This prospective study investigated the relationship between inflammation, damage-associated molecular patterns (DAMPs), and thrombus formation on dialyzer membranes in critically ill patients undergoing renal replacement therapy (RRT) from July 2020 to August 2022, identifying mechanisms and interventions to prevent clotting. The patients were divided into two groups: inflammatory (n = 56, serum C-reactive protein >10 mg/dl) and noninflammatory control (n = 45, serum C-reactive protein <5 mg/dl). Cell-free deoxyribonucleic acid (DNA) levels, high mobility group box 1 protein (HMGB1), histone H3, and myeloperoxidase (MPO) in the lumen of the hollow fiber membrane of the dialyzer were quantified. Immunostaining assessed leukocytes, fibrin fibers, and platelet thrombi on the luminal surface of the hollow fiber membrane. The inflammatory group, compared to controls, exhibited elevated cell-free DNA, HMGB1, and MPO levels, although histone H3 remained unchanged. Damage-associated molecular patterns increased with disseminated intravascular coagulation (DIC) severity. Immunostaining in the inflammatory group revealed leukocytes, amorphous nuclei, neutrophil extracellular trap-like structures, fibrin fibers, and platelet thrombi on the hollow fiber membrane's luminal surface. Elevated DAMP levels in severely inflamed patients' dialyzer membranes, correlating with DIC severity, indicate a link between inflammation, coagulation activation, and dialyzer clotting. Research into thrombus prevention in RRT for DIC-affected critically ill patients is warranted.

Background: Recent practice guidelines strongly recommend evidence-based psychotherapies (EBPs) as the first-line treatment for post-traumatic stress disorder (PTSD). However, previous studies found barriers to the implementation of EBPs and a relatively high dropout rate in clinical settings. After proving the efficacy of prolonged exposure (PE) in Japan [Asukai, N., Saito, A., Tsuruta, N., Kishimoto, J., & Nishikawa, T. (2010). Efficacy of exposure therapy for Japanese patients with posttraumatic stress disorder due to mixed traumatic events: A randomized controlled study. Journal of Traumatic Stress, 23(6), 744-750. https://doi.org/10.1002/jts.20589], we began implementing PE in a real-world clinical setting at the Victim Support Center of Tokyo (VSCT).Objective: We aimed to investigate the effectiveness and benefit of PE for crime-induced PTSD among VSCT clients and what causes dropout from treatment.Method: Of 311 adult clients who received counselling from clinical psychologists at VSCT due to violent or physical crime victimization from April 2008 through December 2019, 100 individuals received PE and participated in this study. Their PTSD symptoms were evaluated before and after treatment using the Impact of Event Scale-Revised and the Clinician-Administered PTSD Scale for DSM-IV.Results: A total of 93 participants completed PE and seven dropped out after six sessions or less. The completers group improved in PTSD symptoms with significant score differences between pre- and post-treatment in IES-R and CAPS-IV. Participants' symptoms did not exacerbate after treatment. Forty of 49 completers who left their workplace or college/school after victimization returned to work or study shortly after treatment. Compared to the completers, all dropout participants were women and younger. The majority were rape survivors, with significantly shorter intervals between victimization and treatment. The reasons for dropout were difficulty scheduling treatment between work/study schedules and manifestation of bipolar disorder or physical illness.Conclusions: PE can be implemented with significant effectiveness and a low dropout rate in a real-world clinical setting if advantages in the system and policies, local organizational context, fidelity support and patient engagement are fortified.


We conducted prolonged exposure (PE) with a low dropout rate for crime-induced PTSD in a non-Western real-world practice setting.Patient outcomes and low dropout rate of PE for PTSD in this study may be due to advantages in the following areas: system and policies, local organizational context, fidelity support and patient engagement.When introducing PE for PTSD, it is important to confirm that patients can be reasonably engaged with PE, and to carefully assess the status of other psychiatric and physical illnesses.

