RESUMEN

Acquired digital fibrokeratoma is a rare, benign tumor that mostly occurs on the fingers and toes and may appear to be a supernumerary rudimentary digit. It generally affects adult men and appears as a dome-shaped papule although it can also be elongated or pedunculated. Trauma is believed to be a triggering factor in some cases. We report a male patient with an acquired digital fibrokeratoma on a finger, shaped like a cutaneous horn, and a history of minimal repeated trauma and spontaneous remissions not previously described in the literature.

RESUMEN

Granuloma annulare (GA) is a benign, self-limited inflammatory skin condition with an unknown etiology. Although it usually presents with characteristic clinical features, a biopsy may be necessary in atypical cases to differentiate it from other granulomatous diseases. We describe a case of a 66-year-old female with two concomitant subtypes of GA, presenting with distinct clinical features but exhibiting similar histopathological findings. The patient had extensive, pruritic erythematous-violaceous lesions on her lower abdomen, buttocks, and proximal thighs, which had been progressing over the course of one year. Biopsies from the abdominal and thigh lesions showed typical histopathological features of GA, with mucin deposition, histiocytic infiltrate, and granulomatous formations. Treatment with oral antihistamines and medium-potency topical corticosteroids effectively controlled the itching but did not alter the lesion's appearance. Five months later, the patient developed new, pruritic, skin-colored, confluent papules on the internal face of her left arm, and a subsequent biopsy confirmed annular GA. Although the patient did not follow the prescribed dapsone treatment, the lesions spontaneously regressed within a year. This case emphasizes the importance of recognizing less common presentations of GA, which can mimic other, more concerning conditions. While various therapeutic options have been explored, none guarantee complete remission; however, GA typically resolves on its own over time. A better understanding of the disease's pathogenesis and the development of targeted treatments are warranted to improve management strategies for GA.

RESUMEN

Pigmentary demarcation lines (PDL), or Voigt-Futcher lines, are lines that mark an abrupt transition between hyperpigmented skin and normal skin. PDLs are more common in Japanese and dark-skinned individuals. Eight types have been described (A-H); Type B is located on the posteromedial aspect of the lower extremities; it is more common in women and is the one most frequently associated with pregnancy. The demarcation lines of pregnancy are of unknown etiology; they appear mainly in the last trimester and disappear spontaneously months after delivery. We report a case of pregnancy-associated PDL with erythema without melanocytic pigmentation in a 23-week-gestational Latin primiparous woman.

El dolor torácico es un motivo de consulta frecuente en los servicios de urgencia. Su espectro de presentaciones y su diagnóstico diferencial es amplio, con patologías de elevada morbilidad y mortalidad asociadas. Es el síntoma principal en pacientes con un síndrome coronario agudo y, ante su sospecha es mandatorio realizar una evaluación inicial centrada en la estratificación de riesgo de sufrir eventos adversos en cada paciente, para así definir su tratamiento y disposición posterior de forma correcta. Objetivo: presentar los elementos que componen la evaluación inicial del dolor torácico ante una sospecha de síndrome coronario agudo y las herramientas disponibles para realizar la estratificación de riesgo y así guiar la disposición desde el servicio de urgencia. Método: Se realizó una revisión bibliográfica de la literatura sobre la estratificación de riesgo del dolor torácico, buscando la evidencia actual respecto a las herramientas diagnósticas utilizadas habitualmente en el servicio de urgencia. Resultados: Se presenta una revisión con generalidades del dolor torácico, sus diagnósticos diferenciales, los elementos de la evaluación inicial y las herramientas clínicas para la evaluación de riesgo de pacientes con dolor torácico y sospecha de síndrome coronario agudo en el servicio de urgencia. Discusión y conclusiones: La presentación del síndrome coronario agudo es variable en la población. Ante la presencia de un cuadro de dolor atípico y/o un electrocardiograma no diagnóstico, recomendamos el uso de un sistema de puntaje validado como el HEART / HEART pathway para reducir la posibilidad de una inadecuada estratificación de riesgo en el servicio de urgencia


Chest pain is a common complaint in emergency departments. The spectrum of presentation and its differential diagnosis are broad, including pathologies associated with high morbidity and mortality, and it is the main symptom in patients suffering from acute coronary syndrome. If suspected, it is mandatory to work out an initial evaluation focused on the risk stratification of adverse events for each patient to define their correct treatment and disposition. Objective: show the elements that involve the initial evaluation of chest pain suspicious of an acute coronary syndrome, the clinical tools available to perform risk stratification, and guide the disposition from the emergency department. Method: a review of the literature on chest pain risk stratification was performed, looking for current evidence of the most commonly used diagnostic tools in emergency departments. Results: we present a literature review of generalities about chest pain and its differential diagnoses, the elements to consider in the initial evaluation, and clinical tools for risk stratification of patients with suspected acute coronary syndrome at the emergency department. Discussion and conclusions: the presentation of acute coronary syndrome is variable in the population. In the presence of atypical chest pain or a non-diagnostic electrocardiogram, we recommend using a validated score as the HEART / HEART Pathway to reduce the chance of inadequate risk stratification in the emergency department.

Superficial angiomyxomas, also known as cutaneous myxomas, are rare, benign soft tissue tumors that present as papulonodular or polypoid, asymptomatic, slow-growing lesions. They typically occur in the head, neck, trunk, and extremities of adults and may be isolated tumors or part of the Carney Complex. We present a case of SA with an uncommon area of presentation and a brief discussion of the importance of ruling out the presence of systemic syndromes such as the Carney Complex.

RESUMEN

En Diciembre 2019, surge una enfermedad emergente, COVID-19, que es debida a un coronavirus (SARS-CoV-2) que empieza en Wuhan, China y que se ha propagado rápidamente a nivel mundial. La Organización Mundial de la Salud (OMS) lo declara como pandemia el 12 de Marzo del 2020. En Chile, el día 14 de Marzo se decretó fase 3 y el 16 de Marzo se pasó a fase 4. Al 24 de Marzo del 2020, en Chile existen 922 casos confirmados de COVID-19, con una mediana de edad de 34 años, correspondiendo 51 casos (6%) a adultos mayores (AM).
