RESUMEN

The objective was to understand experiences of double motherhood during antenatal, childbirth and postpartum healthcare, using a qualitative method involving individual online interviews and asynchronous, online focus groups of cisgender women, mostly in same-sex relationships. The results revealed how these women's experiences of parenting were marginalised, highlighting institutional violence in Brazilian healthcare services, which are presented here in two thematic dimensions: 1) Cisheteronormativity and its impact on experiences of double motherhood; and 2) Institutional violence in healthcare services: from curiosity to LGBTQIA+phobia. It was concluded that cisheteronormativity hinders healthcare for these experiences, especially by rendering the non-gestational mother invisible. This underscores the urgent need to train healthcare personnel, rethink and challenge cisgender and heterosexual norms and promote inclusive policies to ensure equitable care and combat institutional violence.

Background There are increasing reports of cases of Guillain-Barré syndrome (GBS), as an adverse event of an immune checkpoint inhibitor (ICI) but postmarket data on the incidence of this remains scarce. This study sought to conduct a comprehensive review of GBS events arising as a secondary outcome of ICI treatments in real-world patients, using the Food and Drug Administration Adverse Event Reporting System (FAERS). Methods Data covering the period from the third quarter of 2003 to the second quarter of 2023 were extracted from the FAERS database. GBS cases (associated with the usage of avelumab, atezolizumab, ipilimumab, nivolumab and pembrolizumab) were subjected to disproportionality analysis to detect potential signals. Results A total of 2208 reports of GBS were identified within the FAERS database, with 242 of these cases (10.9%) being associated with ICIs. All five drugs exhibited a disproportionality in the reporting of adverse events, with the highest observed for avelumab (reporting OR, ROR: 29.8), followed by atezolizumab (ROR: 17.0), ipilimumab (ROR: 16.0), pembrolizumab (ROR: 11.9) and nivolumab (ROR: 8.2). Conclusion These checkpoint inhibitors are associated with a statistically significant disproportionate number of reports of GBS as an adverse event, with avelumab being the ICI with the highest association. The present pharmacovigilance study serves as a valuable tool, offering a more comprehensive and nuanced perspective on GBS associated with ICIs. This study contributes to a deeper comprehension of this rare adverse drug effect.

Resumo Nesta revisão objetivou-se investigar e sintetizar o que há na literatura científica sobre as relações entre as masculinidades e as pessoas moradoras de rua, com enfoque em relações sociais, trajetória e processo saúde-doença. Foi feita uma busca sistemática em dez bases de dados com os descritores: "Masculinities", "Hegemonic Masculinities", "Gender Identity" e "Homeless Persons", "Homeless, Roofless" e "Houseless". Obteve-se 2.459 resultados, com 25 referências finais após aplicação dos critérios de inclusão e exclusão, compostas em grande parte por estudos internacionais. Identificou-se seis temas-chave: Masculinidade enquanto prática performativa nas ruas, Masculinidades e relações de violência nas ruas, Masculinidades entre homens moradores de rua: percepções de liberdade e encarceramento, Paternidade dos homens moradores de rua, Estigmas e deságios no processo de busca pela masculinidade hegemônica nas ruas e Masculinidades como determinante social de saúde nas ruas. A revisão aborda as influências das masculinidades entre a População Moradora de Rua (PMR), destacando sua transversalidade nas vivências e atitudes do grupo, e explora a relação entre masculinidades e violência, os desafios na paternidade e os estigmas ligados à condição de vida nas ruas e impactos na saúde. Conclui-se que as normas sociais relacionadas ao masculino atuam como uma forma de se adaptar as vidas nas ruas, mas influenciam negativamente no processo saúde-doença da PMR, em especial dos homens.


