RESUMEN

Skeletal muscle wasting has been well-documented among hemodialysis patients. This catabolic condition can be induced by numerous factors, including low-grade inflammation, and is associated with impairments in functional capacity and quality of life, as well as an increased mortality risk. We previously showed that 12 weeks of intradialytic resistance training increases lean mass, functional capacity, and the quality of life of hemodialysis patients. This chapter provides the details of a protocol of intradialytic exercise that leads to improvements in inflammatory status, body composition, and functional capacity.

PURPOSE: Regular physical activity is a good strategy to maintain the health of athletes, and prevent pain and decreased joint flexibility during the pandemic. On the other hand, higher sedentary time during the pandemic period can have deleterious effects. The objective of this study was to compare physical activity levels, sedentary time, and sleep parameters during the pre-COVID period and the COVID-19 pandemic period in young badminton athletes. METHODS: Fifteen young badminton athletes were evaluated during a pre-COVID period (July 2019) and during the COVID-19 period (July 2020). Sleep parameters, physical activity level, and sedentary time were measured using a tri-axial accelerometer. Participants wore the accelerometer on their dominant wrist for 7 days consecutively. In addition, the average of each sleep parameter [time in bed and total sleep time in hours per day, sleep efficiency (%), wake after sleep onset (WASO, total per day), and sleep latency (minutes per day)] was reported over the 7-day period. RESULTS: Athletes presented increased sedentary time (pre-COVID = 7.0 ± 1.1 vs.COVID-19 = 8.9 ± 1.9 h/day, p = 0.004, d = 1.30) and significant decreases in the total PA observed in counts per day (pre-COVID = 2,967,064.4 ± 671,544.1 vs. COVID-19 = 1,868,210.2 ± 449,768.4 counts/day, p = 0.001, d = 1.99), time in vigorous PA (pre-COVID = 7.7 ± 0.9 vs. COVID-19 = 6.1 ± 1.2 h/day, p = 0.001, d = 1.56), and time in moderate-to-vigorous PA (pre-COVID = 8.1 ± 0.9 vs. COVID-19 = 6.5 ± 1.3 h/day, p = 0.001, d = 1.48). There were no significant differences for time in light and moderate PA or in sleep parameters (p > 0.05). CONCLUSION: Young badminton athletes presented increased sedentary time, and decreased total physical activity, time in MVPA, and time in vigorous activities during the COVID-19 pandemic compared to the pre-COVID period, however, there were no significant differences in sleep parameters.

The purpose of this study was to test if different intensities of aerobic exercise could influence abdominal fat, isoforms of BDNF and executive function. Twenty obese men (30.0 ± 5.4 years old; 34.4 ± 3.5 kg/m2) were randomized to moderate-intensity continuous training (MICT, n = 10) and high-intensity intermittent training (HIIT, n = 10) three times a week for 6 weeks, with isoenergetic energetic expenditure for each exercise session (~ 300 kcal) between conditions. Abdominal fat was assessed pre- and post-intervention; executive function (Coding subtest from BETA-III non-verbal intelligence test and Stroop Color and Word Test), concentrations of mBDNF and proBDNF were assessed in response to acute exercise pre- and post-intervention. Abdominal fat did not change in either group. There was a significant increase in mBDNF immediately after acute exercise in both groups before and after intervention. proBDNF did not present changes acutely nor after 6 weeks. Executive function presented a main effect of time at pre- and post-intervention time-points Stroop Word and Stroop Color and Coding subtest presented improved performance from pre- to post-acute exercise session, in both groups. In conclusion, executive function improvements and acute exercise session-induced increases in mBDNF concentration were found from pre- to post-exercise intervention similarly between MICT and HIIT in obese men.

