RESUMEN

Nontuberculous mycobacteria (NTM) are widespread environmental organisms found in both natural and man-made settings, such as building plumbing, water distribution networks and hospital water systems. Their ubiquitous presence increases the risk of transmission, leading to a wide range of human infections, particularly in immunocompromised individuals. NTM primarily spreads through environmental exposures, such as inhaling aerosolized particles, ingesting contaminated food and introducing it into wounds. Hospital-associated outbreaks have been linked to contaminated medical devices and water systems. Furthermore, the rising global incidence, prevalence and isolation rates highlight the urgency of addressing NTM infections. Gaining a thorough insight into the sources and epidemiology of NTM infection is crucial for devising novel strategies to prevent and manage NTM transmission and infections.


Non-tuberculous mycobacteria (NTM) are environmental pathogens affecting humans and animals, with a substantial public health impact. These bacteria have been frequently identified in various natural and human-engineered settings, contributing to their potential transmission.

This paper proposes a cognitive radio network (CRN)-based hybrid wideband precoding for maximizing spectral efficiency in millimeter-wave relay-assisted multi-user (MU) multiple-input multiple-output (MIMO) systems. The underlying problem is NP-hard and non-convex due to the joint optimization of hybrid processing components and the constant amplitude constraint imposed by the analog beamformer in the radio frequency (RF) domain. Furthermore, the analog beamforming solution common to all sub-carriers adds another layer of design complexity. Two hybrid beamforming architectures, i.e., mixed and fully connected ones, are taken into account to tackle this problem, considering the decode-and-forward (DF) relay node. To reduce the complexity of the original optimization problem, an attempt is made to decompose it into sub-problems. Leveraging this, each sub-problem is addressed by following a decoupled design methodology. The phase-only beamforming solution is derived to maximize the sum of spectral efficiency, while digital baseband processing components are designed to keep interference within a predefined limit. Computer simulations are conducted by changing system parameters under different accuracy levels of channel-state information (CSI), and the obtained results demonstrate the effectiveness of the proposed technique. Additionally, the mixed structure shows better energy efficiency performance compared to its counterparts and outperforms benchmarks.

Background: The objectives were to assess the impact of dental caries on the daily living of the geriatric population and determine the factors that influence the relationship between dental health and the daily living of the geriatric population. Methods: A descriptive cross-sectional study was carried out over six months at Rawalpindi's public and private dental hospitals. Participants aged≥60 years, both male and female, were selected. The calculated sample size was 281. Desired sample from one of the dental hospitals was collected using a non-probability consecutive sampling strategy. Data about sociodemographic characteristics and the DMFT index were collected. Adapted validated tool dental impact on daily living (DIDL) was used to assess the impact of dental health on daily living. Results: Chi-squared test of association showed a positive association between the DIDL and sociodemographic variables, including age (P=0.001), gender (P=0.001), education (P=0.001), income (P=0.001), occupation (P=0.029), marital status (P=0.001), living arrangement (P=0.001), and history of chronic illnesses (P=0.001). The association between the DMFT index and DIDL also showed statistically significant results (P=0.001). Binary logistic regression analysis indicated that gender (OR=6.98, P=0.005) and the individual's dental health (OR=6.43, P=0.001) were the strongest predictors of the impact experienced in daily life activities. The overall model was statistically significant (χ2=51.24, P=0.001), and the variables were responsible for 32.4% of the variance in the outcome variable. Conclusion: The study provides strong evidence that sociodemographic factors, DMFT index, gender, and individual dental health significantly contribute to the impact of dental health on daily living. Gender and individual dental health emerge as particularly influential predictors. These findings emphasize the need for targeted interventions and awareness programs, especially for groups with a higher risk of experiencing a significant impact on daily life due to dental issues.