Abstract This study aimed to investigate and synthesize the scientific literature regarding the relationships between masculinities and individuals experiencing homelessness by focusing on social relationships, life trajectories, and the health-disease process. A systematic search was conducted in 10 databases using the following descriptors: "Masculinities," "Hegemonic Masculinities," "Gender Identity," "Homeless Persons," "Homeless, Roofless," and "Houseless." In total, 2459 results were retrieved. Overall, 25 studies remained after the application of inclusion and exclusion criteria, most of which were international studies. This study identified six key themes: 1) Masculinity as a performative practice on the streets, 2) Masculinities and relationships of violence on the streets, 3) Masculinities among men experiencing homelessness: perceptions of freedom and imprisonment, 4) Fatherhood among men experiencing homelessness, 5) Stigmas and challenges in the pursuit of hegemonic masculinity on the streets, and 6) Masculinities as a social determinant of health on the streets. This review addresses the influences of masculinities on individuals experiencing homelessness, emphasizing the cross-cutting nature of these ideals in the experiences and attitudes of this group. It explores the relationship between masculinities and violence, as well as addressing challenges in fatherhood. It highlights stigmas linked to the condition of being homeless that impact health and quality of life. It is concluded that social norms related to masculinity act negatively as social determinants of health for the homeless population, especially men.

Resumo: Introdução: A Política Nacional de Saúde Integral de Lésbicas, Gays, Bissexuais, Travestis e Transexuais (PNSILGBT) é formada por um agrupamento de diretrizes e planos, sendo um eixo importante para formação dos profissionais de saúde, por meio de ações e estratégias específicas, para minimizar os efeitos da discriminação de gênero e sexualidade de uma população historicamente marginalizada. Na literatura, encontram-se estudos que evidenciam a falta de carga horária específica para gênero e sexualidade, a falta de transversalidade da temática LGBTI+ ou ainda a ausência da abordagem de aspectos socioeconômicos, políticos e raciais da saúde LGBTI+ dentro dos currículos de Medicina no Brasil e no mundo. Objetivo: Este estudo teve como objetivo comparar a percepção entre discentes assumidamente LGBTI+ e discentes heterossexuais no que concerne à formação dos médicos sobre a saúde de minorias sexuais e de gênero. Método: Trata-se de um estudo qualitativo, em profundidade, com análise de discurso de grupos focais, um com alunos LGBTI+ e outro com alunos não LGBTI+, em que se aplicou um questionário semiestruturado com base na análise de práticas discursivas de Spink. Resultado: A análise dos grupos identificou como questões mais pertinentes: pouca carga horária programática; falta de transversalidade; abordagem do tema com olhar pejorativo e preconceituoso; associação da população LGBTI+ com doenças infectocontagiosas ou psiquiátricas; ausência de abordagem dos aspectos socioeconômicos, culturais e raciais da temática; e atenção primária à saúde como espaço de maior abertura para discussões sobre gênero e sexualidade; Conclusão: Há uma percepção, em ambos os grupos, de que o ensino de saúde LGBTI+ é insuficiente e há um despreparo dos alunos para a abordagem da temática de gênero e sexualidade, o que gera impacto direto na assistência em saúde dessa população. Além disso, são necessários mais estudos sobre educação médica em saúde LGBTI+.


Abstract: Introduction: Brazil national public policy for the comprehensive healthcare of the LGBTi+ population consists of several guidelines and plans that include the training of health professionals. Through actions and strategies, it aims to minimize the effects of gender and sexuality discrimination on this historically marginalized population. Studies were found that show a lack of specific hours for gender and sexuality, a lack of transversality of the LGBTI+ topic and a lack of addressing the socioeconomic, political and racial aspects of LGBTI+ health in the curricula of Faculties of Medicine in Brazil and in the world. Objective: To compare the perception between admittedly LGBTI+ students and heterosexual students regarding the training of medical students on the health of sexual and gender minorities. Methodology: An in-depth qualitative study was carried out through two focus groups: one with LGBTI+ students and the other with non-LGBTI+ students. In addition, a semi-structured questionnaire was also applied. The analysis was made through the analysis of Spink's Discursive Practices. Result: Current problems were identified: low number of programmatic hours on the topic; lack of transversality; addressing the topic through a pejorative and prejudiced look; association of LGBTI+ people with infectious or psychiatric diseases; lack of addressing socioeconomic, cultural and racial aspects; Primary Care as a space of greater openness to fight against the cisheteronormative hegemony; Conclusion: LGBTI+ health education is still insufficient, as students feel unprepared to address the issue of gender and sexuality, which directly impacts the healthcare of this population. Additionally, further studies on medical education in LGBTI+ health are needed.

