BACKGROUND: The available evidence regarding the predictive value of troponins and natriuretic peptides for early postoperative outcomes in pediatrics is limited, controversial, and based on small sample sizes. We aimed to investigate the association of N-terminal pro B-type natriuretic peptide (NT-proBNP) and high-sensitivity troponin T (hs-TnT) with the in-hospital adverse outcomes after congenital cardiac surgeries. METHODS: A secondary analysis based on a prospective study of pediatric congenital heart disease (CHD) patients was conducted to investigate the association of NT-proBNP and hs-TnT tested within 6 hours postoperatively with in-hospital adverse events. A multivariate logistic regression analysis with a minimum P value approach was used to identify the optimal thresholds of NT-proBNP and hs-TnT for risk stratification. RESULTS: NT-proBNP and hs-TnT are positively correlated with cardiopulmonary bypass time, mechanical ventilation duration, and pediatric intensive care unit stay. The predictive performance of NT-proBNP is excellent for adverse events in both patients younger than 1 year [area under the curve (AUC): 0.771, 0.693-0.850] and those older than 1 year (AUC: 0.839, 0.757-0.922). However, hs-TnT exhibited a satisfactory predictive value solely in patients aged over 1 year. (AUC: 0.784, 0.717-0.852). NT-proBNP levels of 2000 to 10000 ng/L [Odds Ratio (OR): 3.79, 1.47-9.76) and exceeding 10000 ng/L (OR: 12.21, 3.66-40.80) were associated with a higher risk of postoperative adverse events in patients younger than 1 year. Patients older than 1 year, with NT-proBNP higher than 500 ng/L (OR: 15.09, 6.05-37.66) or hs-TnT greater than 1200 ng/L (OR: 5.50, 1.47-20.59), had a higher incidence of postoperative adverse events. CONCLUSIONS: NT-proBNP and hs-TnT tested within postoperative 6 hours demonstrated significant predictive value for postoperative adverse events in CHD patients older than 1 year. However, among CHD patients younger than 1 year, only NT-proBNP exhibited commendable predictive performance for postoperative adverse events.

BACKGROUND: Staged repair is common for complex conotruncal defects, often involving bidirectional Glenn (BDG) procedure. Following the cavopulmonary shunt, both Fontan completion and biventricular conversion (BiVC) serve as definitive approaches. The optimal strategy remains controversial. METHODS: The baseline, perioperative and follow-up data were obtained for all paediatric patients with conotruncal defects who underwent BDG procedure as palliation in Fuwai Hospital from 2013 to 2022. Patients with single ventricle were excluded. The primary outcome was mortality. The secondary outcome was reintervention, including any cardiovascular surgeries and non-diagnostic catheterisations. RESULTS: A total of 232 patients were included in the cohort, with 142 underwent Fontan (61.2%) and 90 underwent BiVC (38.8%). The median interstage period from BDG to the definitive procedure was 3.83 years (IQR: 2.72-5.42) in the overall cohort, 3.62 years (IQR: 2.57-5.15) in the Fontan group and 4.15 years (IQR: 3.05-6.13) in the BiVC group (p=0.03). The in-hospital outcomes favoured the Fontan group, including duration of cardiopulmonary bypass, aortic cross-clamp, mechanical ventilation and intensive care unit stay. Postoperative mortality was generally low and comparable, as was the reintervention rate (HR=1.42, 95% CI: 0.708 to 2.85, p=0.32). The left ventricular size was smaller at baseline and within the normal range at follow-up for both Fontan and BiVC groups; however, it was significantly larger with BiVC at follow-up. CONCLUSION: In paediatric patients with conotruncal heart defects who underwent BDG procedure, BiVC is a feasible option, especially for patients with certain Fontan risk factors, and are not ideal candidates for successful Fontan completion.