Noble metals such as gold (Au), zinc (Zn), and iron (Fe) are highly significant in both fundamental and technological contexts owing to their applications in optoelectronics, optical coatings, transparent coatings, photodetectors, light-emitting devices, photovoltaics, nanotechnology, batteries, and thermal barrier coatings. This study presents a comprehensive investigation of the optoelectronic properties of Fe(111) and Au, Zn/Fe(111) materials using density functional theory (DFT) first-principles method with a focus on both materials' spin orientations. The optoelectronic properties were obtained employing the generalized gradient approximation (GGA) and the full-potential linearized augmented plane wave (FP-LAPW) approach, integrating the exchange-correlation function with the Hubbard potential U for improved accuracy. The arrangement of Fe(111) and Au, Zn/Fe(111) materials was found to lack an energy gap, indicating a metallic behavior in both the spin-up state and the spin-down state. The optical properties of Fe(111) and Au, Zn/Fe(111) materials, including their absorption coefficient, reflectivity, energy-loss function, refractive index, extinction coefficient, and optical conductivity, were thoroughly examined for both spin channels in the spectral region from 0.0 eV to 14 eV. The calculations revealed significant spin-dependent effects in the optical properties of the materials. Furthermore, this study explored the properties of the electronic bonding between several species in Fe(111) and Au, Zn/Fe(111) materials by examining the density distribution mapping of charge within the crystal symmetries.

BACKGROUND: Extrapulmonary tuberculosis (EPTB) without symptomatic pulmonary involvement has been thought to be non-transmissible, but EPTB with asymptomatic pulmonary tuberculosis (PTB) could transmit tuberculosis (TB). Genomic investigation of Mycobacterium tuberculosis (Mtb) isolates from EPTB may provide insight into its epidemiological role in TB transmission. METHODS: Between January 2017 and May 2020, 107 Mtb isolates were obtained from surgical drainage of bone TB patients at the Beijing Chest Hospital, and 218 Mtb strains were isolated from PTB cases. These 325 Mtb isolates were whole-genome sequenced to reconstruct a phylogenetic tree, identify transmission clusters, and infer transmission links using a Bayesian approach. Possible subclinical PTB in the bone TB patients was investigated with chest imaging by two independent experts. RESULTS: Among 107 bone TB patients, 10 were in genomic clusters (≤12 SNPs). Phylogenetic analysis suggested that three bone TB patients transmitted the infection to secondary cases, supported by epidemiological investigations. Pulmonary imaging of 44 bone TB patients revealed that 79.5 % (35/44) had radiological abnormalities suggestive of subclinical PTB. CONCLUSIONS: This study provides genomic evidence that bone TB patients without clinically diagnosed PTB can be sources of TB transmission, underscoring the importance of screening for subclinical, transmissible PTB among EPTB cases.

This review covers the significance of green banana resistant starch (RS), a substantial polysaccharide. The food industry has taken an interest in green banana flour due to its 30% availability of resistant starch and its approximately 70% starch content on a dry basis, making its use suitable for food formulations where starch serves as the base. A variety of processing techniques, such as heat-moisture, autoclaving, microwaving, high hydrostatic pressure, extrusion, ultrasound, acid hydrolysis, and enzymatic debranching treatments, have made significant advancements in the preparation of resistant starch. These advancements aim to change the structure, techno-functionality, and subsequently the physiological functions of the resistant starch. Green bananas make up the highest RS as compared to other foods and cereals. Many food processing industries and cuisines now have a positive awareness due to the functional characteristics of green bananas, such as their pasting, thermal, gelatinization, foaming, and textural characteristics. It is also found useful for controlling the rates of cancer, obesity, and diabetic disorders. Moreover, the use of GBRS as prebiotics and probiotics might be significantly proved good for gut health. This study aimed at the awareness of the composition, extraction and application of the green banana resistant starch in the future food products.

