RESUMEN

AIM: To detect noise levels, generated by high-powered tools in orthopaedic theatres at varying distances from the operating site, and its impact on hearing in staff and patients. METHODS: Sound-level meter was used to measure the sound level generated by various high-powered tools in routine orthopaedic procedures, at varying distances from the operating site. These recorded noise levels were compared against the UK noise safety guidelines. RESULT: We found that the noise generated was often significantly higher than the safety guidelines. CONCLUSION: Noise levels in orthopaedic theatres can be at unacceptable levels, which could potentially lead to hearing problems in the staff and patients. We believe that all patients, especially the elderly patients, (who are more prone to hearing problems) should have hearing protection during orthopaedic operations that use high-powered tools. Surgeons and scrubbed theatre staff should take precautions.

RESUMEN

We investigated the changes in surface roughness of retrieved femoral components in 18 men and four women at revision knee surgery. The mean age at revision was 68.4 years and the mean period of implantation was for 55.6 months. Eighteen implants were retrieved for aseptic loosening and four for infection. The surface changes in the articulating areas were inspected visually and the roughness (Ra) analysed with a profilometer. Parallel scratching and burnishing were the two main forms of damage. The mean Ra measurements in the articulating areas showed no statistically significant difference when compared with those in a control area on either side of the patellar groove at the apex of the femoral flange. This suggests that it is not essential to revise a well-fixed and correctly aligned femoral component where the polished surface has become burnished or bears fine parallel scratches, if the revision is conducted solely for failure of the tibial component.

RESUMEN

A case of giant ganglion cyst arising from the quadriceps femoris tendon, following total knee replacement is presented in this case report. The ganglion cyst was successfully excised. The formation of a ganglion cyst following total knee replacement has not been reported.

RESUMEN

[reaction: see text] This work describes the synthesis of two novel macrocyclic taxoid constructs by ring-closing olefin metathesis (RCM) and their biological evaluation. Computational studies examine conformational profiles of 1 and 2 for their fit to the beta-tubulin binding site determined by electron crystallography. The results support the hypothesis that paclitaxel binds to microtubules in a "T" conformation.