Background: The prevalence and risk factors for failure to thrive (FTT) in pediatric patients with congenital heart disease (CHD) remain ambiguous. We aimed to investigate the prevalence, growth profiles, risk factors, and vulnerable subtypes of CHD associated with FTT in pediatric patients with CHD. Methods: This was a cross-sectional study based on Chinese Database for Congenital Heart Surgery. FTT was defined as either stunting or underweight (height or weight standard deviation score <-2), and they were standardized by references of normal Chinese population. Risk factors was determined with logistic regression model, and growth profiles were delineated in each subgroup. Findings: A total of 13,256 CHD patients were included in this study, with 3994 patients of mild CHD, 7195 patients of moderate CHD and 2067 patients of complex CHD. The prevalence of stunting, underweight and FTT was 24%, 29.3% and 36.9%, respectively. Preoperative anaemia, left ventricle systolic dysfunction, younger age, more complex CHD types, lower birth weight and genetic syndrome were found to be the risk factors for FTT in CHD patients. Interrupted aortic arch was revealed to be the most severe group associated with FTT. Interpretation: FTT is ubiquitous in patients with CHD and exacerbated in high-risk subgroups. Our findings hinted the necessity of early identification and intervention for FTT in patients with CHD during daily practice of pediatrics, as it has the potential to improve outcomes and enhance their quality of life. Furthermore, we advocate for the initiation of prospective research with longitudinal data to comprehensively investigate the association between FTT and CHD across the lifespan. OBJECTIVE: To assess management strategies for pediatric patients with the challenging combination of congenital heart diseases (CHDs) and airway anomalies. STUDY DESIGN: Patients diagnosed with CHD and airway anomalies in the Pediatric Cardiac Surgery Centre of Fuwai Hospital from January 2016 to December 2020 were included in this retrospective study. Patients were divided into three groups based on different management, including the conservative group, the slide group (slide tracheoplasty), and the suspension group (suspension with external stenting). Patients' data and computed tomography measurements from medical records were reviewed. RESULTS: A total of 139 patients were included in the cohort; 107 had conservative airway treatment (conservative group), 15 had slide tracheoplasty (slide group), and 17 had tracheal suspension operation (suspension group). The top three associated intracardiac anomalies were ventricular septal defect (n = 34, 24%), pulmonary artery sling (n = 22, 16%), and tetralogy of Fallot (n = 15, 11%). Compared with patients with conservative airway management (100 minutes [median], 62-152 [IQR]), the extra airway procedure prolonged cardiopulmonary bypass duration, with 202 minutes (IQR, 119-220) for the slide group and 150 minutes (IQR, 125-161) for the suspension group. Patients who underwent slide tracheoplasty required prolonged mechanical ventilation (129 minutes [median], 56-328 [IQR]). Of the total cohort, 6 in-hospital deaths, all in the conservative group, and 8 mid-to long-term deaths, with 6 in the conservative group, occurred. CONCLUSIONS: Both conservative and surgical management of CHD patients with airway anomalies have promising outcomes. Extra tracheobronchial procedures, especially the slide tracheoplasty, significantly prolonged cardiopulmonary bypass duration. Based on multidisciplinary team assessment, individualized management strategies should be developed for these patients.

BACKGROUND: The modified L-shaped incision technique (MLIT) was successfully applied to the repair of supracardiac total anomalous pulmonary venous connection (TAPVC) with promising mid-term outcomes. It is, however, unclear whether or not MLIT could be an alternative to sutureless technique (ST). METHODS: All patients ( n =141) who underwent MLIT or ST repair for supracardiac TAPVC between June 2009 and June 2022 were included and a propensity score-matched analysis was performed to reduce the heterogeneity. RESULTS: MLIT was performed in 80.9% (114/141), whereas ST was performed in 19.1% (27/141). Patients who underwent MLIT repair had a lower incidence of pulmonary veinous obstruction (PVO)-related reintervention (1.8 vs. 18.5%, P =0.002), and late mortality (2.6 vs. 18.2%, P =0.006). Overall survival at 10 years was 92.5% (87.7-97.7%) for MLIT and 66.8% (44.4-100%) for ST ( P =0.012). Freedom from postoperative PVO at 10 years was 89.1% (83.2-95.5%) for MLIT and 79.9% (65.6-97.4%) for ST ( P =0.12). Cox proportional hazards regression identified prolonged mechanical ventilation duration, postoperative PVO, respiratory dysfunction, and low cardiac output syndrome were associated with postoperative death and PVO-related reintervention. CONCLUSIONS: The MLIT strategy is a safe, technologically feasible, and effective approach for supracardiac TAPVC, which is associated with more favorable and promising freedom from death and PVO-related reintervention.