AIMS: Patients with chronic obstructive pulmonary disease (COPD) frequently exhibit an inability to maintain postural balance. However, the contribution of increased intestinal permeability or leaky gut to the postural imbalance in COPD is not known. METHODS: We measured plasma zonulin, a marker of leaky gut, with relevance to postural balance in male controls (n = 70) and patients with mild (n = 67), moderate (n = 66), and severe (n = 58) COPD. We employed a short physical performance battery to evaluate postural balance in supine, tandem, and semi-tandem positions. We also measured handgrip strength (HGS), gait speed, plasma c-reactive proteins (CRP), and 8-isoprostanes as potential mechanistic connections between postural imbalance and leaky gut. RESULTS: COPD patients demonstrated higher plasma zonulin, CRP, and 8-isoprostanes levels and lower balance, HGS, and gait speed than controls (all p < 0.05). These findings were more robust in patients with moderate and severe than mild COPD. In addition, plasma zonulin exhibited significant potential in diagnosing poor balance, low HGS, and gait speed in COPD patients (all p < 0.05). We also found significant correlations of plasma zonulin with CRP and 8-isoprostanes, providing heightened inflammation and oxidative stress as mechanistic connections between leaky gut and postural imbalance. CONCLUSION: Plasma zonulin may be helpful in evaluating postural imbalance in COPD patients. Repairing intestinal leaks can be a therapeutic target to improve postural control in COPD.

Ethnopharmacological relevance of Saussurea species for anti-cancer compounds instigated us to develop chemotherapeutic herbal tablets. This study was an ongoing part of our previous research based on the scientific evaluation of Saussurea heteromalla (S. heteromalla) for anti-cancer lead compounds. In the current study, S. heteromalla herbal tablets (500 /800 mg) were designed and evaluated for anti-cancer activity. Arctigenin was found as a bioactive lead molecule with anti-cancer potential for cervical cancer. The in vitro results on the HeLa cell line supported the ethnopharmacological relevance and traditional utilization of S. heteromalla and provided the scientific basis for the management of cervical cancer as proclaimed by traditional practitioners in China. LD50 of the crude extract was established trough oral acute toxicity profiling in mice, wherein the minimum lethal dose was noticed as higher than 1000 mg/kg body weight orally. Chromatographic fingerprint analysis ensured the identity and consistency of S. heteromalla in herbal tablets in terms of standardization of the herbal drug. About 99.15% of the drug (S. heteromalla crude extract) was recovered in herbal tablets (RSD: 0.45%). In vitro drug release profile was found to be more than 87% within 1 h, which was also correlated with different mathematical kinetic models of drug release (r2 = 0.992), indicating that drug release from matrix tablets into the blood is constant throughout the delivery. The dosage form was found stable after an accelerated stability parameters study which may be used for anti-cervical cancer therapy in the future, if it qualifies successful preclinical investigation parameters.

Combining natural polysaccharides with synthetic materials improves their functional properties which are essential for designing sustained-release drug delivery systems. In this context, the Aloe vera leaf mucilage/hydrogel (ALH) was reacted with acrylic acid (AA) to synthesize a copolymerized hydrogel, i.e., ALH-grafted-Polyacrylic acid (ALH-g-PAA) through free radical copolymerization. Concentrations of the crosslinker N,N'-methylene-bis-acrylamide (MBA), and the initiator potassium persulfate (KPS) were optimized to study their effects on ALH-g-PAA swelling. The FTIR and solid-state NMR (CP/MAS 13C NMR) spectra witnessed the formation of ALH-g-PAA. Scanning electron microscopy (SEM) analysis revealed superporous nature of ALH-g-PAA. The gel fraction (%) of ALH-g-PAA was directly related to the concentrations of AA and MBA whereas the sol fraction was inversely related to the concentrations of AA and MBA. The porosity (%) of ALH-g-PAA directly depends on the concentration of AA and MBA. The ALH-g-PAA swelled admirably at pH 7.4 and insignificantly at pH 1.2. The ALH-g-PAA offered on/off switching properties at pH 7.4/1.2. The metoprolol tartrate was loaded on different formulations of ALH-g-PAA. The ALH-g-PAA showed pH, time, and swelling-dependent release of metoprolol tartrate (MT) for 24 h following the first-order kinetic and Korsmeyer-Peppas model. Haemocompatibility studies ascertained the non-thrombogenic and non-hemolytic behavior of ALH-g-PAA.

