RESUMEN

Sexual disorders such as erectile dysfunction (ED), sterility, and sexual inappetence represent some of the complex reproductive challenges that require addressing the underlying causes. The aim of this paper was to systematically synthesize literature on the ethnobotany, phytochemistry, bioactivities, and safety of plants used as remedies for managing sexual dysfunction and infertility, and improving fertility and virility in the EAC. Through an extensive review conducted in multidisciplinary electronic databases, 171 plant species were identified to have been reported for the management of sexual inappetence (i.e., used as aphrodisiacs, 39.4%), ED (35.9%), infertility (18.7%), and increasing fertility (6.0%). The most used plants are Mondia whitei, Acalypha villicaulis, Combretum illairii, Erythrina abyssinica, Pappea capensis, Rhus vulgaris, and Warburgia ugandensis while roots (44.9%), leaves (21.8%), stem and root barks (16.7%) of shrubs (35%), trees (31%), herbs (26%), and climbers (8%) are the preferred organs for making decoctions (69%). The research strides to date indicate that Citropsis articulata, Cola acuminata, Ekebergia capensis, Plumbago zeylanica, Tarenna graveolens, Urtica massaica, and Zingiber officinale have been assessed for their bioactivity. The majority (71.4%) of the plants either increased testosterone levels and mounting frequency or elicited prosexual stimulatory effects in male rats. More studies investigating the relevant pharmacological activities (aphrodisiac, fertility, and phosphodiesterase-5 inhibitory activities), safety aspects, responsible compounds, and clinical studies are warranted to establish the pharmacological potential of the unstudied species and elucidate the mechanism of action of the bioactive compounds.

BACKGROUND: Spermacoce princeae (K. Schum) has been used in the treatment of bacterial skin infections in Uganda. Pharmacological studies revealed that extracts of S. princeae exhibited antibacterial, antioxidant, and sun protection potential. This study aimed at isolating and identifying pure compounds from the extracts based on comprehensive analytical characterization by multiple analytical techniques. METHODS: The plant samples were extracted by sequential maceration using n-hexane, ethyl acetate, methanol, and distilled water. The compounds were isolated using a combination of chromatographic techniques and their structures were elucidated by multiple spectroscopic techniques. The antibacterial and antifungal activity determination of the isolated compounds was carried out using an agar well diffusion and potato dextrose assay against Pseudomonas aeruginosa, Staphylococcus aureus, Escherichia coli, Klebsiella pneumoniae, Candida albicans, and Aspergillus flavus while the antioxidant activity was screened with the 2,2-diphenyl-2-picryl-hydrazyl (DPPH) radical scavenging assay. The sun protection factor was determined using a Shimadzu Ultra Violet-visible (UV-VIS) double beam spectrophotometer between 290 to 320 nm. RESULTS: Eleven compounds; quercetin (1), kaempferol-3-O-rutinoside (2), rutin (3, 12), myo-inositol (4), asperulosidic acid (5), hexadecanoic acid (6), ß-sitosterol (7), stigmasterol (8), campesterol (9), ursolic acid (10), and ß-sitosterol glucoside (11) were identified in the S. princeae extracts. Compound 2 had good antifungal activity against C. albicans (zone of inhibition, 23.0 ± 0.1 mm). Compound 10 showed antibacterial and antifungal activity against S. aureus, P. aeruginosa, C. albicans, and A. flavus. Compound 2 had a good percentage radical scavenging effect (IC50 = 64.81 µg/ml) and a good sun protection factor (SPF = 26.83). CONCLUSION: This study reports the first-time isolation and identification of compounds 1 to 11 from S. princeae, which contribute to its antimicrobial, antioxidant, and sun protection potential.