Serum dehydroepiandrosterone sulfate (DHEA-S) levels reflect the state of adrenocorticotropic hormone (ACTH) secretion. However, it is difficult to use serum DHEA-S to diagnose hypothalamic-pituitary-adrenal (HPA) axis insufficiency due to its non-normal and highly skewed distribution. In this study, we focused on HPA insufficiency caused by hypothalamic and/or pituitary dysfunction and evaluated the usefulness of the standard deviation score of log-transformed DHEA-S (ln DHEA-S SD score), which was calculated from the established age- and sex-specific reference values. We retrospectively reviewed the medical records of 94 patients suspected of having HPA insufficiency, in whom serum DHEA-S measurement and the rapid ACTH stimulation test were performed, and included 65 patients who met our criteria in this study. The ln DHEA-S SD scores were distributed more normally than measured DHEA-S levels and were significantly higher in patients with a peak cortisol level ≥18 µg/dL than in those below this value, suggesting that this score is a legitimate and strong indicator of adrenocortical function. The optimal cut-off value for impaired HPA function was -0.853, with a sensitivity of 70.3% and a specificity of 100%. Among the 37 patients whose peak cortisol levels were below 18 µg/dL, 11 patients with ln DHEA-S scores ≥-0.853 exhibited significantly higher basal ACTH and basal and peak cortisol levels than the 26 patients with scores <-0.853. Thus, this score plays a supportive role in evaluating HPA axis function, particularly in patients with borderline cortisol responses to ACTH.

BACKGROUND: Food frequency questionnaires are considered an effective method for assessing habitual dietary intake, but they must be developed or validated with the target population. Portion size, supplement use and food choice are thought to be especially important methodological considerations for assessing athletes' dietary intake. This study aimed to develop and validate a food frequency questionnaire for Japanese athletes using data from this population. METHODS: We used dietary records from 440 Japanese athletes involved in our previous projects. Food items were analyzed using cumulative percentage contributions and multiple regression analysis, to give a selection of 62 basic food items and four supplemental items. The validity of the questionnaire was evaluated among another 77 Japanese athletes by comparing nutrient intakes assessed using the questionnaire with dietary records. Reproducibility was evaluated by comparing a second questionnaire completed 2-3 weeks later by 36 of the athletes in the validation study. Validity was assessed using crude Spearman's correlation coefficients (CCs), energy-adjusted CCs, intraclass CCs (ICCs), and Kappa index values. Reproducibility was assessed by CCs, energy-adjusted CCs, and ICCs. RESULTS: In the validation analysis, the median crude CC for all of the nutrients was 0.407, ranging from 0.222 for dietary fiber to 0.550 for carbohydrate. The median energy-adjusted CC was 0.478, and the median ICC was 0.369. When we divided the athletes into quartiles, 65% (vitamin B1) to 86% (iron) of athletes were classified into the same or adjacent categories using the questionnaire and dietary records, with a median Kappa statistic of 0.32. In the reproducibility analysis, the median crude CC between the two completed questionnaires was 0.654, ranging from 0.582 (carbohydrate) to 0.743 (vitamin B2). The median energy-adjusted CC was 0.643, and the median ICC was 0.647. CONCLUSIONS: The new 62-item food frequency questionnaire is both reliable and valid and may be useful for assessing food intake in Japanese athletes.

BACKGROUND: Implantable Cardioverter-Defibrillator (ICD) shocks have been associated with mortality. However, no study has examined the relation between total shock energy and mortality. The aim of this study is to assess the association of total shock energy with mortality, and to determine the patients who are at risk of this association. METHODS: Data from 316 consecutive patients who underwent initial ICD implantation in our hospital between 2000 and 2011 were retrospectively studied. We collected shock energy for 3 years from the ICD implantation, and determined the relation of shock energy on mortality after adjusting confounding factors. RESULTS: Eighty-seven ICD recipients experienced shock(s) within 3 years from ICD implantation and 43 patients had died during the follow-up. The amount of shock energy was significantly associated with all-cause death [adjusted hazard ratio (HR) 1.26 (per 100 joule increase), p < 0.01] and tended to be associated with cardiac death (adjusted HR 1.30, p = 0.08). The survival rate of patients with high shock energy accumulation (≥182 joule) was lower (p < 0.05), as compared to low shock energy accumulation (<182 joule), likewise to no shock. Besides, the relation between high shock energy accumulation and all-cause death was remarkable in the patients with low left ventricular ejection fraction (LVEF ≤40%) or atrial fibrillation (AF). CONCLUSIONS: Increase of shock energy was related to mortality in ICD recipients. This relation was evident in patients with low LVEF or AF.