Vaccines are used for the control of infectious diseases of animals. Over other types of vaccinations like live attenuated or killed vaccines, mRNA-based vaccines have significant advantages. As only a small portion of the pathogen's genetic material is employed and the dose rate of mRNA-based vaccines is low, there is the least possibility that the pathogen will reverse itself. A carrier or vehicle that shields mRNA-based vaccines from the host's cellular RNases is necessary for their delivery. mRNA vaccines have been shown to be effective and to induce both a cell-mediated immune response and a humoral immune response in clinical trials against various infectious diseases (viral and parasitic) affecting the animals, including rabies, foot and mouth disease, toxoplasmosis, Zikavirus, leishmaniasis, and COVID-19. The current review aims to highlight the use of mRNA-based vaccines both in viral and parasitic diseases of animals.

The world is facing water crises because freshwater scarcity has become a global issue due to rapid population growth, resulting in the need for more industries, agriculture, and domestic sectors. Therefore, it is challenging for scientists and environmental engineers to treat wastewater with cost-effective treatment techniques. As compared to conventional processes (physical, chemical, and biological), advanced oxidation processes (AOP) play an essential role in the removal of wastewater contaminants, with the help of a powerful hydroxyl (OH•) through oxidation reactions. This review study investigates the critical role of O3-based Advanced Oxidation Processes (AOPs) in tackling the complex difficulties of wastewater treatment. Effective treatment methods are critical, with wastewater originating from various sources, including industrial activity, pharmaceutical manufacturing, agriculture, and a wide range of toxins. O3-based AOPs appear to be powerful therapies capable of degrading a wide range of pollutants, including stubborn organics, medicines, and pesticides, reducing environmental and human health risks. This review sheds light on their efficacy in wastewater treatment by explaining the underlying reaction mechanisms and applications of several O3-based AOP processes, such as O3, O3/UV, and O3/H2O2. Ozone, a powerful oxidizing agent, stimulates the breakdown of complex chemical molecules by oxidation processes, which are aided further by synergistic combinations with ultraviolet (UV) radiation or hydrogen peroxide (H2O2). Notably, while ozonation alone may not always produce the best outcomes, it acts as an essential pretreatment step prior to traditional treatments, increasing total treatment efficiency. Furthermore, O3-based AOPs' transformational capacity to convert organic chemicals into simpler, more stable inorganic forms with little sludge creation emphasizes its sustainability and environmental benefits. This study sheds light on the processes, uses, and benefits of O3-based AOPs, presenting practical solutions for sustainable water management and environmental protection. It is a valuable resource for academics, engineers, and politicians looking for new ways to combat wastewater contamination and protect water resources.

PURPOSE: Statins medications negatively affect age-associated loss of muscle mass and strength, termed sarcopenia, and neuromuscular junction (NMJ) integrity. However, their association with the sarcopenia-related-quality-of-life (SarQoL) is unknown. METHODS: In this cross-sectional, case control study, we recruited male nonusers (n = 75 and age 75.2 ± 5.9 years) and users (n = 77 and age 77.1 ± 6.2 years) of statins to evaluate SarQoL and handgrip strength (HGS). We also measured plasma C-terminal agrin fragment-22 (CAF22) as a marker of NMJ degradation. RESULTS: Statin users had higher CAF22, and lower HGS, and cumulative SarQoL scores than non-users (all p < 0.05). Plasma CAF22 exhibited negative correlations with SarQoL scores for physical and mental health, locomotion, functionality, activities-of-daily-living, and cumulative SarQoL in statins users and non-users (all p < 0.05). Lastly, the cumulative SarQoL scores exhibited positive associations with HGS and gait speed in the study participants (all p < 0.05). CONCLUSIONS: Collectively, statin usage was associated with NMJ degradation and reduced SarQoL. Statins should be cautiously prescribed in patients with sarcopenia with reduced QoL.

