt % did not significantly affect the compressive strengths of mortar or cement paste. However, the compressive strengths of P-25-containing specimens were more consistent than those of NP-400-containing materials. The nitrogen oxide (NO) removal efficiencies by mortar with 5 and 10 wt % TiO2 were similar at ca. 14⁻16%; the removal efficiency by mortar with 20 wt % NP-400 was ca. 70%. Although the NP-400 cluster size was almost halved by ultrasonication, NO removal efficiency was not enhanced. Removal was enhanced by the presence of accessible surface area: NP-400 dispersed in these surfaces readily adsorbed NO, aided by the large surface areas of the top and bottom faces. Scanning electron microscopy coupled with energy dispersive X-ray analysis (SEM⁻EDX) confirmed that NP-400 tended to sink when added to cement, fine aggregates, and water because the true densities of P-25, NP-400, and cement powder differed (3.41, 3.70, and 3.15 g/mL). The true density of NP-400 was thus the highest of all ingredients. The relatively low apparent density of P-25 compared to that of NP-400 was associated with a more bulky distribution of P-25 within cementitious materials. Nevertheless, NP-400 could be a viable alternative to the definitive product, P-25.

We have investigated the effect of reaction temperature of hot-injection method on the structural properties of CsPbX3 (X: Br, I, Cl) perovskite nanocrystals (NCs) using small- and wide-angle X-ray scattering. It is confirmed that the size of the NCs decreased as the reaction temperature decreased, resulting in stronger quantum confinement. The cubic-phase perovskite NCs formed despite the fact that the reaction temperatures increased from 140 to 180 °C; however, monodispersive NC cubes that are required for densely packing self-assembly film were formed only at lower temperatures. From the X-ray scattering measurements, the spin-coated film from more monodispersive perovskite nanocubes synthesized at lower temperatures resulted in more preferred orientation. This dense-packing perovskite film with preferred orientation yielded efficient light-emitting diode (LED) performance. Thus the dense-packing structure of NC assemblies formed after spin-coating should be considered for high-efficient LEDs based on perovskite quantum dots in addition to quantum confinement effect of the quantum dots.

In this study, the synthesis of porous titania-silica (TiO2-SiO2) composite aerogel at ambient pressure by using non-hazardous chemicals as a source of silica was investigated. TiO2-SiO2 composite aerogels were characterised and their photocatalytic performances were investigated for the removal efficiency of acetaldehyde and NO(x) under UV light. Results showed that porous composite aerogel with aggregated morphology, high surface area and an increased mesoporosity were formed. TiO2-SiO2(1.8) composite, with high Ti/Si ratio, showed the best results in terms of photocatalytic removal of acetaldehyde and nitrogen oxide.

Potassium polytitanate nanofibres prepared by a hydrothermal method were investigated for their possible application in removing toxic metals from aqueous solution. Particular attention was paid to employing the titanate as a novel effective adsorbent for the removal of Pb(II). Batch adsorption experiments demonstrated that the adsorption was influenced by various conditions such as solution pH, adsorbent dosage and initial Pb(II) concentration. The results showed that the adsorption rate was faster in the first 5 min and equilibrium was achieved after 180 min. The maximum amount of adsorption was detected at pH 5. Potassium titanate showed much higher adsorption capacity compared to P25. The kinetic studies indicated that the adsorption of Pb(II) onto titanate best fit the pseudo-second-order kinetic model. FTIR spectra revealed that the hydroxyl groups in titanate were responsible for Pb(II) adsorption. The principal mechanism of the adsorption of Pb(II) in the present study is attributed to both ion exchange and oxygen bonding. The adsorption-desorption results demonstrated that the titanate could be readily regenerated after adsorption. Therefore, the present titanate exhibits great potential for the removal of Pb(II) from wastewater.

PURPOSE: The prevalence of antibiotic-resistant bacteria on rectal swabs in patients undergoing transrectal ultrasound (TRUS)-guided prostate biopsy and the factors affecting resistance to antibiotics were evaluated. MATERIALS AND METHODS: Two hundred twenty-three men who underwent TRUS-guided prostate biopsy from November 2011 to December 2012 were retrospectively evaluated. Rectal swabs were cultured on MacConkey agar to identify antibiotic-resistant bacteria in rectal flora before TRUS-guided prostate biopsy. All patients were admitted and received intravenous antibiotics before prostate biopsy. Clinical variables including underlying disease, infectious complications, and antibiotics associated with resistance were evaluated. Logistic regression was used to determine the factors influencing antibiotic resistance. RESULTS: Of the 233 patients, 161 had positive rectal cultures. Escherichia coli was cultured in 130 (80.7%) and Klebsiella pneumonia in 16 (9.9%). The prevalence of quinolone resistance was 16.8% and the prevalence of extended-spectrum beta-lactamase (ESBL) positivity was 9.3%. A previous history of prostatitis was correlated with quinolone resistance and ESBL positivity (both p=0.001). The factors affecting quinolone resistance in the univariate analysis were a previous history of prostatitis (p=0.003) and previous exposure to antibiotics (p=0.040). Only a previous history of prostatitis was statistically significant in the multivariate analysis (p=0.014). Four patients had infectious complications. CONCLUSIONS: The prevalence of quinolone resistance was 16.8% in rectal swabs performed before TRUS-guided prostate biopsy. A previous history of prostatitis was influential. In patients with a history of prostatitis, selection of prophylactic antibiotics before the biopsy may be reconsidered.