The 17 alpha methyltestosterone (MT) hormone is fed to Oreochromis niloticus larvae in fish farms with the purpose of inducing sex reversal. The aim of this study was to evaluate the toxicity and sub-lethality of MT (99.9% purity) and cMT (a commercial MT with 90% purity) in zebrafish (Danio rerio) adults, where the animals were exposed to concentrations of 0, 4, 23, 139, 833 and 5000 µg/L for 96 hours. Genotoxicity was evaluated by micronucleus test (MN), nuclear abnormalities (NA) and comet assay. A low genotoxic potential of MT was showed, inducing micronucleus, nuclear abnormalities and DNA damage in Danio rerio, depending on the use of MT or cMT, gender and tested concentrations. In the sub-lethality trials, there was a basal difference in the activity of the enzymatic biochemical markers for males and females, while the Glutatione S transferase (GST) activity decreased in all analyzed tissues, and for males the enzymatic activity decreased only in the intestine. Results suggest that MT has a toxic potential to fish because it alters enzymatic metabolic pathways and may pose a risk to the ecosystems.

OBJECTIVE: To evaluate both the performance and acceptability of a method coupling self-sampling with detection of cervical malignancy via elevated HPV 16 and 18 E6 oncoproteins (OncoE6™ Cervical Test) in remote areas in Brazil. METHODS: Women living in rural villages in proximity to Coari city, Amazonas, Brazil were invited to participate in a cervical cancer screening study. 412 subjects were enrolled; there were no refusals. In addition to E6 protein detection, DNA was extracted from the brushes and evaluated for HPV genotypes by PCR (PGMY09/11), followed by typing by the Papillocheck™ if positive. Subjects who were found to be positive for OncoE6 or HPV-DNA were referred for colposcopy. RESULTS: For 110 subjects (27%) this was the first cervical cancer exam. Overall the HPV-DNA prevalence was 19.1% (n = 79); 1.4% (n = 6) were positive by the OncoE6 Test. Fifty-six women attended the invitation for colposcopy where nine had an abnormal cervix and were subsequently biopsied. Histopathological analysis revealed 2 CIN3, 2 carcinomas and 5 CIN1. OncoE6 called two out of the three HPV 16 or 18 associated CIN3+ lesions. CONCLUSIONS: The findings suggest that self-administered sample collection in combination with OncoE6 Test is feasible in this population. This could enable expanded screening coverage while ensuring a high specificity which is imperative given the remote geographic location, since women bearing abnormal test results would necessitate travel and logistical burden to access colposcopy and treatment.

PURPOSE: The aim of the study was to compare the success rates of osseointegration among irradiated and nonirradiated cases submitted to implant placement for anchorage of orbit prostheses from 2003 to 2011. MATERIALS AND METHODS: Charts of 45 consecutive patients were analyzed, 31 men and 14 women, and they were divided in two groups, considering previous irradiation therapy. Nonirradiated group had 33 patients, and irradiated group had 12 patients. In total, 138 implants were installed, 42 (30.4%) in previously irradiated bone. RESULTS: The overall implant survival rate was 96.4% with a success rate of 99.0% among the nonirradiated patients and 90.5% among the irradiated patients (p = 0.03). CONCLUSIONS: Results showed that irradiated sites had a worse prognosis related to success of osseointegration, although the 90.5% survival rate in this group indicates that implant placement is a feasible alternative to anchor orbit prostheses considering the benefits that this technique offers to patients.

RESUMEN

Reconstruction of craniofacial defects following cancer surgery may be performed using several techniques and materials. This case report describes surgery for a large tumor, as well as the rehabilitation process which involved a craniofacial prosthesis covering the whole defect of the anterior brain, orbit, mid-face and hard palate. The results suggest that a craniofacial prosthesis anchored on titanium implants is a viable alternative as a retention system, and also a good alternative to other reconstructive surgeries.

