RESUMEN

Presentation of one case of obstructive anuria secondary to perianeurysmal retroperitoneal fibrosis (PRF). Diagnosis was confirmed by abdominal ultrasound and CAT. The condition's initial treatment consisted in bilateral percutaneous nephrostomy. Following clinical stabilization of the patient, surgical resection of aorta aneurysm and ureterolysis with bilateral intraperitonealization was performed. A revision of the specific literature revealed 17 cases similar to ours. Clinical, diagnostic, and pathophysiologic aspects as well as recommended therapeutical procedures are analyzed.

RESUMEN

Contribution of one case of Spontaneous Perirenal Haematoma (SPH) secondary to hydronephrosis accompanied by lithiasis in pelvic kidney. SPH can result from multiple causes, the most frequent ones being renal tumours, most particularly angiomyolipoma and hypernephroma. Hydronephrosis is an uncommon cause of SPH, and very few cases are described in the literature. The present paper addresses the diagnostic usefulness of the various radiological tests as well as the therapeutical approach to be followed.

RESUMEN

Presentation of an uncommon paratesticular tumour, located in the spermatic cord and originated in the smooth muscle of the cremaster, deferent, or the cord's vascular structures. In 1966, KYLE in Canada reviewed 22 cases of paratesticular leiomyosarcoma from the English literature, and in 1984 PUTS in the Netherlands extended this catalogue to 39. TAKAHA in 1991 presented 18 cases found in the Japanese literature. The theory of malignant degeneration from a previously existing leiomyomatous tumour is generally accepted. This entity is clinically associated with a clear tendency to local relapse, which does not imply a worse prognosis, and with a high percentage of blood metastasis three years after exeresis. The choice therapy is oncologic inguinal orchiectomy. Radiotherapy and chemotherapy, as coadjuvant therapies, appeared to be more effective in children's than in adult's sarcoma. Prognosis of these neoplasias in the reviewed literature is highly variable, ranging between 10% and 62% survival at 5 years, possibly due to the diversity of therapies and the patient's stage at the time of diagnosis.

RESUMEN

A case of sarcomatoid carcinoma of the bladder is described. Its interest basically stems from the fact that this extremely aggressive tumor is uncommon in the bladder and there is existing controversy as to its anatomopathological distinction from other carcinosarcomas, spindle cell sarcoma and pseudosarcomatous bladder lesions.

RESUMEN

We evaluated 287 cases of blunt renal trauma that had been treated at the Urology Department of the Valle de Hebrón Hospital from 1974 to 1987. Data gleaned from patient work up, diagnosis and treatment during the acute phase are presented, highlighting the early and long-term results. Patient follow-up ranging from 1 to 10 years and data from statistical analyses show that conservative treatment achieved better early and late results than surgical treatment of the compromised renal unit. Hypertension was an uncommon secuela in our series (1% incidence). Evaluation with radioisotopes has proved to be the most reliable diagnostic method in the long-term assessment of secuelae in the renal unit.

RESUMEN

Various surgical procedures have been described for correction of stress urinary incontinence (SUI) which have afforded different results. Modifications to these procedures have been made in an attempt to reduce morbidity and the operating time without detriment to the efficacy of the procedures. Herein we describe two procedures, the modified Cobb-Radge and Raz techniques, and report on the results achieved. In patients with no cystocele that had been submitted to the modified Cobb-Radge technique, we achieved 80.4% good results. In patients with significant cystocele, our modified Cobb-Radge technique and anterior colporrhaphy achieved a success rate of 80%, and we achieved good results in 88.3% of these cases with our modified Raz procedure.

RESUMEN

Herein we describe two additional cases of non-acquired arteriovenous renal fistula, one was congenital or cirsoid and the other was idiopathic or aneurysmal. Patient clinical work up prompted us to suspect this condition which was confirmed by renal arteriography. The usefulness of the diagnostic tests are highlighted and the incidence and management of this type of vascular malformation are discussed.

RESUMEN

Post-traumatic renal hypertension is an uncommon sequela of renal trauma that may appear late making long follow-up advisable. The pathophysiology of this condition, the diagnostic methods, its different modes of presentation including transient and reversible or irreversible permanent hypertension are discussed. The usefulness of renal function tests in its diagnosis and the criteria for reversibility with surgical treatment are analyzed. Three cases of hypertension that presented late (2, 12, and 20 years after renal trauma) are described and the literature reviewed.