The thermal stability of polyurethanes, known for its limitations, was addressed in this research by seeking improvement through the introduction of carbohydrate-based chain extenders. In this research paper, we systematically sought to improve the thermal resistance of polyurethanes by incorporating carboxymethyl cellulose and chitosan, representing a pioneering application of the mixture design approach in their preparation. In this synthesis, hydroxyl-terminated polybutadiene and isophorone diisocyanate (IPDI) were reacted to prepare -NCO terminated prepolymer, which was subsequently reacted with varying mole ratios of CMC and CSN to develop a series of five PU samples. The prepared PU samples were characterized using the Fourier-transformed infrared spectroscopic technique. Thermal pyrolysis of PU samples was examined using thermal gravimetric analysis (TGA). It was observed that, among all the samples, PUS-3 showed remarkable thermal stability over a wide temperature range. A comprehensive statistical analysis was conducted to substantiate the experimental findings. It was estimated that CMC and CSN significantly enhance the thermal stability of the samples when involved in an interaction fashion. The ANOVA Table for the mixture design demonstrates that over 90 % of the total variation in thermal stability is explained by the mixture model across a wide temperature range. Moreover, PSU-3 exhibited 4 % more thermal stability over a wide range of temperatures on average, as compared to contemporary samples.

This study investigated the habits and attitudes of individuals towards poultry consumption, utilizing primary data collected through a survey of 5 households from 285 localities in Tehsil Shakargarh, Punjab, Pakistan (n = 1425). Household selection was randomized, and personal visits were conducted for data collection via formal interviews employing a structured questionnaire. Coordinates for each site were obtained using a Garmin eTrex device, in conjunction with meteorological data, to determine global positioning system (GPS) coordinates. A notable portion of respondents (38.8 %) possessed basic knowledge, while the majority (61.2 %) demonstrated intermediate knowledge regarding commercial broilers (chickens raised for meat production). A significant proportion (70.3 %) harbored misconceptions about the inclusion of hormones/antibiotics in poultry feed, with a minority (0.2 %) misinformed about broiler chickens' leg weakness. Some respondents (17.3 %) held both misconceptions, while others (12.2 %) had none. The majority (97.6 %) favored egg consumption, with 51.7 % preferring commercial chicken eggs and 48.3 % opting for domestic chicken eggs. Preference for white-colored eggs (51.5 %) slightly outweighed that for brown-colored eggs (48.5 %). A minority (1.3 %) speculated that poultry consumption could be a potential cause of COVID-19, while the majority (65.7 %) disagreed, and a portion (33.0 %) remained uncertain. Nearly all respondents (99.9 %) believed in the immunity-boosting properties of protein intake, with 65 % associating such benefits with poultry meat and eggs. Similarly, 99.7 % did not encounter difficulties in accessing poultry products during lockdowns. Approximately half (46.3 %) of respondents believed that consuming well-cooked and safely handled poultry meat was safe during outbreaks. Poultry meat and eggs emerged as potentially efficient sources of nutrition during the COVID-19 pandemic, especially for protein-deficient populations like Pakistan. Therefore, initiatives should focus on enhancing commercial poultry production and educating the populace about its advantages.

OBJECTIVES: Metformin protects against age-related muscle decline, termed sarcopenia. However, the effects on sarcopenia quality-of-life (SarQoL) are unknown. We investigated the effects of metformin on SarQoL and associated mechanisms in older adults. METHOD: This double-blind randomized, placebo-controlled trial included geriatric adult men, divided into non-sarcopenic controls (age = 72.2 ± 4.3 years, n = 52) and two groups of patients with sarcopenia randomized into placebo (age at baseline = 74.4 ± 5.7 years, n = 54) and metformin (age at baseline = 71.2 ± 3.9 years, n = 47) groups. Patients in the metformin group received 1.7 grams twice daily for four months. We evaluated SarQoL, handgrip strength (HGS), plasma zonulin, c-reactive protein (CRP), and 8-isoprostanes. RESULTS: Patients with sarcopenia had lower HGS and SarQoL than controls (both p <0.05). Metformin improved the HGS and the SarQoL domains related to physical and mental health, locomotion, and leisure activities, as well as cumulative SarQoL scores (all p <0.05). Metformin also prevented the decline in the SarQoL domains for functionality and fear. Among plasma biomarkers, metformin reduced the levels of zonulin, CRP, 8-isoprostanes, and creatine kinase. We also found a significant correlation of plasma zonulin with cumulative SarQoL in patients with sarcopenia taking metformin, suggesting a role for intestinal repair in improving SarQoL. Finally, metformin did not affect body composition and gait speed. CONCLUSION: Overall, metformin improved HGS and SarQoL by repairing intestinal leakage. Our data have clinical relevance for improving the quality of life in older adults with sarcopenia.

