RESUMEN

PURPOSE: To establish a quantitative relationship between the Blackburne-Peel index and posterior tibial slope in both skeletally mature and skeletally immature individuals and to evaluate the rate at which variation in tibial slope influences changes in patellar height categorization as normal, patella alta, and patella baja. METHODS: A consecutive series of lateral knee radiographs were retrospectively reviewed. Radiographs were excluded for rotation, inadequate visible proximal tibia length, and obstructive hardware/pathology. Modified tibial slopes of 0°, 5°, 10°, and 15° were projected anteriorly from the medial tibial plateau as described by Blackburne-Peel. The Blackburne-Peel index was determined at each modified tibial slope interval. Caton-Deschamps and Insall-Salvati indices also were measured for comparison. The rate of Blackburne-Peel index change with increase in posterior tibial slope was quantitatively analyzed. RESULTS: Fifty skeletally mature and 50 skeletally immature radiographs were included. In the skeletally mature, Blackburne-Peel indices decreased on average by 0.037, 0.044, and 0.049 as posterior tibial slope increased from 0-5°, 5-10°, and 10-15°, respectively. In the skeletally immature, Blackburne-Peel indices decreased on average by 0.045, 0.053, and 0.059 as posterior tibial slope increased from 0-5°, 5-10°, and 10-15°, respectively. Overall, 29 individuals with 0° of tibial slope were categorized as patella alta by the Blackburne-Peel index, and only 16 (55%) remained categorized as patella alta after increasing their posterior tibial slope to 15°. CONCLUSIONS: This study quantitatively demonstrates the relationship between posterior tibial slope and the Blackburne-Peel index. As expected, as posterior tibial slope increases, the Blackburne-Peel index decreases. While the change in the Blackburne-Peel index per 5° change in tibial slope appears to be small, nearly half (45%) of individuals categorized as patella alta with 0° of tibial slope were categorized as normal when their posterior tibial slope was systematically increased from 0° to 15°. When evaluating patellar height, it is important to understand how tibial slope affects the Blackburne-Peel Index measurement. CLINICAL RELEVANCE: As posterior tibial slope increases, the numerator of the Blackburne-Peel ratio decreases, and vice versa. This relationship can lead to incorrect assessment of patellar height. Objectively placing individuals into patella alta and baja categories may influence patient care and decision making.

RESUMEN

Insertional Achilles tendinopathy is one of the most common Achilles tendon disorders and often results in substantial heel pain and functional disability. There is consensus that treatment of insertional Achilles tendinopathy should start with nonoperative modalities. Surgery should be reserved for patients who fail exhaustive conservative treatment for a period of 3 months to 6 months and include débridement of insertional calcifications. Intratendinous degenerative tissue should be débrided and any Haglund deformity resected. Different surgical techniques have been described for reattachment of the distal Achilles tendon. The authors' preferred surgical technique includes the knotless double-row footprint reconstruction. Postoperative complications are not rare.

BACKGROUND: Precise correlations between medial malleolar fracture geometry and fracture mechanism have not been thoroughly described. This study sought to determine the prevalence of different medial malleolar fracture types and to elucidate the association between fracture geometry and fracture mechanism. METHODS: The records of 112 medial malleolar ankle fractures were reviewed. For each fracture, the direction of the fracture line in the medial malleolus (transverse, oblique, vertical, or comminuted), the Lauge-Hansen classification, and the presence or absence of syndesmotic injury was recorded. Bivariate correlation analysis was performed to determine if correlations existed. RESULTS: Transverse fractures were the most prevalent type of medial malleolar fracture [n=64 (57%)], and they correlated with supination-external rotation injuries. These were followed by oblique fractures [29 (26)], which correlated with pronation-external rotation injuries [29 (26)], and vertical fractures [7 (6)], which correlated with supination-adduction injuries [9 (8)]. Comminuted fractures [12 (11)] and pronation-abduction injuries [22 (20)] did not correlate with any other categories. Syndesmotic injuries were correlated with transverse fractures, bimalleolar fractures, and pronation-external rotation injuries. CONCLUSION: Medial malleolar fractures can be divided into four fracture types: transverse fractures, which correlated with supination-external rotation injuries; oblique fractures, which correlated with pronation-external rotation injuries; vertical fractures, which correlated with supination-adduction injuries; and comminuted fractures, which did not correlate with a particular type of injury. Syndesmotic injury was positively correlated with transverse fractures of the medial malleolus, bimalleolar fractures, and pronation-external rotation injuries. These findings suggest that medial malleolar fracture geometry can provide valuable information for the clinician when classifying and managing ankle fractures.

BACKGROUND: Evaluation of operative techniques used for medial malleolar fractures by classifying fracture geometry has not been well documented. METHODS: One hundred eleven patients with medial malleolar fractures (transverse n = 63, oblique n = 29, vertical n = 7, comminuted n = 12) were included in this study. Seventy-two patients had complicating comorbidities. All patients were treated with buttress plate, lag screw, tension band, or K-wire fixation. Treatment outcomes were evaluated on the basis of radiological outcome (union, malunion, delayed union, or nonunion), need for operative revision, presence of postoperative complications, and AOFAS Ankle-Hindfoot score. RESULTS: For transverse fractures, tension band fixation showed the highest rate of union (79%), highest average AOFAS score (86), lowest revision rate (5%), and lowest complication rate (16%). For oblique fractures, lag screws showed the highest rate of union (71%), highest average AOFAS score (80), lowest revision rate (19%), and lowest complication rate (33%) of the commonly used fixation techniques. For vertical fractures, buttress plating was used in every case but 1, achieving union (whether normal or delayed) in all cases with an average AOFAS score of 84, no revisions, and a 17% complication rate. Comminuted fractures had relatively poor outcomes regardless of fixation method. CONCLUSIONS: The results of this study suggest that both tension bands and lag screws result in similar rates of union for transverse fractures of the medial malleolus, but that tension band constructs are associated with less need for revision surgery and fewer complications. In addition, our data demonstrate that oblique fractures were most effectively treated with lag screws and that vertical fractures attained superior outcomes with buttress plating. OBJECTIVE: To investigate whether blood haemoglobin A 1c (HbA1c) levels was predicative of diabetic patients' responsiveness to surgical treatment of ankle fractures. METHODS: The relationship between blood HbA1c levels and surgical treatment outcomes of 21 diabetic patients undergoing open reduction internal fixation (ORIF) for ankle fractures was analyzed with Pearson correlation testing and t testing. All patients were treated with ORIF using standard surgical techniques. Treatment outcomes were defined using radiological outcome, the American Orthopaedic Foot and Ankle Score (AOFAS) ankle-hindfoot scale score, surgical revision rate, and complication rate. RESULTS: HbA1c levels were found to have a statistically significant correlation with poor radiological outcomes (r = 0.547) and AOFAS ankle-hindfoot scores (r = -0.592). Additionally, though rates of poor radiological outcome, revision, and complication were high in the diabetic population as a whole, these rates were considerably higher among individuals with elevated HbA1c (≥6.5%) and considerably lower among individuals with lower HbA1c (<6.5%) levels. CONCLUSION: Blood HbA1c levels appear to be predictive of risk and complication rates in the surgical treatment outcomes of diabetic patients with ankle fractures.