Twenty agroforestry systems consisting of different management practices (conventional and organic) and shade types were set up for coffee plantations in 2,000 at the Tropical Agricultural Research and Higher Education Center (CATIE), Turrialba, Costa Rica. The physical (density, bulk density, moisture content, and roasting loss) and chemical attributes (mineral, total lipid, fatty acids, caffeine, and carbohydrate contents) of harvested green coffee beans were investigated. The full sun and Erythrina shade tree systems significantly improved (p < 0.05) the density of the green coffee beans and decreased (p < 0.05) the moisture content and roasting loss of the green coffee beans. The intensive organic (IO) management practice significantly increased some mineral contents, such as K, P, and Ca, in green coffee beans. The full sun system also significantly promoted (p < 0.05) some mineral contents, such as Ca and Mn, in green coffee beans. In terms of total lipid and fatty acids (FAs), compared with the moderate conventional (MC) management practice, the IO management practice was beneficial as it significantly increased (p < 0.05) the total lipid and FAs contents in the green coffee beans, while the Erythrina shade tree system significantly increased (p < 0.05) the total lipid and FAs contents of green coffee beans more efficiently than the other shade types. The caffeine content of green coffee beans was significantly higher (p < 0.05) under the intensive conventional (IC) and IO management practices than under the MC management practice and higher under the full sun system than under the shaded system. The Erythrina shade tree system significantly improved (p < 0.05) the carbohydrate content of green coffee beans. Overall, in consideration of sustainability, the IO management practice associated with the Erythrina shade tree system would be a useful combination for the local farmers to grow coffee trees.

BACKGROUND: This study focused on congenital heart disease (CHD) patients complicated with airway stenosis (AS) without airway intervention and aimed to identify the patients with potential risks. METHODS: Patients diagnosed with CHD and AS were enrolled in this retrospective study. The primary outcome was defined as a postoperative mechanical ventilation duration of more than two weeks. We constructed a prediction model to predict the risk of prolonged mechanical ventilation (PMV). RESULTS: A total of 185 patients diagnosed with CHD and AS in Fuwai Hospital from July 2009 to December 2022 were included in the study. Weight at CHD surgery, cardiopulmonary bypass (CPB) duration, complex CHD and comorbid tracheobronchomalacia were identified as risk factors and included in the model. The ROC curve showed a good distinguishing ability, with an AUC of 0.847 (95% CI: 0.786-0.908). According to the optimal cut-off value of the ROC curve, patients were divided into high- and low-risk groups, and the subsequent analysis showed significant differences in peri-operative characteristics and in-hospital deaths. CONCLUSIONS: With the predictive model, several factors could be used to assess the risky patients with PMV. More attention should be paid to these patients by early identification and routine surveillance.

Soil contamination by polycyclic aromatic hydrocarbons (PAHs) is an increasing problem in many countries, impacting the ecological environment's sustainable development. This study investigated the effects of fluoranthene (Fla) on soil aggregate stability. A possible mechanism for the interaction of Fla with soil aggregates was proposed by characterizing the aggregate structure. The results showed that Fla significantly improved the aggregate stability in the concentration range of 0-30.0 mg/kg. The content of macro-aggregates reached the maximum value at 10 mg/kg of Fla, which increased by 24.25% compared with the control group, while the content of large-aggregates decreased by 12.11%. Meanwhile, the mean weight diameter (MWD) and geometric mean diameter (GMD) increased by 56.63% and 37.66%, respectively. However, the macro-aggregates zeta potential value and specific surface area (SSA) decreased by 12.68% and 13.61%, respectively. The cracks of macro-aggregates were also significantly reduced. In addition, Fla-based free radicals were detected on the macro-aggregates. The absorption peak of the C-O group significantly increased, indicating that Fla may be covalently bound to the aggregates by aromatic ether bonds, which is a possible mechanism for the interaction between Fla and aggregates. This study provides theoretical support for revealing the effects of PAHs on soil.