The aim of this study was to evaluate the inflammatory (peripheral and lipopolysaccharide (LPS)-stimulated released from whole blood) and metabolic (glucose and insulin) profile of inactive obese men in response to two isoenergetic models of aerobic exercise training (~300  kcal each exercise session). Twenty-two participants (28.7 ± 1.6 years; BMI = 34.4 ± 0.1 kg/m2) were randomized into two groups: I) HIIT: high-intensity interval training (10× 1 bout: 1 min - 100% Maximal Aerobic Velocity) or II) MICT: moderate-intensity continuous training (65% Maximal Aerobic Velocity; kcal equal to HIIT). Both groups trained three times per week for 6-weeks. Fasting blood samples were collected before and 0, 30, and 60 min after exercise during the first and last training sessions for evaluation of: I) MIP-1ɑ, insulin, glucose, visceral and subcutaneous fat depots, oral glucose tolerance test, and homeostatic model assessment of insulin resistance (HOMA-IR) index; II) Peripheral (TNF-α, IL-6, and IL-10) and LPS-stimulated release of TNF-α and IL-10 were analyzed before, 0, and 60 min after sessions. IL-6 concentration remained elevated up to 60-min after the acute exercise session (p < 0.001), and IL-10 concentration was higher after 30 and 60-min (p = 0.001) compared to rest, independent of training period and protocol. AUC of IL-10 presented effect of type of training (p = 0.023) with MICT group showed significantly higher values than the HIIT. The ex-vivo assay showed higher IL-10 secretion in response to LPS immediately (p = 0.003) after both acute MICT and HIIT exercise sessions, independent of training period. Fifteen subjects presented decreased HOMA-IR after 6-weeks and seven presented an increase in this index. When we excluded the two least responsive subjects, it was possible to observe a decrease in HOMA-IR (p = 0.020) after training. Taken together, our results suggest that both HIIT and MICT (with same energy expenditure) promote similar effects on HOMA-IR and led to elevations in IL-10 production in LPS-stimulated whole blood, suggesting that leukocytes had an enhanced ability to secrete anti-inflammatory cytokines after the exercise bout.

This study compared the effects of two exercise programs performed in different intensities, but equal overall energy expenditure (EE). Adult men with obesity (29.6 ± 4.9 years; BMI = 35.1 ± 3.3 kg/m2) were randomised to one of three groups: High-intensity interval training (HIIT- 10 × 1:1 min 100% VO2max; n = 13); Moderate-intensity continuous training (MICT - ˜35 min 65% VO2max; n = 13) or Control (no training; n = 6). The session EE (HIIT = 278.0 ± 37.1; MICT = 299.4 ± 17.8 kcal) was calculated by adding the aerobic contribution (VO2 of the session minus VO2 at rest) and anaerobic (difference between the VO2 estimated and VO2 measured in session). The anaerobic contribution in HIIT was 30%, showing that a substantial portion of the energy for 10 x 1 min HIIT comes from non-oxidative metabolism. VO2max improved in both trained groups (p = 0.006), while systolic blood pressure decreased (p < 0.001) and diastolic blood pressure was not altered. Visceral and subcutaneous fat stores did not change after the intervention, indicating a longer intervention may be necessary for changes in adiposity. Six weeks of HIIT or MICT were effective in improving cardiorespiratory fitness and blood pressure in previously inactive obese men.

BACKGROUND & AIMS: Studies have postulated High Intensity Intermittent Training (HIIT) as a superior strategy to reduce body fat. The purpose of this study was to compare the effects HIIT and steady-state training (SST) on body composition, leptin, soluble leptin receptor (sOB-R) levels, and hunger perception in physically active non-obese men. METHODS: Twenty men performed five weeks of HIIT (5 km - 1 min running at 100% speed correspondent to VÈ©O2peak - v VÈ©O2peak - interspersed with 1-min passive recovery; n = 10) or SST (5 km at 70% of vVÈ©O2peak continuously; n = 10) three times a week. Body composition, and hunger perception were assessed at pre- and post-training and were compared by a two-way analysis (group and training period) with repeated measures in the second factor. A fasting time-course (baseline, 24 h, and 48 h after an experimental session of exercise) of leptin and sOB-R levels were measured at pre- and post-five weeks of training and assessed by a three-way analysis (group, period and time of measurement) with repeated measures in the second and third factors. RESULTS: There was no effect on body composition and hunger perception. Leptin was reduced in both groups, while sOB-R was increased post-five weeks of training in HIIT but not in the SST. CONCLUSIONS: Although both training groups exerted alterations in leptin levels, only HIIT was able increased sOB-R levels, this suggest a superior impact on central responses in physically active non-obese men.