BACKGROUND: Age-associated muscle decline, termed sarcopenia, is a common systemic effect of chronic obstructive pulmonary disease (COPD). Circulating Neurofilament light chain (NfL) levels reflect neuronal degradation and may be relevant to sarcopenia phenotype. However, such an association in COPD patients remains elusive. METHODS: We investigated male, 60-76 years old controls (n = 50) and COPD patients (n = 139) for plasma NfL levels in relation to sarcopenia and physical capacity markers. We measured handgrip strength (HGS), body composition, and short physical performance battery (SPPB) to evaluate sarcopenia and physical capacity. RESULTS: COPD patients had higher plasma NfL and lower HGS and SPPB performance than controls. Plasma NfL levels demonstrated negative associations with HGS and gait speed in COPD patients (all p < 0.05). Further, NfL levels were negatively associated with total SPPB scores in controls and patients with advanced COPD (p < 0.05). Plasma NfL also demonstrated an acceptable accuracy in diagnosing sarcopenia in controls (AUC = 0.757, p < 0.05) and COPD (AUC = 0.806, p < 0.05) patients. CONCLUSION: Collectively, plasma NfL may be helpful in evaluating sarcopenia phenotype and physical capacity in geriatric patients with COPD.

This study displays the effect of reduced graphene oxide (rGO) nanofiller and polystyrene-b-poly(ethylene-ran-butylene)-b-polystyrene-grafted maleic anhydride (SEBS-g-MA) on the optical, thermal, and mechanical features of expanded polystyrene (EPS). First, the thin films of pristine EPS and composites were prepared using solution cast method. The prepared films were subjected to fourier-transform infrared (FTIR), SEM, UV-visible spectrophotometer, thermogravimetric analysis/differential scanning calorimetry, and universal testing machine for structural, morphological, optical, thermal, and mechanical characterizations. Optical study revealed a significant increase in refractive index and absorption of composites than EPS. Indirect band-gap energy of EPS (~4.08 eV) was reduced to ~1.61 eV for rGO composite and ~ 2.23 eV for composite composed of rGO and SEBS-g-MA. Thermal analyses presented improvement in characterization temperatures such as T10, T50, Tp, Tm, and Tg of composites, which ultimately lead to the thermal stability of prepared composites than pristine EPS. Stress-strain curves displayed higher yield strength (46.62 MPa), Young's modulus (96.29 MPa), and strain at break (0.54%) for EPS+rGO composite than pure EPS having stress at break (1.01 MPa), Young's modulus (12.44 MPa), and strain at break (0.08%). Moreover, ductility with relatively higher strain at break (0.61%) and lower Young's modulus (79.32 MPa) and yield strength (32.98 MPa) was noticed in EPS+rGO+SEBS-g-MA composite than EPS+rGO composite film. Morphological analysis revealed a change in globular morphology of EPS and inhomogeneous dispersion of rGO in EPS to homogeneously dispersed rGO in EPS matrix without globules owing to the addition of SEBS-g-MA. The increase in compatibility of EPS and rGO due to SEBS-g-MA was also observed in FTIR spectra. RESEARCH HIGHLIGHTS: Here, the solution casting approach was used to create the composite film of EPS and rGO with globules of various sizes. After adding SEBS-g-MA, the shape altered to globular free films exhibiting homogenous dispersion of rGO in EPS matrix. An optical investigation showed that composite materials had a significantly higher refractive index and absorption than EPS. The optical, thermal, and mechanical investigations suggest that the produced composites may be a great substitute for virgin EPS, allowing for a wider range of applications.