ABSTRACT Introduction: The research and development of new methods of injury diagnosis and rehabilitation training in benefits to the health and training of athletes is a contemporary priority. Among modern techniques, diagnostic imaging technology stands out. Objective: Study the auxiliary effect of imaging technology in the diagnosis and rehabilitation training of functional knee joint injuries in basketball practice. Methods: 50 basketball players diagnosed and treated in the past four years for knee joint function injuries in a special hospital were selected, including 28 male and 22 female athletes, aged 17 to 25 years. PerkinElmer Spotlight-300 FTIR spectral imaging system was used to scan the articular cartilage sections, visible light and total absorption images of the samples were obtained by reflection mode imaging. Results: The peak infrared feature of the articular cartilage measured by the infrared spectrum imaging technology is very consistent with the FTIR spectrum. After rehabilitation training, the Y test score of female patients increased from 96.72 ± 6.05 to 105.40 ± 4.23, and the Y test score of male patients increased from 98.34 ± 5.33 to 105.51 ± 4.89, showing significant differences. Conclusion: Infrared spectrum imaging technology is an effective technique for identifying functional knee joint injury, and is of great use for diagnosis and training update in basketball players with knee trauma. RESUMO Introdução: A pesquisa e o desenvolvimento de novos métodos de diagnóstico de lesões e treinamento de reabilitação em benefícios para a saúde e o treinamento dos atletas é uma prioridade contemporânea. Entre as técnicas modernas, destaca-se o diagnóstico por tecnologia de imagem. Objetivo: Estudar o efeito auxiliar da tecnologia de imagem no diagnóstico e treinamento de reabilitação das lesões funcionais das articulações do joelho na prática do basquetebol. Métodos: Foram selecionados 50 jogadores de basquetebol diagnosticados e tratados nos últimos quatro anos por lesões da função articular do joelho em um hospital especial, incluindo 28 atletas masculinos e 22 atletas femininos, com idade entre 17 e 25 anos. O sistema de imagem espectral PerkinElmer Spotlight-300 FTIR foi usado para escanear as seções de cartilagem articular, a luz visível e as imagens de absorção total das amostras foram obtidas através da imagem em modo de reflexão. Resultados: O pico da característica infravermelha da cartilagem articular medida pela tecnologia de imagem de espectro infravermelho é muito consistente com o espectro FTIR. Após o treinamento de reabilitação, o escore do teste Y de pacientes do sexo feminino aumentou de 96,72 ± 6,05 para 105,40 ± 4,23, e o escore do teste Y de pacientes do sexo masculino aumentou de 98,34 ± 5,33 para 105,51 ± 4,89, mostrando diferenças significativas. Conclusão: A tecnologia de imagem de espectro infravermelho é uma técnica eficaz para identificar a lesão funcional da articulação do joelho, sendo de grande utilidade para o diagnóstico e atualização de treinamento em praticantes de basquetebol com traumatismos no joelho. RESUMEN Introducción: La investigación y el desarrollo de nuevos métodos de diagnóstico de lesiones y entrenamiento de rehabilitación en beneficios para la salud y el entrenamiento de los deportistas es una prioridad contemporánea. Entre las técnicas modernas destaca el diagnóstico por tecnología de imagen. Objetivo: Estudiar el efecto auxiliar de la tecnología de imagen en el diagnóstico y entrenamiento de rehabilitación de lesiones funcionales de la articulación de la rodilla en la práctica del baloncesto. Métodos: Se seleccionaron 50 jugadores de baloncesto diagnosticados y tratados en los últimos cuatro años por lesiones funcionales de la articulación de la rodilla en un hospital especial, incluidos 28 atletas de sexo masculino y 22 de sexo femenino, con edades comprendidas entre los 17 y los 25 años. Se utilizó el sistema de imágenes espectrales FTIR PerkinElmer Spotlight-300 para escanear las secciones de cartílago articular, y se obtuvieron imágenes de luz visible y de absorción total de las muestras mediante imágenes en modo de reflexión. Resultados: El pico infrarrojo del cartílago articular medido mediante la tecnología de imagen de espectro infrarrojo es muy coherente con el espectro FTIR. Tras el entrenamiento de rehabilitación, la puntuación de la prueba Y de las pacientes femeninas aumentó de 96,72 ± 6,05 a 105,40 ± 4,23, y la puntuación de la prueba Y de los pacientes masculinos aumentó de 98,34 ± 5,33 a 105,51 ± 4,89, mostrando diferencias significativas. Conclusión: La tecnología de imágenes del espectro infrarrojo es una técnica eficaz para identificar lesiones funcionales de la articulación de la rodilla, y es de gran utilidad para el diagnóstico y la actualización del entrenamiento en jugadores de baloncesto con traumatismo de rodilla. A long experiment has been established since 2000 at CATIE (Tropical Agricultural Research and Higher Education Center), Turrialba, Costa Rica. Twenty agro-forestry systems with different shade types and managements (organic and non-organic) consisting of an incomplete randomized block-design with shade tree as main effect and subplots represented by management were set up. The effects of different managements and shade types on the aroma and color generation of roasted coffee beans were investigated. The total protein content was significantly higher (P < 0.05) under the intensive conventional (IC) (168 g/Kg) and intensive organic (IO) (167 g/Kg) managements than under the moderate conventional (MC) (153 g/Kg in IC vs. MC group, 157 g/Kg in MC vs. IO group). Comparing with the moderate conventional (MC) management, the intensive organic (IO) management had a stronger ability to generate more flavor and color. The total protein content was significantly higher (P < 0.05) under the full sun system (172 g/Kg) than under the shaded (159 g/Kg) and Erythrina system (155 g/Kg), under the service system (165 g/Kg) than under the timber system (146 g/Kg), under the legume timber system (170 g/Kg) than under the non-legume timber system (152 g/Kg). The full sun system had a greater flavor generation and color after roasting. Comparing with the timber system, the service system produced roasted beans with the more flavor and color. Comparing with the non-legume shade tree, the legume shade tree improved the performance of flavor and color in the roasted coffee beans.