Over the recent years, a particular interest was shown towards understanding the roles of excessive hepatic fat accumulation and the development of obesity-related diseases. While hepatic triacylglycerol accumulation seems to be a response to the systemic increase of insulin release, fatty acid metabolites contribute to the progression of non-alcoholic fatty liver disease (NAFLD) to non-alcoholic steatohepatitis (NASH). It is widely accepted that NAFLD is a polygenic and multifactorial disease under the influence of critical behavioral factors such as overeating and sedentary lifestyles. The progression of the disease is proposed to include the accumulation of lipids in hepatocytes, but liver damage would be mainly initiated through an exaggerated activation of the immune system. This inflammatory response would be triggered by the increase in cytokine production followed by TLR-4 activation and NF-kB pathways. Interestingly, cytokines as IL-1ra, IL-4, IL-6 and IL-10 act as antiinflammatory in response to exercise and thus, could play an important role in the restoration of liver functions in diseased conditions. Strategies for healthy life behaviors including nutrition and regular physical exercise are recommended to counteract the dreadful effects of NAFLD. To beyond the classical effect of exercise for increasing energy expenditure and/or inducing negative energy balance, exercise also prevents and reverses the effects of disorders related to the immunometabolic profile. This suggests that exercise prescription may be an attractive alternative for the prevention of obesity and NAFLD. Thus, this review seeks to shed light on the inflammatory pathways regulating the beneficial effects of physical activity on obesity and NAFLD. We will clarify how physical activity intervenes to normalize inflammatory processes and prevent obesity and NAFLD. Finally, the exercise interventions should be individualized to facilitate behavioral and cognitive strategies in order to promote long-term adherence. A multidisciplinary approach including lifestyles, diet and exercise training interventions is considered as a "best practice" and displays the strongest liver benefits when it occurs simultaneously with weight loss.

Abstract Physical exercise is one of the forms of prevention and treatment of obesity and associated diseases such as hepatic steatosis. The aim of this study was to analyze whether hepatic steatosis interferes in the effect of physical exercise on the body composition of obese children and adolescents. The sample consists of 40 obese individuals, 13 children (8.17±1.33 years) and 27 adolescents (12.28±1.36 years). Total and segmental body composition was estimated by DEXA. Anthropometric measurements were performed, as well as ultrasound examination of the liver to measure intra-abdominal and subcutaneous fat, and to diagnose hepatic steatosis (HS). The intervention consisted of 20 weeks, with recreational/competitive activities being applied to children and, for adolescents, concurrent training (aerobic and resistance). The Levene's, repeated-measures ANOVA and effect size tests (ES) by Eta-Squared were performed. It was observed that, although not statistically significant, analyzing the effect size, physical activity in both children (body fat ES effect time= 0.210 and ES effect group= 0.208; fat mass ES effect group= 0.338; fat android ES effect Interaction= 0.267), and adolescents (intra-abdominal fat ES effect group= 0.230) regardless of whether or not HS was effective in reducing body fat. Therefore, exercise was effective in improving the body composition of obese children and adolescents, regardless of HS.


Resumo O exercício físico é uma das formas de prevenção e tratamento da obesidade e doenças associadas como a esteatose hepática. Objetivou-se analisar se a esteatose hepática interfere no efeito do exercício físico sobre a composição corporal de crianças e adolescentes obesos. A amostra consiste em 40 indivíduos obesos, sendo 13 crianças (8,17±1,33 anos) e 27 adolescentes (12,28±1,36 anos). A composição corporal total e por segmento foi estimada pelo DEXA. Realizou-se medidas antropométricas, bem como, o exame de ultrassom do fígado para mensurar a gordura intra-abdominal e subcutânea, assim, diagnosticar a esteatose hepática (EH). A intervenção foi composta por 20 semanas, sendo aplicadas nas crianças, atividades lúdico-recreativas/competitivas e para os adolescentes, treinamento concorrente (aeróbio e resistido). Foram realizados os testes de Levene, ANOVA de medidas repetidas e o tamanho do efeito (TE) pelo Eta-Squared. Observou-se que, apesar de não obter significância estatística, analisando o tamanho do efeito, o exercício físico tanto nas crianças (gordura corporal TE efeito tempo= 0,210 e TE efeito grupo= 0,208; massa gorda TE efeito grupo= 0,338; gordura andróide TE efeito interação= 0,267), quanto nos adolescentes (gordura intra-abdominal TE efeito grupo=0,230), independentemente se tinham ou não a EH foi eficaz para reduzir a gordura corporal. Observa-se que o exercício físico foi eficaz na melhora da composição corporal de crianças e adolescentes obesos independente da EH.