BACKGROUND: Ultrasound-guided transversus ab- dominis plane (TAP) and rectus sheath (RS) blocks are peripheral nerve blocks that diminish somatic pain of the abdominal wall and are useful for postoperative analgesia. Here, we retrospectively compared the effi- cacy of ultrasound-guided TAP and RS blocks in com- bination with continuous intravenous fentanyl for postoperative analgesia of laparoscopic colectomy. METHODS: The ethics committee of our institute ap- proved the study. In our hospital, postoperative analge- sia is performed with continuous intravenous fentanyl administration at three concentrations : 12.5, 18.75, and 31.25 µg · hr⁻¹. TAP and RS blocks were applied using 30-40 ml of 0.19-0.25% ropivacaine. We selected 43 patients who underwent laparoscopic colectomy from May to October 2015. We compared the fentanyl only group (F group, n=26) and block combination group (F+B group, n=17). Statistical analysis was performed with the Mann-Whitney U test P<0.05 was considered significant Results : Patient characteristics including age, height, body weight, duration of anesthesia, and surgery did not significantly differ between the two groups. The concentration of intravenous fentanyl was significantly lower in the F+B group than in the F group (F group : 19.5±8.4µg · hr⁻¹, F+B group : 14.7?4.9 µg · hr⁻¹, P=0.02). In the present study, TAP and RS blocks significantly reduced the dose of administered fentanyl. - Conclusions : Our findings suggest that a combina- tion of TAP and RS blocks lower the dose of continu- ous intravenous fentanyl needed, and may provide better postoperative analgesia after laparoscopic colec- tomy.

We report a patient with multiple myeloma who developed severe hemostatic dysfunction during spinal fusion surgery. A 74-year-old man presented with lower limb paralysis due to thoracic supine pathological fracture and was scheduled for spinal fusion surgery. He suffered from multiple myeloma for decades but did not present with any significant abnormalities in a preoperative blood exam. Prothrombin time international normalized ratio (PT-INR, 0.97) and activated partial thromboplastin time (APTT, 31 sec) were normal. Surgeons confronted hemostatic difficulty during surgery. At the point of 1,000 ml blood loss, PT-INR was 9.99, APTT was 300 sec, and platelet count was 116,000 x µl(-1). The patient was administered 1,400 ml of frozen plasma concentrate; PT-INR and APTT recovered to 1.04 and 39.6 sec, respectively. Hemostatic dysfunction in this patient may have resulted from an inherent coagulation deficiency associated with multiple myeloma.

PURPOSE: During out-of-hospital cardiopulmonary resuscitation, several factors can render tracheal intubation more difficult, such as when rescuers must secure the airway in complete darkness or with limited illumination. The purpose of this study was to evaluate the efficacy of six supraglottic devices (SGDs), ProSeal(®) (ProSeal), Classic(®) (Classic), Supreme(®) (Supreme), Laryngeal Tube(®) (LT), air-Q(®) (air-Q), and i-gel(®) (i-gel), for airway management under light and dark conditions using a manikin. METHODS: Seventeen novice doctors and 15 experienced doctors performed insertion of six SGDs under light and dark conditions using an adult manikin. Insertion time, successful ventilation rate, and subjective insertion difficulty on a visual analogue scale (VAS) were measured. RESULTS: Both novice and experienced doctors had a significantly lower ventilation success rate in the dark than in the light when ProSeal and Classic were used, but not with the other four SGDs. Novice doctors required a significantly longer insertion time in the dark than in the light with all SGDs. Experienced doctors required a significantly longer insertion time in the dark than in the light with ProSeal or Classic, but not with the other four SGDs. VAS was significantly higher for both novice and experienced doctors when ProSeal and Classic were used, as compared with the other four SGDs in the dark. CONCLUSIONS: Compared to ProSeal and Classic, Supreme, i-gel, LT, and air-Q are more effective for airway management in the dark. Our findings suggest that anatomically shaped SGDs may help novice doctors secure the airway under dark conditions.

We report a case of successful double-lumen endotracheal tube exchange with a soft-tipped extra firm exchange catheter in a patient with severe subcutaneous emphysema. A 70-year-old man underwent right lower lobectomy for primary lung cancer under general anesthesia. He developed pneumothorax on postoperative day (POD) 14, which led to subcutaneous emphysema. An emergent operation was performed on POD20 to close the pulmonary fistula under general anesthesia with a single-lumen endotracheal tube and bronchial blocker. Subcutaneous emphysema became worse and pharyngeal emphysema was also suspected; re-operation to close the pulmonary or bronchial fistula was planned. We decided to place a double-lumen tube to precisely detect the fistula. Under the guide of a Pentax-AWS Airwayscope, the single-lumen endotracheal tube was exchanged uneventfully to a 35 Fr double-lumen endotracheal tube with a 110 cm soft-tipped extra firm exchange catheter. The fistula was detected by a leak test and the operation was performed uneventfully, leading to improvement of subcutaneous emphysema.
