RESUMEN

The aim of this paper was to confirm the efficacy of adrenocorticotropin depot (ACTH-depot) therapy in pregnancies with threatened miscarriage and preterm delivery through the desired stimulation of the adrenal glands controlled by the rest of organism. The activity of hypothalamic-pituitary-adrenal axis plays a key role in pregnancy. Such naturally stimulated endogenous corticosteroid hormones are free from unwanted side effects of their synthetics analogs. Low level of maternal blood ACTH and insufficient increase of induced by hypothalamic hormones oxytocinases (cystine-ß-aminopeptidases) were indication to ACTH-depot therapy (0.5 mg/week) in our consecutive prospective studies. Contrary to antenatal use of synthetic corticosteroids, there are no temporal limits of this therapy, which has to be more often recommended into clinical prevention of fetal morbidity, treatment of premature delivery, and finally elimination of the newborn's mortality caused by the neuroendocrinological gestoses.

Cancer appears in the new form of cell life as a direct consequence of self-organising pre-cancer cells (dissipathogenic systems), whose further existence is disabled by extreme impairment of their metabolism. Life itself is the highest value, since instead of necrosis or apoptosis, the cellular system in an unfavourable environment can change its genetic identity provided that the improvement in its own metabolism leads to increased chaos by higher dissipation of matter and energy in its environment. Prolongation of human life has resulted by a longer period of old age which is favourable for self-organisation of dissipative neoplastic cells. Modern medicine has explained the relation of the cervical cancer to preterm births and to inadequate use of oral contraceptives and/or replacement, instead of supplementary, neurohormonal therapy. Therefore, as early as in the period of pregnancy reproductive cells should be protected due to their prime importance in the intergenerational passage of life. Disturbance of systemic autoregulation causes development of dissipathogenic state of cells. A single zygote, whose environment is also important for the future development of the next two generations that are initiated with its formation, defines the unique identity of each person, whose life is determined by free will and neoplasms.

Cervical cancer can be not only prevented, but also effectively treated. Decreased efficiency of biochemical, neurohormonal and/or immunological mechanisms leads to infectious states which, irrespective of their bacterial, viral or parasitic aetiology, are only the necessary, but not the sufficient causes of neogenesis. The cause of cancer is the natural and common phenomenon of the self-organization of systems, endangered by ending of their existence, into more efficient time-space structures at the expense of their surrounding. Infected cells or infectiously changed tissues in their final phase of existence are often recognized as a precancerous state, but their genome does not differ from other organism cells, and that is why the carcinogenesis can still be prevented by direct fighting of pathogenic microorganisms, and indirectly by strengthening the body by neurohormonal therapy or vaccine immunopotentialization. Primary prophylaxis of neoplasms requires that not only the dissipathogenic state of cells be prevented, but also their tissue surrounding be normalized to head off the risk of the self-organization of neoplastic forms of life, differing in their genetic identity from the surrounding cells. Lactovaginal immunopotentialization complements the conservative and operative methods of oncological treatment, as well as has prophylactic application in women with the history of miscarriages, premature deliveries, lack of or significantly shortened lactation, neurohormonal menstruating disorders, chronic and recurrent inflammations of the reproductive organs, long-term hormonal contraception and hormone replacement therapy during menopause, or only deficiency of Lactobacillus vaginalis, as indicators of risk of cervical cancer.

RESUMEN

Oncology--just as every field of medicine that deals with etiology, diagnostics, pathomechanism and treatment of diseases--is only a part of the general human knowledge, whose all significant achievements must be used to protect human health. This pursuit has as its object not only the benefits form practical discoveries (L. Pasteur, W.C. Roentgen, P. Curie and M. Sklodowska-Curie, V. Schally etc.), but also theoretical generalizations (A. Einstein, W.K. Heisenberg and I. Prigogine). Unfortunately it is the lack and/or slow adaptation of that information, that is responsible for the still unsatisfactory progress in clinical oncology. Responsibility rests not only with oncologists, but primarily with editors of medical journals and textbooks, who have a moral duty to follow the entire general knowledge, especially in the field of the basic research. On the basis of an analysis of the contents of the Polish oncology textbooks and materials from the specialist conferences in gynaecologic oncology, they were found to: 1. Omit the current, particularly domestic literature, 2. Contain mostly works, whose conclusions are textbook information, 3. Rarely include studies in the area of medical thermodynamics, 4. Attempt to explain the effects of the modern technologies, e.g. fotodynamics or nanotechnology using theoretical generalizations which are inadequate for them, and 5. Disregard the rule primum non nocere not only in prevention but even in the treatment of neoplasms. Neoplastic disease has many conditionings and types because of the unique identity of the neoplasms which cause it and which are caused by universal and natural phenomena of the self-organizing dissipative structures. It requires not only early diagnosing but also causative treatment already in the precancerous states, which are better detected by modern methods based on the quantum thermodynamics (lasers, fotodynamics, nuclear magnetic resonance, genetic nanotechnology etc.).