The purposes of this study were to determine: (i) the extent of an acute session of high-intensity intermittent exercise (HIIE) followed by a concurrent strength session (Conc) on the increase of systemic inflammatory cytokines and chemokines, and (ii) whether eight weeks of high intensity interval training plus concurrent strength training alters the acute inflammatory response and immune status. Ten recreationally active males (aged 26.9±4.3years) performed two experimental exercise sessions interspersed by eight weeks of HIIT plus concurrent strength training. The experimental exercise session was composed of a 5-km run on a treadmill (1:1 at 100% of maximal aerobic speed (MAS)), and after 10min of passive recovery, back squat exercises were performed (80% 1RM, four sets until exhaustion). Serum samples were collected after fasting, pre-HIIE, post-HIIE, Pre-Conc, Post-Conc, and 30 and 60min post-exercise session. The comparison between both concurrent exercise sessions was performed using repeated measure ANOVA, with the Bonferroni Post-hoc when necessary. Interleukin-6 (IL-6) presented a moment effect (F=6.72; p<0.05), with Post-Conc significantly higher than pre-HIIE, Post-HIIE, and 60min, only a tendency was found between pre-HIIE and post-HIIE (difference=-5.99; p=0.09). MCP-1 and IL-1ra did not present effects for condition, moment, or interaction. Interleukin-10 (IL-10) presented both moment and interaction effects (F=5.31 and 2.50; p=0.005 and 0.036). Pre-Conc and Post-Conc were significantly higher than Pre-HIIE. The interaction between before and after eight weeks of concurrent training probably occurred at Post-Conc (11.42±3.09pgmL-1 and 8.88±1.29pgmL-1). In addition, maintenance of immune function was observed. Therefore, HIIE and concurrent strength exercise lead to an increase in cytokines response, but eight weeks of training program promoted anti-inflammatory response after an acute session of concurrent exercise.

Abstract Different types of physical activity programs have been used with the purpose of improving body composition and increasing resting energy expenditure (REE) in obese adolescents. The aim of the present study was to compare the effects of two training models on REE and body composition in this population. The study included 20 obese male adolescents, who were randomly assigned to follow two training models: strength training (n=8, age=13,4±1.0) and functional training (n=12, age= 13.0±1.1). Body composition variables were estimated by dual-energy X-ray absorptiometry. REE was assessed by indirect calorimetry using the QUARK-PFT equipment (COSMED, Rome, Italy). The training protocol consisted of 30 minutes of aerobic training followed by 30 minutes of strength training (ST) or functional training (FT), both with a duration of 20 weeks. There were no significant differences between the two training models with regard to body composition (fat mass, FT= -7.6±5.5% vs. ST= -8.9±6.2%; p=0.620), (lean body mass, FT= 9.0±5.3% vs. ST= 6.8±6.7%; p=0.431) and to REE (FT= 19.6±15.3% vs. ST= 10.7±24.5%; p=0.331). Moreover, lean body mass (p=0.01) and fat mass (0.01) had an influence on REE. No differences were observed between the two training models, but both were effective in improving body composition and increasing REE in obese adolescents. Furthermore, the present study showed the importance of systematic physical training, since lean body mass and fat mass contributed to the increase in REE after the training period.


Resumo Diferentes tipos de programas de exercícios físicos têm sido utilizados na tentativa de melhorar a composição corporal e aumentar o gasto energético de repouso (GER) de adolescentes obesos. O objetivo foi comparar os efeitos de dois modelos de treinamento sobre o gasto energético de repouso e a composição corporal de adolescentes com obesidade. Participaram do estudo vinte adolescentes obesos do sexo masculino, foram divididos de forma aleatória em dois modelos de treinamento: treinamento contra resistência (n=8, idade=13,4±1,0) e treinamento funcional (n=12, idade= 13,0±1,1). As variáveis de composição corporal foram estimadas pela densitometria radiológica de dupla energia. O GER foi realizado por meio da calorimetria indireta usando o equipamento QUARK-PFT (COSMED, Roma, Itália). O protocolo de treinamento consistiu de 30 minutos de treino aeróbio seguidos de 30 minutos de treino contra resistência (TC) ou funcional (TF), ambos durante 20 semanas. Não houve diferenças significantes entre os dois modelos de treinamento na composição corporal (massa gorda, TF= -7,6±5,5% x TC= -8,9±6,2%; p=0,620), (massa corporal magra, TF= 9,0±5,3% x TC= 6,8±6,7%; p=0,431) e no GER (TF= 19,6±15,3% x TC= 10,7±24,5%; p=0,331). Além disso, a massa corporal magra (p=0,01) e massa gorda (p=0,01) influenciam o GER. Não há diferença entre os dois modelos de treinamento, porém ambos são eficazes na redução da composição corporal e aumento do GER de adolescentes obesos. Adicionalmente, foi verificada a importância do treinamento físico sistematizado, uma vez que a massa corporal magra e massa gorda contribuíram para o aumento do GER após o treinamento.

