RESUMEN

The main objective of this study was to investigate the dynamic of SARS-CoV-2 viral excretion in rectal swab (RS), saliva, and nasopharyngeal swab (NS) samples from symptomatic patients and asymptomatic contacts. In addition, in order to evaluate the replication potential of SARS-CoV-2 in the gastrointestinal (GI) tract and the excretion of infectious SARS-CoV-2 from feces, we investigated the presence of subgenomic nucleoprotein gene (N) mRNA (sgN) in RS samples and cytopathic effects in Vero cell culture. A prospective cohort study was performed to collect samples from symptomatic patients and contacts in Rio de Janeiro, Brazil, from May to October 2020. One hundred and seventy-six patients had samples collected at home visits and/or during the follow up, resulting in a total of 1633 RS, saliva, or NS samples. SARS-CoV-2 RNA was detected in 130 (73.9%) patients who had at least one sample that tested positive for SARS-CoV-2. The presence of replicating SARS-CoV-2 in RS samples, measured by the detection of sgN mRNA, was successfully achieved in 19.4% (6/31) of samples, whilst infectious SARS-CoV-2, measured by the generation of cytopathic effects in cell culture, was identified in only one RS sample. Although rare, our results demonstrated the replication capacity of SARS-CoV-2 in the GI tract, and infectious viruses in one RS sample. There is still a gap in the knowledge regarding SARS-CoV-2 fecal-oral transmission. Additional studies are warranted to investigate fecal or wastewater exposure as a risk factor for transmission in human populations.

The Brazilian "National School Lunch Program" (PNAE) must provide healthy food for students in public primary education. Thus, it is necessary to ensure both nutritional and parasitological quality, reducing health risks. Vegetables must be clean, presenting no parasites and larvae, according to Brazilian legislation. Lettuce (Lactuca sativa L.) is one of the favorite vegetables among students, and is eaten raw in salads. This paper aims to evaluate the parasitological quality of lettuce served in school lunches. 72 samples of leaf lettuce, 36 from conventional agriculture (CA) and 36 from family agriculture (FA), were analyzed. Half the samples in each group were sanitized by immersion in a sodium hypochlorite solution, with 2% active stabilized chlorine for 10 minutes (n=18/each) and half were not sanitized (n=18/each). Parasite load evaluation was performed by two methods in each subgroup: spontaneous sedimentation (SS) and sedimentation by centrifugation (SC). The parasite frequencies found were evaluated by the chi-squared test. Medically relevant parasites identified were helminths (Strongyloides stercoralis, Ascaris lumbricoides and Hookworms) as well as protozoa (Balantidium coli and Entamoeba coli). 44.6% of the FA samples presented some form of parasite by SS evaluation and 33.4% by SC evaluation; 66.7% of the CA samples presented parasites by SS evaluation, and 44.5% by SC evaluation. No significant differences were noted between the FA and CA groups in either technique. No parasites were found in any of the sanitized lettuces, regardless of the subgroup or technique applied. These results evidence the importance of adequate training and guidance for vegetable growers, food handlers as well as the general population regarding proper hygiene of lettuce leaves prior to consumption.

Fulminant hepatic failure (FHF) is characterized by massive hepatocellular injury, whose physiopathology is still unclear. Hepatitis B (HBV) is probably the most common viral cause of FHF, while hepatitis A (HAV) virus seem occurs less frequently. However, the host and viral factors that determine the outcome of these infections are poorly understood. In the present study, viral load and genotyping determining regions of HAV and HBV genomes were sequenced. Eight FHF patients and one patient with severe acute hepatitis (SAH) were included. Liver and blood samples were collected during liver transplantation or necropsy procedures. HAV-RNA and HBV-DNA were extracted from serum, biopsy and paraffin liver. Nucleotide sequencing of HAV-RNA was performed from VP1/2A and HBV-DNA from PreS/S region. The amplified samples were quantified by Real-Time PCR. The cases of HAV infection were due to subgenotype IA. The cases of HBV infection were due to genotype A2 and D4. The case of HAV/HBV coinfection was infected by genotype IA and D3. Hepatitis A and B infection were associated with genotypes most prevalent in Brazil. In hepatitis A infection the mean of period evolution was 13 days. In hepatitis B, FHF patients infected by genotype D have a shorter period of evolution than FHF patients infected by genotype A (mean 15 v. 53 days). There was no association with genotype-determining region with the severity of hepatitis, however nucleotide differences and high viral load could be observed among FHF.