Exosomes are small vesicles derived from cells used as cell-to-cell communication goods in numerous diseases including tumorigenesis, neurological diseases, cardiovascular diseases and other diseases. Circular RNAs (circRNAs) are an innovative constituent of non-coding endogenous RNAs generated through backsplicing, catalyzed by RNA polymerase Ⅱ. These non-coding RNAs have been suggested to control gene expression through miRNA sponging, RNA-binding protein regulation and translational capabilities. Genome-wide RNA sequence analyses observed that circRNAs were stably improved in exosomes in association to parental cells. Little attention has been dedicated to exosomal circRNAs (exo-circRNAs). However, research has demonstrated that exo-circRNAs may have important regulatory functions because of their stability in cells and within exosomes. If well understood, the precise roles and mechanisms of exo-circRNAs might surge the impending clinical applications of these molecules as markers in the identification, prediction and treatment of various diseases. In this review, we outline recent findings regarding exo-circRNAs which includes their functions and highlights their potential applications and therapeutic targets in human diseases.

Gliomas are a type of malignant central nervous system tumor with poor prognosis. Molecular biomarkers of gliomas can predict glioma patient's clinical outcome, but their limitations are also emerging. C-X-C motif chemokine ligand family plays a critical role in shaping tumor immune landscape and modulating tumor progression, but its role in gliomas is elusive. In this work, samples of TCGA were treated as the training cohort, and as for validation cohort, two CGGA datasets, four datasets from GEO database, and our own clinical samples were enrolled. Consensus clustering analysis was first introduced to classify samples based on CXCL expression profile, and the support vector machine was applied to construct the cluster model in validation cohort based on training cohort. Next, the elastic net analysis was applied to calculate the risk score of each sample based on CXCL expression. High-risk samples associated with more malignant clinical features, worse survival outcome, and more complicated immune landscape than low-risk samples. Besides, higher immune checkpoint gene expression was also noticed in high-risk samples, suggesting CXCL may participate in tumor evasion from immune surveillance. Notably, high-risk samples also manifested higher chemotherapy resistance than low-risk samples. Therefore, we predicted potential compounds that target high-risk samples. Two novel drugs, LCL-161 and ADZ5582, were firstly identified as gliomas' potential compounds, and five compounds from PubChem database were filtered out. Taken together, we constructed a prognostic model based on CXCL expression, and predicted that CXCL may affect tumor progression by modulating tumor immune landscape and tumor immune escape. Novel potential compounds were also proposed, which may improve malignant glioma prognosis.

RESUMEN

Circular RNAs (circRNAs) are a class of non­coding RNAs with a circular, covalent structure that lack both 5' ends and 3' poly(A) tails, which are stable and specific molecules that exist in eukaryotic cells and are highly conserved. The role of circRNAs in viral infections is being increasingly acknowledged, since circRNAs have been discovered to be involved in several viral infections (such as hepatitis B virus infection and human papilloma virus infection) through a range of circRNA/microRNA/mRNA regulatory axes. These findings have prompted investigations into the potential of circRNAs as targets for the diagnosis and treatment of viral infection­related diseases. The aim of the present review was to systematically examine and discuss the role of circRNAs in several common viral infections, as well as their potential as diagnostic markers and therapeutic targets.