BACKGROUND: The prevalence of obesity in pediatric population is increasing at an accelerated rate in many countries, and has become a major public health concern. Physical activity, particularly exercise training, remains to be a cornerstone of pediatric obesity interventions. The purpose of our current randomized intervention trial was to compare the effects of two types of training matched for training volume, aerobic and concurrent, on body composition and metabolic profile in obese adolescents. Thus the aim of the study was compare the effects of two types of training matched for training volume, aerobic and concurrent, on body composition and metabolic profile in obese adolescents. METHODS: 32 obese adolescents participated in two randomized training groups, concurrent or aerobic, for 20 weeks (50 mins x 3 per week, supervised), and were compared to a 16-subject control group. We measured the percentage body fat (%BF, primary outcome), fat-free mass, percentage of android fat by dual energy x-ray absorptiometry, and others metabolic profiles at baseline and after interventions, and compared them between groups using the Intent-to-treat design. RESULTS: In 20 weeks, both exercise training groups significantly reduced %BF by 2.9-3.6% as compare to no change in the control group (p = 0.042). There were also positive changes in lipid levels in exercise groups. No noticeable changes were found between aerobic and concurrent training groups. CONCLUSIONS: The benefits of exercise in reducing body fat and metabolic risk profiles can be achieved by performing either type of training in obese adolescents. REGISTRATION NUMBER: RBR-4HN597.

OBJECTIVE: The purpose of the present study was to analyze the effects of a 20-week concurrent training (20 WCT) intervention program on gender-specific body composition and metabolic variables in obese adolescents. SUBJECTS AND METHODS: Sample was composed of twenty-five obese adolescents, aged between 12 and 15 (13.4 ± 0.96) years. Fat-free mass (FFM), percentage trunk fat mass (TFM%) and percentage fat mass (%FM) were evaluated through dual-energy X-ray absorptiometry (DXA). Measurement of intra-abdominal adiposity (IAAT) was performed using ultrasound. Blood pressure was measured and blood samples analyzed for total cholesterol (TC), high-density lipoprotein cholesterol (HDL-c), low-density lipoprotein cholesterol (LDL-c), triglycerides (TG) and plasma glucose. All participants performed the concurrent training (combination of weight training and aerobic training) three times per week, one hour per day, for 20 weeks. Descriptive analysis and analysis of variance (ANOVA) for repeated measures were used to compare baseline, 10 week and 20 week moments using the Bonferroni post-hoc test. Statistical significance was set at p < 0.05. Significant decrease in TC, LDL-c and TFM% were verified in both genders after the 10 initial weeks of concurrent training. RESULTS: A significant increase in height was found in both the male and female groups (p = 0.001 and p = 0.047, respectively), after 20 weeks of concurrent training. In addition, several modifications were observed in body composition and metabolic variables, with a significant decrease in BMI (p = 0.002 and p = 0.017), BMI z-score (p = 0.033 and p = 0.004), FM% (p = 0.002 and p = 0.002), TFM% (p = 0.009 and p = 0.018), TC (p = 0.042 and p = 0.001) and LDL-c (p = 0.006 and p = 0.001) in the male and female groups, respectively, after 20 weeks of intervention when compared with baseline. CONCLUSION: Our results identified that concurrent training was an effective intervention for treating metabolic variable and body composition disorders, in both genders, by decreasing adiposity with consequent improvement in BMI and BMI z-scores, and enhancement in lipid profile variables.

BACKGROUND & AIMS: condition of hypoxia caused by hypertrophy of adipose cells in obesity triggers macrophages recruitment and production of cytokines. Additionally, high consumption of saturated fatty acids (SFA) and high glycemic index meals may contribute to oxidative stress and chronic low-grade inflammation by increases NF-kB activation. Thus, the aim of the study was to analyze the contribution of the macronutrients intake in the metabolic and inflammatory profile, by levels of lipoproteins, insulin resistance, anti and pro inflammatory cytokines, in obese adolescents according the gender. METHODS: sample was composed by 37 adolescents, both genders, identified as obese by body mass index (BMI). Body composition was assessed by Dual-energy X-ray absorptiometry (DEXA) and measures of intra-abdominal adiposity (IAAT) and subcutaneous adiposity tissue (SAT) were done by ultrasound. Biochemical analyses were done and the measurement of cytokines; fatty acids and insulin were performed by the technique of immunoassay ELISA. The estimation of macronutrients consumption was made by 3 day food register regarding food intake. Statistical significance was set at p-value < 5% and the statistical software SPSS version 17.0 (SPSS Inc, Chicago, IL) performed all analyses. RESULTS: BMI (p = 0.316), FM (p = 0.416), IAAT (p = 0.505) and SAT (p = 0.935) presented similarities between genders. Cytokines and metabolic variables values were similar between the groups. Only in the male group, metabolic variables and cytokines were significant correlated with the consumption of total lipids or its fractions. Was observed that insulin concentration had significant interaction with MUFA(g) (= -18.4; p = 0.004) and adiponectin with CHO(g) (= -58.2; p = 0.032) in the group male and female, respectively. CONCLUSIONS: macronutrients intake is associated with low-grade inflammation in obesity, by production of inflammatory cytokines and alteration of the lipid profile, especially male obese adolescents which seem to be more responsive of this consumption when compared with female obese adolescents.