OBJECTIVES: Newborns of both high and low body weight, as well as of higher or lower gestational age and length are born within the wide range of the six weeks of the biological norm. The correlation between the fetal mass and height is high, however the correlation of those two basic features with the fetal maturity is half as high. In view of the above, in order to verify the proper time of pregnancy ending, particularly if it is an instrumental one, it is necessary to assess the newborn's maturity index, which is also an indicator of the mother's health. Consequently, the index has a prognostic value for assessment of both the mother's and the infant's morbidity. MATERIALS END METHODS: Human maturation process was evaluated by means of at least two ultrasonographic measurements of fetal parameters < or = 28th week to determine an individual birth term in 2500 cases, construed as the day when the fetus reaches full maturity to self-dependent life and the mother becomes ready for the labor. Regardless of the method of assessment, the most important criterion of the sufficient pregnancy duration is the degree of the fetal maturity assessed immediately after the labor, i.e. Klimek's score (K) during the routine neonatal evaluation accompanied by the assessment of the newborn infant's adaptive efficiency expressed in the popular Apgar score (A). RESULTS AND CONCLUSIONS: It was found that the point count in K score is a more sensitive index of the newborn infant's wellbeing than the A score. Fetal development in the mother's womb progresses according to the auxological laws, to which the K score point-count conforms, unlike the A score. This study summarizes the latest studies on this subject in infants born both from physiological and pathological labors.

RESUMEN

Apgar score remains permanent conquest of medicine due its objective evaluation of available point counting of the level and quickness of newborn's breathing-circulation system adaptation to the new environment. For its example the score of fetal and newborn's maturation evaluation has been described. The matter is in this so that Apgar score don't be using for evaluation of fetal maturity because it is easy to determine it directly. Of course, the higher fetal maturity level the higher probability of high Apgar score, and the quicker and better post-partum child's adaptation for independent life in new circumstances takes place.

Labour is the most important caesura in the life of every human being, as it is the ultimate test of the child's fetal maturity to self dependent life, whose onset is connected with a violent change in the circulatory system and the commencement of breathing. Maturity assessment scale comprises such features as, among others: posture, angle forearm, pulling an elbow to the middle line of the body, distribution of lanugo, plantar creases, breast development. They can be objectified through their technical quantification the same way as we give the appropriate number of grams and centimeters when assessing the weight and length of the newborn. However, two equally mature fetuses call differ in mass by 800g, body length by 6 cm, and gestational age by 6 weeks. Relativity of pregnancy duration clearly demonstrates that only the quantitativization of maturity enabled its prenatal assessment on the basis of the rate of increase of the spatial parameters of the baby developing in the mother's womb. The date of the end of pregnancy is determined depending equally on the child's fetal maturity and the mother's readiness to birth, where the placenta plays an important role, as its existence coincides with the individual duration of every pregnancy. The use of industrially synthesized hormones means that those pharmaceuticals contain the whole mixture of side-products of the synthesis of the hormones. The biologically alien corticoid analogues like dexamethasone and betamethasone must, when exerting prolonged action, cause unwanted side effects. In contrast the ACTH-depot administration leads to desired production and secretion of corticoids through the control of the whole body function of the adrenals. Such naturally stimulated endogenous steroid hormones are free from unwanted side effects of drugs from the group of their synthetic analogues. An enormous body of evidence supports the link between the administration of corticotrophin and fetal body mass, maturity and age of neonates.