In this study, titania photocatalyst was produced from the wastewater sludge of Ti-salt flocculation. The high photocatalytic activity of titania reported was evaluated based on a single organic substrate. However, the photocatalytic activity is a substrate-specific; one photocatalyst showed different photocatalytic degradation rates for different substrates. Thus, to investigate the substrate-specific photocatalytic activity of titania, various substrates of humic acid (HA), dichloroacetic acid (DCA), rhodamine B (RhB), metsulfuron methyl (M&M) and phenol were used under simulated solar light irradiation. Results showed titania had a high activity rate for RhB, moderate activity for HA and DCA and low activity for M&M and phenol indicating substrate-specific activities. When compared with Degussa-P25, titania showed higher activity for M&M, while the opposite was observed for HA and phenol. The specific-substrate behaviour of titania could depend on specific physicochemical and electronic interactions between titania, substrates, and their intermediates compounds formed.

The main drawback of flocculation process with dye wastewater is the large amount of unrecyclable sludge which needs disposal. A novel process using Ti-salt flocculation to purify wastewater was developed to produced sludge that can be calcined to produce titania. In this study, iron-doped TiO2 nanomaterial was successfully produced from sludge obtained by the flocculation of dye wastewater with a composite floculant including TiCl4 and FeSO4. The titania was characterised using scanning electron microscopy (SEM/EDX), transmission electron microscopy (TEM), X-ray diffraction (XRD), and the photodecomposition of acetaldehyde. The XDR results showed that the anatase and rutile structures were found after sludge calcination at 550 degrees C and 640 degrees C respectively. The elemental analyses were carried out using EDX. The rutile titania sample consisted of Ti (35.7 wt.%), Fe (14.7 wt.%), O (42.3 wt.%), P (2.6 wt.%) and Ca (4.7 wt.%). The photocatalytic activity was monitored for the photodecomposition of gas acetaldehyde. Iron-doped titania seems to play an important role in increasing the photocatalytic activity under UV light irradiation.

In this study, the removal of phosphorous (P) using Ti-salt flocculation of biologically treated sewage effluent (BTSE) was investigated for a year. The pH, alkalinity and concentration of P, before and after Ti-salt flocculation, were measured and compared. The sludge of Ti-salt flocculation was incinerated at 600 degrees C to produce titania nanoparticles which found to be doped with P Titania nanoparticles were characterised and their photocatalytic activity under UV light irradiation were also tested. Results indicated that the removal of P, which exceeded 97% in average, was not affected by the pH and the alkalinity of BTSE. The concentration of P in titania had no effect on the characteristics of titania nanoparticles in different seasons. Titania nanoparticles exhibited superior properties in terms of BET surface area and photocatalytic activity.

Dye wastewater flocculation using TiCl4 and FeSO4 coagulants was studied for organic removal and precipitation efficiency. Moreover, TiCl4 was assessed in a pilot-scale flocculation process to investigate organic removal and solution pH effect. A large amount of TiO2 was produced from sludge of Ti-salt flocculation in dye wastewater. This flocculation process simultaneously reduces a large amount of sludge produced from wastewater. The TiO2 nanoparticle was characterized in terms of physical and chemical properties. Results showed that 77.6% of organic matter was removed from dye wastewater when using FeSO4 as coagulant, while TiCl4 degraded 75.9% of organic matter. On the other hand, floc precipitation efficiency was better for TiCl4 if compared with FeSO4. The optimum concentration of TiCl4 for pilot-scale flocculation was found to be equal to 2.1 g/L. DWT (Dye wastewater titania) size was 15-20 nm, mainly doped with carbon atoms and exhibited a dominant anatase structure. DWT was similar to P-25 in decomposing acetaldehyde under UV-irradiation and complete photocatalytic degradation was achieved after 140 min.