BACKGROUND: Adolescent girls and young women (AGYW) account for 29% of new HIV infections in Uganda despite representing just 10% of the population. Peer support improves AGYW linkage to HIV care and medication adherence. We evaluated the feasibility and acceptability of peer delivered HIV self-tests (HIVST) and oral pre-exposure prophylaxis (PrEP) to young women in Uganda. METHODS: Between March and September 2021, we conducted a pilot study of 30 randomly selected young women, aged 18-24 years, who had received oral PrEP for at least three months, but had suboptimal adherence as measured by urine tenofovir testing (< 1500 ng/ml). Participants were offered daily oral PrEP and attended clinic visits three and six months after enrollment. Between clinic visits, participants were visited monthly by trained peers who delivered HIVST and PrEP. Feasibility and acceptability of peer-delivered PrEP and HIVST (intervention) were measured by comparing actual versus planned intervention delivery and product use. We conducted two focus groups with young women, and five in-depth interviews with peers and health workers to explore their experiences with intervention delivery. Qualitative data were analyzed using thematic analysis. RESULTS: At baseline, all 30 enrolled young women (median age 20 years) accepted peer-delivered PrEP and HIVST. Peer delivery visit completion was 97% (29/30) and 93% (28/30) at three and six months, respectively. The proportion of participants with detectable tenofovir in urine was 93% (27/29) and 57% (16/28) at months three and six, respectively. Four broad themes emerged from the qualitative data: (1) Positive experiences of peer delivered HIVST and PrEP; (2) The motivating effect of peer support; (3) Perceptions of female controlled HIVST and PrEP; and (4) Multi-level barriers to HIVST and PrEP use. Overall, peer delivery motivated young women to use HIVST and PrEP and encouraged persistence on PrEP by providing non-judgmental client-friendly services and adherence support. CONCLUSION: Peer delivery of HIVST and oral PrEP was feasible and acceptable to this sample of young women with suboptimal PrEP adherence in Uganda. Future larger controlled studies should evaluate its effectiveness among African AGWY.

High solids anaerobic digestion (AD) of chicken manure (CM) is often challenging due to ammonia-N inhibition and accumulation of volatile fatty acids (VFAs). This study evaluated the effect of adding biochars from different feedstock to ameliorate semi-dry AD of fresh CM during batch fermentation. Experiments were performed in 300 mL at two total solid (TS) levels (12% and 15%) under mesophilic (36 ±1ᵒC) conditions for 55 d, using activated sludge as inoculum. Treatments included: fresh CM (at 12% or 15% TS) mixed separately with rice husks char (RB), wood char (WB) and bamboo char (BB) at biochar dosages of 2.5%, 5% and 10% of TS in the CM, inoculum only and inoculum plus CM without addition of char as the control. Results indicated that addition of biochar reduced the lag phases to 4-5.4 d and AD performances were significantly improved with total volatile solids removal of 53-67% and 62-71%, and cumulative methane of 277-380 mL/gVS (CH4 content ≈ 51-63%) and 297-438 mL/gVS (CH4 content ≈ 49-67%) at 12% and 15% TS, respectively. Biochar buffered over acidification and stabilized pH in the range of 6.5-7.8 but mild ammonia inhibition still occurred in all biochar treatments due to the high residual total ammonia-N (4.3 g-5.6 g/L). For all the investigated parameters, WB amended digesters exhibited the best results owing to its high specific surface area, porosity, cationic exchange capacity, and elemental composition which were superior to those of RB and BB. At 10% dosage of all tested biochars, the AD process was more stable and methane content neared optimal of >65% CH4. Therefore, addition of biochar from lignocellulosic materials at a given threshold dosage could promote semi-dry and dry biogas production from chicken manure and thus add value to this waste which in most cases is improperly managed.