A inibição muscular (IM) tem sido reportada como um dos fatores associados à fraqueza muscular presente na osteoartrite (OA) e condromalácia patelar (CP), sendo inclusive associada com a etiologia e a progressão. Entretanto, parece existir uma lacuna na literatura em relação a estudos de revisão que avaliaram o grau de IM de sujeitos acometidos por CP e OA. O objetivo do estudo foi reunir os resultados de estudos que investigaram o grau de IM na OA e CP e identificar possíveis diferenças na IM que estejam associadas aos estágios do processo degenerativo. Foram incluídos nesta revisão sistemática estudos transversais e/ou experimentais publicados nas bases de dados PubMed, Scopus, SciELO e Cochrane entre 1990 e 2010 que avaliaram a IM por meio da técnica de interpolação de abalo publicados. Os dados referentes à população, protocolo de IM, qualidade dos estudos e resultados de IM foram sumariados e apresentados em Tabelas. Para análise da qualidade, utilizou-se a escala de PEDro. Após a aplicação dos critérios de inclusão, 13 artigos foram incluídos na revisão sistemática (OA=9 e CP=4). A partir da análise dos dados, observou-se uma IM maior na CP em comparação à OA. Contudo, a variabilidade metodológica e a falta de informações sobre os protocolos de IM indicam a necessidade de novos estudos experimentais a fim de que se possa determinar com maior precisão a relação entre a IM e as doenças degenerativas articulares.


Muscle inhibition (MI) has been reported as one of the factors associated with muscle weakness present in osteoarthritis (OA) and chondromalacia patellae (CP), including being associated with the etiology and progression. However, there seems to be a gap in the literature regarding the review studies that assessed the degree of IM subjects affected by CP and OA. The main objective of study was bringing together the results of studies that investigated the degree of OA in the MI and CP and to identify possible differences in IM which are linked to stages of the degenerative process. We included cross-sectional and/or experimental studies published in the databases PubMed, Scopus, SciELO and Cochrane between 1990 and 2010 that evaluated the MI through interpolation twitch techniques. Data on population characteristics, MI protocol, quality of studies and MI results were summarized and presented in Tables. For quality analysis, it was used the scale PEDro. After applying the inclusion criteria, 13 articles were included in the systematic review (OA=9 and CP=4). Based on the analysis of the data, there MI>CP compared to OA. However, the variability of protocols, as well as the lack of information about the IM protocols, indicates the need for further experimental studies in order that we can determine more precisely the relationship between IM and degenerative joint diseases.

The aim of the present study was to investigate the immediate effects of static and proprioceptive neuromuscular facilitation (PNF) stretching on the flexibility of hip adductors in female ballet dancers. Forty-five subjects (age: 28.5 ± 8.0 years; minimum two years of ballet training) were randomly assigned to three groups: PNF (contract-release technique), Static, and Control. Subjects in the PNF and Static groups performed four sets of 30 second stretching with an interval of 30 seconds between sets. The control group stayed at rest for the same time spent by the PNF and Static groups during the stretching sessions. Maximal range of motion was measured before and immediately after the experimental and control protocols in all groups. The results indicated significant differences between pre- and post-stretching flexibility in both PNF and Static groups (p < 0.0001; effect size = 0.24 and 0.39, respectively), whereas no change was identified in the Control group (p = 0.265). However, no differences in post-exercise flexibility were found between PNF and Static groups (p = 0.235). It is concluded that static and PNF stretching methods provoked similar post-exercise acute effects on the maximal range of motion of hip adductors in highly flexible female ballet dancers.

Turner syndrome is caused by haploinsufficiency of the short arm of X-chromosome, and is usually diagnosed by karyotyping. This procedure is time-consuming, expensive and unfeasible for population screening. We propose molecular detection of 45XO Turner patients based on the ability of HpaII, a methylation sensitive endonuclease, to induce the cleavage of non-methylated DNA in the active X-allele. Genomic DNA was obtained from 22 patients with Turner syndrome confirmed by karyotype (45XO, N = 18; 45XO/46XX, N = 4). After digestion, DNA was amplified with primers directed to exon 1 of the androgen receptor (AR) gene and to the GAPDH control gene. Normal control females or mosaic patients, with a second methylated X-chromosome, escaped from HpaII digestion and produced a band corresponding to AR gene amplification. 45XO patients have just one active non-methylated X-chromosome, completely digested by HpaII, thus preventing the amplification of the AR gene. Three of the 45XO cases gave amplified bands, suggesting low-frequency mosaicisms that are not detected by karyotyping. Compared to classical karyotype studies for the detection of 45XO Turner patients, this new molecular method is simpler, faster and less expensive.
