RESUMEN

Detection of Mycoplasma hominis and Ureaplasma urealyticum in the samples obtained from the cervical canal of the uterus and from the pouch of Douglas in 71 women in whom underwent diagnostic and operative laparoscopy is presented. Ureaplasma urealyticum in 33 cases diagnosed for infertility was present in 6 (18.2%) patients. In cervical canal and the pouch of Douglas in 5 and 2 women respectively. In 1 (3.0%) woman Ureaplasma urealyticum was present in both places. In the women from the control group U. urealyticum was detected in 6 (15.5%) cases only in cervical canal. Mycoplasma hominis was present only in cervical canal more frequently in the control group 4 (cases) than among infertile women 1 (3.0%).

RESUMEN

The results of microbiological examination of samples from cervical canal of the uterus and from pouch of Douglas in 71 women who underwent diagnostic and operative laparoscopy is presented. In 33 cases diagnosed because of infertility Chlamydia trachomatis was present in 2 (6.1%) women and in one woman both in cervical canal and pouch of Douglas. In second woman only in cervical canal Chlamydia trachomatis was present. There was not statistically significant correlation between control and study groups.

RESUMEN

DESIGN: 64 women, with PCOS underwent treatment by bilateral ovarian wedge resection, during laparotomy, or by tunica albuginea electrocauterisation under laparoscopic control were examined. RESULTS: In treated women 2 losses of obtained 25 pregnancies were observed. After bilateral ovarian wedge resection were observed 17 pregnancies (50%), and after laparoscopic ovulation induction were observed 8 pregnancies (27%). One pregnancy was unsuccessfully terminated by premature labor, three pregnancies were terminated by caesarean section. CONCLUSION: Abortion rates and rates of pathology of delivery in women with PCOS is not elevated compared with that of non-PCOS subjects.

RESUMEN

OBJECTIVES: The early pregnancy disorders are most frequent diagnostic problem in a GP-practice. This observation and commonness of the problem inclined the authors to assume eutopic physiological pregnancy, eutopic complicated pregnancy, ectopic pregnancy and trophoblastic diseases as important diagnostic hypotheses for our knowledge based consultation system, intended as decision support tool for first contact physician (GP) when diagnosing early pregnancy disorders. DESIGN: Defining 81 diagnostic tests to be included in the knowledge acquisition process, interviewing clinical experts and developing knowledge base (set of clinical diagnostic rules), implementing the knowledge base for early pregnancy disorders--related amenorrhea patients, in particular: determining clinical importance of symptoms and lab/diagnostic tests. MATERIALS AND METHODS: The clinical knowledge acquisition process has been performed taking into account 81 diagnostic features. Moreover clinical data on 220 cases, including 96 early pregnancy disorders cases, have been collected to enable validation of the knowledge base. ELSA approach to creating environment-self-adjustable medical knowledge bases, developed by Ruszkowski has been applied and the eleven main diagnostic hypotheses have been defined. We believe that the proposed knowledge base and PC-based consultation system offers such an approach for improving GP diagnostic effectiveness. RESULTS AND CONCLUSIONS: 1. The knowledge base and the consultation system could contribute to improving diagnostic effectiveness, when used by a GP for diagnosing early pregnancy disorders such an approach could also be used to determine relative diagnostic power of specific tests or symptoms, 2. The laboratory and diagnostic test confirmed high diagnostic accuracy in early pregnancy disorders. The anamnesis has a low diagnostic effectiveness for particular diagnostic features in ectopic pregnancy patients and physical examination confirmed low diagnostic accuracy in eutopic physiological pregnancy and eutopic complicated pregnancy.

RESUMEN

Authors described a case of the endometriosis presence in the bladder wall in the patient who underwent the caesarean section. In this patient the appropriate surgery was executed. The bladder wall fragment containing the endometriosis focus was cut out. The period after surgery was uncomplicated. As the complementary efficace treatment the pharmacotherapy with Danol-Danazol was administered.

The concentration of haptoglobin in sera of healthy women and patients with non-malignant and malignant ovarian tumours was measured by two methods, i.e. complex formation with haemoglobin and complex formation with concanavalin A. High correlation (r = 0.74) between both the methods was found in the group of healthy women, but correlation coefficients were much lower in the group of non-malignant and malignant tumours (r = 0.42 and 0.37, respectively). Direct determinations of haptoglobin, and calculation of the ratio of haptoglobin bound to concanavalin A to haptoglobin bound to haemoglobin revealed statistically significant differences among the examined groups. Comparison of two methods of haptoglobin quantitation suggest that processes connected with ovarian disorders may alter the glycosylation of haptoglobin oligosaccharide chains.

In the serum of 21 women with malignant ovarian tumours, 13 with uterine myomas and 16 with cysts the concentrations were determined of alpha 1 inhibitor of proteases and alpha 2 macroglobulin by immunodiffusion, and the functional activity of both inhibitors which made possible calculation of the specific activity (units of inhibitory activity per one gram of inhibitor protein). In women with malignant tumours the specific activity of alpha 1 inhibitor was decreased, suggesting that a part of the inhibitor was functionally inactive. This decrease was statistically significant in relation to the control group and the groups of myomas and cysts, but was independent of the degree of disease progression. The specific activity of alpha 2 macroglobulin was significantly decreased only in patients with grade IV degrees of clinical progression (according to FIGO) in relation to the control group and to the group with cysts.