Introducción y objetivo: la hipoxia causada por la hipertrofia de las células adiposas en la obesidad desencadena macrófagos de reclutamiento y la producción de citoquinas. Además, el alto consumo de ácidos grasos saturados (AGS) y las comidas con alto índice glucémico pueden contribuir al estrés oxidativo y la inflamación crónica de bajo grado por los aumentos de activación de NF-kB. Por lo tanto, el objetivo del estudio fue analizar la contribución de la ingesta de macronutrientes en el perfil metabólico e inflamatorio, por niveles de lipoproteínas, resistencia a la insulina, citoquinas anti y pro inflamatorias, en adolescentes obesos según el género. Métodos: la muestra estaba compuesta por 37 adolescentes, de ambos géneros, identificados como obesos según el índice de masa corporal (IMC). La composición corporal se evaluó mediante absorciometría de energía dual de rayos X (DEXA) y medidas de adiposidad intraabdominal (IAAT) y del tejido adiposo subcutáneo (SAT) se realizaron mediante ecografía. Los análisis bioquímicos se realizaron mediante la medición de citoquinas; los ácidos grasos y la insulina se realizaron por la técnica de ELISA. La estimación del consumo de macronutrientes fue llevado a cabo durante tres días mediante el registro de alimentos con respecto a la ingesta total. La significación estadística se estableció en el valor p.

Objective The purpose of the present study was to analyze the effects of a 20-week concurrent training (20 WCT) intervention program on gender-specific body composition and metabolic variables in obese adolescents.Subjects and methods Sample was composed of twenty-five obese adolescents, aged between 12 and 15 (13.4 ± 0.96) years. Fat-free mass (FFM), percentage trunk fat mass (TFM%) and percentage fat mass (%FM) were evaluated through dual-energy X-ray absorptiometry (DXA). Measurement of intra-abdominal adiposity (IAAT) was performed using ultrasound. Blood pressure was measured and blood samples analyzed for total cholesterol (TC), high-density lipoprotein cholesterol (HDL-c), low-density lipoprotein cholesterol (LDL-c), triglycerides (TG) and plasma glucose. All participants performed the concurrent training (combination of weight training and aerobic training) three times per week, one hour per day, for 20 weeks. Descriptive analysis and analysis of variance (ANOVA) for repeated measures were used to compare baseline, 10 week and 20 week moments using the Bonferroni post-hoc test. Statistical significance was set at p < 0.05. Significant decrease in TC, LDL-c and TFM% were verified in both genders after the 10 initial weeks of concurrent training.Results A significant increase in height was found in both the male and female groups (p = 0.001 and p = 0.047, respectively), after 20 weeks of concurrent training. In addition, several modifications were observed in body composition and metabolic variables, with a significant decrease in BMI (p = 0.002 and p = 0.017), BMI z-score (p = 0.033 and p = 0.004), FM% (p = 0.002 and p = 0.002), TFM% (p = 0.009 and p = 0.018), TC (p = 0.042 and p = 0.001) and LDL-c (p = 0.006 and p = 0.001) in the male and female groups, respectively, after 20 weeks of intervention when compared with baseline.Conclusion Our results identified that concurrent training was an effective intervention for treating metabolic variable and body composition disorders, in both genders, by decreasing adiposity with consequent improvement in BMI and BMI z-scores, and enhancement in lipid profile variables.

BACKGROUND: Nonalcoholic fatty liver disease (NAFLD) is a disorder associated with excessive fat accumulation, mainly in the intra-abdominal region. A simple technique to estimate abdominal fat in this region could be useful to assess the presence of NAFLD, in obese subjects who are more vulnerable to this disease. The aim of this cross-sectional study was to verify the reliability of waist circumference and body composition variables to identify the occurrence of NAFLD in obese children and adolescents. METHODS: Sample was composed of 145 subjects, aged 11 to 17 years. Assessments of waist circumference (WC), trunk fat mass (TFM) and fat mass (FM) by dual-energy X-ray absorptiometry (DXA) and ultrasound for diagnosis of NAFLD and intra-abdominal adipose tissue (IAAT) were used. Correlation between variables was made by Spearman's coefficients; ROC curve parameters (sensitivity, specificity, area under curve) were used to assess the reliability of body composition variables to assess the presence of NAFLD. Statistical significance was set at 5%. RESULTS: Significant correlations were observed between NAFLD and WC (p = 0.001), TFM (p = 0.002) and IAAT (p = 0.001). The higher values of area under the ROC curve were for WC (AUC = 0.720), TFM (AUC = 0.661) and IAAT (AUC = 0.741). CONCLUSIONS: Our findings indicated that TFM, IAAT and WC present high potential to identify NAFLD in obese children and adolescents.

