BACKGROUND: Due to lack of vitamin D absorption in patients with cystic fibrosis (CF), vitamin D supplementation becomes necessary. Our aim was to study the association between serum vitamin D levels and key clinical factors, such as nutritional status, pulmonary function and pulmonary exacerbations (PEx) frequency, in an adult CF population. METHODS: Prospective analysis of a published vitamin D (VitD3) supplementation protocol (N = 200 adult patients) over a follow-up period of 5 years. Data were collected from the medical files before (baseline) and after (follow-up) the implementation of the VitD3 supplementation protocol, between 2009 and 2014. Serum samples to measure vitamin D were also collected at baseline and follow-up. RESULTS: A positive relationship between serum vitamin D and lung function was observed at baseline (R = 0.158, P = 0.027), but it disappeared at follow-up (P = 0.454). There was no association between serum vitamin D levels and body mass index. At follow-up, patients with significantly higher serum vitamin D levels were women, older in age, had CF-related diabetes or had a history of recurring PEx. CONCLUSION: No direct link was observed between heightened serum vitamin D and lung function or BMI in an adult CF population. We suggest that better compliance to treatments and closer follow-up from health professionals could partially explain why such patients reached higher vitamin D serum levels.

Malnutrition in cystic fibrosis (CF) is associated with increased mortality and can lead to fat-free (FFM) and fat mass (FM) loss. Dual-energy X-ray absorptiometry (DXA) is used and validated to measure FFM and FM. DXA's high cost has led to the utilization of less costly techniques such as bioelectrical impedance analysis (BIA). The aim of this study was to determine the agreement of FFM, FM and %FM measurements taken with DXA and BIA in adults with CF. We measured FFM, FM and %FM in 34 adults with CF with a leg-to-leg BIA and an iDXA and determined agreement using Bland-Altman analysis. While DXA and BIA measurements were well correlated (r > 0.8), mean biases between both methods were between 8 and 11%. BIA underestimated FM and %FM and overestimated FFM. In a clinical research setting where these measurements are used to phenotype patients, BIA cannot replace DXA.

RESUMEN

Seclusion with or without restraint is a measure for managing aggressive or agitated clients and promoting site security, particularly in an emergency psychiatric setting. The decision to control a potentially dangerous person's behaviour by removal or seclusion seems ethically justifiable in such a setting. However, although the decisions on these restrictive measures are based on rational needs, they are also influenced by the healthcare team's perceptions of the client and by the characteristics of the team and the environment. The purpose of this paper is to set out and categorize the factors in play in aggression- and agitation-management situations as perceived by the healthcare teams, particularly the nurses. The first part of the paper deals briefly with the settings in which control measures are applied in a province in eastern Canada and the effect of such measures on patients and healthcare teams. The second part identifies the factors involved in the management of agitation and aggression behaviour. The final part discusses the current spin-offs from this knowledge as well as promising paths for further research on the factors involved. The ultimate objective is to reduce recourse to coercive measures and enhance professional practices.

OBJECTIVE: Given the need for continuing education in medical informatics for mid-career professionals, the authors aimed to implement and evaluate distance learning courses in this area. DESIGN: The authors performed a needs assessment, content and technology planning, implementation, and student evaluation. MEASUREMENTS: The needs assessment and student evaluations were assessed using a combination of Likert scale and free-form questions. RESULTS: The needs assessment indicated much interest in a medical informatics distance learning program, with electronic medical records and outcome research the subject areas of most interest. The courses were implemented by means of streaming audio plus slides for lectures and threaded discussion boards for student interaction. Students were assessed by multiple-choice tests, a term paper, and a take-home final examination. In their course evaluations, student expressed strong satisfaction with the teaching modalities, course content, and system performance. Although not assessed experimentally, the performance of distance learning students was superior to that of on-campus students. CONCLUSION: Medical informatics education can be successfully implemented by means of distance learning technologies, with favorable student satisfaction and demonstrated learning. A graduate certificate program is now being implemented.

CONTEXT: There is a growing interest in and need for continuing education in medical informatics delivered by distance learning. OBJECTIVES: Implement and evaluate a distance learning introductory course in medical informatics. METHODS: A Web-based version of our on-campus "Introduction to Medical Informatics" course was implemented using streaming audio lectures, threaded discussion boards, and several other teaching modalities. Evaluation was performed using an adaptation of our on-campus course evaluation instrument. RESULTS: The course was implemented with no major technological or pedagogical problems. Student satisfaction with teaching modalities and other course modalities was high. CONCLUSIONS: The learning technologies used in this course were implemented successfully and a Graduate Certificate Program is planned to further meet educational needs in medical informatics.

