RESUMEN

PURPOSE: Clinicians are tasked with using culturally and linguistically appropriate tools to evaluate oral and written language development accurately. However, limited tools account for linguistic diversity in writing. This gap can lead to under- and overdiagnosis of students who speak nonmainstream dialects. This study addressed that gap by developing a writing task to identify nonmainstream dialect features in the writing of early elementary school students. We describe the development, feasibility, and results of pilot testing of the task. METHOD: One hundred fifty-one first and second graders participated in the study as part of a larger study of nonmainstream dialect use. Students completed standardized literacy and language assessments and the researcher-developed writing task. The writing task used a novel fill-in-the-blank format to identify morphological features that vary between Mainstream American English and nonmainstream varieties such as African American English. RESULTS: Second-grade students performed better on the writing task than first graders, and writing performance was strongly related to standardized literacy scores. Literacy skills were the strongest predictor of Mainstream American English use in writing, but spoken dialect use also correlated with written dialect use. CONCLUSIONS: The writing task captured dialect use in early elementary school students' writing, and students' performance on standardized literacy measures predicted written dialect features. These results are a first step toward developing a standardized measure to help professionals appropriately diagnose written expression disorders within linguistically diverse students. PURPOSE: Stuttering-like disfluencies (SLDs) and typical disfluencies (TDs) are both more likely to occur as utterance length increases. However, longer and shorter utterances differ by more than the number of morphemes: They may also serve different communicative functions or describe different ideas. Decontextualized language, or language that describes events and concepts outside of the "here and now," is associated with longer utterances. Prior work has shown that language samples taken in decontextualized contexts contain more disfluencies, but averaging across an entire language sample creates a confound between utterance length and decontextualization as contributors to stuttering. We coded individual utterances from naturalistic play samples to test the hypothesis that decontextualized language leads to increased disfluencies above and beyond the effects of utterance length. METHOD: We used archival transcripts of language samples from 15 preschool children who stutter (CWS) and 15 age- and sex-matched children who do not stutter (CWNS). Utterances were coded as either contextualized or decontextualized, and we used mixed-effects logistic regression to investigate the impact of utterance length and decontextualization on SLDs and TDs. RESULTS: CWS were more likely to stutter when producing decontextualized utterances, even when controlling for utterance length. An interaction between decontextualization and utterance length indicated that the effect of decontextualization was greatest for shorter utterances. TDs increased in decontextualized utterances when controlling for utterance length for both CWS and CWNS. The effect of decontextualization on TDs did not differ statistically between the two groups. CONCLUSIONS: The increased working memory demands associated with decontextualized language contribute to increased language planning effort. This leads to increased TD in CWS and CWNS. Under a multifactorial dynamic model of stuttering, the increased language demands may also contribute to increased stuttering in CWS due to instabilities in their speech motor systems.

RESUMEN

We describe guest speaker presentations that we developed to bring language science to elementary school students via videoconference. By using virtual backgrounds and guided discovery learning, we effectively engage children as young as 7 years in in-depth explorations of language science concepts. We share the core principles that guide our presentations and describe two of our outreach activities, Speech Detectives and Bilingual Barnyard. We report brief survey data from 157 elementary school students showing that they find our presentations interesting and educational. While our pivot to virtual outreach was motivated by the Covid-19 pandemic, it allows us to reach geographically diverse audiences, and we suggest that virtual guest speaker presentations will remain a viable and effective method of public outreach.

PURPOSE: To describe one institution's experience in the rehabilitation of children with acute flaccid myelitis (AFM). This study reviews the medical and rehabilitative course and functional outcomes of a cohort of children who underwent Activity Based Restorative Therapy (ABRT) at a single center. METHODS: Children with AFM presenting for rehabilitation between March 2005 and January 2017 were identified and a retrospective chart review was conducted. Changes in medical and functional status were assessed using multiple standardized instruments, as well as a chart review of medical progress. RESULTS: Thirty-one children with AFM treated at our institution in the study time period were identified. Of these, seventeen received inpatient treatment, and fourteen received solely outpatient interventions. Their medical and functional outcomes are described with use of standardized measures when available. CONCLUSIONS: Children with flaccid paralysis due to AFM undergoing structured, comprehensive rehabilitation interventions, even when these are initiated long after paralysis onset, can make significant neurologic and functional gains. Recovery of function and prevention of comorbidities are the main therapeutic targets for interventions in this population.