In this paper, we successfully constructed a ternary metal/non-metal nanomaterial which can synergize Z-scheme heterojunction and plasmonic metal/semiconductor for enhancing visible light photoreduction of aqueous Cr(VI). The as-formed ternary metal/non-metal nanomaterial was composed of g-C3N4 nanosheets mutual modified by Ag nanoparticles and Bi4O7. In the ternary nanomaterial, g-C3N4 nanosheets and Bi4O7 fabricated Z-scheme heterojunction, g-C3N4 nanosheets and Ag nanoparticles formed plasmonic metal/semiconductor system, synchronously. The photodeposition experiments were designed to indirect demonstrate the Z-scheme heterojunction via photocatalytic oxidation and reduction deposition reactions, AgNO3 was used as the electron acceptor and Pb(NO3)2 as the electron donor, photocatalytic reduction product and oxidation product were selectively deposited on g-C3N4 nanosheets and Bi4O7, respectively, revealing the Z-scheme heterojunction was definite formed. In addition, X-ray photoelectron spectroscopy (XPS), photoluminescence (PL), UV-vis absorption, and Fourier-transform infrared spectroscopy (FTIR) were studied to indicate that the plasmonic metal/semiconductor system was existent in the ternary nanomaterial. The visible light photoreduction of ternary Ag/Bi4O7/g-C3N4 nanosheets was evaluated by photoreducing aqueous Cr(VI). The photoreduction results show that the ternary Ag/Bi4O7/g-C3N4 nanosheets is much higher than the pristine bulk g-C3N4 and g-C3N4 nanosheets.

An electrochromic device with the as-obtained nanoporous NiO /ZnO nanoarray as a working electrode was constructed and assembled. The nanoporous NiO/ZnO nanoarray film with a three-dimensional structure was prepared on indium tin oxide (ITO) glass substrate through a two-step route that combined chemical bath deposition method with a hydrothermal method. The nanoporous NiO/ZnO nanoarray electrode reveals a noticeable improvement in electrochromism compared with that of nanoporous NiO alone, including higher optical modulation (81 %), higher coloration efficiency (78.5 cm2 C-1 ), faster response times (2.6 and 9.7 s for coloring and bleaching, respectively), and favorable durability performance. Such enhancements are mainly attributed to the three-dimensional structures of nanoporous NiO coated on ZnO nanoarray, namely, 1) the uniform hexagonal ZnO nanoarray loads more nanoporous NiO, 2) nanoporous NiO cross-linked with ZnO nanorods provides a loose interspace morphology, 3) stronger adhesion between ZnO nanorods and ITO covered with ZnO seed, 4) core-shell and cross-linked structures promote electrolyte infiltration, and 5) appropriate band gaps improve charge transfer.

OBJECTIVE: To observe the effect of solar infrared ray (IR) radiation on the expressions of c-Jun and collagens I and III in cultured human skin fibroblasts (HSFs) and explore the molecular mechanism by which IR radiation causes aging of the skin. METHODS: Primarily cultured HSFs exposed to IR radiation were examined for changes of the cell viability with MTT assay. The mRNA and protein expressions of c-Jun and collagens I and III was detected with real-time quantitative PCR and immunocytochemistry. RESULTS: MTT assay showed that IR irradiation caused inhibition of cell proliferation compared with the control cells. The mRNA and protein expression of collagen I was decreased significantly by IR irradiation with the increase of the irradiation dose (P<0.01). HSFs irradiated by IR for 12 h showed a dose-dependent reduction of the expression of collagen type III mRNA and protein (P<0.05, P<0.01), but the expression increased dose-dependently in response to IR exposure for 24 h (P<0.05 or 0.01). IR irradiation enhanced the mRNA and protein expression of c-Jun in a dose-dependence manner (P<0.05 or 0.01). CONCLUSIONS: IR irradiation can increase the expression of c-Jun, inhibit the expression of collagen I, and cause disturbance in collagen III expression in human skin fibroblasts, which may be one of the mechanism of IR radiation to initiate and promote skin photoaging.