BACKGROUND: Rural populations in Uganda rely heavily on medicinal plants for the treatment of bacterial skin infections. However, the efficacy of these medicinal plants for their pharmacological action is not known. The study aimed at evaluating the antibacterial, antioxidant, and sun protection potential of Spermacoce princeae, Psorospermum febrifugum, Plectranthus caespitosus, and Erlangea tomentosa extracts. METHODS: The plant samples were extracted by maceration sequentially using hexane, dichloromethane, ethyl acetate, methanol, and distilled water. Antibacterial activity of each extract was carried out using an agar well diffusion assay against Pseudomonas aeruginosa, Staphylococcus aureus, Escherichia coli, Klebsiella pneumonie, Streptococcus pyogenes, and Salmonella typhi. Acute dermal toxicity of the aqueous extract of S. princeae and P. febrifugum, and E. tomentosa was assessed in young adult healthy Wistar albino rats at a dose of 8000 and 10,000 mg/kg body weight. The antioxidant activity of each extract was carried out using a 1,1-diphenyl-2-picryl-hydrazyl (DPPH) radical scavenging assay. The sun protection factor was determined using Shimadzu UltraViolet-Visible double beam spectrophotometer between 290 and 320 nm. RESULTS: The plant extracts showed good antibacterial activity against the tested bacterial strains with minimum inhibitory concentration (MIC) ranging between 3.12 and 12.5 mg/ml. There was no significant change in the levels of creatinine, alanine aminotransferase, and aspartate aminotransferase in the rats even at a higher dose of 10,000 mg/kg, which was related to the results of biochemical analysis of the blood samples from the treated and control groups. The aqueous and methanol extracts of S. princeae showed potential antioxidant properties, with half maximal inhibitory concentration (IC50) values of 59.82 and 61.20 µg/ml respectively. The organic and aqueous extracts of P. caespitosus showed high levels of protection against Ultraviolet light with sun protection potential values ranging between 30.67 and 37.84. CONCLUSIONS: The study demonstrated that the selected medicinal plants possessed good antibacterial, antioxidant, and sun protection properties. Therefore, the plants are alternative sources of antibacterial, antioxidant, and sun protection agents in managing bacterial skin infections.

The impact of oil exploration and production activities on the environment of sub-saharan African countries is not well studied. This study aimed at determining concentrations, sources, and bioaccumulation of 13 polycyclic aromatic hydrocarbons (PAHs) in sediments and fish from the White Nile near Melut oil fields, South Sudan. The study also assessed the ecological and human health risk associated with PAHs in this aquatic system. Total (∑13) PAH concentrations ranged from 566 to 674 ng g-1dry weight (dw) in sediments, while those in fish were 191-1143 ng g-1 wet weight (ww). ∑13PAH concentrations were significantly higher in C. gariepinus than in other fish species. Low molecular weight PAHs (LPAHs) dominated the profile of PAHs in sediments (constituted 95% of ∑13PAHs) and fish (97% of ∑13PAHs). Compared to Sediment Quality Guidelines of the United States Oceanic and Atmospheric Administration, the levels of LPAHs in this study were all above the threshold effect limits, but below the probable effect level, while those of high molecular weight PAHs (HPAHs) were all below the lowest effect levels. The carcinogenic potency equivalent concentrations of PAHs in L. niloticus and C. gariepinus were above the US EPA screening level; suggesting consumption of these species could adversely affect human health. Biota-sediment accumulation factor values (range: 0.006-3.816 g OC g-1 lipid) for PAHs showed high bioaccumulation of LPAHs in fish muscle, and that bioaccumulation decreased with increase in hydrophobicity of the compounds. This is possibly because LPAHs have higher aqueous solubilities which increases their bioavailability through water-gill transfers compared to HPAHs. Profiles of PAHs in the White Nile environment indicate predominant contribution from petrogenic sources, which could be attributed to presence of crude oil reservoirs and oil production operations. More research into the levels of other environmental pollutants in the oil-rich area is recommended.

There is little information on the distribution of microcystins (MCs) in Oreochromis niloticus (ON) and Lates niloticus (LN) obtained from L. Mburo and Murchison Bay of L. Victoria. These fishes are harvested and sold both for local human consumption and for export. The presence of MC-RR, MC-LR and MC-YR in different organs (gut, muscle and liver) was determined using Liquid Chromatography coupled with a Mass Spectrometry Detector (LC/MS/MS). The total MCs in ON gut, muscle and liver were 1312.08, 208.65 and 73.10 ng g(-1) from L. Mburo and 1479.24, 9.65 and 48.07 ng g(-1) from Murchison Bay, respectively, while for LN from Murchison Bay they were 27.78, 1.86 and 3.74 ng g(-1). Generally, in both lakes, MC-RR was the most dominant followed by MC-YR and MC-LR, respectively. Gut showed a high MC content, followed by liver and muscle, in that order. The presence of MCs in muscle indicated possible fish contamination, which implied that it was possible to transfer the toxins to humans who are at the end of the food chain. This poses a risk to them since the MCs are heat stable. The local authorities should warn the public of the risk of possible poisoning by eating the contaminated fish.
