RESUMEN

Background Distal biceps tendon (DBT) rupture is not one of the most common upper limb injuries. Surgical intervention is recommended for these injuries to restore muscular strength and functionality. Multiple different techniques are documented in the literature, however there is no definitive consensus on the most effective surgical treatment. The objective of this study was to assess the functional results of patients who underwent repair of DBT utilizing cortical button fixation procedures. Methods This study is a retrospective single-unit case series consisting of 54 patients who underwent DBT repair at Heartlands Hospital in Birmingham, United Kingdom. The patients' functional outcomes was assessed by the Mayo Elbow Performance Score (MEPS). Results The mean age was 51±11.01 years. Patients were operated on 4.72±7.083 days after the injury. The mean pain Visual Analogue Scale (VAS) 6 months after the surgery was 0.54±0.50. At 6 months follow-up, the average extension deficit was 2.69° (0-10), flexion 132° (120-140), supination76° (50- 85), and 77° for pronation (78-95). Patients were followed up routinely for 6 months. Mayo Elbow Performance (MEP) Score was utilized to assess the functional outcome and the mean MEP score was 91.43±8.26 which showed excellent functional outcomes for the cohort. Conclusion DBT repair with cortical button fixation yielded favorable functional outcomes at 6 months, notably restoring supination strength. This approach offers anatomical reinsertion while minimizing nerve damage risk.

Haemonchus contortus is a blood-sucking gastrointestinal nematode that infects all ruminants and causes significant economic losses in production. Characterizing the genetic variability of H. contortus populations is crucial for understanding patterns of disease transmission and developing effective control strategies against haemonchosis. This study aimed to identify the genetic variability of H. contortus isolates in small ruminants from slaughterhouses in Bangladesh. During January to December 2015, 400 abomasa samples were collected and 186 were found to be positive for Haemonchus. A 321-bp fragment of the second internal transcribed spacer (ITS-2) of nuclear ribosomal DNA and an 800-bp fragment of the mitochondrial nicotinamide dehydrogenase subunit-4 gene (nad4) were amplified using polymerase chain reaction (PCR) and directly sequenced. The results showed 10 genotypes (ITS-2) and 45 haplotypes (nad4) among the 186 worms. The sequences were 98.5 to 100% identical to reference sequences from the GenBank database. ITS-2 sequence analysis revealed four nucleotide substitutions at positions 30, 41, 42, and 216. There was one transition (C/T) at position 42 and three transversions (C/A at position 30, G/C at position 41, and T/A at position 216). The nad4 gene sequences showed 15 substitutions, all of which were transitions. The pairwise distance of ITS-2 between H. contortus populations ranged from 0.005 to 1.477. The nucleotide diversity (µ) among the populations was 0.009524 using ITS-2 and 0.00394 using nad4. This study indicated low genetic deviation among H. contortus populations in Bangladesh.

RESUMEN

The incidence of stroke in young individuals has been showing an increasing trend. In such cases, atypical mechanisms of stroke should be considered. Here, we report a case of a 37-year-old healthy female who presented with complaints of right-sided ataxia and clumsiness that started after an amusement park ride where she was swung rapidly. Imaging revealed an infarct in the right cerebellar hemisphere. This case report highlights certain mechanisms that can cause a stroke in this situation, with special emphasis on recognition, timely medical management, public awareness of such risk factors, and the prevention of complications.

Potassium (K) improves the stress tolerance of crop plants, which varies on the timing of K application and crop varieties. Soybean is a promising crop that can easily fit with the cropping pattern during kharif I season, when water logging occurs due to sudden rain. Therefore, an experiment was conducted to determine the effect of K management on the productivity and seed quality of soybean under normal and waterlogged conditions. The treatments comprised three factors, namely soybean genotypes (BU Soybean-1 and BU Soybean-2), waterlogging (WL) (control and WL for 4 days at the flowering stage (FS)), and K application (full dose as basal and 50% as basal +50% as top dress after termination of the flooding). The trial was laid out in a randomized complete block design with three replications. Findings revealed that BU Soybean-1 produced a higher number of pods and seeds pod-1 under control conditions with basal application of K. On the other hand, BU Soybean-2 produced taller plants and heavier grain, improving grain and straw yield under WL conditions when K was top dressed. The varieties absorbed a higher amount of nitrogen, phosphorus, and potassium under control conditions compared to WL when K was top dressed. Similarly, the seed protein content of both varieties was higher in the control condition with a top dressing of K. However, a higher percentage of seed germination was obtained from BU Soybean-2 in the control condition with a top dressing of K. Further, more electrical conductivity and more mean germination time were recorded in the case of BU Soybean-2 under WL with the basal application of K. Split application of 50% of recommended K fertilizer after the recession of flood water could be suggested for improved grain yield in flood-affected soybean growing areas.

OBJECTIVE: To determine the association of the studied variables on mortality in pyogenic meningitis in paediatric age group. DESIGN: An observational analytical study. PLACE AND DURATION OF STUDY: Paediatrics Medicine, Unit-2 of B.V. Hospital, attached with Quaid-e-Azam Medical College, Bahawalpur, from December 2000 to December 2002. PATIENTS AND METHODS: Sample size was 300 admitted cases of pyogenic meningitis, aged 2 months to 12 years, meeting the inclusion criteria. The association of the different variables with mortality in pyogenic meningitis was studied. Data was collected and tabulated. X2 test of significance was applied to determine the p-value. p<0.05 was taken as statistically significant. RESULTS: The mortality rate was 10% (30/300). The association of different variables, with outcome at the time of admission was statistically significant in prolonged duration of illness (chronicity prior to admission and increased cerebrospinal fluid (CSF) neutrophils after 1st lumbar puncture. Among the etiological microorganisms isolated from culture of CSF, only Streptococcus pneumoniae had statistically significant association. CONCLUSION: Mortality in pyogenic meningitis was significantly related to pre-admission illness duration and CSF neutrophilia. Mortality can be reduced by early and appropriate recognition. Antibiotic and supportive therapy should be initiated promptly followed by specific medication when the reports of the CSF culture is available.