RESUMEN

The recent improvements in capabilities of desktop computers and communications networks give impetus for the development of clinical image repositories that can be used for patient care and medical education. A challenge in the use of these systems is the accurate indexing of images for retrieval performance acceptable to users. This paper describes a series of experiments aiming to adapt the SAPHIRE system, which matches text to concepts in the UMLS Metathesaurus, for the automated indexing of image reports. A series of enhancements to the baseline system resulted in a recall of 63% but a precision of only 30% in detecting concepts. At this level of performance, such a system might be problematic for users in a purely automated indexing environment. However, if the ability to retrieve images in repositories based on content in their reports is desired by clinical users, and no other current systems offer this functionality, then follow-up research questions include whether these imperfect results would be useful in a completely or partially automated indexing environment and/or whether other approaches can improve upon them.

RESUMEN

This article describes techniques and strategies used to judge the potential applicability of new information management technologies in the clinical setting and to develop specific design recommendations for new features and services. We focus on a project carried out to identify the potential uses of handheld computers (i.e., the Palm Pilot or a small WinCE-based device) in the ambulatory practice setting. We found that the potential for a robust handheld computing device to positively affect the outpatient ambulatory clinical setting is enormous, and that the information derived from the exploratory research project is useful in creating specific design recommendations for further development.

This paper describes preliminary work evaluating automated semantic indexing of radiology imaging reports to represent images stored in the Image Engine multimedia medical record system at the University of Pittsburgh Medical Center. The authors used the SAPHIRE indexing system to automatically identify important biomedical concepts within radiology reports and represent these concepts with terms from the 1998 edition of the U.S. National Library of Medicine's Unified Medical Language System (UMLS) Metathesaurus. This automated UMLS indexing was then compared with manual UMLS indexing of the same reports. Human indexing identified appropriate UMLS Metathesaurus descriptors for 81% of the important biomedical concepts contained in the report set. SAPHIRE automatically identified UMLS Metathesaurus descriptors for 64% of the important biomedical concepts contained in the report set. The overall conclusions of this pilot study were that the UMLS metathesaurus provided adequate coverage of the majority of the important concepts contained within the radiology report test set and that SAPHIRE could automatically identify and translate almost two thirds of these concepts into appropriate UMLS descriptors. Further work is required to improve both the recall and precision of this automated concept extraction process.

OBJECTIVES: Since the 1960's, marked sociocultural and economic changes have affected lifestyle, religious practices, and family structure among French Canadians in Quebec. Recent nutritional assessment was unavailable, despite indications of change. METHODS: A survey was carried out in 1988 in a representative sample living in Greater Montreal, to obtain current data on food habits, nutrient intakes and sociodemographic factors, using interviewer-administered questionnaires and seven-day food records. RESULTS: Some 845 families (1450 individuals from different age groups) were studied in two phases: summer/fall and winter/spring. Mean household size in the study population was 2.7. Among adults, extremes of educational level were observed, with 33% having elementary school only and 28% having completed university. On average, each household spent $CAN 89.90 a week for food, $26.00 for tobacco, and $13.50 for alcohol. Breakfast was eaten regularly by 90% of subjects while 96% ate lunch and 99% ate dinner. Morning snacks were consumed by 36% and afternoon snacks were taken by 50%. Milk was consumed with breakfast by 24% of respondents, with lunch by 19%, and with dinner by 24%. On weekdays, 81% of subjects ate their main meals at home, while on weekends this figure was 95%. Time spent for meals varied by meal, and was slightly longer on weekends. Specialty diets, including vegetarianism, were followed by 7% of the study subjects, while 22% adhered to health-related diets. CONCLUSIONS: The food record analyses revealed adequate nutrient intakes overall in relation to the 1990 Canadian Recommended Nutrient Intakes, although further investigation is needed before addressing dietary quality in specific age-sex groups. Protein comprised 16% of energy, fat 38%, and carbohydrates composed 45% or 46% for males and females, respectively. Subsequent analyses will evaluate nutrient intakes in relation to health and sociodemographic indicators in this population.

In order to determine calcium intake, 1,450 French-Canadians of both sexes, all age groups and various socio-economic status, were recruited in Montreal and Laval using a stratified randomized selection. One thousand and sixty-five (73%) subjects completed food records: 82.4% for a period of seven days, the others for periods varying from one to six days, providing 6,590 days of food records. Subjects ranged in age from one month to 95 years with a mean age of 35.1 years. No seasonal variation in calcium intake was observed. On average, men consumed more calcium than women. This difference was significant for age groups 13 to 15, 19 to 24 and 25 to 49 (p < 0.05). Womens' diets, however, had a higher calcium density than those of men, except among 10-15-year-olds. Mean calcium intake exceeded the RNI for the Canadian population in all age groups except females aged 10 to 15 years, and both men and women aged 50 years and over. The results of this study suggest that those in the study region most at risk for insufficient calcium intake are adolescent females and older men and women.