OBJETIVO Analizar los efectos de 20 semanas de entrenamiento concurrente sobre las variables de composición corporal, perfil lipídico y diagnóstico de la esteatosis hepática en adolescentes obesos. MÉTODOS Se realizó un ensayo clínico abierto con 34 adolescentes obesos con edades entre los 12 y los 15 años (13,7±1,17). Se analizaron grasa corporal total y de tronco, colesterol total y sus fracciones (HDL, LDL y VLDL) y triglicéridos, siendo realizado examen de ultrasonografía abdominal superior para diagnosticar esteatosis hepática. Los participantes fueron sometidos al entrenamiento concurrente (asociación de entrenamiento con pesos y ejercicio aeróbico) tres veces por semana, con duración de una hora clase durante 20 semanas. Para el tratamiento estadístico, se realizaron análisis descriptivo, prueba t de Student pareada y análisis de frecuencia, a fin de verificar las reducciones relativa y absoluta del diagnóstico de la esteatosis hepática, adoptando p<0,05. RESULTADOS Los adolescentes estudiados presentaron mejoras significativas de la composición corporal, con reducción del porcentaje de grasa total, de la masa gorda total, de la grasa de tronco y del aumento de masa magra, además de reducción del tamaño de los lóbulos del hígado, de los índices de prevalencia de la esteatosis hepática, del colesterol total y del LDL-colesterol. CONCLUSIONES El entrenamiento concurrente fue efectivo por promover mejorías significativas de variables de la composición corporal y del perfil lipídico, además de reducir la prevalencia de la esteatosis hepática. .

Foram analisados gordura corporal total e de tronco, colesterol total e suas frações (HDL, LDL e VLDL) e triglicérides, sendo realizado exame de ultrassonografia de abdome superior para diagnosticar esteatose hepática. Os participantes foram submetidos ao treinamento concorrente (associação de treino com pesos e exercício aeróbio) três vezes por semana, com duração de uma hora-aula durante 20 semanas. Para o tratamento estatístico, foram realizados o teste t de Student pareado e a análise de frequência, a fim de verificar as reduções relativa e absoluta do diagnóstico da esteatose hepática, adotando-se p<0,05. RESULTADOS Os adolescentes estudados apresentaram melhoras significativas da composição corporal, com diminuição do percentual de gordura total, da massa gorda total, da gordura de tronco e do aumento da massa magra, além de redução do tamanho dos lóbulos do fígado, dos índices de prevalência da esteatose hepática, do colesterol total e LDL-colesterol. CONCLUSÕES O treinamento concorrente foi efetivo por promover melhorias significativas de variáveis da composição corporal e do perfil lipídico, além de reduzir a prevalência da esteatose hepática. .

BACKGROUND: Previous studies have shown an association between adiposity, especially intra-abdominal adipose tissue, and hemodynamic/metabolic comorbidities in adults, however it is not clear in pediatric population. The aim of the study was to analyze the relationship between non-alcoholic fatty liver disease (NAFLD) and components of metabolic syndrome (MS) with values of intra-abdominal (IAAT) and subcutaneous (SCAT) adipose tissue in obese children and adolescents. METHODS: Cross-sectional study. SUBJECTS: 182 obese sedentary children and adolescents (aged 6 to 16 y), identified by the body mass index (BMI). MEASUREMENTS: Body composition and trunk fat by dual-energy X-ray absorptiometry- DXA; lipid profile, blood pressure and pubertal stage were also assessed. NAFLD was classified as absent (0), mild (1), moderate (2) and severe (3), and intra-abdominal and subcutaneous abdominal fat thickness were identified by ultrasound. The MS was identified according to the cut offs proposed by World Health Organization adapted for children and adolescents. The chi-square test was used to compare categorical variables, and the binary logistic regression indicated the magnitude of the associations adjusted by potential cofounders (sex, age, maturation, NAFLD and HOMA-IR). RESULTS: Higher quartile of SCAT was associated with elevated blood pressure (p = 0.015), but not associated with NAFLD (p = 0.665). Higher IAAT was positively associated with increased dyslipidemia (p = 0.001), MS (p = 0.013) and NAFLD (p = 0.005). Intermediate (p = 0.007) and highest (p = 0.001) quartile of IAAT were also associated with dyslipidemia, independently of age, sex, maturation, NAFLD and HOMA-IR (homeostatic model assessment-insulin resistance). CONCLUSION: Obese children and adolescents, with higher IAAT are more prone to develop MS and NAFLD than those with higher values of SCAT, independent of possible confounding variables.

OBJETIVO: Analisar o efeito de 12 semanas de intervenção envolvendo prática de atividade física, orientações alimentar e psicológica sobre fatores de risco para o desenvolvimento da síndrome metabólica em crianças e adolescentes obesos. MÉTODOS: Estudo longitudinal com 23 crianças e adolescentes obesos, com idade entre seis e 16 anos (12,0±3,2 anos). Foram mensurados: gordura corporal total e de tronco, glicemia, colesterol total e triglicérides, pressão arterial sistólica e diastólica. Os jovens foram submetidos a três sessões semanais de 60 minutos de exercício físico (atividades esportivas recreativas, ginástica, circuitos e caminhadas), durante 12 semanas. O teste do qui-quadrado foi usado para comparar dados categóricos daqueles que apresentaram valores acima das recomendações para cada fator de risco. O teste t para dados pareados foi aplicado para comparar os dois momentos do estudo. RESULTADOS: Em indivíduos com alterações metabólicas no início do estudo, observou-se, após a intervenção, a diminuição de 11,6 por cento na glicemia (105 para 93mg/dL; p=0,046) e de 24,9 por cento no triglicérides (217 para 163mg/dL; p=0,013); porém, não houve diferenças na pressão arterial e no colesterol total. CONCLUSÕES: O programa de exercício físico aplicado nas crianças e adolescentes foi eficiente para melhorar os valores de glicemia e triglicérides.


OBJECTIVE: To analyze the effects of a 12-week intervention program based on physical activity practice, alimentary and psychological orientation on the risk factors for the development of metabolic syndrome in obese children and adolescents. METHODS: A longitudinal study was conducted with 23 obese children and adolescents, aged six to 16 years (12.0±3.2 years). Total body and trunk fat mass, glycemia, total cholesterol, triglycerides, systolic and diastolic blood pressure were measured. The children were submitted to physical activity three times a week during 12 weeks. Each session of 60 minutes consisted of recreational sports activities, gymnastics, circuits and track-walking. Chi-square test was used to compare categorical data for those patients who had values above the recommendations for each risk factor and Student's t-test for paired data was used to compare the two moments of the study. RESULTS: In subjects who had altered baseline values in the beginning of the study, the glycemia levels decreased 11.6 percent (from 105 to 93mg/dL; p=0.046) and the triglycerides decreased 24.9 percent (from 217 to 163mg/dL; p=0.013) after the intervention. On the other hand, no differences were noted for blood pressure and total cholesterol after the intervention. CCONCLUSIONS: The intervention was effective in improving blood glucose and triglycerides levels in children and adolescents enrolled in the study.

MÉTODOS: Estudio longitudinal con 23 niños y adolescentes obsesos, con edad entre seis y 16 años (12,0±3,2 años). Se midió: grasa corporal total y de tronco, glucemia, colesterol total y triglicéridos, presión arterial sistólica y diastólica Los jóvenes fueron sometidos a tres sesiones semanales de 60 minutos de ejercicio físico (actividades deportivas recreativas, gimnástica, circuitos y caminatas) durante 12 semanas. En el tratamiento estadístico, se realizó el análisis descriptivo, con promedio, desviación estándar y error estándar. La prueba de chi cuadrado fue usada para comparar datos categóricos con aquellos que presentaron valores por encima de las recomendaciones para cada Factor de Riesgo y la prueba t para datos pareados fue usada para comparar los dos momentos del estudio. RESULTADOS: En individuos con alteraciones metabólicas en el inicio del estudio, se observó, después de la intervención, reducción del 11,6 por ciento en la glucemia (1053mg/dL para 93mg/dL; p=0,046) y del 24,9 por ciento en los triglicéridos (217mg/dL para 163mg/dL; p=0,013); sin embargo, no hubo diferencias en la presión arterial y en el colesterol total. CONCLUSIÓN: El programa de ejercicio físico aplicado en los niños y adolescentes fue eficiente para mejorar los valores de glucemia y triglicéridos.
